1.Impact of number of positive regional lymph nodes in N1 stage on the prognosis of patients with non-small cell lung cancer: A propensity score matching study

Objective To explore the impact of number of positive regional lymph nodes (nPRLN) in N1 stage on the prognosis of non-small cell lung cancer (NSCLC) patients. Methods Patients with TxN1M0 stage NSCLC who underwent lobectomy and mediastinal lymph node dissection from 2010 to 2015 were screened from SEER database (17 Regs, 2022nov sub). The optimal cutoff value of nPRLN was determined using X-tile software, and patients were divided into 2 groups according to the cutoff value: a nPRLN≤optimal cutoff group and a nPRLN>optimal cutoff group. The influence of confounding factors was minimized by propensity score matching (PSM) at a ratio of 1 : 1. Kaplan-Meier curves and Cox proportional hazards models were used to evaluate overall survival (OS) and lung cancer-specific survival (LCSS) of patients. Results A total of 1316 patients with TxN1M0 stage NSCLC were included, including 662 males and 654 females, with a median age of 67 (60, 73) years. The optimal cutoff value of nPRLN was 3, with 1165 patients in the nPRLN≤3 group and 151 patients in the nPRLN>3 group. After PSM, there were 138 patients in each group. Regardless of before or after PSM, OS and LCSS of patients in the nPRLN≤3 group were superior to those in the nPRLN>3 group (P<0.001). N1 stage nPRLN>3 was an independent prognostic risk factor for OS [HR=1.52, 95%CI (1.22, 1.89), P<0.001] and LCSS [HR=1.72, 95%CI (1.36, 2.18), P<0.001]. Conclusion N1 stage nPRLN>3 is an independent prognostic risk factor for NSCLC patients in TxN1M0 stage, which may provide new evidence for future revision of TNM staging N1 stage subclassification.
2.Dimethyl fumarate alleviates nerve damage in a mouse model of Parkinson's disease

BACKGROUND:Parkinson's disease is a multifactorial neurological disorder characterized by progressive loss of dopaminergic neurons,and dimethyl fumarate(DMF)has potent neuroprotective and immunomodulatory effects in neurodegenerative diseases. OBJECTIVE:To explore the neuroprotective mechanism of DMF in a mouse model of 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine(MPTP)-induced Parkinson's disease. METHODS:Twenty-four C57BL/6 mice were selected and randomly divided into control group,model group,low-dose DMF,and high-dose DMF groups.An animal model of Parkinson's disease was established in the latter three groups by intraperitoneal injection of 30 mg/kg MPTP,once a day for 5 consecutive days.Intragastric administration was given 30 minutes after each injection of MPTP.Mice in the low-dose DMF group(30 mg/kg)and high-dose DMF group(50 mg/kg)were intragastrically administered once a day for 7 consecutive days.The control and model groups were initially administered the same dose of normal saline.Behavioral testing,western blot,oxidative stress marker detection,and immunohistochemical staining were used to analyze the regulatory effects of DMF on oxidative stress and Keap1/Nrf2 signaling pathway in MPTP-induced Parkinson's disease mice,as well as the protective mechanism of DMF on degeneration of dopamine neurons. RESULTS AND CONCLUSION:Compared with the model group,mice in the low-dose DMF group exhibited significant improvements in motor retardation and postural imbalance(P<0.01),with even more remarkable improvements observed in the high-dose DMF group(P<0.01).Compared with the control group,the model group showed a significant increase in the oxidative stress marker malondialdehyde and a decrease in superoxide dismutase expression(P<0.01).Compared with the model group,the low-dose DMF group reduced malondialdehyde production and increased superoxide dismutase expression(P<0.01),and similar improvements were observed in the high-dose DMF group(P<0.01).Immunohistochemical and western blot assays demonstrated a significant decrease in the number of dopaminergic neurons and tyrosine hydroxylase protein expression in the substantia nigra of mice in the model group compared with the control group(P<0.01).However,in the low-dose DMF group,there was an increase in the number of dopaminergic neurons and tyrosine hydroxylase protein expression in the substantia nigra(P<0.01),with even more significant improvements in the high-dose DMF group(P<0.01).Western blot results revealed that the model group exhibited elevated Keap1 protein expression and decreased Nrf2 protein expression.In contrast,the DMF groups showed reduced Keap1 protein expression and increased Nrf2 protein expression compared to the model group(P<0.01).To conclude,DMF regulates the Keap1/Nrf2 pathway in the substantia nigra of mice with Parkinson's disease,and this regulatory effect is positively correlated with the dose of DMF(P<0.01).Therefore,we infer that DMF exerts neuroprotective effects through the Keap1/Nrf2 signaling pathway.
3.Effect of EGCG on MPTP-induced Parkinson's model mice via autophagy-lysosomal pathway

