1.Application of organ dose modulation technique in CTA for both lower extremities

Objective:To investigate the effect of organ dose modulation(ODM)technique on radiation dose and image quality in computed tomography angiography(CTA)for arteries of both lower extremities.Methods:A total of sixty patients with suspected lower extremity arterial occlusion who underwent CTA examination on both lower extremities at Beijing Friendship Hospital affiliated to Capital Medical University from March 2024 to June 2024 were selected.They were randomly divided into an ODM group and a non-ODM group,with 30 patients in each group.All patients underwent CTA examination with the same scanning parameters.The tube currents in the pelvic region,which included 4 directions(anterior,left,posterior and right),of two groups of patients were recorded,and the CT values,SD values,volumetric CT dose index(CTDIvol)and dose-length product(DLP)of femoral artery and external iliac artery of the regions of interest(ROI)in the pelvic region were respectively measured,and the image quality of them was analyzed by subjective scores.Results:The tube current in front of the pelvic cavity was(109.17±35.62)mA in ODM group,which decreased by 42%than(185.58±55.38)mA in non-ODM group,and the difference was statistically significant(t=-4.781,P<0.05).There were no statistically significant differences in CTDIvol values,DLP values,and CT values and signal noise ratio(SNR)of femoral artery and external iliac artery of the regions of interest(ROI)in the pelvic region between two groups(P>0.05).The subjective scores for image quality of two groups were≥4 points,and there was not statistically significant difference in that between two groups(P>0.05).Conclusion:The application of ODM technique in CTA examination for both lower extremities can realize protective function for surface gonadal organs when ensures image quality,which can meet the needs of clinical diagnosis.It has clinical application value.
2.Influence of different ratios of CA to saline on CCTA image quality in dual-flow injection technique

Objective:To investigate the influence of different ratios of contrast agent(CA)to saline on image quality of coronary computed tomography angiography(CCTA)in dual-flow injection technique.Methods:A prospective study included 102 patients who were clinically diagnosed as suspected coronary heart disease(CHD)to undergo CCTA scan at Beijing Friendship Hospital,Capital Medical University,between March 2024 and October 2024.Ultimately,90 patients were enrolled,who adopted CA injection protocol of dual-flow injection technique.Patients were randomly divided into three groups using a random number table,with 30 patients in each group.The different ratios of CA to saline were adopted,which were respectively Group A(1:9),Group B(2:8),and Group C(3:7).For the reconstructed images,the computed tomography(CT)values of heart-related anatomical structures,which included superior vena cava,right atrium,right ventricle,left atrium,left ventricle,interventricular septum and each segment of the coronary artery,were measured respectively.At the same time,two radiologists,who possessed over 10 years of diagnostic experience of CCTA examination,conducted subjective evaluations for image quality.For the cases of disagreement,the consensus between them was reached through discussion.Results:In three groups,the total CA doses were respectively(42.75±5.22)ml,(47.08±5.83)ml and(50.64±6.29)ml,and the differences of that among the three groups was statistically significant(F=13.964,P<0.05).The differences of CT values of superior vena cava,right atrium,right ventricle,left atrium,left ventricle and aorta among three groups were significant(F=29.141,24.194,39.308,9.643,8.178,6.881,P<0.05).There were significant differences in the CT values of the proximal,mid,and distal segments of the left anterior descending(LAD),and the proximal and mid segments of the left circumflex(LCx)and right coronary artery(RCA)among three groups(F=7.114,9.106,3.851,6.075,4.025,7.349,5.756,P<0.05).However,there was not statistically significant difference was found among the three groups in the CT value of the distal segments of the LCx and RCA(P>0.05).There were significant differences in the subjective scores for the superior vena cava,right atrium and right ventricle among the three groups(H=8.939,29.105,32.180,P<0.05).The number of cases with 3-4 points at the superior vena cava,right atrium and right ventricle were respectively 25(83.3%),29(96.7%)and 30(100%)in group B,there was no case with 1 point.In addition,the imaging quality of right heart in group B was better than that in group A and C.There were no significant differences at left atrium,left ventricle,and each segment of coronary artery among three groups(P>0.05).Conclusion:For patients with clinically non-extreme body weight,the selection of the ratio(2:8)of CA to saline can effectively show relevantly anatomic structures of heart,and coronary artery vessels,including the right heart system,which belongs to the optimal solution.
3.Analysis of factors influencing right atrial strain in patients with pulmonary arterial hypertension

