1.5G-enabled remote robot-assisted thoracic surgery: Clinical outcomes, current challenges, and future perspectives

With the integration of 5G communication technology and robotic surgical systems, remote robot-assisted thoracic surgery is overcoming geographical barriers, offering an innovative approach to addressing the uneven distribution of medical resources. This study conducted a systematic literature review—using databases such as PubMed and CNKI, with the search period extending up to 2025—incorporating clinical studies, case reports, and review articles to comprehensively evaluate the clinical efficacy and safety of 5G-enabled remote robot-assisted thoracic surgery (5G-RRATS). The analysis also examined current technological limitations and potential future development trajectories. Existing evidence indicates that, given adequate technical support, 5G-RRATS can achieve perioperative outcomes comparable to those of conventional local robotic surgeries across procedures including pulmonary wedge resection, lobectomy, and esophagectomy. Furthermore, it demonstrates potential advantages in minimizing surgical incisions and reducing intraoperative blood loss. Nevertheless, challenges related to network stability, latency control, interdisciplinary collaboration between medical and engineering teams, and legal, regulatory, and ethical considerations continue to hinder widespread clinical adoption. Looking ahead, the emergence of a "one-to-many" remote surgical model, combined with the integration of artificial intelligence and augmented reality technologies, as well as advancements in low-orbit satellite communications, may enable 5G-RRATS to further advance precision and efficiency in thoracic surgery, thereby facilitating equitable access to high-quality care for a broader patient population.
2.Efficacy and Mechanism of Shenling Qushi Prescription in Treatment of Hyperuricaemia Mice

ObjectiveTo investigate the urate-lowering effect of Shenling Qushi prescription in the mouse model of hyperuricaemia. MethodsSixty-three C57BL/6N mice were randomly assigned to the following groups (n=9): Control, model, low-dose (6 g·kg-1) Shenling Qushi prescription, medium-dose (12 g·kg-1) Shenling Qushi prescription, high-dose (24 g·kg-1) Shenling Qushi prescription, medium-dose Shenling Qushi prescription plus febuxostat (12+0.003 g·kg-1), and febuxostat (3 mg·kg-1). A mouse model of persistent hyperuricaemia was established via intraperitoneal injection of potassium oxazolamide (250 mg·kg-1) combined with oral administration of allopurinol (300 mg·kg-1) over a 28-day period. Concurrently, the prescription was administered orally at the specified doses once daily for 21 consecutive days. Throughout the experiment, mouse body weight, food intake, water consumption, and urine output were measured and recorded. Kidney and liver indices were calculated. Renal histopathological changes were observed by haematoxylin and eosin staining. Serum uric acid (SUA) and urinary uric acid (UUA) levels, along with xanthine oxidase (XOD) and adenosine deaminase (ADA), alanine aminotransferase (ALT) and aspartate aminotransferase (AST) activities, were determined. Real-time PCR was employed to assess the mRNA levels of urate anion transporter 1 (URAT1) and glucose transporter 9 (GLUT9) in the renal tissue, alongside those of XOD and ADA in the liver tissue. The protein levels of URAT1 and GLUT9 in the renal tissue were determined by Western blot. ResultsCompared with the normal group, model mice exhibited reduced food intake and body weight (P<0.01), decreased water consumption yet increased urine output, no significant change in the liver index but increased kidney index (P<0.01). Pathological findings revealed renal tissue damage including glomerular atrophy, epithelial cell detachment, and inflammatory cell infiltration. Furthermore, the model group exhibited elevated UUA and SUA levels (P<0.01), enhanced activities of ALT, AST, XOD, and ADA in the liver tissue (P<0.05, P<0.01), upregulated mRNA and protein levels of URAT1 and GLUT9 in the renal tissue (P<0.05), alongside markedly elevated mRNA levels of XOD and ADA in the liver (P<0.01). Compared with the model group, the Shenling Qushi prescription groups showed no significant changes in body weight or food intake, with water consumption and urine output remaining within normal ranges, alleviated glomerular atrophy, with reduced pathological damage such as inflammatory infiltration, declined UUA and SUA levels (P<0.01), and increased activities of ALT, AST, XOD, and ADA in the serum (P<0.01). Real-time PCR results demonstrated that Shenling Qushi prescription downregulated the mRNA levels of URAT1 and GLUT9 in the renal tissue (P<0.05, P<0.01) and XOD and ADA in the hepatic tissue (P<0.05, P<0.01). Western blot results demonstrated Shenling Qushi prescription reduced the protein levels of URAT1 and GLUT9 (P<0.05, P<0.01). ConclusionShenling Qushi prescription modulates fluid balance and reduces uric acid levels, thereby alleviating renal injury in hyperuricaemic mice. This effect is highly likely mediated through inhibition of uric acid synthesis and reabsorption.
3.Hydraulic sinus floor elevation for clinical efficacy: a systematic review and Meta-analysis

