1.Value of dual-source CT in prognosis evaluation after anterior cruciate ligament injury and reconstruction

Objective To explore the clinical application value of dual-source CT (DSCT) in imaging evaluation after anterior cruciate ligament (ACL) injury and reconstruction. Methods A total of 93 patients with ACL injury confirmed in our hospital between December 2023 and December 2024 were enrolled as the research subjects. All patients underwent arthroscopic single-bundle anatomical reconstruction of ACL, as well as DSCT and multi-slice spiral CT (MSCT) examinations at 1 month after surgery. ACL scan parameters, bone tunnel aperture center, and radiation dose parameters (CT dose index volume, dose-length product, effective dose) under DSCT and MSCT were compared. At 3 months after surgery, according to postoperative curative effect by International Knee Documentation Committee (IKDC) scoring method, patients were divided into favorable outcome group (IKDC ≥ 70 points, n = 69) and unfavorable outcome group (IKDC < 70 points, n = 24). The two groups were compared for bone tunnel angles [angle between femoral tunnel and joint line (γ angle), angle between femoral tunnel and femoral shaft (θ angle), angle between tibial tunnel and tibial shaft (α angle), angle between tibial tunnel and joint line (β angle)]. The predictive value of bone tunnel angles under DSCT in poor prognosis of patients after ACL reconstruction was analyzed by receiver operating characteristic curves. Results The intraclass correlation coefficients for image quality of ACL length, ACL angle, long diameter of femoral attachment point, long diameter of tibial attachment point, Fx, Fy, Tx, Ty, γ angle, θ angle, α angle, and β angle were 0.812, 0.835, 0.823, 0.834, 0.836, 0.847, 0.841, 0.834, 0.835, and 0.864, respectively. The long diameters of femoral attachment point and tibial attachment point under DSCT were shorter than those under MSCT (P < 0.05), Tx and Ty were lower than those under MSCT (P < 0.05), CT dose index volume, dose-length product, and effective dose were lower than those under MSCT (P < 0.05). The γ angle and θ angle in the favorable outcome group were lower than those in the unfavorable outcome group (P < 0.05). Receiver operating characteristic curve analysis showed that after ACL reconstruction, the areas under the receiver operating characteristic curves of γ angle, θ angle, and their combination under DSCT for predicting poor prognosis were 0.711, 0.712, and 0.783, respectively, with sensitivities of 76.81%, 79.71%, and 92.75% as well as specificities of 70.83%, 75.00%, and 62.50%, respectively (P < 0.05). Conclusion DSCT can evaluate the poor prognosis of patients after ACL reconstruction through ACL scan parameters, bone tunnel aperture center, γ angle, and θ angle, which also can effectively reduce radiation dose.
2.Clinical and genetic analysis of a patient with FSIP2 compound heterozygous variants causing multiple morphological abnormalities of sperm flagella.

OBJECTIVE:
The aim of this study is to analyze the clinical features and genetic etiology of a patient with multiple morphological abnormalities of the sperm flagella (MMAF) retrospectively.
METHODS:
A severely oligospermic patient from the Reproductive Center of the First Affiliated Hospital of Ningbo University was selected as the study subject. Clinical data and examination results were collected. High-throughput sequencing and bioinformatics were used to analyze the genetic etiology. And Sanger sequencing was employed to validate findings in the family. Transmission electron microscopy (TEM) was used to observe the sperm ultrastructure, and immunofluorescence analysis was performed to examine the localization of FSIP2 protein in the sperm.
RESULTS:
The patient presented with severe oligospermia, and sperm morphology displayed MMAF. TEM revealed fibrous sheath and 9+2 microtubule structural disruptions in the sperm. Sequencing identified compound heterozygous variants in the FSIP2 gene (c.17798C > T, c.5927T > G), inherited from the father and mother, respectively. According to the guidelines of the American College of Medical Genetics and Genomics, the variants were classified as pathogenic. The patient's spouse underwent intracytoplasmic single sperm injection, resulting in one embryo, but no clinical pregnancy occurred after embryo transfer.
CONCLUSION
This study reported the mutation of FSIP2 gene c.17798C > T, c.5927T > G in a patient with MMAF. These findings expand the mutational spectrum of the FSIP2 gene and provide insights for genetic and assisted reproductive counseling for patients with MMAF.