Objective To analyze the correlation between right atrial strain at various stages and various influencing factors in patients with pulmonary hypertension,and to explore the role of right atrial strain in the assessment of pulmonary hypertension.Methods A total of 239 cases diagnosed with pulmonary hypertension who underwent echocardiography and complete right heart catheterization at hospital from October 2021 to December 2023 were included.Conventional ultrasound parameters such as right heart strain,right atrial area(RA area),inferior vena cava diameter(IVC diameter),and collapse rate of the inferior vena cava(IVC diameter changes)were measured.The heart rate(HR)corresponding to the ultrasound images were recorded.General information such as age and gender,as well as catheter data including mean right atrial pressure(mRAP),mean pulmonary artery pressure(mPAP),and pulmonary vascular resistance(PVR),were collected.The relationship between right atrial strain and its influencing factors was analyzed,and further analysis was conducted by dividing into shunt group and non-shunt group based on the presence or absence of left-to-right shunt disease.Results The correlation with RA reservoir strain(RASr)from high to low is RV global strain(RV4CSL),RV free wall strain(RVFWSL),RA area,IVC diameter,mRAP,age,HR,and PVR;the correlation with RAconduit strain(RAScd)from high to low is RV4CSL,RVFWSL,RA area,IVC diameter,mRAP,age,PVR,and HR;the correlation with RA contraction strain(RASct)from high to low is RA area,RV4CSL,RVFWSL,mRAP,IVC diameter,and HR.The collapse rate of the inferior vena cava is correlated with strain at various stages of the right atrium;gender is correlated with RASr and RASct.Conclusions Right atrial strain can reflect changes in right atrial function,with the highest correlation to right ventricular strain and right atrial area.Right atrial strain can indicate the severity of right ventricular function and right atrial remodeling,serving as an evaluative index for the condition and treatment outcomes of pulmonary arterial hypertension.
4.Application of predictive nursing based on root cause analysis in cesarean section patients

Objective:To investigate the effectiveness of predictive nursing based on root cause analysis in patients undergoing cesarean section.Methods:A convenience sampling method was used to select 180 women who underwent cesarean section under combined spinal-epidural anesthesia in the Affiliated Hospital of Jining Medical University from September 2021 to October 2022. According to the random number table method, they were divided into a control group ( n=90) and an observation group ( n=90). The control group received routine nursing care, while the observation group received predictive nursing based on root cause analysis. Compared the pain intensity at 24 hours after cesarean section and the incidence of postoperative complications between the two groups of parturients. Results:The Visual Analog Scale scores at 24 hours post-cesarean section and the overall incidence of postoperative complications were lower in the observation group than those in the control group, and the differences were statistically significant ( P<0.05) . Conclusions:Predictive nursing based on root cause analysis can effectively relieve postoperative pain and reduce the incidence of complications in patients undergoing cesarean section with combined spinal-epidural anesthesia.
5.Exploring experiment on the impact of splenectomy on full-thickness skin defect healing