Objective The present study aimed to evaluate the clinical efficacy of hydraulic sinus floor elevation for the treatment of insufficient bone height in the posterior maxilla through a systematic review and Meta-analysis, so as to provide evidence for clinical practice. Methods Databases including PubMed, Embase, Cochrane Library, and Clinical Trials.gov were searched up to May 2025. Randomized controlled trials, non-randomized controlled trials, cohort studies, cross-sectional studies, and case-control studies that involved patients receiving hydraulic sinus floor elevation with simultaneous or delayed implant placement were included. Exclusion criteria comprised in vitro studies, animal experiments, narrative reviews, unpublished data, commentaries or expert opinions, studies with unclear sample sizes, and publications without full-text access or unavailable outcome data. Three investigators independently performed literature screening and quality assessment. A single-arm Meta-analysis was conducted using Stata 18.0 for studies meeting the inclusion criteria. The outcome measures were the incidence of sinus membrane perforation, implant survival rate, and endo-sinus bone gain. The Cochrane Risk of Bias 2 (RoB2) tool and the Joanna Briggs Institute (JBI) critical appraisal checklist were used to assess the methodological quality of randomized controlled trials and non-randomized controlled trials, respectively. The overall certainty of evidence was assessed using the GRADE(Grading of Recommendations Assessment, Development and Evaluation)system. Results Nineteen studies with 982 patients, 1 151 elevated sites, and 1 263 implants were finally included. The Meta-analysis showed that the intraoperative sinus membrane perforation rate of hydraulic sinus floor elevation was 2.0% [ES = 0.02, 95% CI (0.00, 0.04)] (18 studies, 1 061 sites); the implant survival rate was 99.0% [ES = 0.99, 95% CI (0.97, 1.00)] (17 studies, 1 253 implants); and the mean vertical bone gain was 7.80 mm. For this outcome, heterogeneity among studies was extremely high (I2 = 98.6%), thus only descriptive analysis was performed. Egger's test revealed no significant publication bias for the three outcomes. GRADE grading indicated very low certainty of evidence for mucosal perforation, implant survival rate, and endo sinus bone gain. Conclusion Current preliminary evidence suggests that hydraulic sinus lift may be a safe minimally invasive technique, but the evidence is very low, and more high quality, long term follow up randomized controlled clinical trials are still needed for further verification.

Intentional tooth replantation (ITR) is an advanced treatment modality and the procedure of last resort for preserving teeth with inaccessible endodontic or resorptive lesions. ITR is defined as the deliberate extraction of a tooth; evaluation of the root surface, endodontic manipulation, and repair; and placement of the tooth back into its original socket. Case reports, case series, cohort studies, and randomized controlled trials have demonstrated the efficacy of ITR in the retention of natural teeth that are untreatable or difficult to manage with root canal treatment or endodontic microsurgery. However, variations in clinical protocols for ITR exist due to the empirical nature of the original protocols and rapid advancements in the field of oral biology and dental materials. This heterogeneity in protocols may cause confusion among dental practitioners; therefore, guidelines and considerations for ITR should be explicated. This expert consensus discusses the biological foundation of ITR, the available clinical protocols and current status of ITR in treating teeth with refractory apical periodontitis or anatomical aberration, and the main complications of this treatment, aiming to refine the clinical management of ITR in accordance with the progress of basic research and clinical studies; the findings suggest that ITR may become a more consistent evidence-based option in dental treatment.

Instrument separation is a critical complication during root canal therapy, impacting treatment success and long-term tooth preservation. The etiology of instrument separation is multifactorial, involving the intricate anatomy of the root canal system, instrument-related factors, and instrumentation techniques. Instrument separation can hinder thorough cleaning, shaping, and obturation of the root canal, posing challenges to successful treatment outcomes. Although retrieval of separated instrument is often feasible, it carries risks including perforation, excessive removal of tooth structure and root fractures. Effective management of separated instruments requires a comprehensive understanding of the contributing factors, meticulous preoperative assessment, and precise evaluation of the retrieval difficulty. The application of appropriate retrieval techniques is essential to minimize complications and optimize clinical outcomes. The current manuscript provides a framework for understanding the causes, risk factors, and clinical management principles of instrument separation. By integrating effective strategies, endodontists can enhance decision-making, improve endodontic treatment success and ensure the preservation of natural dentition.

