OBJECTIVETo evaluate the efficacy of specific sublingual immunotherapy (SLIT) with Dermatophagoides farinae drops on children with allergic asthma and allergic rhinitis of the preschool and school-age groups of children and adolescents.
METHODThis study analyzed the efficacy of SLIT in 122 children (aged 3-14 yr) with house dust mites-induced allergic asthma and allergic rhinitis. According to the age, patients were defined as the preschool group ( ≤ 6 years old, n = 59) and school-age group (> 6 years old, n = 63). All children were treated with Dermatophagoides farinae drops for at least 2 years. Clinical observation and follow-up study were conducted during the treatment. Before and after SLIT for half a year, 1 year and 2 years, asthma symptom scores (day and night), rhinitis symptom scores and medication scores were evaluated. The adverse events were assessed during the study.
RESULTAfter SLIT for half a year, 1 year and 2 years, there were no significant differences between the preschool group (0.3 ± 0.5,0.0 ± 0.1,0.0 ± 0.0) and school-age group (0.3 ± 0.4,0.0 ± 0.1,0.0 ± 0.0) in day scores of asthma (Z value was -1.687, -0.613,0.000, all P > 0.05). There were no significant differences between the preschool group (0.2 ± 0.5,0.1 ± 0.3,0.0 ± 0.0) and school-age group (0.2 ± 0.4,0.1 ± 0.3, 0.0 ± 0.0) in night scores of asthma (Z value was -0.496, -0.486,0.000, all P > 0.05). There was no significant differences between the preschool group (1.4 ± 0.9,0.4 ± 0.5,0.1 ± 0.3) and school-age group (1.3 ± 0.9,0.5 ± 0.6,0.2 ± 0.4) in symptom scores of allergic rhinitis (Z value was -0.394, -1.166, -1.075, all P > 0.05). There were no significant differences between the preschool group (1.6 ± 0.8,0.0 ± 0.0,0.0 ± 0.0) and school-age group (1.7 ± 0.7,0.0 ± 0.0,0.0 ± 0.0) in medication scores of allergic rhinitis (Z value was -0.655,0.000,0.000, all P > 0.05). After SLIT for 2 years, most children in the preschool and school-age groups were no longer using asthma controlling medication, with no significant difference between the two groups (Z value was 0.000, P > 0.05). The overall incidence of adverse reactions was only 7%, and there was no significant difference in the incidence of adverse reactions between the two groups (χ(2) = 0.000, P > 0.05). The local adverse events were mild gastrointestinal discomfort and rash, a majority of local adverse events in the preschool group were diarrhea. No severe adverse events happened in the treatment.
CONCLUSIONSLIT with Dermatophagoides farinae drops is safe and effective in children with allergic asthma and allergic rhinitis of the preschool and school-age groups of children and adolescents, which provides evidences for early SLIT intervention of the disease.

Journal of Clinical Otorhinolaryngology Head and Neck Surgery 2013;27(16):899-904
OBJECTIVE:
In this study, we investigated the anti-inflammation effects of Xuebijing in OVA-induced murine allergic rhinitis model. Furthermore, we determined whether heme oxygenase (HO)-1 is required for the protective activity of Xuebijing.
METHOD:
Airways of OVA-sensitized mice exposed to OVA challenge developed eosinophilia, mucus hypersecretion and increased cytokine levels. Levels of interleukin IL-4, IL-5, IL-13, and tumor necrosis factor (TNF)-alpha in nasal lavage fluid were measured using enzyme-linked immunosorbent assays (ELISAs). Lung tissue and nasal mucosa sections were stained with Mayer's hematoxylin and eosin for assessment of cell infiltration and mucus production, Immunohistochemistry, Real-time PCR and Western Blot analyses for HO-1 protein expression.
RESULT:
Orally administered Xuebijing significantly inhibited the number of OVA-induced inflammatory cells and IgE production, along with reduced T-helper (Th) 2 cytokine levels, such as IL-4, IL-5 and IL-13, improved the level of IFN-gamma, in nasal lavage fluid. In addition, Xuebijing induced a marked decrease in OVA-induced inflammatory cell infiltration and mucus production in nasal and lung tissues. These effects were correlated with HO-1 mRNA and protein induction.
CONCLUSION
Our results indicate that Xuebijing protects against OVA-induced airway inflammation, at least in part, via HO-1 upregulation.

