1.Expression characteristics of placental steroidogenic enzymes in preeclampsia

ObjectiveTo investigate the expression patterns of key steroidogenic enzymes (CYP11A1, HSD3B1, and CYP19A1) in placental tissues from preeclampsia (PE) patients and evaluate their potential as biomarkers for PE. MethodsA case-control study was conducted using placental tissues from PE patients (PE group, n=3) and normotensive pregnant women (control group, n=3). RNA sequencing was performed to identify differentially expressed genes (DEGs), followed by validation of target gene expression using Western blot, qRT-PCR, and immunohistochemistry. Steroid hormone levels in placental tissues and serum were measured by ELISA. Spearman correlation analysis was used to assess associations between protein expression and clinical parameters, and receiver operating characteristic (ROC) curve analysis was performed to evaluate the predictive value of target genes for PE. ResultsThe results showed that 501 DEGs (P<0.05 and |Log2Fold Change| >1) were identified in PE placental tissues, with KEGG enrichment analysis revealing significant involvement in steroid hormone biosynthesis pathways. Compared with the control group, PE placental tissues exhibited significantly decreased protein and mRNA expression of HSD3B1 and CYP19A1 (P<0.01), while CYP11A1 expression was upregulated (P<0.05). Serum estradiol (E2) and progesterone (P4) levels were significantly reduced in the PE group (P<0.01), whereas testosterone (T) levels were elevated (P<0.05). Correlation analysis demonstrated that CYP11A1 expression was positively associated with systolic blood pressure (SBP), diastolic blood pressure (DBP), and soluble fms-like tyrosine kinase-1 (sFlt-1) (P<0.01), but negatively correlated with placental growth factor (PLGF) (P<0.01). Conversely, HSD3B1 and CYP19A1 expression showed inverse correlations with these clinical parameters (P<0.01). ROC analysis revealed that CYP11A1, HSD3B1, and CYP19A1 each had an area under the curve (AUC) of 0.778 for predicting PE, with 95%CI ranging from 0.614 to 0.942 (P=0.004). The sensitivity was 94.4%, and the specificity was 66.7%. ConclusionThese findings suggest that dysregulated expression of steroidogenic enzymes in PE placental tissues contributes to hormonal imbalance, thereby playing a role in PE pathogenesis. Furthermore, CYP11A1, HSD3B1, and CYP19A1 may serve as potential diagnostic biomarkers for PE.
2.Cost-effectiveness and mortality risk impact on elderly health management of essential public health services:A case study in Henan Province

Objective:To evaluate the cost-effectiveness and impact on mortality of health management services for the elderly aged 65 years and older in national essential public health service project.Methods:Based on the data of county-level medical institutions in Henan Province from 2019 to 2024,the Random Forest Method was used to construct a counterfactual framework to predict the hospitalization expenses under the unmanaged scenario,and then the cost-benefit ratio(BCR)and net income were calculated.Time-dependent Cox proportional hazards model was used to evaluate the effect of health management on all-cause mortality and cardiovascular and cerebrovascular disease mortality in the elderly.Results:A total of 962 955 elderly patients were included,451 119(46.85%)were included in the management group.The average hospitalization cost of the management group was significantly lower than that of the non-management group(P<0.05).Except for 2020-2021,BCRS in 2019 and 2022-2024 were 6.34,2.05,4.45 and 6.60,respectively.The risk of all-cause death was reduced by 76.96%,and the risk of cardiovascular and cerebrovascular death was reduced by 75.57%in the elderly patients included in the management group compared with those not included in the management group.Suggestions:It is necessary to establish a health outcomes-based evaluation system and promote the transformation and upgrading of the service model from single chronic disease management to"integrated health services with multi-disease management".
3.Research Progress on the Pharmacological Effects of Artesunate in the Prevention and Treatment of Periodontitis

Periodontitis as a common chronic inflammatory oral disease,seriously threatens oral and systemic health.Currently,the clinical conventional treatment options for periodontitis mainly include mechanical therapy and drug therapy.Mechanical treatment,such as scaling and curettage,can to some extent remove dental plaque and tartar,but it was difficult to completely remove bacteria in deep periodontal pockets,leading to inflammation that was prone to recurrence.Traditional drug treatments,such as the use of antibiotics,have limited long-term use due to issues such as drug resistance and side effects,while restricted its long-term use.As an important derivative of artemisinin,artemether has various unique pharmacological activities such as anti-inflammatory,antibacterial,immune regulation,and promotion of tissue repair.Currently,multiple basic studies have shown that artemether can be used for the treatment of periodontitis.This article reviews the pharmacological effects of artemether in the prevention and treatment of periodontitis,aiming to provide theoretical basis for the clinical treatment of periodontitis and assist in the development of new treatment plans.
4.Bibliometrics and Visualization of the Regulation of Notch Signaling Pathway by TCM Based on Global Perspective

Objective To analyze the research status,hot directions and frontier trends of traditional Chinese medicine in the prevention and treatment of diseases by regulating Notch signaling pathway based on bibliometrics.Methods Based on Citespace and Vosviewer,the literature on the regulation of Notch signaling pathway by traditional Chinese medicine in CNKI and WoSCC was visually analyzed.Results 362 and 85 related literatures were published in Chinese and English respectively until January 2024.Since 2013,the number of literatures published in this field has shown a fluctuating increasing trend.China is the country with the most publications;Hunan University of Chinese Medicine were the institutions with the most publications in the Chinese database,and Beijing University of Chinese Medicine was the institution with the most publications in the English literature database.Combined with the research direction of each research team and keyword clustering and burst analysis,the research hotspots of traditional Chinese medicine regulating Notch signaling pathway are focused on cerebral ischemia,myelosuppression and hepatic fibrosis.Diseases such as Asthma,colorectal cancer have become emerging research directions in recent years.Electroacupuncture therapy to promote stem cell proliferation and treat neurological diseases is one of the frontier research trends in this field.Conclusion Recent years have seen a rapid development of traditional Chinese medicine's disease prevention and treatment that targets Notch signaling pathway.Various expert teams have obtained rich research results,and the research hotspots show a diversified trend.In-depth exploration of this can provide strong evidence for the molecular mechanism of traditional Chinese medicine in the treatment of various diseases.

