A 61-year-old woman presented with a 2-month history of non-painful left eye proptosis. Imaging studies showed a superotemporal mass in the left orbit with intracranial extension. Surgical excision of the orbitocranial mass was performed and histopathologic examination revealed metastatic papillary thyroid carcinoma. She subsequently underwent total thyroidectomy. Orbital metastasis from thyroid carcinoma is rare and can be the initial manifestation of occult disease in 63% of cases.

3.Development of core curriculum for cytopathology fellowship training in the Philippines

Cytopathology in the Philippines is currently practiced by board-certified anatomic pathologists who receive only a three-month rotation in the discipline during their four-year combined Anatomic and Clinical Pathology residency training. At present, the country lacks a formal fellowship program in cytopathology, and no national core curriculum has been developed for this subspecialty. Local pathologists with advanced training and extensive experience in cytopathology recognized by the Philippine Society of Pathologists, Inc. (PSP) developed the first national core curriculum for cytopathology fellowship training. This initiative marks the first locally developed guideline for cytopathology fellowship training in the Philippines, representing a significant milestone in the advancement of subspecialty pathology education in the country.

4.Cytopuncture microbiopsy during transcanalicular endoscopic lacrimal duct recanalization: A novel approach

Objective:
To describe the cytopuncture microbiopsy (CM) technique performed during transcanalicular endoscopic lacrimal duct recanalization (TELDR) in patients with primary acquired nasolacrimal duct obstruction (PANDO) as an alternative to open biopsy, the standard method used in collecting specimen.
Methods:
This is a noncomparative, interventional case series with histopathologic correlation. Patients diagnosed with complete PANDO who underwent TELDR with balloon dacryoplasty and silicone intubation with CM at University of Santo Tomas Hospital from October 2014 to January 2017 were included.
Results:
Twenty (20) tissue specimens from the lacrimal sac and nasolacrimal duct were obtained from 18 patients. There were 16 females and 2 males included in the study. Mean age was 57.5 years. All specimens revealed few clusters of benign epithelial cells with few degenerated mononuclear cells and lymphocytes, and singly scattered lymphocytes that are set in fibrinous background. Tissue cytology studies were negative for malignant cells.
Conclusion
CM is a minimally invasive procedure that offers an alternative to open biopsy technique that can be done routinely during TELDR.

5.A Three-Year Review of the Clinical and Pathologic Profile of Patients with Colonic Polyps in a University Hospital in Metro Manila (2014-2016)

Background :
The data on the prevalence and distribution of polyps in Asians is limited with conflicting data about the most common type.
Objective:
To obtain recent data about the histologic types of endoscopically labeled as polyps by
colonoscopy and correlate with the clinico-pathologic profile
Method:
Retrospective cross-sectional review of histopathologic and endoscopic reports of colonoscopy with biopsy of colonic polyps of patients in the
University of Santo Tomas Hospital (January 2014-
December 2016).
Results:
3910 colonoscopies were performed and
a total of 302 patients were retrieved and 500 polyps were resected. The most common indication was
hematochezia and hemorrhoids. The colonic polyps
were solitary in 36% of the cases. Majority were seen
in 50-69 years old, left sided region (78%), particularly the sigmoid (37%), and sessile (77.3%). The
most common type is tubular adenoma (45%) and
majority of the adenomatous polyps were seen in
the sigmoid. There is significant association between
age and presence of an adenomatous polyp where
≥70 years old are about 2.5 times more likely to
have adenomatous polyp and pedunculated polyps
are 2 times more likely to be adenomatous. There is
no significant association between presence of an
adenomatous polyp and polyp size and gender.
Conclusion
The type and distribution of colorectal
polyps are similar with others and vigilant approach
of the left side should be exercised. Absence of any
significant difference between size and adenomatous nature necessitate the need for early identification and removal of colorectal polyp in preventing
morbidity and mortality from cancer.

7.The correlation of the percentage of positive core biopsy with pathological stage and grade in patients who underwent radical retropubic prostatectomy for prostate cancer.