Objective To investigate the effect of splenectomy on the repair of full-thickness skin tissue defects,as well as the impact of different recovery times after splenectomy on the healing of skin tissue defects.Methods According to a random number table,39 8-week-old female C57 mice were randomly divided into three groups:sham surgery group(sham group,n=13),splenectomy group with 3 days of recovery(Spx3d group,n=13),and splenectomy group with 3 weeks of recovery(Spx3w group,n=13).Full-thickness skin defects were created on the backs of the mice in each group.The wound healing conditions at different times after skin defects were observed,and the wound healing rates after the injury were calculated.Peripheral blood cell analysis was performed on day 14 after the defect,and tissue samples from the wound area were taken for hematoxylin and eosin(HE)staining to observe the granulation tissue thickness at the defect site and the re-epithelialization rate.Masson's trichrome staining was used to observe the proportion of collagen fibers.Results After splenectomy and sham surgery,the mice recovered well without significant discomfort.From 1 to 14 days after the skin defect modeling,the wound areas of the mice in all three groups gradually decreased.Compared with sham group,the wound areas were smaller in Spx3d and Spx3w groups at 3,5 and 7 days after the injury,and the differences were statistically significant(P<0.05).The wound healing rates were also significantly higher(P<0.05).Moreover,at 3 days and 5 days after the injury,the wound healing rates of Spx3d group were significantly higher than those of Spx3w group(P<0.05 or P<0.01).The peripheral blood white blood cell(WBC)count in Spx3w group was significantly higher than that in sham group and Spx3d group(P<0.01).The platelet counts in both sham group and Spx3w group were significantly higher than that in Spx3d group(P<0.05).Additionally,the lymphocyte and neutrophil counts in Spx3w group were markedly higher than those in sham group(P<0.05).No statistically significant differences in red blood cell(RBC)counts were observed among the three groups(P>0.05).HE staining results showed that compared with sham group,the wound healing of the mice in Spx3d and Spx3w groups were better,and the thickness of the granulation tissue in Spx3d group were better than that in Spx3w group.At 7 days,the thickness of the granulation tissue in Spx3d and Spx3w groups was significantly higher than that in sham group(P<0.01,P<0.05)and the re-epithelialization rate in Spx3d group was significantly higher than that in sham group and Spx3w group(P<0.05).At 14 days,the re-epithelialization rates of Spx3d and Spx3w groups were significantly higher than those of sham group(P<0.05).The results of Masson's staining showed that the collagen fiber proportion in the wounds of Spx3d group at 7 and 14 days and that of Spx3w group at 14 days were significantly higher than that in sham group(P<0.05).Conclusion The healing of skin defects in mice is accelerated after splenectomy,and the recovery time after splenectomy has a certain effect on the healing of skin defects.
6.Assessment of clinal target volume deformation in online adaptive radiotherapy for prostate cancer

Objective:To assess intrafractional errors of the deformation of clinical target volumes (CTVs) during online adaptive radiotherapy (OART) for prostate cancer patients, aiming to provide a basis for online plan optimization.Methods:A retrospective analysis was conducted for 13 prostate cancer patients who received 1.5 T magnetic resonance imaging (MRI)-guided OART (8 Gy × 5 fractions, totaling 65 fractions) at the Department of Radiation Oncology, Cancer Hospital, Chinese Academy of Medical Sciences. The MRI images were collected at the beginning and end of various treatment fractions. Then, the CTVs and organs at risk (OARs) were delineated by the same radiation oncologist. After rigid registration and triangle mesh generation, the surface vertices were extracted. The deformable registration for the CTV surfaces was performed using the thin-plate spline robust point matching (TPS-RPM) algorithm, yielding vertex correspondences. Last, both systematic and random intrafractional errors of CTV deformation were calculated.Results:The average Hausdorff distance (HD) for deformable registration of treatment fractions was (1.68 ± 0.28) mm. The intrafractional systematic errors of CTV deformation were (0.25 ± 3.18) mm (anterior-posterior direction; A-P), (0.89 ± 3.85) mm (left-right direction, L-R), and (-1.98 ± 6.69) mm (superior-inferior direction, S-I). The intrafractional random errors of CTV deformation were determined at (-0.26 ± 1.89) mm (A-P), (-0.08 ± 0.88) mm (L-R), and (-0.04 ± 1.86) mm (S-I).Conclusions:During OART, CTV deformations primarily occur in the S-I direction. Therefore, it is necessary to consider the expanded size of margins in this direction during OART for prostate cancer.
7.Noninvasive right ventricular pressure-strain loop for quantitative evaluation on right ventricular myocardial function changes in liver cirrhosis patients after TIPS