7.Study on toxicity and mechanism of Epimedium sagittatum(Sieb.et Zucc.)Maxim based on"serum toxicology"

Aim To evaluate the toxicity and mecha-nism of Epimedium sagittatum(Sieb.et Zucc.)Maxim aqueous extract(ESMAE)on HepaRG cells based on serum toxicology.Methods MTT assay was used to detect the activity of HepaRG cells after treatment with the serum containing ESMAE from SD rats.Western blot was used to detect the effects of the serum contai-ning drug on the expression of endoplasmic reticulum stress-related proteins(PERK,eIF-2α,ATF-4,GRP78,CHOP)and pyroptosis related proteins(NL-RP1,caspase-1,cleaved caspase-1,GSDMD,GSDMD-N).MTT assay was used to detect the activity of Hep-aRG cells after treatment with the liver homogenate containing ESMAE from SD rats.Results Twenty percent serum containing drug significantly decreased the viability of HepaRG cells,with the cells exhibiting swelling,rupture,and vesicle-like pyroptosis.The ex-pression levels of endoplasmic reticulum stress-related proteins PERK,eIF-2α,ATF-4,GRP78,and CHOP significantly increased.The expression levels of pro-teins involved in the NLRP1-mediated classical pyrop-tosis pathway were significantly increased.Finally the liver homogenate containing drug decreased the cell ac-tivity,and cells exhibited swelling,rupture,and vesicle-like pyroptosis.Conclusions After administration of ESMAE,the serum containing drug and the liver ho-mogenate containing drug of rats show toxicity to Hep-aRG cells,and can induce endoplasmic reticulum stress and activate the NLRP1/caspase-1/GSDMD pyroptosis pathway in HepaRG cells.

High altitude heart disease(HAHD)is a chronic mountain sickness in which the body is exposed to high altitude(>2 500 m)hypobaric hypoxia environment for a long time.HAHD has high morbidity and poor prognosis,and pulmonary hypertension is the main causative mechanism for its develop-ment.The phosphodiesterase-5 inhibitor sildenafil has become a hot drug for the treatment of pulmonary hypertension.This paper reviews the progress of HAHD and discusses the mechanism of action and effectiveness of sildenafil in the treatment of HAHD,with a view to providing a basis for the treatment of HAHD with sildenafil.
10.Application value of serum Hsp90α combined with β2-MG detection in early diagnosis and prognosis of colorectal cancer

Objective To explore the application value of serum heat shock protein 90α(Hsp90α)combined with β2-microglobulin(β2-MG)detection in the early diagnosis and prognosis of colorectal cancer(CRC).Methods A total of 101 patients with CRC who received their first treatment were selected as the CRC group(no surgery,radiotherapy or chemotherapy before enrollment),100 patients with benign colon tumor admitted to our hospital during the same period were selected as the benign tumor group and 94 healthy individuals who underwent physical examinations in our hospital were used as the control group.The levels of serum Hsp90α and β2-MG in all subjects were detected by ELISA.Follow-up was conducted every three months within the first two years after surgery,and every six months in the third year,for a total follow-up period of 3 years,and the survival status of CRC patients within 3 years was recorded.ROC curve analysis was used to evaluate the diagnostic value of serum Hsp90α and β2-MG for CRC.The Kaplan-Meier method was used to draw the survival curves of CRC patients with different expression levels of Hsp90α and β2-MG.Results The serum levels of Hsp90α and β2-MG increased successively in the control group,the benign tumor group and the CRC group(P<0.05).There were no significant differences in serum levels of Hsp90α and β2-MG between CRC patients of different genders,ages and lesion locations.The levels of serum Hsp90α and β2-MG in patients with tumor diameter>5 cm,low differentiation degree,TNM stage Ⅲ and lymph node metastasis were higher than those in patients with tumor diameter≤5 cm,moderate to high differentiation degree,TNM stage Ⅰ-Ⅱ and no lymph node metastasis(P<0.05).The ROC results showed that the combined diagnostic efficacy of serum Hsp90α and β2-MG was superior to that of serum Hsp90α or β2-MG alone(Z=2.433,2.435,P<0.05).Based on the average expression levels of serum Hsp90α and β2-MG in CRC patients(76.66 μg/L and 6.52 μg/L),patients were divided into the high expression group of Hsp90α(59 cases),the low expression group of Hsp90α(42 cases),the high expression group of β2-MG(63 cases)and the low expression group of β2-MG(38 cases).Among the 101 CRC patients,31 died and 70 survived,with a total survival rate of 69.31%.The Kaplan-Meier survival curve showed that the 3-year survival rates of patients with high expression levels of Hsp90α and β2-MG were lower than theose of patients with low expression levels of Hsp90α and β2-MG(P<0.01).Conclusion The expression levels of serum Hsp90α and β2-MG are both elevated in CRC patients,and they are closely related to the occurrence,development and prognosis of CRC.