Journal of Clinical Otorhinolaryngology Head and Neck Surgery 2013;27(8):404-406
OBJECTIVE:
To investigate the correlation of serum specific IgE and skin prick test in allergic rhinitis patients sensitive to dust mite and the difference between the results.
METHOD:
Data of 349 allergic rhinitis patients who had positive result in either Dermatophagoides pteronyssinus(Der p) or Dermatophagoides farinae (Der f) by serum specific IgE and skin prick test were statistically analyzed.
RESULT:
Grades of Skin prick test and specific IgE levels were notably relevant in these two dust mites(Der p r= 0. 568, Der f r= 0. 506, P<0. 01). There was significant difference of positive rates between serum specific IgE and skin prick test in Der p(Chi2 = 11. 605, P<0. 01)but not in Der f(Chi2 =0, P>0.05). There was no correlation between positive degree of two methods and score of clinical symptoms in allergic rhinitis.
CONCLUSION
Serum specific IgE and skin prick test were notably relevant in allergic rhinitis patients sensitive to dust mites. The positive rates of the two methods were different due to different allergen. Therefore, they could not substitute for each other. The level of serum specific IgE and positive degree of skin prick test could not reflect the degree of symptom in allergic rhinitis.

Journal of Clinical Otorhinolaryngology Head and Neck Surgery 2013;27(5):255-263
OBJECTIVE:
The aim of this study was to explore the changes of the extracellular matrix in nasal mucosa by a guinea pig model of prolonged allergic-induced rhinitis.
METHOD:
Thirty-two male Hartley guinea pigs were randomly divided into four groups: allergen challenged groups (Group 2 w, Group 6 w and Group 12 w) and a control group. Ovalbumin-sensitized guinea pigs were repeatedly challenged with allergen twice a week from 2 weeks to 12 weeks. Matched control groups were challenged with physiological saline. Nasal mucosa were obtained from the animals killed. Hematoxylin-Eosin, Masson's trichrome, and immunohistochemical staining against transforming growth factor-beta1 (TGF-beta1), Collagen III and Collagen I were performed to nasal mucosa.
RESULT:
(1) Pathological examination showed obvious infiltration of eosinophils and the enlarged thickness of epithelial layer of nasal mucosa in the experiment groups. (2) The area ratios of blue stained in the extracellular matrix of nasal mucosa were increased. The area ratios of blue stained were statistically different in Group 6 w and Group 12 w compared with the control group. (3) The increasing absorbance of TGF-beta1 were statistically different in the experiment groups with the control group. The absorbance of Collagen III and Collagen I showed a rising trend along prolonged allergen challenged in the experiment groups.
CONCLUSION
Prolonged allergen challenge and the inflammation of nasal mucosa, can lead to the increasing of the inflammation relevant factors and the deposit of collagen in the extracellular matrix of nasal mucosa.

