Ischemic stroke (IS) represents a major global health challenge with complex pathological mechanisms. Although modern therapies such as intravenous thrombolysis and endovascular thrombectomy have advanced, their application remains constrained by narrow therapeutic time windows, hemorrhagic risks, and uneven distribution of medical resources. Traditional Chinese medicine (TCM) demonstrates unique value in the prevention and treatment of IS, owing to its multi-component, multi-target, and holistic regulatory characteristics. This review summarized the molecular mechanisms by which active ingredients and compound formulations of TCM exert therapeutic effects against IS through the regulation of inflammatory responses, oxidative stress, excitatory toxicity, apoptosis, and autophagy. Studies have indicated that components such as curcumin, baicalin, and astragaloside Ⅳ inhibit microglial activation and the nucleotide-binding oligomerization domain (NOD)-like receptor protein 3 (NLRP3) inflammasome to attenuate neuroinflammation, activate the nuclear factor erythroid 2-related factor 2(Nrf2)/heme oxygenase-1 (HO-1) pathway to alleviate oxidative stress, modulate glutamate receptor function to counteract excitatory toxicity, and regulate the B-cell lymphoma 2(Bcl-2)/Bcl-2-associated X protein (Bax), cysteine aspartate-specific protease (Caspase), and phosphatidylinositol 3 kinases (PI3K)/protein kinase B (Akt) signaling pathways to suppress neuronal apoptosis. Recent research has further revealed that TCM can modulate ferroptosis by targeting key proteins glutathione peroxidase 4 (GPX4) and acyl-coenzyme A synthetase long-chain family member 4 (ACSL4) to maintain iron homeostasis, intervene in the "microbiota-gut-brain axis" to ameliorate dysbiosis and reduce neuroinflammation, utilize exosomes for brain-targeted drug delivery, and influence neural repair processes through epigenetic regulation. Furthermore, the review discussed the integrated mechanisms of compound formulations, such as Buyang Huanwu Decoction, in improving cerebral microcirculation and promoting neurovascular remodeling via multi-component synergy. It also analyzed the strategy and advantages of integrating TCM with Western medicine for IS treatment, providing a novel theoretical foundation and research directions for future investigations and clinical translation of TCM in IS management.

BackgroundPersistent cognitive impairment is prevalent among patients with stable schizophrenia. While serum total bile acid (TBA) level in acute-phase patients are known to be associated with cognitive dysfunction, the relationship between serum TBA and multi-dimensional cognitive functions in stable phase patients remains unclear. ObjectiveTo investigate the correlation between serum TBA level and cognitive function in patients with stable schizophrenia, and to evaluate its predictive value for cognitive impairment, thereby providing a serological biomarker for the timely identification and objective assessment of cognitive dysfunction. MethodsA cross-sectional study was conducted on 137 inpatients with stable schizophrenia at The Fourth People's Hospital of Yancheng from March to December 2024. All participants met the diagnostic criteria of the Diagnostic and Statistical Manual of Mental Disorders, fifth edition (DSM-5). Cognitive function was evaluated using the Chinese Brief Cognitive Test (C-BCT), patients were categorized into four groups: normal cognition (n=28), mild impairment (n=28), moderate impairment (n=47), and severe impairment (n=34). Fasting venous blood samples were collected, and serum TBA level was quantified using an enzymatic cycle assay. Spearman correlation analysis was ultilized to determine the relationship between serum TBA level, overall cognitive function, and specific cognitive domains. Binary Logistic regression model was used (adjusting for covariates such as age, gender, and disease duration) to analyze the impact of serum TBA level on overall and individual cognitive functions. The predictive value of serum TBA level for overall cognitive impairment was evaluated using receiver operating characteristic (ROC) curve. ResultsSerum TBA levels differed significantly among the four groups (H=18.677, P<0.01). Specifically, serum TBA levels in both the moderate and severe cognitive impairment groups were significantly higher than those in the normal cognitive group (adjusted P<0.01). Serum TBA level was positively correlated with the severity grading of overall cognitive impairment (rs=0.354, P<0.05), and negatively correlated with T-scores on the trail making test (rs=-0.328, P<0.05), continuous performance test (rs=-0.247, P<0.05), digit span (rs=-0.265, P<0.05), and symbol coding (rs=-0.221, P<0.05). Binary Logistic regression analysis identified serum TBA level as an independent risk factor for overall cognitive impairment (OR=1.322, 95% CI: 1.021 - 1.713, P=0.034), with a particularly robust predictive ability for impaired information processing speed (OR=1.325, 95% CI: 1.057 - 1.661, P=0.015). The area under ROC curve (AUC) for serum TBA level in predicting overall cognitive impairment was 0.738, with a sensitivity of 60.61% and a specificity of 78.64%. ConclusionIn patients with stable schizophrenia, elevated serum TBA levels are associated with worse overall cognitive function, as well as deficits in information processing speed, attention, working memory, and executive function. Serum TBA serves as an independent risk factor and exhibits moderate predictive value for overall cognitive impairmen, particularly in the domain of information processing speed. Cholestatic liver disease (CLD) is closely associated with bile acid homeostasis imbalance, oxidative stress, inflammatory responses, mitochondrial damage, and hepatic fibrosis. Nuclear factor-erythroid 2-related factor 2 (Nrf2) can regulate molecules related to antioxidation, detoxification, and bile acid transport, and together with signaling pathways such as farnesoid X receptor (FXR), silent information regulator 1/AMP-activated protein kinase, and nuclear factor-κB, participates in the development and progression of CLD. This review summarizes the role of Nrf2 in the development and progression of CLD, as well as the research progress on traditional Chinese medicine (TCM) active constituents and formulas/Chinese patent medicine in regulating Nrf2 for the intervention of CLD. The results indicate that TCM active constituents such as terpenoids, saponins, flavonoids, and polysaccharides, as well as formulas/Chinese patent medicine with the effects of heat-clearing and dampness-draining, liver-soothing and choleretic-promoting, and body-strengthening and liver-protecting, can improve bile acid metabolism and transport and alleviate oxidative stress, inflammatory responses, and hepatic fibrosis by regulating Nrf2-related signaling pathways. However, existing studies still primarily judge based on Nrf2 expression, nuclear translocation, and downstream molecular expression changes, and the understanding of the regulatory relationship between Nrf2 and signaling pathways such as FXR, as well as the synergistic mechanisms of multiple constituents in formulas, remains insufficient. Future studies can strengthen gene intervention and functional validation, utilize liver or bile duct organoids, co-culture systems, and other approaches, and combine bile acid profiling and multi-omics analysis to further elucidate the mechanism by which TCM regulates Nrf2 to ameliorate CLD.