6.Quality of inpatient medical records based on quality control indicators for medical record manage-ment

Objective Guided by"Medical Record Management Quality Control Indicators(2021 Edition)"(hereafter regarded as Medical Record Quality Indicators),this study aims to evaluate the quality of inpatient hospice medical records at a tertiary hospital in Guangzhou.Methods A total of 1,071 inpatient hospice medical records from the year 2023 in a tertiary gen-eral hospital in Guangzhou were selected for evaluation.The evaluation focused on three aspects:documentation compliance of documentation of critical examinations(including CT/MRI,pathology,and pathogen examinations),the compliance rate of treat-ment behavior records(encompassing antibiotic usage,chemotherapy or radiotherapy or targeted or immunotherapy for malignant tumors,and surgical records),and the incidence of unreasonable duplication within medical record.Results The compliance rate for major examination records ranged from 47.7%to 100.0%,with the lowest compliance rate(47.7%)observed in docu-mentation of pathogen culture analysis.Treatment behavior documentation compliance varied from 49.1%to 100.0%,with the lowest compliance rate of 49.1%observed in the recording of antibiotic usage.Rates of inappropriate duplication ranged from 1.0%to duplication(63.1%),with the highest rate of 63.1%occurring when initial progress notes replicated admission histo-ries without synthesis(63.1%).Conclusion The Medical Record Management Quality Control Indicators serves as an effective tool for evaluating the dimensions of medical record quality and offers a systematic framework for enhancing documentation integri-ty within hospitals.
7.Factors influencing occupational burnout and career choice among resident physicians during standardized training

Objective:To investigate the prevalence,related factors,and career implications of occupational burnout among resident physicians during standardized training in Inner Mongolia.Methods:This cross-sectional study used convenience sampling to enroll 2 891 resident physicians,assessing them with a self-developed general information questionnaire,socio-psychological scales,and a universal burnout inventory.Latent profile analysis clas-sified participants based on burnout dimensions,while logistic regression models examined the factors influencing burnout and their relationship with career choices.Results:Resident physicians were divided into low(59.0%),moderate(28.9%),and high(12.1%)burnout subgroups.Logistic regression showed that,compared to the low burnout group,significant risk factors included professional master's students(OR=0.44),working>70 hours weekly(OR=0.63),>4 night shifts monthly(OR=0.66),anxiety(mild OR=0.49;moderate OR=0.26;se-vere OR=0.14),depression(mild OR=0.38;moderate OR=0.24;severe OR=0.11),insomnia(mild OR=0.51;moderate OR=0.44;severe OR=0.38),low social support(OR=0.23),and low happiness index(OR=0.42).Male residents(OR=1.25)were less likely to experience burnout.High burnout residents were 5.11 times more likely to quit the medical profession.Conclusion:Training intensity and socio-psychological factors signifi-cantly contribute resident physicians' burnout levels,with severe burnout increase career attrition.
8.The prognostic value of TNM stage and L3-skeletal muscle mass index combined score in elderly patients with colorectal cancer

Objective:We study the prognostic value of TNM stage and L3-skeletal muscle mass index in overall survival(OS)in elderly patients with colorectal cancer after surgery.Furthermore, investigate the prognostic value of combined scoring system(TNM-SMI score)in patients with colorectal cancer.Methods:From January 1, 2018 to January 31, 2019, the clinical data of elderly patients who were diagnosed with colorectal cancer for the first time in Longyan First Hospital, Fujian Medical University were retrospectively analyzed.The baseline data were compared in patients with different TNM stage and L3SMI.The relevant factors that might affect the prognosis of the patients were analyzed to determine the independent risk factors in cox regression analyses.Survival curves were plotted by using the Kaplan-Meier method, and Log-rank test was used to compare Overall Survival.Predictive efficacy was compared by plotting (ROC)curve and calculating the area under the curve(AUC).Results:A total of 128 eligible patients aged 65-88(73.96±6.20)years were enrolled.Among them, 82 were male.Univariate and multifactorial cox regression analyses showed that TNM stage( HR=3.944, 95% CI: 1.519-10.237, P<0.05)and L3SMI( HR=3.194, 95% CI: 1.222-8.349, P<0.05)were the independent risk factors for OS in elderly colorectal cancer patients; Survival curves plotted by Kaplan-Meier method showed that the 5-year survival rate(OS rate)of patients in TNM(Ⅲ and Ⅳ)group was lower than in TNM(Ⅰ and Ⅱ)group(58.8% vs.88.9%, P<0.001), and the 5-year survival rate(OS rate)of patients in low L3SMI group was lower than in high L3SMI group(51.0% vs.88.3%, P<0.001); Elderly colorectal patients were risk stratified by TNM-SMI score, the patients with higher score had worse 5-year OS rate; The AUC of L3SMI, TNM stage, and TNM-SMI score are 0.729, 0.695, and 0.800. Conclusions:The TNM stage and L3SMI are independent risk factors of OS after colorectal cancer surgery; TNM-SMI score, which combines TNM stage and L3SMI, can predict prognosis more efficiency compare to single-factor.The higher TNM-SMI score had the worse prognosis; TNM stage in combination with L3SMI improves the efficiency prediction of OS after colorectal cancer surgery.
9.Discriminating Tumor Deposits From Metastatic Lymph Nodes in Rectal Cancer: A Pilot Study Utilizing Dynamic Contrast-Enhanced MRI

