1.Buyang Huanwutang Ameliorates Idiopathic Pulmonary Fibrosis by Inhibiting Bleomycin-induced Senescence of A549 Cells

ObjectiveTo investigate the effect of the Buyang Huanwutang (BHT)-containing serum on the bleomycin-induced senescence of A549 cells and explore the potential mechanism by which BHT ameliorates pulmonary fibrosis. MethodsA549 cells were cultured in vitro and modeled for senescence through the application of bleomycin. SD rats were administrated with BHT by gavage for the preparation of BHT-containing serum, and the effect of BHT-containing serum on the viability of the cell model was studied through a cell experiment designed with the following groups: blank group, model group, 2.5%, 5%, and 10% BHT-containing serumgroups. The effect of BHT-containing serum on the BLM-induced senescence of A549 cells was studied by the experiment designed with blank group, model group, positive group (PC,pirfenidone,600 mg·L-1), 2.5%, 5%, 10% BHT-containing serum groups. SA-β-Gal staining was used to reveal the area of senescence-positive cells, and flow cytometry was employed to analyze the cell cycle distribution. Real-time PCR and the immunofluorescence assay were adopted to determine the mRNA and protein levels of cell senescence markers p16 and p21. Western blot was employed to quantify the protein levels of senescence-associated secretory phenotype (SASP) molecules interleukin (IL)-1β, IL-6, matrix metalloproteinase (MMP)-3, chemokine ligand (CCL)2, tumor necrosis factor (TNF)-α, and transforming growth factor-β (TGF-β)/Smad pathway molecules TGF-β1, Smad2, phosphorylated (p)-Smad2, Smad3, and p-Smad3. ResultsCompared with the blank group, the model group showed a decrease in cell survival rate (P<0.01), increases in the SA-β-Gal-positive area, the proportion of cells in G0/G1 phase, the mRNA and protein levels of P16 and P21, and the protein levels of IL-1β, IL-6, TNF-α, MMP-3, CCL2, TGF-β1, p-Smad2, and p-Smad3 (P<0.05,P<0.01). Compared with the model group, the BHT-containing serum at each dose increased the cell survival rate (P<0.01) and decreased the SA-β-Gal-positive area, the proportion of cells in G0/G1 phase, the mRNA and protein levels of p16 and p21, and the protein levels of IL-1β, IL-6, TNF-α, MMP-3, and CCL2 (P<0.05, P<0.01). Moreover, the inhibitory effect of BHT-containing serum on cell senescence showed a dose-dependent manner, with the 10% BHT-containing serum showing the most obvious effects and inhibiting the expression of TGF-β1, p-Smad2, and p-Smad3 (P<0.01). The effect of 10% BHT-containing serum was comparable to that of pirfenidone, and the serum even outperformed pirfenidone in inhibiting the expression of TNF-α and Smad2 (P<0.05). ConclusionThe BHT-containing serum can inhibit BLM-induced senescence of A549 cells in a dose-dependent manner by regulating the TGF-β/Smad signaling pathway.
2.Nutritional insufficiency and appropriate catch-up growth in extremely preterm infants within 24 months of corrected age: a retrospective cohort study

