1.Comprehensive geriatric assessment-based screening and integrated intervention for osteosarcopenia risk factors in older adults: an application analysis in patients aged ≥80 years

Objective To explore the utility of comprehensive geriatric assessment (CGA) in screening risk factors for osteosarcopenia (OS) among older adults (≥80 years old) and to evaluate the therapeutic efficacy of CGA-guided integrated interventions for OS. Methods A total of 420 patients aged ≥80 years, recruited from the Department of Geriatrics, General Practice of The Affiliated Jiangyin Hospital of Nantong University, and community health centers from January 2022 to October 2024, were enrolled. Participants were classified into OS (n=139) and non-OS (n=281) groups based on diagnostic criteria. CGA was utilized to compare differences in general characteristics, laboratory indicators, comorbidities between groups. Binary logistic regression analysis identified independent risk and protective factors. Subsequently, 40 OS patients were randomly assigned to an intervention group (n=20) receiving integrated interventions including nutritional support, exercise training, and psychological management or a control group (n=20, receiving routine care). Appendicular skeletal muscle mass index (ASMI), grip strength, gait speed, and bone mineral density (BMD) T-score were compared between groups after 3 months. Results The prevalence of OS in this cohort was 33.1%. Compared to the non-OS group, the OS group exhibited significant differences in age, body mass index (BMI), smoking history, comorbidity index, concomitant medication, cognitive impairment, visual and hearing impairment, sleep disorders, depression, marital status, social participation, activities of daily living, nutritional risk, total cholesterol, uric acid, and constipation (P<0.05). Logistic regression analysis identified age and comorbidity index as significant risk factors for OS, while BMI, married status, total cholesterol, and activities of daily living (assisted and independent) served as protective factors. The intervention group demonstrated significant improvements in grip strength, gait speed, BMD T-score, and male ASMI compared to controls (P<0.05). Conclusions CGA demonstrates clinical utility in systematically identifying risk factors for OS in the old population. Multimodal interventions guided by CGA effectively improve musculoskeletal function in elderly OS patients.

3.Epidemiology of urticaria in China: a population-based study

Background::Urticaria is a common skin disease characterized by episodes of wheals, and it has a negative effect on patients’ quality of life. Large-scale population-based epidemiological studies of urticaria are scarce in China. The aim of this survey was to determine the prevalence, clinical forms, and risk factors of urticaria in the Chinese population.Methods::This survey was conducted in 35 cities from 31 provinces, autonomous regions, and municipalities of China. Two to three communities in each city were selected in this investigation. Participants completed questionnaires and received dermatological examinations. We analyzed the prevalence, clinical forms, and risk factors of urticaria.Results::In total, 44,875 questionnaires were distributed and 41,041 valid questionnaires were collected (17,563 male and 23,478 female participants). The lifetime prevalence of urticaria was 7.30%, with 8.26% in female and 6.34% in male individuals ( P < 0.05). The point prevalence of urticaria was 0.75%, with 0.79% in female and 0.71% in male individuals ( P < 0.05). Concomitant angioedema was found in 6.16% of patients. Adults had a higher prevalence of urticaria than adolescents and children. Living in urban areas, exposure to pollutants, an anxious or depressed psychological status, a personal and family history of allergy, thyroid diseases, and Helicobacter pylori infection were associated with a higher prevalence of urticaria. Smoking was correlated with a reduced risk of urticaria. Conclusion::This study demonstrated that the lifetime prevalence of urticaria was 7.30% and the point prevalence was 0.75% in the Chinese population; women had a higher prevalence of urticaria than men. Various factors were correlated with urticaria.

