ObjectiveTo evaluate the antitumor activity of Periplaneta americana extract(PAE) against human-derived lung adenocarcinoma organoids(LUAD-PDOs) and to elucidate its potential mechanism based on transcriptomics. MethodsFresh tumor and adjacent normal tissues from patients with LUAD were collected to construct LUAD-PDOs and normal lung organoid(Nor-PDOs) models using 3D organoid culture technology. The effective intervention concentration of PAE was determined using the cell counting kit-8(CCK-8) assay. Experimental groups included the model group(LUAD-PDOs), normal group, model administration group(LUAD-PDOs+PAE), and normal administration group(Nor-PDOs+PAE). Hematoxylin-eosin(HE) staining was used to observe the pathological structures of PDOs, immunohistochemistry(IHC) was performed to detect the expressions of the proliferation marker Ki-67 and lung adenocarcinoma differentiation markers cytokeratin-7(CK-7) and Napsin A, TUNEL staining was applied to detect cell apoptosis. RNA sequencing(RNA-Seq) was conducted to identify differentially expressed genes(DEGs), followed by Gene Ontology(GO), Kyoto Encyclopedia of Genes and Genomes(KEGG), and Gene Set Enrichment Analysis(GSEA), alongside protein-protein interaction(PPI) network analysis to screen core mechanisms. Finally, key targets were validated by integrating external database analysis with immunofluorescence(IF). ResultsNor-PDOs and LUAD-PDOs that highly recapitulated the pathological characteristics of the primary tissues were successfully established. The CCK-8 assay determined that the effective intervention concentration of PAE was 16 g·L-1. Morphological observation showed that Nor-PDOs exhibited lumen-forming structures, whereas LUAD-PDOs displayed dense, solid structures. CCK-8 and TUNEL assays revealed that, compared with the model group, PAE intervention inhibited the proliferation of LUAD-PDOs and promoted apoptosis in LUAD cells, while showing no significant effect on the viability of Nor-PDOs. Transcriptomic analysis identified 719 DEGs that were significantly reversed after PAE intervention(347 up-regulated and 372 down-regulated)(P<0.05). GO enrichment analysis indicated that DEGs in the model administration group were significantly enriched in biological processes related to cell cycle regulation compared to the model group. KEGG pathway analysis revealed that PAE affected pathways related to proliferation and metabolism, including pathways in cancer and the p53 signaling pathway. GSEA further confirmed that PAE significantly enhanced the activity of the p53 signaling pathway(P<0.05). PPI network analysis indicated that breast cancer type 1 susceptibility protein(BRCA1) and checkpoint kinase 1(CHEK1) were the core down-regulated targets in the p53 pathway. IF verified the high expression of BRCA1 and CHEK1 in LUAD-PDOs and their significant downregulation after PAE intervention(P<0.05). Furthermore, survival analysis based on The Cancer Genome Atlas(TCGA) database indicated that low expression of BRCA1 and CHEK1 was significantly associated with prolonged overall survival in patients with LUAD(P<0.05). ConclusionPAE effectively inhibits proliferation of LUAD-PDOs and promotes their apoptosis, its anti-tumor mechanism is potentially associated with the activation of the p53 signaling pathway, with BRCA1 and CHEK1 genes likely serving as key downstream targets for the effects of PAE.

Background:
This study was performed to characterize the incidence, costs, and risk factors associated with renal and cardiovascular adverse outcomes following primary percutaneous coronary intervention (pPCI) in patients with ST-elevation myocardial infarction (STEMI).
Methods:
Patients with STEMI who underwent pPCI using iso-osmolar contrast were enrolled at 39 centers. The incidence of acute kidney injury (AKI) and major adverse renal and cardiovascular events (MARCE) was analyzed, as well as inpatient costs. Logistic regression analysis was performed to identify risk factors.
Results:
Among 2,293 patients, the incidence of AKI and MARCE within 72 hours post-pPCI was 4.14% (n = 95) and 4.40% (n = 101), respectively. AKI and/or MARCE were associated with systolic blood pressure (AKI: odds ratio [OR], 1.009; 95% confidence interval [CI], 1.000–1.018), hypertension (AKI: OR, 1.815; 95% CI, 1.133–2.906; MARCE: OR, 1.760;95% CI, 1.118–2.769), anterior wall infarction (AKI: OR, 1.895; 95% CI, 1.196–3.004; MARCE:OR, 1.939; 95% CI, 1.240–3.032), Killip class (AKI: OR, 1.465; 95% CI, 1.117–1.922; MARCE:OR, 1.467; 95% CI, 1.131–1.903), and serum creatinine (SCr; MARCE: OR, 1.006; 95% CI, 1.000–1.012). Hospitalization costs for patients with STEMI who developed AKI or MARCE were significantly higher than for those without AKI (9,595 ± 5,795 vs. 8,279 ± 3,872 USD, P = 0.003) or without MARCE (9,890 ± 5,616 vs. 8,255 ± 3,859 USD, P < 0.001).
Conclusions
In patients with STEMI undergoing pPCI with iso-osmolar contrast, the incidence of AKI and MARCE was associated with higher hospitalization costs. Systolic blood pressure, hypertension, anterior wall infarction, Killip class, and SCr were identified as risk factors for these outcomes.