Objective To observe the value of noninvasive right ventricular pressure-strain loop(RVPSL)for quantitative evaluation on right ventricular myocardial function changes in liver cirrhosis patients after TIPS.Methods Totally 26 cases of liver cirrhosis who would undergo TIPS were prospectively recruited as liver cirrhosis group.Echocardiography was performed before and 1 week,1 month after TIPS,and parameters of right ventricular myocardial function,including routine right ventricular echocardiography,right ventricular strain and right ventricular myocardial work were acquired.Meanwhile,30 healthy adults were recruited as control group,and the above parameters were recorded.Then these parameters were compared between groups,also before and after TIPS within liver cirrhosis group,and the changes of right ventricular myocardial function after TIPS were evaluated.Results Right ventricular global work index(RVGWI),right ventricular global constructive work(RVGCW)and right ventricular global wasted work(RVGWW)in liver cirrhosis group were higher than those in control group at all time points(all P<0.05).One weak and 1 month after TIPS,right ventricular global longitudinal strain(RVGLS)and right ventricular free wall longitudinal strain(RVFWLS)in liver cirrhosis group were all higher than those in control group(all P<0.05).In liver cirrhosis group,RVGWI,RVGCW and RVGWW 1 week after TIPS were all higher than those before TIPS and 1 month after TIPS(all P<0.05),while RVGLS 1 weak and RVGWI 1 month after TIPS were both higher than those before TIPS(both P<0.05).Conclusion Noninvasive RVPSL could be used to sensitively and quantitatively evaluate right ventricular function changes in patients with liver cirrhosis after TIPS.
8.Impact of returned migration experience on prevalence of non-suicidal self-injury behavior and its association with relevant psychosocial factors among middle school students

Objective:To examine the impact of returned migration experience on the prevalence of non-sui-cidal self-injury(NSSI)and its associations with childhood emotional maltreatment(EM),social support and sleep quality.Methods:A total of 3 901 middle school students in Guizhou Province were investigated with the Adoles-cent NSSI behavior Questionnaire,Childhood Trauma Questionnaire-short Form(CTQ-SF),Adolescent Social Sup-port Scale,and Pittsburgh Sleep Quality Index(PSQI).Results:The prevalence of NSSI among middle school students in Guizhou province was 22.8%,with the rate of 27.3%among returned migrant middle school students.Social support and sleep quality partially mediate the relationship between childhood EM and NSSI in mid-dle school students,with effect sizes of 0.06.The EM scores of returned migrant middle school students(β=-0.62)and non-returned migrant middle school students(β=-0.50)were negatively correlated with social sup-port scores in childhood.The sleep quality scores of returned migrant students(β=0.22)and non-returned migrant students(β=0.14)were positively correlated with NSSI scores.Conclusion:The prevalence of NSSI in returned migrant students is higher.Social support and sleep quality play an important role in the relationship between child-hood EM and NSSI in middle school students.The relationship between childhood EM and social support,sleep quality and NSSI in returned migrant middle school students is stronger than that in non-returned migrant middle school students.
9.Clinicopathological characteristics and prognostic factors in 36 cases of early-stage gastric mixed adenoneuroendocrine carcinoma