Objective To assess the efficacy and side effects of intravesical instillation of BCG after transurethral resection of the bladder tumor (TURBT) in non-muscle invasive bladder cancer (NMIBC) patients.Methods The clinical data of patients treated with BCG 120 mg per course induced perfusion or more after TURBT from December 2013 to October 2016 in 18 hospitals of northeast China region,were analyzed retrospectively.The first part,data of 106 patients with moderate,high-risk NMIBC were collected.A total of 83 patients were male,while the other 23 patients were female.The average age was 66.7 years old.The clinical staging were T1 in 86(81.1%) cases,Ta in 20(18.9%) cases and carcinoma in situ in 6 (5.7%) patients.Intravesical instillation of BCG was executed after transurethral resection of the bladder tumor.The incidence rate of recurrence and progression during more than 6 months' follow-up time were observed.Multivariate analyses were done by using logistic analysis and Cox proportional hazards regression model with Kaplan-Meier method.The second part,treatment compliance of 276 patients with bladder cancer,including moderate/high-risk NMIBC in 263 cases,moderate/high-risk NMIBC followed with renal pelvis/ureteral carcinoma in 8 cases were and moderate/high-risk NMIBC with renal pelvis/ureteral carcinoma in 5 cases who treated with BCG after the surgeries,were observed.Patients consisted of 211 males and 65 females with average age of 68.3 years.Results With a median follow-up of 12 months,9 (8.5%) patients experienced tumor recurrence and 2 (1.9%) patients were found progression in the first part.The one-year cancer free recurrence rate of the patients was 91.5%.Statistically significant prognostic factors for recurrence identified by multivariable analyses were prior recurrence of the tumors (OR =3.214,95%CI0.804-12.845,P =0.099).In the second port,an incidence rate of adverse effects was 64.1% (177/276).The Ⅲ/Ⅳ degree complications were occurred in 11 patients and satisfactory outcomes achieved with active treatment.A total of 36 patients withdrawal with the major causes were recurrence and progression of bladder tumor in 12 cases (4.4 %),9 cases (3.3 %) with economic reasons and 11 cases (4.0%) with serious complications.Conclusions NMIBC patients treated with intravesical BCG therapy have approving cancer free recurrence rates and acceptable adverse effects.Prior recurrence may be prognostic factor of recurrence after intravesical BCG therapy.
5.Efficacy analysis of ruptured posterior communicating artery aneurysms with stent-assisted vs.non-stent-assisted coil embolization

Objective To compare the safety and efficacy of stent-assisted coil and non-stent-assisted coil for the treatment of ruptured posterior communicating artery aneurysms.Methods A total of 121 consecutive patients with ruptured posterior communicating artery aneurysm treated at the Department of Neurosurgery,Heze Municipal Hospital between June 2014 and June 2017 were enrolled retrospectively.They divided into either a stent group (n =63) or non-stent group (n =58) according to whether they used stent treatment or not.The two groups were compared,and their clinical data,surgical related complications,the degree of embolism immediately after aneurysm surgery,and the follow-up results of clinical and imaging were analyzed.Results Interventional embolization therapy was successfully performed in all the patients.(1) The proportion of wide-necked aneurysms of the stent group was higher than that of the non-stent group.There was significant difference (92.1% [58/63] vs.8.6% [5/58],x2 =84.249,P < 0.01).There were no significant differences in age,aneurysm size,sex,and HuntHess grade (all P > 0.05).(2) The incidences of intraoperative aneurysm rupture of the stent group and non-stent group were 4.8% (3/63) and 3.4% (2/58) respectively.There was no significant difference (x2 =0.132,P =0.717).The incidence of thromboembolie complication was 12.7% (8/63) and 5.2% (3/58)respectively.There was no significant difference (x2 =2.070,P =0.150).In the stent group,1 patient disabled and 1 died,and in the non-stent group,there were no cases with surgical related disability,and 1 died.(3) The comparison of immediate results after procedure between the stent group and non-stent group:there were no significant differences in the complete embolic rate (39.7% [25/63] vs.37.9%[22/58],x2 =0.039),aneurysm neck residual rate (25.4% [16/63] vs.39.7% [23/58],x2 =2.811],and residual rate of aneurysm (34.9% [22/63] vs.22.4% [13/58],x2 =2.298,all P > 0.05).(4) The recurrence rate of aneurysms (4.3% [2/46]) of the stent group was lower than that of the non-stent group (31.8% [14/44]).There was significant difference between the two groups (x2 =112.610,P =0.01).No rebleeding occurred in all aneurysms after procedure.Conclusion Compared with the non-stent-assisted coil embolization,the stent assisted embolization of ruptured posterior communicating artery aneurysms in acute phase may reduce the recurrence rate of aneurysms and without significantly increasing the incidence of surgery-related complications.
6.The diagnostic value of ProGRP for small cell lung cancer