Objective To explore the recognition capabilities of electronic nose combined with machine learning in identifying the breath odor map of benign and malignant pulmonary nodules and Traditional Chinese Medicine (TCM) syndrome elements. Methods The study design was a single-center observational study. General data and four diagnostic information were collected from 108 patients with pulmonary nodules admitted to the Department of Cardiothoracic Surgery of Hospital of Chengdu University of TCM from April 2023 to March 2024. The patients' TCM disease location and nature distribution characteristics were analyzed using the syndrome differentiation method. The Cyranose 320 electronic nose was used to collect the odor profiles of oral exhalation, and five machine learning algorithms including random forest (RF), K-nearest neighbor (KNN), logistic regression (LR), support vector machine (SVM), and eXtreme gradient boosting (XGBoost) were employed to identify the exhaled breath profiles of benign and malignant pulmonary nodules and different TCM syndromes. Results (1) The common disease locations in pulmonary nodules were ranked in descending order as liver, lung, and kidney; the common disease natures were ranked in descending order as Yin deficiency, phlegm, dampness, Qi stagnation, and blood deficiency. (2) The electronic nose combined with the RF algorithm had the best efficacy in identifying the exhaled breath profiles of benign and malignant pulmonary nodules, with an AUC of 0.91, accuracy of 86.36%, specificity of 75.00%, and sensitivity of 92.85%. (3) The electronic nose combined with RF, LR, or XGBoost algorithms could effectively identify the different TCM disease locations and natures of pulmonary nodules, with classification accuracy, specificity, and sensitivity generally exceeding 80.00%.Conclusion Electronic nose combined with machine learning not only has the potential capabilities to differentiate the benign and malignant pulmonary nodules, but also provides new technologies and methods for the objective diagnosis of TCM syndromes in pulmonary nodules.

Iodine is an indispensable trace element in the human body and its intake level is closely related to thyroid function. Iodine deficiency or iodine excess will lead to iodine-related diseases. The implementation of the universal salt iodization policy of China has achieved remarkable results, yet it is still facing the problems of iodine deficiency and iodine excess at present. Since iodine in the human body is mainly metabolized by the kidneys and excreted in urine, urinary iodine test has become an effective way to reflect the recent iodine nutrition status of the body. This article will discuss the current iodine nutrition status of the population in China, the hazards of iodine deficiency and iodine excess, as well as the clinical application of urinary iodine test.

Parathyromegaly refers to chronic enlargement of the parathyroid glands caused by multiple etiological factors. Pathological conditions, such as hyperparathyroidism, parathyroid hyperfunction, parathyroid adenoma, parathyroid cysts, and parathyroid carcinoma may all lead to parathyromegaly. Notably, calcium intake insufficiency and/or vitamin D insufficiency (CVI), which is the predominant etiology of parathyromegaly, now has been recognized as a global public health challenge. Chronic CVI induces negative calcium balance and relative low serum calcium level, stimulating compensatory parathyroid hyperplasia and enlargement. This progression triggers parathyroid hyperfunction and secondary hyperparathyroidism, resulting in bone mass loss, height reduction, kyphosis, osteoporosis, pathological fractures, metastatic vascular calcification and systemic abnormal calcium migration and calcinosis (such as urolithiasis). During the early stages of parathyromegaly, the condition remains preventable and treatable; However, delayed intervention may lead to irreversible tertiary hyperparathyroidism. CVI-associated parathyromegaly exhibits high prevalence and heterogeneous clinical manifestations, representing a critically underrecognized clinical entity. This article will systematically discuss the etiology, pathological characteristics, clinical consequences, and prevention and control strategies for CVI-related parathyromegaly.

