Psychological factors have become significant contributors to the onset and progression of insomnia. This article explored the treatment of insomnia from the perspective of “liver governs wei qi （defensive qi）”. The concept of “liver governs wei qi （defensive qi）” is summarized in three aspects， firstly， the liver assists the spleen and stomach in transformation and transportation， governing the generation of wei qi； secondly， the liver aids lung qi diffusion and dispersion， governing the distribution of wei qi； thirdly， the liver regulates circadian rhythms， governing the circulation of wei qi. It is proposed that the clinical treatment of insomnia should focus on the following methods： for regulating the liver to harmonize the five viscera， and facilitate the circulation of wei qi， medicinals entering the liver channel include Chaihu （Bupleuri radix）， Baishao （Paeoniae Radix Alba）， Zhizi （Gardeniae Fructus）， and Suanzaoren （Ziziphi Spinosae Semen） could be commonly used； for nourishing the liver， the treatment should align with the day-night rhythm， and herbs such as Baihe （Lilium）， Hehuan （Albizia julibrissin）， and Yejiaoteng （Polygoni multiflori caulis） are commonly used； for soothing the liver and address both mental and physical health to calm wei qi， treatment should advocate verbal counseling， psychological regulation， and health education. Ultimately， this treatment approach can free liver qi to flow， soothe qi movement， restore the motion of wei qi， regulate during day and night， balance yin and yang， and resolve insomnia effectively.

Two thousand five hundred and sixty swab samples were collected from December 2022 to December 2023 in China.PCR was used to detect FBoV and amplify its VP2 and NS1 gene cod-ing equences,and bioinformatics was used to analyze the genetic diversity of FBoV.The results showed that the total positive rate of FBoV was 4.6％(119/2 560).Genetic variation analysis showed that FBoV existed in a variety of genotypes,and FBOV-1 was the main epidemic type in China.The 15 FBoV-1 strains,four FBoV-2 strains and one FBoV-3 strains identified in this study were genetically close to the strains identified in China,the United States,Thailand,Australia and Portugal.Sequence analysis showed that the identities of amino acid sequence of NS1 and VP2 genes between the sequenced strains and the reference strains were 59.13％-99.25％and 96.41％-100.00％,respectively.The amino acid identities of NS1 and VP2 among the newly sequenced FBoV strains were 60.00％-100.00％and 96.41％-100.00％,respectively,which indicated that the FBov strains circulating in China had great genetic diversity.This study enriched the data for elucidating the epidemic status of FBoV in China,and provided the basis for the subsequent diag-nosis,prevention and control of FBoV.

Objective To explore the distribution of common TCM syndromes and symptoms of insomnia;To prepare for the construction of the theoretical framework and item pool of syndrome diagnosis and efficacy evaluation scale.Methods TCM guideline standards of insomnia,textbooks and journals over the years were retrieved,the information of TCM syndromes,syndrome elements and symptoms was extracted,the guideline textbook and journal database were established,and descriptive statistics,association rules,systematic clustering,factor analysis,potential categories and implicit structure analysis were carried out.Results Totally 116 guide standards and textbooks over the years were included,and 454 articles of journals were included.The high-frequency symptoms accounted for≥3%of the guide textbooks and journal databases were 87 and 79 categories,respectively,and the cumulative proportion was 87.48%and 87.75%,respectively.According to the analysis results,five common TCM syndromes and their characteristic symptom classification of insomnia were finally deduced.According to the frequency/person time distribution,they were heart and spleen deficiency syndrome,yin deficiency and fire hyperactivity syndrome,liver fire disturbing heart syndrome,phlegm heat disturbing heart syndrome,heart and gallbladder qi deficiency syndrome.Conclusion There are five common TCM syndromes of insomnia,and the characteristic symptoms of each TCM syndrome provide a reference source for the theoretical framework of syndrome diagnosis and efficacy evaluation scale and the establishment of item pool.