Objective:This study analyzes the clinicopathological features and prognostic factors of early-stage gastric mixed adenoneuroendocrine carcinoma (G-MANEC), which is an exceedingly rare malignancy, in an effort to provide evidence-based insights for clinical decision-making.Methods:A retrospective observational study was conducted using the clinical data of 36 patients with early-stage G-MANEC who underwent surgical resection at Zhongshan Hospital, Fudan University, from July 2014 to May 2022. The observed indicators included clinicopathological data and follow-up information on recurrence, metastasis, and overall survival (OS).Results:Among the 36 patients there were 21 males and 15 females, aged 32-84 (65±11) years. The most common initial symptoms were abdominal pain and distension (19/36, 52.8%), followed by incidental findings during physical examinations (7/36, 19.4%). Tumors were located in the proximal stomach in 13 cases (36.1%), the middle stomach in 4 cases (11.1%), and the distal stomach in 19 cases (52.8%). Average tumor diameter was (2.48±1.18) cm. Gross morphology included elevated type in 12 cases (33.3%), flat type in 20 cases (55.6%), and depressed type in 4 cases (11.1%). Ulceration was present in 12 cases (33.3%). There were 11 cases (30.6%) at T1a stage and 25 cases (69.4%) at T1b stage. Lymph node metastasis was positive in 10 cases (27.8%), and the differentiation grades of the adenocarcinoma component were Grade I, II, and III in 3 (8.3%), 10 (27.8%), and 23 (63.9%) cases, respectively. Furthermore, the proportion of neuroendocrine carcinoma component was ≥50% in 18 cases (50.0%) and <50% in 18 cases (50.0%). Lymphovascular or perineural invasion was present in 18 cases (50.0%). Lauren classification included mixed type in 10 cases (27.8%), intestinal type in 19 cases (52.8%), and diffuse type in 7 cases (19.4%), and chromogranin A (CgA) positivity was found in 20 cases (55.6%). Additionally, the Ki-67 index positivity was found in 26 cases (72.2%). Total gastrectomy was performed in 12 cases (33.3%) and partial gastrectomy in 24 cases (66.7%), with a median follow-up duration of 77.5 months. The 3-year and 5-year OS rates were 88.89% and 79.67%, respectively. Univariate analysis revealed that age, gross morphology, ulceration, proportion of neuroendocrine carcinoma component, lymphovascular or perineural invasion, and chromogranin A (CgA) positivity showed statistical significance in their association with OS ( P<0.10). Multivariate Cox regression analysis further identified ulceration (HR=7.74, 95%CI: 1.24-48.30, P=0.028) and CgA positivity (HR=21.76, 95%CI: 1.86-53.97, P=0.014) as independent risk factors of OS. Conclusions:Patients with early-stage G-MANEC are typically asymptomatic, and those with ulceration or positive CgA immunohistochemical staining tend to have a poor prognosis.
10.Current status and influencing factors of decision conflicts among family members of Neurosurgical Intensive Care Unit patients

Objective:To explore the current status of decision conflicts among family members of Neurosurgical Intensive Care Unit patients and its influencing factors, so as to provide a reference for meeting the decision needs of family members of Neurosurgical Intensive Care Unit patients.Methods:Convenience sampling was used to select 150 family members of Neurosurgical Intensive Care Unit patients from Beijing Tiantan Hospital affiliated with Capital Medical University from March to May 2024 for the study. General Information Questionnaire, Decisional Conflict Scale, Control Preference Scale, Preparation Decision Making, and Impact of Event Scale were used to survey the study participants. Multiple linear regression was used to analyze the factors influencing decision conflicts among family members of Neurosurgical Intensive Care Unit patients.Results:A total of 150 questionnaires were distributed, six questionnaires whose answers showed obvious patterns were excluded, 144 valid questionnaires were recovered, and the effective recovery rate of the questionnaires was 96.0% (144/150) . The total score of the Decisional Conflict Scale of family members of Neurosurgical Intensive Care Unit patients was (34.95±9.99) . Multiple linear regression analysis showed that decision readiness and impact of events were influencing factors of decision conflicts among family members of Neurosurgical Intensive Care Unit patients, and the difference was statistically significant ( P<0.05) . Conclusions:There are certain decision conflicts among family members of Neurosurgical Intensive Care Unit patients. Healthcare professionals should pay attention to family members' decision readiness and the impact of events, and develop targeted decision interventions to reduce decision conflicts of family members in order to promote a smooth medical decision process.