Patients with initially diagnosed breast cancer and uncontrolled hyperthyroidism are at high risk of perioperative thyroid crisis. This article reports two cases of early-stage breast cancer initially diagnosed concurrently with uncontrolled primary hyperthyroidism. In Case 1, the patient received neoadjuvant chemotherapy to control breast cancer progression while concurrently taking antithyroid drugs to manage hyperthyroidism. Hyperthyroidism was controlled during chemotherapy, and the patient successfully underwent surgery after neoadjuvant chemotherapy. Case 2 involved recurrent primary hyperthyroidism with leukopenia after antithyroid drug therapy. Since leukopenia is a relative contraindication for antithyroid drugs, the patient underwent radioactive iodine therapy (iodine-131) and endocrine therapy for one month before proceeding with breast cancer surgery. Through a literature review, this article analyzes preoperative management strategies for uncontrolled hyperthyroidism in initially diagnosed breast cancer patients, emphasizing the importance of normalizing thyroid function to prevent thyroid crisis and reduce perioperative risks.

Objective:To construct the supportive care intervention scheme for breast cancer radiotherapy patients based on the empowerment theory and discuss its application effect, so as to provide the reference for the clinical nursing intervention. Methods:According to the investigation results of the current situation of supportive care needs of patients with breast cancer radiotherapy and the results of literature review, a supportive care intervention scheme for breast cancer radiotherapy patients based on the empowerment theory was constructed by Delphi expert consultation method.Adopting experimental research, breast cancer radiotherapy patients in General Hospital of Ningxia Medical University were selected by convenient sampling method. They were divided into the control group from April to May 2023 (40 cases) for routine nursing care, and the experimental group from August to September 2023 (40 cases) for the supportive care intervention scheme for breast cancer radiotherapy patients based on the empowerment theory on the basis of routine nursing care according to the admission time. The 34-item Short-Form Supportive Cancer Needs Survey (SCNS-SF34), Strategies Used by People to Promte Health (SUPPH), Cancer Fatigue Scale(CFS) and Function Assessment of Cancer Therapy-Breast (FACT-B) were collected on the day of admission, the end of radiotherapy and 3 months after radiotherapy to evaluate the effects before and after intervention. Results:After 1, 2 rounds of expert correspondence, the positivity of experts were 95.24%(20/21) and 100.00%(20/20), the expert authority coefficient was 0.804 and 0.825, and the Kendall harmony coefficient was 0.129 and 0.138 (both P<0.05). Finally, a supportive care intervention scheme for breast cancer radiotherapy patients based on the enabling theory was formed, which included 5 primary items, 11 secondary items and 32 tertiary items. The application results showed that there were 37 cases in the control group with an age of (48.16 ± 10.21) years, and 38 cases in the experimental group with an age of (44.50 ± 9.81) years. All patients were female. There was no statistically significant difference in the scores of SCNS-SF34, SUPPH, CFS, and FACT-B between the two groups on the day of admission(all P>0.05). Three months after the end of radiotherapy, the score of SCNS-SF34 in the experimental group was (73.24 ± 11.99) points, which was lower than (79.91 ± 14.79) points in the control group, and the difference was statistically significant ( t=4.43, P<0.05). At the end of radiotherapy and 3 months after the end of radiotherapy, the scores of SUPPH in the experimental group were (80.39 ± 8.55) and (92.37 ± 8.14) points, respectively, which were higher than (73.95 ± 9.86) and (86.24 ± 7.10) points in the control group, and the difference was statistically significant ( t=-3.03, -3.47, both P<0.05). At the end of radiotherapy and 3 months after the end of radiotherapy, the scores of CFS in the experimental group were 30.00 (28.75, 33.00) and (25.29 ± 3.26) points, respectively, which were lower than 34.00 (31.50, 35.00) and (27.59 ± 3.43) points in the control group, and the difference was statistically significant ( Z=-3.51, t=2.98, both P<0.05). At the end of radiotherapy and 3 months after the end of radiotherapy, the scores of FACT-B in the experimental group were (85.08 ± 12.74) and (105.53 ± 6.53) points, respectively, which were higher than (79.03 ± 7.90) and (95.73 ± 7.61) points in the control group, and the difference was statistically significant ( t=-2.48, -5.99, both P<0.05). Conclusions:The supportive care intervention scheme for patients with breast cancer radiotherapy based on the empowerment theory established in this study is scientific and feasible, which can reduce patients' needs for supportive care, alleviate cancer-related fatigue, and improve self-management efficacy and quality of life.