Journal of Clinical Otorhinolaryngology Head and Neck Surgery 2013;27(5):246-250
OBJECTIVE:
To investigate which aeroallergens were prevalent in patients with allergic rhinitis in Yunnan.
METHOD:
Retrospective analysis of the medical records of intradermal test performed in 1893 AR patients. The samples were divided into four age groups: 4-17-year-olds, > 17-35-year-olds, > 35-50-year-olds and > 50-70-year-olds. According to gender, the samples were decided into two groups: the males and the females. The positive rate of aeroallergens were compared among the groups.
RESULT:
(1)The total positive rate of intradermal test was 70.1%. The top ten allergens were as follow: dermatophagoides pteronyssinus, dermatophagoides farina, house dust, pollen of the summer and autumn, alnus nepalensis, chenopodiaceae, mugwort, poaceae, brassica and cockroach. (2) In all groups, patients with triple positive allergens were the most common, followed by dual positive allergens and single positive allergen. (3) Among the top ten allergens, the top three positive ones in all groups were dermatophagoides pteronyssinus, dermatophagoides farina and house dust. And there were significant differences between 4-17-year-olds group and the other age groups (P < 0.01). (4) There was no significant difference between male and female patients in distribution of aeroallergens except House dust and mite.
CONCLUSION
The most common allergen in patients with allergic rhinitis in Yunnan is mite, the follow are pollen of the summer and autumn, alnus nepalensis, chenopodiaceae, mugwort, poaceae, and brassica. The distribution of aeroallergens are different among the age groups, but has no evident correlation with gender.

9.Analysis of total and specific IgE in serum of carvota mitis pollen-induced allergic rhinitis patients.
Journal of Clinical Otorhinolaryngology Head and Neck Surgery 2013;27(2):78-80
OBJECTIVE:
Exploring the relationship between total and specific IgE in serum and allergen skin test of carvota mitis pollen-induced allergic rhinitis patients.
METHOD:
Four hundred and-twenty-nine carvota mitis pollen-induced allergic rhinitis patients and 243 healthy control subjects were recruited. The experimental group carried out skin tests. and pollen-specific IgE were also examined by BSA-ELISA method. Total IgE in serum of all of the subjects were determined by ELISA.
RESULT:
The positive rate of the total IgE level of the patients were much higher than those of the controls (66.2% vs. 15.6%, P < 0.01). No statistically significance was found between the positivity of skin test and serum specific IgE of the experimental group (chi2 = 0.758 8, P > 0.05). The difference between serum-specific IgE and total IgE was statistically significant (chi2 = 50.639, P<0.01). There was no statistical significance of specific IgE and the total IgE in serum between long term residents in Haikou and Hainan Tourisms (P > 0.05).
CONCLUSION
Allergen skin test and carvota mitis pollen-specific IgE are two effective methods for the diagnosis of carvota mitis pollen-induced allergic rhinitis. The detection of total IgE in serum of carvota mitis pollen-induced allergic rhinitis provides a reference value for diagnosis. The relationship between concentration of IgE in serum of the carvota mitis pollen-induced allergic rhinitis and allergen contact duration is waiting for further study.

Zhonghua HUANG ; Yumei LIU ; Huifen LUO
Journal of Clinical Otorhinolaryngology Head and Neck Surgery 2013;27(11):579-581
OBJECTIVE:
To investigate whether sublingual immunotherapy(SLIT) can take effect during the early phase in allergic rhinoconjunctivitis.
METHOD:
Sixty subjects diagnosed as allergic rhinoconjunctivitis were enrolled and divided into the SLIT group and the control group. The daily symptom and medication scores were recorded and analyzed.
RESULT:
After 4 weeks treatments, the daily symptoms score had significantly reduced in SLIT group(P<0. 05). After 6, 8, 10 weeks treatments, the daily symptoms scores of subjects in the SLIT group had all shown continuous reduction, with no consumption of medication. The comparison between daily symptom score at 4th week and those in 6th week or 8th week or 10th week in the SLIT group had shown no significant difference (all P > 0.05). Whereas, the daily symptoms score and daily medication score in control group had shown changes with fluctuations. The correlation analysis had indicated that the daily medication score in the control group had a significant negative correlation with the daily symptom score (F = 554.9, P < 0.01). No serious adverse events occurred in this study.
CONCLUSION
SLIT could take effect in patients with allergic rhinoconjunctivitis sensitive to house dust mite as fast as the 4th week.