Objective To investigate the protective effect of epigallocatechin gallate(EGCG)on 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine(MPTP)-induced Parkinson's disease model mice.Methods Twenty-eight male C57BL/6J mice aged 6~8 weeks were randomly divided into four groups:the control group,the model group,the low-dose EGCG group[25 mg/(kg·d)],and the high-dose EGCG group[50 mg/(kg·d)].A Parkin-son's disease(PD)mouse model was established by intraperitoneal injection of MPTP at a dose of 30 mg/(kg·d)for 7 consecutive days.The protective effect of EGCG on MPTP-induced Parkinson's model mice was analyzed through behavioral index detection and Western blot method.Results(1)In the behavioral tests,compared with the model group,the movement distance and speed of mice treated with low-and high-dose EGCG were significantly improved(both P values<0.001).The mice in the high-dose EGCG treatment group also showed a significant advantage in the percentage of the central path distance(P<0.001).(2)Compared with the control group,the deposition of α-synuclein in the model group increased significantly(P<0.001).Compared with the model group,both the low-and high-dose EGCG groups reduced the deposition of α-synuclein(both P<0.001).(3)Compared with the control group,the expression levels of Beclin 1 and LC3 proteins in the substantia nigra region of mice in the model group decreased significantly(both P<0.001),while the expression level of p62 protein increased significantly(P<0.001).After treatment with EGCG,compared with the model group,the expression levels of Beclin 1 and LC3 proteins in mice of the low-dose EGCG group increased to varying degrees(P<0.01;P<0.001),and the expression level of p62 protein decreased significantly(P<0.001).In the high-dose EGCG group,the expression levels of Beclin 1 and LC3 proteins increased significantly(both P<0.001),and the expression level of p62 protein decreased significantly(P<0.001).Conclusion EGCG reduces alpha-synuclein deposition via the autophagy-lysosomal pathway and protects against MPTP-induced Parkinson's disease model mice.
4.Development and validation of a prognostic nomogram model for patients with the lower third and abdominal oesophageal adenocarcinoma

Objective To establish an individualized nomogram model and evaluate its efficacy to provide a possible evaluation basis for the prognosis of lower third and abdominal part of oesophageal adenocarcinoma (EAC). Methods Lower third and abdominal part of EAC patients from 2010 to 2015 were chosen from the SEER Research Plus Database (17 Regs, 2022nov sub). The patients were randomly allocated to the training cohort and the internal validation cohort with a ratio of 7∶3 using bootstrap resampling. The Cox proportional hazards regression analysis was used to determine significant contributors to overall survival (OS) in EAC patients, which would be elected to construct the nomogram prediction model. C-index, calibration curve and receiver operating characteristic (ROC) curve were performed to evaluate its efficacy. Finally, the efficacy to evaluate the OS of EAC patients was compared between the nomogram prediction model and TNM staging system. Results In total, 3945 patients with lower third and abdominal part of EAC were enrolled, including 3475 males and 470 females with a median age of 65 (57-72) years. The 2761 patients were allocated to the training cohort and the remaining 1184 patients to the internal validation cohort. In the training and the internal validation cohorts, the C-index of the nomogram model was 0.705 and 0.713, respectively. Meanwhile, the calibration curve also suggested that the nomogram model had a strong capability of predicting 1-, 3-, and 5-year OS rates of EAC patients. The nomogram also had a higher efficacy than the TNM staging system in predicting 1-, 3-, and 5-year OS rates of EAC patients. Conclusion This nomogram prediction model has a high efficiency for predicting OS in the patients with lower third and abdominal part of EAC, which is higher than that of the current TNM staging system.
5.A qualitative study on economic toxicity perceptions and experiences of caregivers of colorectal cancer patients from the perspective of social ecological systems theory

