1.Application of an information-based integrated supervision system for the prevention and control of multidrug-resistant bacterial infections

OBJECTIVE To explore the application effect of the"IMS"information-based combined supervision sys-tem in the prevention and control of multidrug-resistant bacterial infections in the comprehensive ICU of a tertiary hospital.METHODS Data related to multidrug-resistant bacterial hospital infections in hospitalized patients in the comprehensive ICU of Shanghai Changzheng Hospital from 2022 to 2023 was retrospectively collected.In 2022,the conventional multidrug-resistant infection management mode was continued and was designated as the conven-tional infection management group.In 2023,on the basis of the aforementioned conventional management mode,the"IMS"information-based integrated supervision strategy was implemented,which included a multidrug-resist-ant critical value alert isolation system,a real-time monitoring system for antibiotic usage,a hand hygiene compli-ance monitoring system,and an automatic disinfection system for environmental cleanliness,and was designated as the information-based integrated supervision group.The rates of MDRO infections and hospital infections be-tween two groups were compared.RESULTS The informatized combined supervision group showed a reduction in The detection rate of five multidrug-resistant bacteria decreased in the information-based integrated supervision group(62.47%vs.69.33%,P<0.05).The utilization rate of special-grade antimicrobial agents among hospital-ized patients decreased(51.46%vs.57.58%,P<0.05).The hand hygiene compliance among ICU healthcare workers improved(95.45%vs.92.32%,P<0.05).Additionally,the rate of pathogenetic delivery prior to anti-microbial in hospitalized patients of other department,as well as the microbiological qualification rate of the envi-ronment,have also shown improvement.CONCLUSION With the help of"IMS"information-based integrated su-pervision system has effectively enhanced the capacity for prevention and control of multidrug-resistant infection in the emergency intensive care department in the hospital.
2.Analysis of the therapeutic effect of precise disconnection of pargastric varices guided by endoscopic ultrasound for the treatment of esophagogastric variceal bleeding(20 cases)

Objective To explore the therapeutic effect of precise disconnection of pargastric varices guided by endoscopic ultrasound in the treatment of esophagogastric variceal bleeding.Method A retrospective analysis was conducted on 20 patients with cirrhosis esophagogastric variceal bleeding treated with endoscopic ultrasound-guided precise disconnection of pargastric varices from January 1,2024 to December 31,2024.The efficacy was analyzed.Result All 20 patients successfully completed the precise disconnection of pargastric varices under the guidance of endoscopic ultrasound.The injection of tissue gel combined with the placement of spring coils(14 cases)and the injection of tissue gel alone(4 cases)successfully blocked the pargastric varices.All patients did not experience perforation,esophageal and cardia stenosis,massive bleeding,septicemia,or ectopic embolization.One patient who received tissue gel alone had slight bleeding from the pargastric varices after surgery and improved after 3 days of treatment to reduce portal vein pressure.Another one patient who received tissue gel alone had a low-grade fever and normal body temperature after 3 days of anti-infection treatment.Conclusion Precise disconnection of pargastric varices under the guidance of endoscopic ultrasound has a good therapeutic effect on esophagogastric variceal bleeding,with fewer complications such as ectopic embolization,massive bleeding,infection,and perforation.However,close follow-up observation is still needed to address the issue of pargastric varices.
3.Relationship between serum prolactin and mild cognitive impairment in postmenopausal women with type 2 diabetes mellitus

Objective To investigate the relationship between prolactin and mild cognitive impairment(MCI)in postmenopausal women with type 2 diabetes mellitus(T2DM).Methods A total of 319 postmenopausal women with T2DM who were hospitalized in the Department of Endocrinology,Drum Tower Hospital,Affiliated Hospital of Medical School,Nanjing University were enrolled in this study from August 2016 to October 2023.All the patients were divided into two groups according whether they had MCI:T2DM group(n=161)and MCI group(n=158).Differences in clinical characteristics were compared between the two groups.Pearson correlation was used to analyze the correlation between sex hormones and cognitive domains,and Logistic regression analysis was used to evaluate the influencing factors for MCI development.Results Serum prolactin levels were significantly lower in the MCI group than in the T2DM group[(5.5±2.1)vs(7.2±2.9)μg/L,P<0.05].Serum prolactin level was positively correlated with mini-mental state examination score,Montreal cognitive assessment score,immediate memory score,visuopatial constructional score,attention score and hippocampal volume(P<0.05),and negatively correlated with processing speed test(time)and executive function test(time)(P<0.05).Logistic regression analysis demonstrated that serum prolactin level was an influencing factor for the risk of MCI in postmenopausal women with T2DM(OR 0.715,95%CI 0.605~0.845,P<0.01).Conclusions The decrease of serum prolactin level is associated with an increased risk of MCI in postmenopausal women with T2DM.