Objective:To explore the changes of serum interferon-inducible protein 10(IP-10) and hypersensitive C-reactive protein(hs-CRP) in children with recurrent respiratory tract infection(RRTI) and their correlation with prognosis.Methods:A total of 62 children with RRTI(RRTI group) and 58 healthy children (healthy control group) who underwent physical examination during the same period from October 2022 to December 2023 in Baoji Maternal and Child Health Hospital were retrospectively selected as the study objects. The levels of serum IP-10 and hs-CRP were compared between the two groups. Univariate analysis and multivariate Logistic regression analysis were used to screen the risk factors for poor prognosis in children with RRTI, and receiver operating characteristic (ROC) curve was used to analyze the predictive value of serum hs-CRP and IP-10 levels for poor prognosis in children with.Results:The serum levels of hs-CRP and IP-10 in the RRTI group were higher than those in the healthy control group: (110.23 ± 21.56) ng/L vs. (29.56 ± 4.53) ng/L, (3.63 ± 0.64) mg/L vs. (2.85 ± 0.52) mg/L, there were statistical differences ( P<0.05). The results of univariate analysis showed that exposure to second-hand smoke, household pets, history of asthma and elevated levels of procalcitonin, serum amyloid, IP-10 and hs-CRP were risk factors for poor prognosis in children with RRTI ( P<0.05). Logistic regression analysis showed that exposure to second-hand smoke and increased levels of serum IP-10 and hs-CRP were independent risk factors for poor prognosis in RRTI children ( P<0.05). ROC curve analysis showed that the area under the curve of serum IP-10 and hs-CRP combined to predict poor prognosis of RRTI children was 0.866, which was higher than any single index. Conclusions:The serum levels of IP-10 and hs-CRP are obviously elevated in children with RRTI and those with poor prognosis. The combined detection of IP-10 and hs-CRP has predictive value for poor prognosis in children with RRTI.

Objective To explore the relationship between the expression of B10 cells and transforming growth factor-β(TGF-β)in peripheral blood of children with chronic gastroenteritis accompanied by Helico-bacter pylori(Hp)infection and their condition and prognosis.Methods A total of 104 children with chronic gastroenteritis accompanied by Hp infection admitted to Xianyang Hospital,Yan'an University from Decem-ber 2021 to December 2023 were retrospectively selected as the research subjects.According to the visual ana-logue scale(VAS)score,endoscopic examination results and histopathological examination results,the chil-dren were divided into mild group(n=37),moderate group(n=41)and severe group(n=26).All children were treated with standard triple therapy for 14 days.Follow-up consultation was conducted 4 weeks after the end of treatment,and the patient's prognosis was determined based on the results of gastroscopy and urease test.The children were divided into good prognosis group(n=82)and poor prognosis group(n=22).The pe-ripheral blood of the children was collected before treatment and 4 weeks after the end of treatment,and the proportion of B10 cells and the level of TGF-β were measured respectively.The general information,the pro-portion of B10 cells in peripheral blood,and the expression of TGF-β were compared between the mild group and the moderate group,among the three subgroups in the moderate group,and between the poor prognosis group and the good prognosis group.The independent sample t test was used to analyze the differences in quantitative parameter values of each group,Spearman correlation coefficient was used to analyze the correla-tion between the disease condition and the peripheral blood B10 cells and TGF-β level,and the receiver operat-ing characteristic(ROC)curve was used to analyze the predictive value of peripheral blood B10 cell proportion and TGF-β level for poor prognosis in pediatric patients.Results Before treatment,the proportion of B10 cells and TGF-β level in the peripheral blood of children decreased sequentially in severe group,moderate group and mild group,and were positively correlated with the severity of the disease(P＜0.05).After treatment,the proportion of B10 cells and TGF-β level in the peripheral blood of the children were significantly reduced com-pared to before treatment(P＜0.05).4 weeks after the end of treatment,the proportion of B10 cells and TGF-β level in the peripheral blood of children in the poor prognosis group were significantly higher than those in the good prognosis group(P＜0.05).After treatment,the proportion of B10 cells and the level of TGF-β in children's peripheral blood had certain predictive value for the prognosis of children.The area under the curve of ROC curve analysis was 0.759 and 0.743,respectively,which was lower than 0.886 of combined prediction(Z=-2.445,2.391,P=0.014,0.017),and the sensitivity and specificity of combined prediction were 77.34％and 91.45％respectively.Conclusion The proportion of B10 cells and TGF-β level before treatment are positively correlated with the severity of the disease,and the combined detection of the two after treatment has good predictive value for the prognosis of children.

