1.Hepatitis virus seroconversion among patients on maintenance hemodialysis in North India: a prospective cohort study

Objectives:
Patients receiving hemodialysis (HD) are at increased risk of acquiring hepatitis B virus (HBV) and hepatitis C virus (HCV) due to repeated blood exposure, vascular access procedures, and blood transfusions. The study aimed to determine the seroconversion rates of HBV and HCV among patients undergoing dialysis and to identify associated risk factors.
Methods:
A total of 220 adult patients receiving dialysis who were seronegative for hepatitis B surface antigen (HBsAg) and anti-HCV antibodies at baseline were enrolled. Serologic testing was repeated at 6 months.
Results:
Sixteen patients (7.3%) seroconverted to HBsAg positivity, and 23 (10.5%) seroconvertedto anti-HCV positivity. HBV seroconversion was significantly associated with multiple dialysis center visits. HCV seroconversion was significantly associated with dialyzer reuse, multiple dialysis center visits, and dialysis duration ≥2 years. Elevated liver enzyme levels were stronglycorrelated with seroconversion for both viruses.
Conclusion
HBV and HCV seroconversion remain concerns in HD units. Standardized infection control practices, minimization of patient transfers, and strict adherence to nationalguidelines are essential to reducing risk.
2.Biofilm Formation and Antibiotic Susceptibility Patterns in Contact Lens-Associated Bacteria

Purpose:
To characterize biofilm-forming capacity, extracellular polymeric substance (EPS), and antibiotic susceptibility (AST) of bacteria isolated from contact lenses (CL) and their accessories.
Methods:
Bacterial isolates from 20 CLs and their cases were evaluated for biofilm formation using crystal violet (CV) staining in tube and tissue culture plate assays. AST was performed by the Kirby-Bauer method. EPS production was assessed using Congo red agar (CRA) and Congo red broth (CRB), while carbohydrate, protein, and extracellular DNA (eDNA) contents were quantified.
Results:
The bacterial isolates included Achromobacter xylosoxidans, Klebsiella pneumoniae, Staphylococcus aureus, Staphylococcus cohnii, Staphylococcus epidermidis, and Staphylococcus saprophyticus, identified and confirmed by matrix-assisted laser desorption/ionization time-of-flight mass spectrometry. Out of 110 isolates, biofilm production by CV staining showed 28.5% strongly positive, 38.1% moderately positive, and 23.3% weakly positive by the tube method, and 32.1%, 18.6%, and 29.0%, respectively by tissue culture plate method; 14.3% were non-biofilm producers. The selected isolates were susceptible to the tested antibiotics, but resistance was noted in A. xylosoxidans. The EPS production was confirmed by black coloration in CRA plates, and CRB tubes with carbohydrate content was found to increase, whereas protein content was slightly reduced, and the presence of eDNA was detected after 48 hours. The scanning electron microscopy images depicted the presence of branched and sheet-like extracellular matrix structures. Physiological conditions like acidic pH and the presence of ribose sugar showed reduced biofilm; conversely, incubation at 28 °C enhanced bacterial attachment. In Staphylococcus species, increased salt concentration supported biofilm formation.
Conclusions
This study showed that bacteria isolated from CL of asymptomatic users had significant biofilm-forming ability. A. xylosoxidans and CoNs Staphylococcus were the predominant biofilm producers. CRB was an effective qualitative method for detecting EPS. These findings indicate bacterial colonization rather than infection and highlight the importance of proper contact lens hygiene and preventive care.

This study aimed to evaluate the efficacy of oil massage as an adjunct therapy for neonatal jaundice during phototherapy.
Methods:
A total of 160 neonates with unconjugated hyperbilirubinemia were enrolled and randomly allocated into two study groups using a computer-generated random number table.Group 1 (Intervention Group) consisted of 84 patients who received both phototherapy and massage therapy, while Group 2 (Control Group) consisted of 76 patients who received only phototherapy. The intervention group underwent massage therapy three times daily for 20 minutes over two consecutive days. Transcutaneous bilirubin levels were measured every eight hours. Additionally, the total duration of phototherapy and the 24-hour stool and urine output frequency were recorded in both groups.
Results:
There were no significant differences in the demographic characteristics between the two groups. However, stool frequency and urine output were significantly higher in the intervention group. The average duration of phototherapy was 19.4±4.17 hours in the intervention group compared to 28.4±6.58 hours in the control group (p<0.001). The rate of Transcutaneous bilirubin reduction was significantly higher in the intervention group during the first 24 hours. A rebound increase in bilirubin levels was observed at 40 and 48 hours in both groups after phototherapy was discontinued.
Conclusion
Phototherapy combined with massage significantly reduced total bilirubin levels in newborns with neonatal hyperbilirubinemia, making it a valuable adjunct treatment for the rapid reduction of bilirubin in neonates.

6.Osteonecrosis following Steroid Therapy in COVID-19Patients: An Outlook on the Emerging Problem

Steroids are used in management of coronavirus disease 2019 (COVID-19) patients with severe illness and their use has been demonstrated to decrease mortality. Although life-saving, steroids are well documented as risk factors for osteonecrosis.Osteonecrosis of the hip can be debilitating and surgery may be required to improve the quality of life. With the increasing number of COVID-19 cases, osteonecrosis of the hip and other joints resulting from steroid use is expected to show a sharp rise in the coming years. In this review we discuss the association between steroids and osteonecrosis, indications for steroid therapy in COVID-19 patients, and incidence, diagnosis, and treatment of osteonecrosis secondary to steroids in COVID-19.