3.Simultaneous content determination of nine constituents in Chaige Changyuan Mixture by UPLC-MS/MS

AIM To establish a UPLC-MS/MS method for the simultaneous content determination of isofraxidin,saikosaponin c,ginsenoside Re,puerarin,rosmarinic acid,praeruptorin A,daidzein,baicalin and 5-O-methylvisammioside in Chaige Changyuan Mixture.METHODS The analysis was performed on a 35 ℃ Titank C18 column(100 mm×2.1 mm,3 μm),with the mobile phase comprising of acetonitrile-0.1%formic acid flowing at 0.3 mL/min in a gradient elution manner,and electron spray ionization source was adopted in positive and negative ion scanning with multiple reaction monitoring mode.RESULTS Nine constituents showed good linear relationships within their own ranges(r≥0.999 0),whose average recoveries were 84.08%-115.40%with the RSDs of 0.21%-4.47%.CONCLUSION This efficient,simple,sensitive and specific method can be used for the quality control of Chaige Changyuan Mixture.
4.Roles of IgG1 and IgG2a subtypes in anti-glycoprotein Ⅰbα antibody-induced platelet clearance

Objective:To investigate the role of the IgG subtypes (IgG1 and IgG2a) in anti-glycoprotein (GP) Ⅰbα antibody-induced platelet clearance.Methods:Venous blood was collected from healthy volunteers, and platelets were separated. The phagocytosis of human platelets by human acute monocytic leukemia cells (THP-1 cells) induced by different anti-GPⅠbα antibodies (AN51, AK2, HIP1, TM60, VM16d, WM23, and SZ2) was detected by flow cytometry. The effects of the AN51 full-length antibody, F (ab') 2, and Fab fragments on platelet phagocytosis by THP-1 cells were detected by flow cytometry. Then, the Fc blocking antibody 2.4G2 and normal rat IgG2a or IgG1 were injected into C57BL/6J mice via the posterior ocular vein, and their effects on platelet reduction induced by R300 were detected by a hematology analyzer. Results:Compared with IgG1, the IgG2a subtype of anti-GPⅠbα antibodies induced the phagocytosis of platelets by THP-1 cells in vitro ( P<0.05). In contrast to the AN51 full-length antibody, neither AN51 F (ab') 2 nor the Fab fragment could induce THP-1 cells to phagocytose platelets ( P<0.05). Compared with the control group, anti-mouse GPⅠbα R300-induced thrombocytopenia in mice was reduced at 2, 4, and 6 h after the injection of Fc blocking antibody 2.4G2 ( P<0.05). Similarly, R300-induced thrombocytopenia in mice was reduced at 2, 4, and 6 h after the injection of rat IgG2a ( P<0.05) . Conclusion:IgG2a plays an important role in anti-GPⅠbα-induced clearance.
5.Study on protection of cerebral ischemia-reperfusion injury by HSYA activated neuronal autophagy based on SIRT1

Objective:To investigate effect and mechanism of hydroxysafflor yellow A(HSYA)activating neuronal autophagy on cerebral ischemia-reperfusion injury through a combination of in vitro and in vivo experiments.Methods:SD rat MCAO/R model was established by improved suture method.Rats were randomly divided into sham surgery(Sham)group,MCAO/R group and MCAO/R+HSYA group,following indicators were detected to determine extent of cerebral ischemia-reperfusion nerve damage:Z-Longa neu-rological function score was detected,TTC staining to measure cerebral infarction area,and TUNEL staining to measure cell apopto-sis;Western blot was used to detect protein expressions of autophagy related markers LC3,Beclin1,P62 and SIRT1 in rat brain tis-sue;immunofluorescence staining was used to observe expression of LC3 co-localization with neurons.OGD/R injury model of SH-SY5Y cells was established and randomly divided into Normal group,OGD/R group,OGD/R+HSYA group,OGD/R+SIRT1 inhibitor(EX-527)group and OGD/R+EX-527+HSYA group.Western blot was used to detect protein expressions of LC3,Beclin1,P62 and SIRT1.Results:Compared with Sham group,model group rats showed impaired neurological function,significantly increased neu-robehavioral scores,widespread cerebral infarction,significantly increased neuronal cell apoptosis,significantly increased autophagy related protein Beclin1 expression and LC3-Ⅱ/LC3-Ⅰ,significantly decreased P62 expression,significantly increased LC3/NeuN co-stained cells,and decreased SIRT1 expression;compared with model group,HSYA intervention group showed a significant decrease in neurological functional scores,a significant reduction in cerebral infarction area,a significant decrease in neuronal cell apoptosis,a further increase in Beclin1 expression and LC3-Ⅱ/LC3-Ⅰ,a further decrease in P62 expression,number of LC3/NeuN and P62/NeuN co-stained cells also increased,and SIRT1 expression significantly increased.Expression trends of Beclin1,LC3-Ⅱ/LC3-Ⅰ,P62 and SIRT1 of cells between normal group,model group and HSYA intervention group were same as animal experiment;compared with model group,expressions of SIRT1,Beclin1 and LC3-Ⅱ/LC3-Ⅰ in OGD/R+EX-527 group were significantly reduced,while expression of P62 was significantly increased;compared with OGD/R+EX-527 group,there was no significant change in SIRT1 expression in OGD/R+EX-527+HSYA group,LC3-Ⅱ/LC3-Ⅰ and Beclin1 expression were significantly increased,and P62 expres-sion was significantly decreased.Conclusion:HSYA can significantly improve neurological deficits in rats after cerebral ischemia-reperfusion,reduce cerebral infarction area,and decrease neuronal cell apoptosis rate,whose neuroprotective effect may be related to its activation of SIRT1,which significantly enhances neuronal autophagy.
6.The relationship between the time within the blood glucose target range and obstructive sleep apnea hypopnea syndrome in type 2 diabetes mellitus patients