Objective:
To evaluate the feasibility of dynamic contrast-enhanced MRI (DCE-MRI) in differentiating tumor deposits (TDs) from metastatic lymph nodes (MLNs) in rectal cancer.
Materials and Methods:
A retrospective analysis was conducted on 70 patients with rectal cancer, including 168 lesions (70 TDs and 98 MLNs confirmed by histopathology), who underwent pretreatment MRI and subsequent surgery between March 2019 and December 2022. The morphological characteristics of TDs and MLNs, along with quantitative parameters derived from DCE-MRI (K trans , kep, and v e) and DWI (ADCmin, ADCmax, and ADCmean), were analyzed and compared between the two groups.Multivariable binary logistic regression and receiver operating characteristic (ROC) curve analyses were performed to assess the diagnostic performance of significant individual quantitative parameters and combined parameters in distinguishing TDs from MLNs.
Results:
All morphological features, including size, shape, border, and signal intensity, as well as all DCE-MRI parameters showed significant differences between TDs and MLNs (all P < 0.05). However, ADC values did not demonstrate significant differences (all P > 0.05). Among the single quantitative parameters, v e had the highest diagnostic accuracy, with an area under the ROC curve (AUC) of 0.772 for distinguishing TDs from MLNs. A multivariable logistic regression model incorporating short axis, border, v e, and ADC mean improved diagnostic performance, achieving an AUC of 0.833 (P = 0.027).
Conclusion
The combination of morphological features, DCE-MRI parameters, and ADC values can effectively aid in the preoperative differentiation of TDs from MLNs in rectal cancer.
10.Modified Maimendong Decoction Inhibits Lung Cancer Metastasis by Up-Regulating Levels of NK and CD8+ T Cells in Peripheral Blood and Tumor Microenvironment

Objective To explore the mechanism of modified maimendong decoction (MMD) in inhibiting lung cancer metastasis from the perspective of immune regulation. Methods CTC-TJH-01 and LLC cells were intervened with different concentrations of modified maimendong decoction. The cell proliferation was detected with a CCK-8 kit, apoptosis was detected with an Annexin V-FITC/PI kit, and cell migration was detected through Transwell assays. A lung metastasis model was established through the tail vein injection of LLC cells into C57BL/6 mice, and body weight change and lung tumor metastasis in the mice were evaluated after continuous gavage intervention with MMD. HE staining, immunohistochemistry, and immunofluorescence were employed to observe the histomorphology, Ki-67 protein level, and NK and T cell levels of metastatic lesions. The levels of NK and T cells in the peripheral blood of mice were detected throughflow cytometry. Results MMD had no significant inhibitory effect on the proliferation, apoptosis, and migration of CTC-TJH-01 and LLC cells in vitro. In mice, MMD could significantly inhibit the lung metastasis of LLC cells, increase the proportion of NK and CD8+ T cells in peripheral blood and tumor microenvironment (P<0.05), and reduce the expression of Ki-67 protein in metastatic tumor tissues (P<0.05). Conclusion MMD may inhibit the growth of metastatic tumors by upregulating the expression levels of NK and CD8+ T cells in peripheral blood to promote the elimination of circulating tumor cells, and regulating the infiltration of NK and CD8+ T cells in the immune microenvironment of metastatic tumors, then play an antimetastatic role in lung cancer.