Objective:To assess the status of undernutrition and appropriate catch-up growth in extremely preterm infants within 24 months of corrected age (CA).Methods:A retrospective cohort study was conducted. A total of 422 extremely preterm infants born at Shenzhen Maternity and Child Healthcare Hospital, Women and Children's Medical Center, Southern Medical University from January 2017 to December 2022 and followed up until 24 months of CA were enrolled. The extremely preterm infants were grouped by gestational age at birth (<25, 25-26, 27 weeks), birth weight (<500, 500-749, 750-999,≥1 000 g), weight for gestational age (large for gestational age (LGA), appropriate for gestational age (AGA), small for gestational age (SGA)) and sex. Weight data within 24 months of CA were collected every 3 months. Nutritional insufficiency, growth rate, and achievement of adequate catch-up growth were analyzed during the period from 0 to 24 months of CA. Z-score method was used to analyze data. Fenton 2013 preterm growth charts (Fenton 2013) were used before 40 weeks of corrected gestational age, and World Health Organization child growth standards (2009) fitted Z-scores were applied from 40 weeks of CA. Changes in weight Z-scores of extremely preterm infants from 0 to 24 months of CA were observed and compared, the occurrence of moderate to severe malnutrition and growth retardation was determined, nutritional insufficiency was assessed, and growth rate as well as the achievement of appropriate catch-up growth were analyzed. The Lambda-mu-sigma method combined with the Z-score fitting model was used to fit and analyze the distribution characteristics of weight percentiles in extremely preterm infants. The Chi-square test was used to compare differences among groups.Results:A total of 422 extremely preterm infants were included, with a gestational age at birth of 26.3(25.4, 27.2) weeks and a birth weight of (880±177) g. Among them, 238 were males and 184 were females; 36 cases (8.5%) were LGA, and 16 cases (3.8%) were SGA. During follow-up within 24 month of CA, 89 cases (21.1%) developed moderate to severe malnutrition. When compared separately among different birth weight and gestational age at birth groups, there had both statistically differences in the incidence of moderate to severe malnutrition ( χ2=42.94 and 9.17, both P<0.05). The incidence was the highest in the birth weight of CA<500 g group and the <25 weeks gestational age at birth group, while it was the lowest in the birth weight of CA≥1 000 g group and the 27 weeks gestational age at birth group in their respective groups. Growth retardation occurred in 5.2% (22/422). However, there had statistically differences in the incidence of growth retardation among different birth weight and gestational age at birth groups, in each grouped time interval ( χ2=21.61 and 4.30, both P<0.05). The proportions of rapid growth were relatively high in the 0-3 months and 3-6 months of CA groups, which were 96 cases (27.4%) and 98 cases (26.6%), respectively. Overall, appropriate catch-up growth was achieved in 341 cases (80.8%) from 0 to 24 months of CA. There had statistically differences in the completion rate of appropriate catch-up growth among different birth weight and gestational age at birth groups ( χ2=23.65 and 7.08, both P<0.05). The completion rate was the highest in the birth weight of CA<500 g group and the <25 weeks of gestational age at birth group, while it was the lowest in the birth weight of CA≥1 000 g group and the 27 weeks of gestational age at birth group. Conclusions:The lower the birth weight and gestational age of extremely preterm infants, the higher the incidence of moderate to severe malnutrition and the lower the achievement rate of adequate catch-up growth within 24 months of CA. The period of 0-6 months of CA is the critical window for catch-up in extremely preterm infants.
3.Indoor environment management and CO 2 volume concentration of primary and secondary school classrooms in winter across three provinces and municipalities of China

Objective:
To evaluate the classroom environmental management and CO 2 volume concentration in primary and secondary schools from Liaoning, Tianjin, and Shanghai, thereby providing a scientific basis for developing targeted strategies to improve classroom air quality.
Methods:
From December 16 to 26, 2024, by using stratified random cluster sampling method, the questionnaire survey was conducted in 72 primary and secondary schools (24 each of primary, junior high, and regular high schools) across Liaoning, Tianjin and Shanghai. Information on heating, ventilation and other classroom environmental management was collected. Additionally, 108 classrooms were selected for on site microclimate measurements, including temperature, humidity, wind speed and CO 2 volume concentration. Univariate analysis and multiple linear regression models were employed to explore related factors of classroom CO 2 volume concentration.
Results:
Among the three provinces/municipalities, 20.8% of schools regularly monitored the microclimate. The overall compliance rate for classroom CO 2 volume concentration was 17.6%. Multiple linear regression analysis showed that CO 2 volume concentration in regular and junior high school classrooms were higher than in primary school classrooms ( β=0.067, 0.046, 95%CI =0.036-0.099, 0.013-0.080); classrooms ventilated regularly in the morning and afternoon had higher CO 2 volume concentration than those ventilated during every break between classes ( β=0.043, 95%CI = 0.007- 0.080); both temperature ( β=0.010, 95%CI =0.004-0.016) and humidity ( β=0.003, 95%CI =0.002-0.004) were positively correlated with CO 2 volume concentration (all P <0.05).
Conclusions
Excessive CO 2 volume concentration in primary and secondary school classrooms is a prominent issue, and ventilation frequency is a key intervenable factor for controlling CO 2 levels. It is recommended to promote ventilation during every break between classes as a core management measure and to emphasize air quality supervision in regular high school classrooms.

