5.Statins Ticagrelor and Rhabdomyolysis: A Coincidence or a Drug Interaction?

Objective:
Statins play a key role in the management of atherosclerotic cardiovascular disease for both primary and secondary prevention. However, their increasing usage has correspondingly led to a higher incidence of adverse effects, with muscle symptoms being the most common. An intriguing drug interaction exists between ticagrelor and high-intensity statins, which may exacerbate the adverse effects of statin-induced rhabdomyolysis, leading to significant consequences. This study was conducted to examine the profile of patients who have experienced statin-induced rhabdomyolysis while undergoing percutaneous transluminal coronary angioplasty (PTCA).
Methods:
This was an observational study that included 1,862 patients who underwent PTCA at our institute over the course of 1 year.
Results:
Over a 1-year period, we encountered four patients who were being treated with high-intensity statin therapy following acute coronary syndrome. These patients presented with muscle weakness and kidney injury. A notable commonality among all patients was the co-prescription of ticagrelor. Two patients died, while the other 2 were successfully managed through hydration, electrolyte balance, dialysis, and alternative lipid management drugs.
Conclusion
The concomitant use of ticagrelor and high-intensity statins should be carefully considered due to the additional risk of rhabdomyolysis and kidney injury. Future pharmacokinetic studies are needed to establish a causal relationship and predict potential drug interactions, which, if not avoided, could be fatal.
6.Accuracy of Femoral Tunnel Placement between Anteromedial and Anterolateral Visualisation Portals in Anterior Cruciate Ligament Reconstruction - Outcomes of a CT based Cross-Sectional Study

Introduction: Anatomical femoral tunnel placement is
critical for anterior cruciate ligament reconstruction
(ACLR). Tunnel placement may vary with different surgical
techniques. The aim of this study was to compare the
accuracy of femoral tunnel placement between the
Anteromedial (AM) and Anterolateral (AL) visualisation
portals on post-operative CT scans among a cohort of ACLR
patients.
Materials and methods: This cross-sectional study was
conducted from January 2018 to March 2020 after obtaining
ethics clearance. Patients who went for arthroscopic ACLR
in our institute were divided into an AM (group 1) and an AL
(group 2) based on the visualisation portal for creating the
femoral tunnel and a 3D CT scan was done. The femoral
tunnel position was calculated in deep to shallow and high to
low direction using the Bernard Hertel grid. Femoral tunnel
angle was measured in the 2D coronal image. Statistical
analysis was done with the data collected.
Results: Fifty patients with an average age of 26.36 (18-55)
years ±7.216 SD were enrolled in the study. In this study, the
AM technique was significantly more accurate (p<0.01) than
the AL technique in terms of femoral tunnel angle.
Furthermore, the deep to the shallow position was
significantly (p= 0.018) closer to normative values, as
determined by the chi-square test. The chances of error in
tunnel angle in femoral condyle are 2.6 times greater in the
AL technique (minimal clinical difference).
Conclusion: To conclude, in ACLR the anteromedial
visualisation portal can facilitate accurate femoral tunnel
placement compared to the anterolateral visualisation portal.
7.Small interfering RNA for cancer treatment: overcoming hurdles in delivery.

In many ways, cancer cells are different from healthy cells. A lot of tactical nano-based drug delivery systems are based on the difference between cancer and healthy cells. Currently, nanotechnology-based delivery systems are the most promising tool to deliver DNA-based products to cancer cells. This review aims to highlight the latest development in the lipids and polymeric nanocarrier for siRNA delivery to the cancer cells. It also provides the necessary information about siRNA development and its mechanism of action. Overall, this review gives us a clear picture of lipid and polymer-based drug delivery systems, which in the future could form the base to translate the basic siRNA biology into siRNA-based cancer therapies.
8.Simultaneous bilateral shoulder and bilateral central acetabular fracture dislocation: What to do?

Musculoskeletal injuries following seizures have a high morbidity and mortality. These injuries are often missed and the diagnosis is delayed due to a lack of clinical suspicion and appropriate investigations.We report a case of 72 years old male with simultaneous bilateral central acetabular fracture dislocation and bilateral posterior shoulder fracture dislocation secondary to an epileptic seizure. Present study high- lights the significance of clinical suspicion and clinico-radiological evaluation for diagnosis of a rare injury following episode of seizures. Simultaneous fracture dislocation of all four limbs treated with a holistic approach can lead to a good functional recovery. Surgical management with open reduction and internal fixation is preferred and replacement arthroplasty should be reserved for cases with implant failure and elderly patients.