Objective To explore the economic toxicity perceptions and experiences of caregivers of colorectal cancer patients from the perspective of social ecological systems theory.Methods Using purposive sampling,18 caregivers of colorectal cancer patients hospitalized in the gastrointestinal sur-gery and oncology departments of a tertiary grade A hospital in Yangzhou were selected for semi-struc-tured interviews.The Colaizzi 7-step analysis method was employed to organize and analyze the data.Results Three main themes and nine sub-themes were extracted regarding the economic toxicity expe-riences and needs of caregivers of colorectal cancer patients.Microsystem included multiple negative experiences,impacted individual health,and difficulties in surrogate decision-making;mesosystem in-cluded heavy family financial burden,altered family lifestyle,and tense family atmosphere;macrosys-tem included needs for the scope and intensity of medical insurance reimbursement,needs for medical resources and services,and a desire for social support.Conclusion The ecological system of caregiv-ers of colorectal cancer patients is not optimistic,and is generally affected by economic toxicity.Inter-vention strategies can be sought from multiple aspects,including alleviating the negative experiences of caregivers,strengthening social support,and paying attention to the needs of caregivers,aiming to re-duce the level of economic toxicity among caregivers of colorectal cancer patients.
6.The effect of different swallowing tasks on post-stroke dysphagia as observed by functional near-infrared spectroscopy

Objective:To explore the effect of different swallowing tasks on cortex activation and functional connectivity in stroke survivors with dysphagia using functional near-infrared spectroscopy (fNIRS).Methods:Thirty stroke survivors with dysphagia performed three different swallowing tasks: swallowing action observation (SO), swallowing action execution (SE), and swallowing action imagination (SI). During each task, fNIRS was used to document the brain concentrations of oxyhemoglobin and deoxyhemoglobin. Cortex activation (β value) and brain functional connectivity were assessed.Results:Compared with the resting state, the areas activated during the SO task included the left primary sensory cortex and the right prefrontal cortex. During the SE and SI tasks the left prefrontal cortex and the left motor cortex were activated as well. Compared with hemorrhagic stroke survivors, ischemic stroke survivors showed significantly greater activation of the right primary sensory cortex, the right motor cortex, and the left primary sensory cortex during the SE task. Functional connectivity during the SO, SE and SI tasks was significantly greater than in the resting state, with the average connectivity values during the SE task significantly higher than during the SI task.Conclusions:Stroke survivors with dysphagia exhibit increased activation in the prefrontal cortex and primary sensory cortex during different swallowing tasks. Such tasks can improve their brain functional connectivity.
7.Analysis of the surveillance results of viral diarrhea among children under 5 years of age in Chengdu,2021—2024

Objective:To determine the pathogens causing viral diarrhea in children under 5 years of age and the epidemiological patterns in Chengdu from 2021 to 2024.Methods:Fecal samples were collected from hospitalized children under 5 years of age with diarrhea in a sentinel surveillance hospital for viral diarrhea in Chengdu from 2021 to 2024. Norovirus(NoV),sapovirus(SaV),astrovirus(AstV),and enteric adenovirus(EAdV)were detected by real-time PCR or real-time RT-PCR. Group A rotavirus(RVA)was detected by enzyme-linked immunosorbent assay. The results were analyzed statistically by using SPSS 20.0 software.Results:RVA,NoV,SaV,AstV,and EAdV were detected in 329 fecal samples from 719 hospitalized children with diarrhea under 5 years of age. The total detection rate was 48.8%(329/719). The detection rate of RVA,NoV,SaV,AstV,and EAdV was 17.0%(122/719),20.9%(150/719),4.0%(29/719),2.8%(20/719),8.6%(62/719),separately. The detection rate of mixed infection with multiple pathogens was 7.1%(51/719),among which RVA combined with NoV infection was the most common(2.2%,16/719). Except for EAdV( χ2=14.368, P=0.002),there was no significant difference in the detection rates of RVA,NoV,SaV,and AstV in different years. Except for SaV and EAdV,the detection rates of RVA( χ2=11.106, P<0.001),NoV( χ2=65.164, P<0.001)and AstV( χ2=19.612, P<0.001)were statistically significant in different quarters. There was no statistically significant difference in the detection rates of the five pathogens between males and females. Except for NoV,there were statistically significant differences in the detection rates of RVA( χ2=32.209, P<0.001),SaV( χ2=13.983, P=0.002),AstV( χ2=9.56, P=0.015)and EAdV,( χ2=8.992, P=0.029)in children of different months of age. Conclusion:RVA and NoV are the main pathogens causing viral diarrhea in children under 5 years of age in Chengdu. The peak infection period of viral diarrhea tends to occur in winter and spring and occurs most frequently in 12-24 months old and 24-59 months old children.