5.Comparative analysis of the characteristics of imported malaria cases in Nanning City in 2024 and the same period of the previous year

Objective To investigate the epidemiological characteristics of malaria and provide a basis for developing improved prevention and control measures. Methods Data were obtained from the Chinese Disease Prevention and Control Information System. Malaria surveillance data for Nanning City from January 1,2023, to December 31,2024, were exported from the Infectious Disease Reporting Information Management Subsystem. The characteristics of the two groups of malaria cases were compared. Results A total of 103 imported malaria cases were reported in Nanning City in 2024, representing a 38.32% decrease compared with the same period of the previous year. No statistically significant difference were observed between cases reported in 2023 and 2024 in terms of average age, gender ratio, proportion of parasite species, and monthly reporting distribution;however, statistically significant differences were found in the proportion of reporting areas and current residence areas(χ2= 13.572 and 10.355, respectively; P = 0.001 and 0.035, respectively). The proportion of cases reported in Shanglin County and the proportion of cases residing in Shanglin County were both lower than those during the same period of the previous year. Conclusions The high aggregation of imported malaria cases in Nanning City has decreased. Medical institutions in areas other than Shanglin County should strengthen their vigilance against malaria.
6.Sodium lactate modulates TLR4/NF-κB signaling pathway for treatment of right heart failure

Aim To investigate the effects of sodium lactate(NALA)on right heart failure induced by monocrotaline(MCT)-induced pulmonary arterial hy-pertension in rats and to reveal the underlying mecha-nisms.Methods Forty male Sprague-Dawley(SD)rats were randomly allocated into four groups,with ten rats in each group,namely,MCT group,NALA group,and NALA+MCT group;the MCT and NALA+MCT groups were administered a single intraperito-neal injection of MCT at 60 mg·kg-1 to induce pul-monary hypertension,and one week later,the NALA and NALA+MCT groups received intraperitoneal in-jections of NALA at 0.1 g·kg-1(once a day,for 5 weeks),while the CON and MCT groups received e-qual volumes of physiological saline(once a day,for 5 weeks);right heart function was assessed using echo-cardiography,right ventricular and pulmonary artery remodeling were evaluated via histopathological sec-tions,and the expression levels of ANP,BNP,and in-flammatory factors were measured by ELISA,along with assessments of oxidative stress levels,Western blot detection of the expression levels of proteins in the TLR4/NF-κB signaling pathway.Results Compared to the CON group,the MCT group exhibited increased RVSP and RVHI,decreased right heart function,in-creased collagen fiber deposition,and elevated oxida-tive stress and inflammatory factor expression,and the expression levels of proteins in the TLR4/NF-κB signa-ling pathway increased(P<0.05);compared to the MCT group,the NALA+MCT group showed reduced RVSP and RVHI,improved right heart function,atten-uated pulmonary vascular remodeling,decreased ex-pression of ANP,BNP,inflammatory factors,and H2O2,along with increased antioxidant enzyme expres-sion,and the expression levels of proteins in the TLR4/NF-κB signaling pathway decreased(P<0.05).Conclusion NALA can inhibit right ventric-ular remodeling in rats with pulmonary hypertension,and the underlying mechanism may involve the allevia-tion of inflammatory responses and oxidative stress through the inhibition of the TLR4/NF-κB signaling pathway.
7.Effect of GLP-1R gene polymorphism on the efficacy of Lirglutide in type 2 diabetes mellitus patients with metabolic associated fatty liver disease

Objective To investigate the effect of the rs3765467 polymorphism of glucagon-like peptide-1 receptor(GLP-1R)gene on the efficacy of Liraglutide(Lir)in patients with type 2 diabetes mellitus(T2DM)and metabolic associated fatty liver disease(MAFLD).Methods A total of 281 patients with T2DM from May 2022 to May 2023 were selected,including 125 patients with simple T2DM(T2DM group)and 156 patients with T2DM combined with MAFLD(T2DM+MAFLD group).120 healthy individuals during the same period were selected as the control(NC)group.The related indexes of glucose and lipid metabolism were detected.The polymorphism of GLP-1R gene rs3765467 was detected.Results BMI,FPG,HbA1c,HOMA-IR and TG in each group increased in turn(P<0.05),while the distribution frequency of genotype GG and allele G decreased in turn(P<0.05).TC and LDL-C in T2DM and T2DM+MAFLD groups were higher than those in NC group(P<0.05).TC and TG levels in genotype GA/AA patients were significantly higher than those in genotype GG patients(P<0.05).Compared with before treatment,the levels of BMI,FPG,HbA1c,HOMA-IR,TC,TG and LDL-C in T2DM patients with MAFLD were significantly decreased after Lir treatment(P<0.05).There was no significant difference in BMI and related indexes of glucose and lipid metabolism in GG and GA/AA patients before and after Lir treatment(P>0.05).Conclusions The distribution frequency of GG and G allele at rs3765467 of GLP-1R gene is reduced in T2DM patients with MAFLD.The carrying of allele A was associated with increased TC and TG levels,but did not affect the efficacy of Lir in reducing weight and improving glycolipid metabolism.
8.Diagnostic value of plasma heparin-binding protein in the bloodstream infection of tumor patients