Objective To explore the clinical significance of serum ProGRP in diagnosis of SCLC.Methods Serum ProGRP were detected in twenty-three SCLC patients,forty NSCLC patients and forty-three patients with benign pulmonary disease whose were definite diagnosis collected from April 15,2016 to July 19,2016,and a contemporary cohort of 40 healthy controls were while recruited.Results The levels of ProGRP in SCLC group were significantly higher than those in NSCLC group,benign pulmonary disease group and healthy control group(P <0.05).The levels of ProGRP in extensive disease group were significantly higher than those in 1imited disease group(P <0.05).There was no significant difference in the positive rates of ProGRP between the sex group and age group (P > 0.05 ).The boundary value of diagnosis of SCLC through ProGRP identified through ROC curve was 65.66 ng/L.The sensitivity and specificity of the ProGRP for the diagnosis of SCLC were respectively 90.9% and 89.9%.Conclu-sion Serum ProGRP can be use as a sensitive and specific index for diagnosis of SCLC,and the level of ProGRP can also be used for the clinical staging of SCLC.
7.Effect of Enteral Nutrition Liquid with Different Dietary Fiber Content on Serum Protein in Patients with Stress Hyperglycemia

Objective To compare the effect of enteral nutrition liquid with different dietary fiber content on serum protein in patients with stress hyperglycemia.Methods A total of 90 patients with stress hyperglycemia were randomly divided into three groups:enteral nutritional suspension (SP) group (Peptisorb),enteral nutritional suspension (TPF-FOS) group (Jevety) and enteral nutritional suspension (TPF-D) group (Glucerna),each group consisting of 30 patients.In the three groups,prealbumi (PA),serum albumin (ALB),retinol binding protein (RBP),transfer-rin (TRF) and hemoglobin (Hb) were continuously measured on the 1st,2nd,3rd,4th,5th,6th and 7th day.Data were analyzed by SPSS 15.0.Variances on time and group were analyzed by repeated measures of general linear model.Results The PA,ALB,RBP,TRF and Hb were significantly different among Peptisorb group,Jevety group and Glucerna group (P< 0.05).The PA,ALB and Hb which were determined in different time,were not significantly impacted by Peptisorb,Jevety or Glucerna,and not significantly changed with time.The RBP and TRF which were determined at different time,were not impacted by Peptisorb,Jevety or Glucema,but time× group from RBP and TRF which were determined in different time,were significantly impacted in Peptisorb group,Jevety group and Glucerna group (P< 0.01).Conclusion The content from different dietary fiber significantly affects serum protein in patients with stress hyperglycemia,and TPF-D has the most significant effect on serum protein.
8.Optimization of automatic spectral imaging mode selection and adaptive statistical iterative reconstruction in upper abdominal enhanced CT with low contrast agent dose

