Objective To analyze the effect of graded exercise rehabilitation in patients with acute exacerbation of chronic obstructive pulmonary disease, and to provide references for clinical nursing practice. Methods A total of 70 patients with acute exacerbation of chronic obstructive pulmonary disease who met the criteria in the Department of Respiratory Medicine of a tertiary hospital in Zunyi City from September to December 2023 were randomly divided into an experimental group and a control group(with 35 cases in each group). The experimental group implemented graded exercise rehabilitation based on the Global Chronic Obstructive Pulmonary Initiative guidelines, and the control group implemented routine exercise rehabilitation. After intervention, the lung function, blood gas analysis, oxygenation index,6 min walking test and the incidence of complications related to non-invasive mechanical ventilation were compared between the 2 groups. Results Finally,34 cases were included in the experimental group and 35 cases in the control group. After intervention, the forced expiratory volume in the first second of the experimental group was improved compared with the control group(P<0.05).The 6-minute walking test of the experimental group was higher than that of the control group(P<0.05).The total incidence of non-invasive mechanical ventilation-related complications in the experimental group was lower than that in the control group(P<0.05).There was no significant difference in blood gas analysis and oxygenation index between the 2 groups(P<0.05).Conclusion The implementation of graded exercise rehabilitation based on the Global Chronic Obstructive Pulmonary Initiative guidelines can help patients with acute exacerbation of chronic obstructive pulmonary disease improve their respiratory function, improve their exercise endurance, and reduce non-invasive mechanical ventilation-related complications.

Objective To investigate the protective effects of paeonol(PAE)on autophagy in human neuroblastoma cells(SH-SY5Y)induced by overexpression of α-synuclein(α-Syn),and to explore its related mechanism. Methods SH-SY5Y cells served as control group, while those induced with A53T-α-Syn mutation were used as model group.Additional groups included PAE(150 μg/ml)group,3-MA(1 mmol/L)group,and PAE(150 μg/ml)+3-MA(1 mmol/L)group.Cell viability was assessed using CCK-8 method, cell morphology was observed under an optical microscope, and protein expressions of α-Syn,LC3-Ⅱ,p62,Beclin-1,phosphorylated c-Jun N-terminal kinase(p-JNK),and p-Bcl-2 were determined by Western blotting. Results Compared with control group, model control exhibited decreased cell survival(P<0.01),increased α-Syn expression(P<0.001),reduced expression of autophagy-related proteins LC3-Ⅱ and Beclin-1(P<0.01,P<0.05),elevated autophagy substrate protein p62(P<0.05),and decreased expression of autophagy pathway-related proteins p-JNK and Bcl-2(P<0.05,P<0.01).Compared with model group,PAE group showed increased cell survival(P<0.01),decreased α-Syn and p62 protein expression(P<0.01,P<0.05),and increased expression of LC3-Ⅱ,Beclin-1,p-JNK and Bcl-2(P<0.05).Compared with PAE group,3-MA+PAE group demonstrated increased α-Syn expression(P<0.05).Conclusions PAE could attenuate the injury of SH-SY5Y cells induced by A53T-α-Syn and eliminate over-expressed α-Syn by activating autophagy pathway, which may be associated with the upregulation of JNK/Bcl-2 mediated autophagy pathway.

Objective To explore the influencing factors on foot function recovery after distraction device-assisted repositioning of small incisions treatment for Sanders Ⅱ and Ⅲ calcaneal fractures, and to provide guidance for timely clinical interventions. Methods A retrospective analysis was conducted on 80 patients with Sanders type Ⅱ and Ⅲ calcaneal fractures admitted to the People's Hospital of Bozhou from January 2021 to June 2023.Data were collected using a general information questionnaire, the hospital's electronic medical record system, the visual analog scale(VAS),the Marland foot function scale(MFS),the perceived social support scale(PSSS),and the perceived stress scale(PSS).The influencing factors of postoperative foot function recovery were analyzed using univariate factor analysis, multivariate linear regression, and structural equation modeling. Results The MFS scores of 80 patients at 6 months post-operation were significantly higher than preoperative scores(P<0.001).The rates of excellent, good, fair, and poor foot function at 6 months post-operation were 56.3%,31.3%,10.0%,and 2.5%,respectively.Univariate analysis revealed significant differences in MFS scores based on different ages, time to start full weight-bearing exercise, bone mineral density, surgical timing, body mass index(BMI),postoperative VAS score,PSSS score,PSS score, and the presence of postoperative peroneal muscle spasm and peroneal muscle tendon adhesion(P<0.05).Multivariate linear regression identified age, time to start full weight-bearing exercise, surgical timing,BMI,postoperative fibular muscle spasm, postoperative fibular muscle tendon adhesion, postoperative VAS score,PSSS score, and PSS score as significant factors affecting foot function recovery(P<0.05).Structural equation modeling indicated that age, time to start full weight-bearing exercise, bone density, operative timing,BMI,postoperative peroneal muscle spasm, postoperative peroneal tendon adhesion, and pain severity directly influenced postoperative foot functional recovery.Additionally, psychological stress not only directly affected postoperative foot functional recovery but also indirectly influenced it through the mediating effect of social support. Conclusions Foot function recovery after surgery for Sanders type Ⅱ and Ⅲ calcaneal fractures is influenced by multiple factors.Psychological stress not only directly affects the recovery of foot function after surgery, but also mitigates the harm to foot function through social support.

Objective To evaluate the changes in postoperative plasma β-endorphin(β-EP)levels in patients who had received perioperative electroacupuncture(EA)treatment in 10 randomized controlled trials(RCTs)and examine the impact of EA on postoperative pain. Methods This meta-analysis evaluated the changes in plasma β-EP levels and visual analog scale(VAS)12,24 and 48 hours after surgery in patients receiving perioperative EA.It also assessed the changes in plasma serotonin(5-hydroxytryptamine,5-HT)and prostaglandin E2(PGE2)levels at 24 hours postsurgery.A comprehensive search was conducted in the China National Knowledge Infrastructure(CNKI),Wanfang,Chongqing VIP database,Chinese Biomedical Database(CBM),Web of Science, and PubMed databases.RCTs on perioperative EA and β-EP published from the inception of the websites up to July 25,2023,were retrieved.Effect size aggregation, literature quality assessment, and bias analysis were performed using RevMan 5.3 software, and sensitivity analysis was conducted via R 4.3.1.Results A total of 10 RCTs involving 706 patients were included.EA in conjunction with conventional anesthesia significantly increased plasma β-EP levels at 12 hours postsurgery[standard mean difference(SMD)=2.79,95%CI(1.85,3.72),Z=5.81,P<0.00001],24hours postsurgery[SMD=1.87,95%CI(0.9,2.83),Z=3.79,P=0.0001],and 48 hours postsurgery[SMD=2.02,95%CI(1.49,2.54),Z=7.50,P<0.00001].EA reduced plasma PGE2 levels at 24 hours postsurgery and plasma 5-HT levels at 24 hours postsurgery, and the VAS at 12,24 and 48 hours after surgery also decreased. Conclusion These findings suggest that perioperative EA markedly elevates plasma β-EP levels, reduces pain-inducing factors in plasma, and effectively alleviates acute postoperative pain.