Creutzfeldt-Jakob disease(CJD)is a rapidly progressive and fatal neurodegenerative disorder caused by prions,with certain infectivity and iatrogenic transmission risks.With the rapid progress and application of new dia-gnostic biomarkers and detection methods,as well as the construction and improvement of surveillance and reporting systems,the detection of CJD in patients domestically and internationally has shown an increasing trend year by year.Due to its long incubation period and heterogeneity of early symptoms,early identification and diagnosis of the disease is difficult,increasing the risk of transmission within medical institutions.Currently,there is a lack of con-sensus on the infection prevention and control of CJD.In order to timely identify and diagnose CJD as well as effec-tively block its transmission in medical institutions,this consensus summarizes 15 clinical concerns and formulates 24 specific recommendations based on the latest domestic and international research findings and clinical evidence,as well as combines with clinical practice,aiming to standardize healthcare-associated infection prevention and control measures for CJD and reduce its transmission risk in medical institutions.
5.Analysis of influencing factors of major adverse cardio-cerebrovascular events after drug-eluting stenting in patients with coronary artery disease

Objective:To explore the influencing factors of major adverse cardio-cerebrovascular events(MACCE)after drug-eluting stenting in patients with coronary artery disease(CAD).Methods:Retrospective study data from First Affiliated Hospital of Zhengzhou University were extracted from the Dryad database.Patients were di-vided into MACCE group(n=160)and control group(n=880)according to whether MACCE occurred during fol-low-up.Clinical data including age,gender,fibrinogen(Fg)level etc.were compared between two groups,and the variables with significant differences between two groups were substituted into Logistic regression model to screen the influencing factors of MACCE after drug eluting stenting in CAD patients.Results:Compared with pa-tients in control group,those in MACCE group had significant higher Fg level[(3.6±1.1)g/L vs.(3.4±1.0)g/L],proportions of triple-vessel coronary disease(25.6% vs.18.3%)and right coronary artery lesion(53.1% vs.41.7%)(P<0.05 or<0.01).Multivariate Logistic regression analysis indicated that Fg level(OR=1.18,95%CI 1.01~1.37,P=0.039)was the independent risk factor of MACCE after drug eluting stenting in CAD patients.The risk of MACCE after drug eluting stenting in CAD patients significantly increased when the Fg level was>3.27 g/L.Conclusion:High Fg level is the independent risk factor of MACCE after drug eluting stenting in CAD patients.
6.Investigation of Treatment Measures for Severe Postpartum Hemorrhage Relat-ed to Massive Transfusion:a Multi-center Retrospective Study

Objective:To investigate the use of treatment measures in patients with severe postpartum hemor-rhage and massive blood transfusion.Methods:A retrospective analysis was conducted on the clinical data of parturients from 18 medical institutions who gave birth between January 2019 and June 2023,with postpartum hemorrhage≥ 1000 ml within 24 hours after delivery and red blood cell(RBC)transfusion≥4 U.According to the amount of RBC transfusion,the patients were divided into massive-transfusion(MT)group(≥ 10 U)and non-massive-transfusion(non-MT)group(4-<10 U).The high-risk factors for postpartum hemorrhage,causes of hemorrhage,and medical and surgical treatment measures were compared between the two groups.Results:A total of 575 patients were included(134 cases in MT group and 441 cases in non-MT group).In the MT group,the proportions of multiparous women,gravidity≥2,preterm birth,previous cesarean section,placenta previa,and placenta accreta were significantly higher than those in the non-MT group,with statistically significant differences(P<0.05).The primary cause of postpartum haemorrhage in the MT group was placental factors(58.2%),whereas uterine atony was the main cause in the non-MT group(52.2%).Analysis of medication treatment showed that carboprost tromethamine was the most frequently used secondary uterotonic in both groups(73.1%and 80.5%).Surgical management analysis revealed that the proportion of surgical interventions was higher in the MT group than that in the non-MT group.Among the 44 patients who underwent hysterectomy,33 cases un-derwent hysterectomy during cesarean section,with the primary cause of postpartum haemorrhage being placen-tal factors(29 cases,87.9%).Conclusion:Patients with severe postpartum haemorrhage requiring massive transfusion need extensive pharmacological and surgical interventions.It is crucial to reduce risk factors,particu-larly by decreasing the incidence of primary cesarean sections,optimize uterotonic use,and ultimately minimize the occurrence of severe postpartum haemorrhage and massive transfusion.
7.Bushen Zhuanggu Formula promotes bone repair in nontraumatic osteonecrosis of the femoral head via regulating PKC-RAS-ERK-ETS1-RANKL signaling axis

Background:Bushen Zhuanggu Formula(BZF),derived from the classic Yougui Pills,has shown favorable clinical efficacy in treating advanced nontraumatic osteonecrosis of the femoral head(NONFH),particularly by promoting bone repair.However,its underlying mechanisms remain unclear.Objective:This study aimed to explore the mechanisms by which BZF promotes bone repair in advanced NONFH.Materials and methods:A total of 518 potential BZF targets were identified from the ETCM v2.0 database.Transcriptomic profiling of clinical cohorts revealed 485 differentially expressed genes in advanced NONFH patients compared to healthy controls.A drug target-disease gene interaction network was constructed to identify candidate BZF targets involved in NONFH pathogenesis.In vivo experiments were conducted to validate the effects of BZF in a rat model of advanced NONFH.Results:Network analysis identified key pathways associated with blood circulation obstruction,immune-inflammatory imbalance,and abnormal bone metabolism.Protein kinase C alpha(PKCA),Ras proto-oncogene(RAS),mitogen-activated protein kinase 3(ERK),ETS proto-oncogene 1(ETS1),and receptor activator of nuclear factor-κB ligand(RANKL)formed a signaling axis implicated in NONFH pathogenesis.BZF treatment alleviated joint inflammation,preserved trabecular bone morphology,reduced bone loss,and promoted bone repair.Mechanistically,BZF significantly downregulated the expression of PKCA,RAS,ERK,ETS1,and RANKL,improved blood circulation,and inhibited osteoclast activation while promoting osteoblast activation.Conclusion:BZF may promote bone repair in advanced NONFH by enhancing blood circulation and modulating the PKC-RAS-ERK-ETS1-RANKL signaling axis,thereby reversing dysregulated bone metabolism.