Objective To explore the correlation between time within target range of blood glucose(TIR)and obstructive sleep apnea hypopnea syndrome(OSAHS)in type 2 diabetes mellitus(T2DM)patients.Methods 435 T2DM patients hospitalized in the Department of Endocrinology,Second People's Hospital of Hefei from January 2019 to September 2023 were selected.According to the sleep apnea hypopnea index(AHI)and the lowest blood oxygen saturation(LSaO2),they were divided into 270 simple T2DM patients(T2DM),100 diabetes patients with mild OSAHS(MOS)and 65 diabetes patients with moderate to severe OSAHS(SOS).The transient glucose monitoring system was used to complete dynamic glucose monitoring and calculate TIR.According to the 25th,50th and 75th percentile values of TIR,the study subjects were divided into four groups:Q1 group(TIR<51%),Q2 group(TIR 51%~65%),Q3 group(TIR 65%~78%)and Q4 group(TIR>78%).Results Compared with T2DM group,the TIR levels of patients in MOS and SOS groups decreased(P<0.05).Spearman correlation analysis showed a negative correlation between TIR and AHI(P<0.05),and a positive correlation with LSaO2(P<0.05).Compared with Q1 group,Q4 group had shorter duration of DM,lower levels of HbA1c,UACR and AHI,lower incidence of coronary heart disease and stroke(P<0.05),but higher levels of eGFR and LSaO2(P<0.05).Logistic regression showed that male,BMI,DBP,TC,UACR and HOMA-IR were the influencing factors for OSAHS.TIR levels were the influencing factors for OSAHS(OR 0.960,95%CI 0.937~0.982,P=0.004)and moderate and Severe OSAHS(OR 0.922,95%CI 0.879~0.974,P=0.002).Compared with Q4 group,the Q1 group had the highest risk of OSAHS(OR 3.745,95%CI 1.128~12.046,P=0.034).Conclusions In type 2 diabetes mellitus patients,men,BMI,DBP,TC,UACR,HOMA-IR are related to the occurrence of OSAHS,OSAHS is significantly related to the decline of TIR level.
7.Regulation of autophagy on diabetic cataract under the interaction of glycation and oxidative stress

Diabetic cataract, a prevalent ocular complication of diabetes mellitus, arises from a complex interplay of pathological mechanisms, with oxidative stress and glycation stress playing central roles. Autophagy, a critical cellular self-protection mechanism, sustains intracellular homeostasis by selectively degrading damaged organelles and misfolded proteins, thereby counteracting the detrimental effects of oxidative and glycation stress under hyperglycemic conditions. Emerging evidence indicates a synergistic interaction between glycation stress and oxidative stress, which may exacerbate autophagic dysfunction and accelerate the onset and progression of diabetic cataract. However, the precise molecular mechanisms underlying this relationship remain incompletely understood. This review systematically examines the regulatory role of autophagy inthe pathogenesis of diabetic cataract, with a particular focus on how autophagic impairment influences disease progression under the combined effects of glycation and oxidative stress. By elucidating these mechanisms, the paper aims to provide novel insights into molecular diagnostic approaches and targeted therapeutic strategies for diabetic cataract.

Coronavirus disease 2019 (COVID-19) is an acute infectious respiratory disease that has been prevalent since December 2019. Chinese medicine (CM) has demonstrated its unique advantages in the fight against COVID-19 in the areas of disease prevention, improvement of clinical symptoms, and control of disease progression. This review summarized the relevant material components of CM in the treatment of COVID-19 by searching the relevant literature and reports on CM in the treatment of COVID-19 and combining with the physiological and pathological characteristics of the novel coronavirus. On the basis of sorting out experimental methods in vivo and in vitro, the mechanism of herb action was further clarified in terms of inhibiting virus invasion and replication and improving related complications. The aim of the article is to explore the strengths and characteristics of CM in the treatment of COVID-19, and to provide a basis for the research and scientific, standardized treatment of COVID-19 with CM.