Objective To investigate the value of automatic spectral imaging mode selection and adaptive statistical iterative reconstruction(ASIR) in upper abdominal enhanced CT with low contrast agent dose, and to optimize the combination of monochromatic energy level and ASIR percentage.Methods 100 patients underwent upper abdominal enhancement CT were randomly assigned into control group (n=50) and study group (n=50).In the control group, tube voltage of 120 kVp and contrast medium of 450 mg I/kg were used with the images reconstructed using 40% ASIR.In the study group, CT spectral imaging with automatic spectral imaging mode selection and contrast medium of 300 mg I/kg were used,and monochromatic images (40-65 keV,intervals of 5 keV) were reconstructed using 40%-60% ASIR (intervals of 10%), respectively.CT value, image noise and contrast-to-noise ratio of the liver, pancreas, aorta and portal vein and radiation dose were compared using two independent samples t test.Overall image quality was assessed by Mann-Whitney U test.Results All image quality indexes in 60 keV with 40% ASIR, 55-60 keV with 50% ASIR in the study group were equal to or better than the control group.There was no significant difference in radiation dose between control group and study group.Conclusion Combined automatic spectral imaging mode selection with ASIR, upper abdominal enhanced CT with low contrast agent dose could improve image quality compared to 120 kVp with 40% ASIR, without increasing radiation dose.
9.The early diagnostic value of corneal laser confocal microscopy for small neuropathy in elderly patients with type 2 diabetes mellitus

Objective To investigate the early diagnostic value of cornel confocal microscopy for the screening of small neuropathy in elderly patients with type 2 diabetic mellitus.Methods In the prospective study,96 elderly patients with diabetes as study group and 46 patients with non-diabetes as the control group were continuously collected from our hospital endocrinology and ophthalmology out patients during May 2014 to February 2016.The 96 cases of type 2 diabetes were subdivided into 47 patients with diabetic peripheral neuropathy (DPN)and 47 patients with nowdiabetic peripheral neuropathy(non DPN).Results The diabetes course was shorter in non-DPN group than in DPN group(P=0.000).The levels of glycosylated hemoglobin and urine albumin were lower in the nonDPN than in the DPN(P =0.072,0.007,respectively).The corneal nerve fiber density was lower in the DPN group than in NDPN group (P =0.000).Corneal nerve fiber density was higher in control group than in DPN and NDPN group.The differences in number of corneal nerve fibers showed no statistical significance between DPN and NDPN group (x2 =2.391,P =0.314).But the number of corneal nerve fibers was significant less in DPN and NDPN group than in control group(x2 =16.014,P =0.000).The negative correlation was found between the course of disease and corneal fibrous density by using single factor linear regression analysis.The number of corneal nerve fibers was lower in smoking group than in non-smoking group(P=0.003).The multiple linear regression analysis showed that duration of diabetes was a risk factor for diabetic neuropathy.Conclusions In some elderly diabetic patients with non-neuropathy,corneal nerve fiber density and number have been significantly decreased before nerve conductive velocity is reduced.Therefore,corneal confocal microscopy can be used to detect and diagnose small diabetic neuropathy in elderly patients with diabetes mellitus.
10.Expression of GABA and glutamate vesicular transporter in depressed mice

Objective To study the expression of vesicular GABA transporter and vesicular glutamate transporter 1 in de‐pression .Methods Mice was divided into control group and defeat group stochastically .By social defeat model and social avoidance , the defeat group was divided into two groups:susceptible group and unsusceptible group .Synaptic proteins were extracted respec‐tively from the 3 groups .We detected the expression abundance of VGAT and VGLUT1 by Western blot .Results Compared with the control group ,in susceptible group ,the residence time in the contact area was significantly reduced ,and the residence time in the corner area was significantly increased ,with statistical difference(P<0 .05) .In the prefrontal cortex and hippocampus ,Com‐pared with the control group and the unsusceptible group ,the expression levels of VGLUT1 and VGAT were increased in the sus‐ceptible group(P<0 .05) .there were no statistically significant in VGLUT1 and VGAT leveles between control group and the un‐susceptible group(P>0 .05) .In the striatum ,although the expression levels of VGAT and VGLUT1 were increased in susceptible group ,but in unsusceptible group ,the expression of these proteins also increased significantly .Conclusion The prefrontal cortex and hippo‐campus excitability and inhibitory vesicle transport were changed in depression ,which may relate to the transcription disorder .