OBJECTIVE: To establish that the percentage of positive core biopsy for prostate cancer is correlated with transrectal ultrasound prostate volume, pathological stage and grade.
MATERIALS AND METHODS: We presented our experience with 47 patients (mean age 65.74 years) with clinically localized prostate cancer who underwent radical retropubic prostatectomy. All patients underwent transrectal ultrasound guided biopsy of the prostate. Multiple cores ranging between 6 - 20 with a mean of 12 were taken and sent for histopathologic studies. Percentage of cores positive for prostate cancer was computed and correlated with the transrectal ultrasound volume of the prostate, pathologic tumor stage and Gleason score.
RESULTS: Correlation of prostate volume with the percentage of positive core biopsy using Pearson's correlation was insignificant having a p-value of 0.197 with the level of significance set at 0.05. Tumor stage and Gleason score were positively correlated with the percentage of positive biopsy core using the Spearman's correlation having p-values of 0.003 and 0.042, respectively, with the level of significance set at 0.05.
CONCLUSION: As the percentage of positive core biopsy increases, tumor stage and Gleason score also increase. Prostatic volume showed no correlation with the percentage of positive biopsy core.

8.Radical retropubic prostatectomy for prostate adenocarcinoma: A case series.

Objectives: This paper aimed to describe the characteristics of patients with clinically localized prostate cancer who underwent radical prostatectomy according to age, transrectal ultrasound-guided biopsy (TRUS) Gleason's score, histopathologic Gleason's score, histopathologic tumor stage, number of transfusions, complications and type of anesthesia used. It further aimed to compare the no. of complications by mean age, PS A levels, TRUS prostate volume, operating time, blood loss and hospital stay among patients with and without complications. Finally, it aimed to determine significant association of TRUS biopsy Gleason's score, histopathologic Gleason's score , histopathologic tumor stage, number of transfusions, and type of anesthesia used with the development of complications. Methods: This is a retrospective cohort study done by examining hospital records of patients with clinically localized prostate cancer (stage 2c and below), who underwent radical prostatectomy from 1996-2005 at UST Hospital, done by a single surgeon. Data were analyzed with SPSS v.ll, using Cramer's V statistic at the 0.05 level of significance. Results/Conclusion: A total of 41 subjects were included in the study. Subjects had a mean age of 65.85 years, with mean PSA levels of 16.22 ng/dL and TRUS volume of 35.64ml. All subjects had Gleason's Biopsy scores of 4 and above, with majority having scores between 4-6 (78.04 percent). Histopathologic results showed that 82.9 percent of the subjects had scores between 4 to 6. The majority of subjects had tumor stage of T2c (75.6 percent). Thirty-two of 41 cases (78 percent) did not require blood transfusions and only 17 percent (7/41) had post-operative complications, in the form of bladder neck contractures, clot retention, iatrogenic external iliac vein lacerations, postoperative ileus and incontinence. Majority (40/41) required only epidural anesthesia. The mean operating time was 3.65 hours with blood loss of 847.56 ml. Average hospital stay was 5.29 days. TRUS biopsy Gleason's score, histopathologic Gleason's score, histopathologic tumor stage, type of anesthesia and blood transfusions were not significantly associated with development of postoperative complications. 9.Scrimp technique in the rapid cytologic diagnosis of surgical specimens - A local experience.

A rapid tissue preparation to establish the diagnosis of surgical specimens was reported. The specific reason to request such a preparation usually relates to a suspicion of a neoplastic lesion. This study was conducted to evaluate the role of scrimp technique, a modification of imprint cytology, in the rapid intraoperative diagnosis of tumors as compared with frozen section. Scrape and imprints of tumors were made from 86 unfixed specimens at the time of frozen section. Rapid and permanent staining methods were employed and the accuracy of diagnosis was compared with frozen and paraffin preparations. The results showed that the sensitivity and specificity of scrimp preparation were 95% each as compared with frozen section which has 98% and 100% respectively which manifested a negligible inferiority over frozen section. This technique was simple, fast, easy, and reliable and does not require special instruments or time consuming set-up. While not meant to replace frozen section, it can be an excellent adjunct or alternative especially in remote and secondary hospitals unequipped with sophisticated apparatus. (Author)