9.Seroepidemiological of hepatitis B among outpatients in medical institutions in Jiaxing City

Objective:
To investigate the seroepidemiological characteristics of hepatitis B among outpatients in medical institutions in Jiaxing City, Zhejiang Province, so as to provide a reference for formulating region-specific hepatitis B prevention and control strategies.
Methods:
From April to June 2024, outpatients were selected as study subjects from sentinel medical institutions in Jiaxing City. Information such as gender and age was collected. Venous blood samples were obtained and serological markers including hepatitis B surface antigen (HBsAg), hepatitis B surface antibody (HBsAb), hepatitis B e antigen (HBeAg), hepatitis B e antibody (HBeAb), and hepatitis B core antibody (HBcAb) were tested. Positive rates of hepatitis B virus (HBV) serological markers were analyzed by genders and ages.
Results:
A total of 1 468 outpatients were included, among whom 721 were males (49.11%) and 747 were females (50.89%). The mean age was (46.41±19.66) years. The positive rates of HBsAg, HBsAb, HBeAg, HBeAb, and HBcAb were 7.29%, 44.75%, 1.84%, 23.50%, and 42.03%, respectively. The HBcAb positive rate in males was significantly higher than in females (46.05% vs. 38.15%, P<0.05), while no statistically significant gender differences were observed in the positive rates of other four HBV serological markers (all P>0.05). Except for HBsAb, the positive rates of the other four HBV serological markers showed statistically significant differences across different age groups (all P<0.05). Pairwise comparisons results showed that the HBsAg positive rates in age groups of 20-<40 years and 40-<60 years were 9.48% and 9.57%, respectively, which were higher than those in age groups of <20 years (1.43%) and ≥60 years (2.75%) (all P<0.05). A total of 17 HBV serological marker patterns were observed, among which the proportion of all markers negative was the highest, at 39.65%. The proportions of "small three positive" (HBsAg+, HBeAb+, HBcAb+) and "large three positive" (HBsAg+, HBeAg+, HBcAb+) patterns were 4.77% and 1.50%, respectively. Among HBsAg-positive individuals, the proportions of the "small three positive" pattern across age groups were 0, 45.45%, 90.00%, and 81.82%, while those of the "large three positive" were 0, 36.36%, 5.00%, and 0, with statistically significant differences across age groups (both P<0.05).
Conclusions
The positive rate of HBsAg among outpatients in medical institutions in Jiaxing City is relatively high, with a notable proportion of individuals showing either no immunity or non-response to vaccination. It is recommended to strengthen hepatitis B immunization efforts among the population aged 20-<60 years, and to enhance monitoring and interventional treatment for "small three positive" and "large three positive" patterns.
10.The clinical value of the procalcitonin-to-albumin ratio in predicting short-term prognosis in patients with acute cerebral infarction

Objective:To explore the predictive value of procalcitonin-to-albumin ratio (PAR) in the short-term prognosis of sequential treatment with butylphthalide for acute cerebral infarction (ACI) after interventional surgery.Methods:The clinical data of ACI patients admitted to 970th Hospital of the Joint Logistics Support Force of the People′s Liberation Army of China from January 2020 to June 2022 were retrospectively included. According to the prognosis of sequential treatment of butylphthalide after interventional therapy, 80 patients with poor prognosis were included in the poor group, and 80 patients with good prognosis were included in the good group. The detection data of procalcitonin, albumin and other laboratory indexes of the two groups of patients on admission were collected, the PAR value was calculated, and the predictive value of PAR on the short-term prognosis of butylphthalide sequential therapy after ACI intervention was analyzed.Results:The levels of procalcitonin (PCT), PAR, D-dimer (D-D), and low density lipoprotein cholesterol (LDL-C) in the poor group were higher than those in the good group: (24.74 ± 5.37) ng/L vs. (20.96 ± 4.11) ng/L, 0.91 ± 0.35 vs. 0.62 ± 0.19, (0.75 ± 0.22) mg/L vs. (0.56 ± 0.17) mg/L, (3.28 ± 0.43) mmol/L vs. (3.03 ± 0.45) mmol/L, while the levels of albumin (ALB) were lower than those in the good group: (29.43 ± 4.25) g/L vs. (33.71 ± 4.53) g/L, with a statistical significant difference ( P<0.05). Multivariate Logistic regression analysis showed that PCT, ALB, PAR, D-D, and LDL-C were all factors influencing the short-term prognosis of sequential treatment with butylphthalide after ACI intervention ( P<0.05). The results of receiver operating characteristic curve (ROC) showed that PCT, ALB, PAR, D-D and LDL-C had certain predictive value for the short-term prognosis of butylphthalide sequential therapy after ACI intervention, and PAR had the best predictive value. Conclusions:PAR is an influential factor for the sequential short-term prognosis of butylphthalide after ACI intervention, and it has certain predictive value for the short-term prognosis of patients.