Psoriasis arthritis (PsA) is a chronic inflammatory musculoskeletal disease intricately linked to psoriasis (PsO), with a multifaceted etiology encompassing genetic, environmental, and immunological factors. Characterized by complex clinical manifestations, PsA often follows a protracted course with a propensity for relapses, potentially culminating in joint deformity and disability. The condition is further complicated by associated comorbidities such as inflammatory bowel disease, uveitis, cardiovascular disease, and metabolic syndrome, which significantly diminish patients′ quality of life. Early detection and screening of PsA are crucial for its management and prevention of adverse outcomes. However, in China, there is a notable deficiency in the recognition and early diagnosis of PsA, with missed or incorrect diagnoses being relatively common. The consensus comprises four overarching statements and sixteen detailed recommendations, with the overarching goal of enhancing the early diagnosis and treatment of PsA by clinical physicians, thereby improving patient outcomes.
10.Cancer Incidence and Mortality in Henan Province in 2020 and Trends from 2010 to 2020

[Purpose]To analyze the cancer incidence and mortality in 2020 and trends from 2010 to 2020 in Henan Province.[Methods]Data from cancer registries in Henan Province from 2010 to 2020 were collected and evaluated.Incidence and mortality rates were calculated by urban/rural areas,sex and age,and the incidence and mortality of cancers in the whole province in 2020 were estimated based on population data released by Henan Provincial Bureau of Statistics.Age-standardized rates were calculated according to the age-standardized rate of Chinese standard population(ASIRC/ASMRC)and world standard population(ASIRW/ASMRW).Joinpoint 5.4.0 soft-ware was used to construct a regression model to analyze the changing trends of malignant tumors from 2010 to 2020,and the average annual percentage change(AAPC)and 95%confidence in-terval were calculated.[Results]In 2020,the estimated number of new cancer cases in Henan Province was 299 148,with a crude incidence rate of 259.38/105,ASIRC of 201.09/105(204.56/105 for males and 200.45/105 for females)and ASIRW of 196.46/105(203.43/105 for males and 192.22/105 for females).The ASIRC was higher in urban areas(208.10/105)than that in rural areas(197.74/105).The top five cancer types in male were lung,stomach,liver,esophagus,and colorectal cancers,while the top five in female were breast,lung,thyroid,cervical,and esophageal cancers.The estimated number of cancer deaths was 172 070,with a crude mortality rate of 149.20/105 and ASMRC of 106.52/105(137.22/105 for males and 78.04/105 for females)and ASMRW of 106.24/105(137.05/105 for males and 77.91/105 for females).The ASMRC was higher in rural areas(109.92/105)than that in urban areas(99.49/105).The top five causes of cancer death in male were lung,stomach,liver,esophagus,and colorectal cancers,and those in female were lung,esophagus,stomach,liver,and breast cancers.From 2010 to 2020,the trends of ASIRC remained stable(AAPC=0.14%,P=0.572),while the ASMRC showed a significant decreasing trend(AAPC=-1.46%,P=0.011).[Conclusion]Lung cancer,breast cancer and digestive system cancers are the main malignant tumors threatening the health of residents in Henan Province.The incidence and mor-tality of common malignant tumors show significant gender and urban-rural differences.It is neces-sary to further optimize the prevention and control of malignant tumors,formulate targeted inter-vention strategies based on population characteristics,and improve the health awareness of the whole population.