ObjectiveTo investigate the pharmacodynamic characteristics and explore the molecular mechanism of Honghua oral liquid （HOL） in relieving neuropathic pain （NP）. MethodHealthy male SD rats were randomly assigned into sham group， model group， low-， medium-， high-dose （0.5， 1.0， 2.0 mL·kg^-1·d^-1， respectively） HOL groups， and a positive drug （pregabalin， 25 mg·kg^-1·d^-1） group， with 6 rats in each group. Spinal nerve ligation （SNL） of L5 was conducted in other groups except the sham group. Drug administration was performed 3 days after the SNL surgery for 2 consecutive weeks， and samples were collected after the end of the administration. During the treatment period， the mechanical pain threshold and cold pain threshold were determined to measure the pain-relieving effect of HOL. Transcriptome sequencing was performed on hippocampal tissue samples from the sham， model， and high-dose HOL groups， and differentially expressed genes between the sham group and the model group as well as the model group and HOL high-dose group were obtained. After pathway enrichment analysis， we selected the targets which were closely related to neuroinflammation for validation， and predicted the specific binding sites of the major active components in HOL with the targets through molecular docking. In addition， the serum levels of tumor necrosis factor-α （TNF-α） and interleukin-10 （IL-10） were determined by enzyme-linked immunosorbent assay （ELISA） to evaluate the effect of HOL on neuroinflammation in NP rats. ResultCompared with the sham group， SNL decreased the mechanical pain threshold and cold pain threshold （P<0.05）. Compared with the model group， HOL recovered the mechanical pain threshold and cold pain threshold （P<0.05）. The transcriptome data showed that 376 differentially expressed genes （DEGs） were identified between the model group and the sham group， including 124 upregulated genes and 252 downregulated genes， and 194 DEGs between the model group and the high-dose HOL group， including 33 upregulated genes and 161 downregulated genes. Among them， insulin-like growth factor 1（IGF1）， matrix metallopeptidase-2 （MMP-2）， matrix metallopeptidase-14 （MMP-14）， erb-B2 receptor tyrosine kinase 2 （ERBB2）， and integrin subunit alpha 5 （ITGA5） associated with NP were selected for further validation. The Real-time fluorescence quantitative polymerase chain reaction（Real-time PCR） results showed that compared with the sham group， the modeling up-gurelated the mRNA levels of the above five molecules in the hippocampus （P<0.01）. Compared with model group， HOL down-regulated the mRNA levels of these molecules （P<0.01）. The molecular docking results showed that the main active components of safflower， hydroxysafflor yellow A， kaempferol， and quercetin， formed stable hydrogen bonds with the amino acid residues of IGF1， MMP-2， MMP-14， ERBB2， and ITGA5. The enzyme-linked immunosorbent assay（ELISA） results showed that compared with those in the sham group， the serum levels of TNF-α and IL-10 were out of balance in the model rats （P<0.01）. Compared with the model group， HOL lowered the level of the pro-inflammatory cytokine TNF-α （P<0.01） and elevated that of the anti-inflammatory cytokine IL-10 （P<0.05）. ConclusionHOL exerts analgesic effect on SNL rats by inhibiting neuroinflammation.

Chorea is a rare adverse effect of phenytoin and is common in children, with widely variable clinical presentations. This article reported a case of an elderly woman presented chorea without nystagmus, ataxia, dysarthria and other typical vestibular-cerebellar symptoms, who took compound theophylline and ephedrine contained phenytoin to treat asthma for 1 year. The serum phenytoin concentrations were at toxic levels and chorea disappeared within 3 days after discontinuation of the drug. The clinical features of previously reported cases of phenytoin-induced chorea were also summarized.