Objective To investigate the diagnostic value of plasma heparin-binding protein(HBP)in the bloodstream infection of tumor patients.Methods The tumor patients treated at Ruijin Hospital,Shanghai Jiao Tong University School of Medicine from Janu-ary 2023 to December 2023 were selected and further divided into the following three groups:124 patients with positive blood cultures(bloodstream infection group),95 with negative blood cultures but positive cultures from other body fluids(culture-positive group),and 100 with negative bacterial cultures(culture-negative group).The levels of HBP,procalcitonin(PCT),C-reactive protein(CRP),interleukin 6(IL-6),white blood cell(WBC),and neutrophil(NE)in the three groups were detected.The level differences of each indicator among three groups were compared.The differences in the expression levels of HBP in the bloodstream infection group with different pathogens such as Gram-negative bacteria,Gram-positive bacteria,and fungi were also compared.The Logistic regression analysis was used to evaluate the predictive efficacy of each indicator for the risk of bloodstream infection.The receiver operating char-acteristics(ROC)curve was used to assess the diagnostic value of each indicator alone and in combination for the bloodstream infection of tumor patients.Results The expression level of HBP in the bloodstream infection group(32.14[14.01,110.40]ng/mL)was sig-nificantly higher than that in the culture-positive group(23.00[5.90,54.87]ng/mL,t=5.39,P<0.05),and both of them were sig-nificantly higher than that in the culture-negative group(6.80[5.90,16.05]ng/mL,Z=47.248,P<0.05).HBP had a certain clini-cal predictive value for the occurrence of bloodstream infection in tumor patients(OR=1.017,95%CI:1.008~1.027,P<0.001).In addition,the levels of HBP in the Gram-negative bacterial infection group(78.04[21.60,157.20]ng/mL)were significantly higher than that in the Gram-positive bacterial infection group(51.00[18.45,106.50]ng/mL,t=2.01,P<0.05),and both of them were significantly higher than that in the fungal infection group(5.90[5.90,14.17]ng/mL,Z=3.40,P<0.05).The area under the ROC curve(AUCROC),sensitivity,and specificity of HBP for the diagnosis of bloodstream infection in tumor patients were 0.717,67.37%,and 67.26%,respectively,which were superior to those of PCT(0.700,77.17%,and 52.14%),CRP(0.667,63.16%,and 62.39%),and IL-6(0.629,60.47%,and 60.47%).The AUCROC,sensitivity,and specificity of the combination of HBP,CRP,PCT,and IL-6 were 0.739,70.24%,and 68.42%,respectively,which were better than those of each indicator alone.Conclusion The detection of HBP has high clinical application value in the diagnosis of bloodstream infections of tumor patients and may serve as a potential biomarker for the bloodstream infection of tumor patients.

10.A cohort study on the association of healthy lifestyle with co-morbidity of overweight/obesity and myopia in adolescents

Objective:To explore the association between healthy lifestyle and co-morbidity of overweight/obesity and myopia in adolescents.Methods:Based on the Shanghai Municipal Dynamic Cohort of Student Common Diseases, we used the group-based trajectory model (GBTM) to establish a model for the development of adolescents' healthy lifestyle trajectories aged 11-14 years and the Cox proportional hazard regression model to assess the effects of different healthy lifestyles on the co-morbidity of overweight/obesity and myopia in adolescents.Results:A total of 2 331 adolescents were included in the analysis. The average follow-up time was 1.87 years, with age of (11.44±0.45) years at the initial follow-up, among 1 106 boys and 1 225 girls. During the 2-year follow-up period, the incidence rates of overweight/obesity, myopia, and co-occurrence of overweight/obesity and myopia were 25.9%, 77.0%, and 13.7%, respectively. GBTM identified 3 healthy lifestyles: the unhealthy group (7.3%), the relatively healthy group (50.2%), and the healthy group (42.5%). The risk of overweight/obesity was lower in the relatively healthy group than in the unhealthy group ( HR=0.694, 95% CI: 0.508-0.947). There was no statistically significant association between the GBTM lifestyle subgroups and myopia ( P>0.05). The risk of co-occurrence of overweight/obesity and myopia was lower in both relatively healthy group and healthy group than in the unhealthy group (relatively healthy group: HR=0.515, 95% CI: 0.348-0.763; healthy group: HR=0.571, 95% CI: 0.384-0.849). Sensitivity analyses showed this result to be stable for myopia, overweight/obesity and myopia co-morbidities. Conclusion:Persistent unhealthy lifestyle increases the risk of overweight/obesity and myopia co-occurrence in adolescents.
