1.Mechanism and clinical research progress of calcineurin inhibitor-induced hyperglycemia

Calcineurin inhibitor(CNI) is potent immunosuppressive agents and serve as cornerstone therapies in the treatment of organ transplantation and autoimmune diseases, with cyclosporine A and tacrolimus being the representative drugs. Long-term use of CNI can lead to drug-induced hyperglycemia, severely affecting patients’ prognosis. The pathogenesis involves multilevel pathological alterations: at the pancreatic β-cell level, CNI directly damage β-cell by inducing calcium overload, oxidative stress, and mitochondrial dysfunction, suppressing the expression of key insulin synthesis factors and promoting apoptosis; in peripheral tissues, CNI interfere with insulin receptor substrate phosphorylation and inhibit the phosphatidylinositol 3 kinase/protein kinase B signaling pathway, resulting in decreased glucose uptake and insulin resistance; additionally, CNI can also induce β-cell injury by suppressing the secretion and receptor signal transduction of glucagon-like peptide-1, as well as by activating the nuclear factor kappa B pathway to promote inflammatory responses. Clinical studies demonstrate that the incidence of CNI-associated hyperglycemia is closely related to drug type, dosage, and individual patient factors. For high-risk patients, dose adjustment of CNI, switching to agents with lower metabolic toxicity when necessary, and selection of appropriate glucose-lowering regimens based on glycemic levels are recommended. Future research should further elucidate the molecular mechanisms of CNI metabolic toxicity and optimize individualized pharmacotherapy strategies to improve long-term patient outcomes.
2.Application of imaging evaluation in endovascular recanalization of non-acute symptomatic intracranial large artery occlusion

Many studies have shown that endovascular recanalization is feasible for the treatment of non-acute symptomatic intracranial large artery occlusion. However, its incidence of perioperative complications is relatively high. Appropriate imaging evaluation can better observe the occluded segments of the vessels before procedure, thereby guiding the choice of clinical treatment.
3.Application of whole exome sequencing technology in fetuses with congenital structural abnormalities.

OBJECTIVE:
To investigate the application value of whole exome sequencing technology in fetuses with congenital structural abnormalities.
METHODS:
The chromosomal abnormalities of 1147 families were analyzed. According to the follow-up results, the data of fetuses with new phenotypes in late pregnancy or after birth were reanalyzed. Subgroups were divided according to the organs involved and whether single malformation or not. The gene regulatory network map was drawn by using string database and Cytoscape software. Fisher exact probability method was used to compare the difference of the diagnostic rate of pathogenic genes among the groups.
RESULTS:
A total of 160 fetal cases received positive molecular diagnosed, involving 178 variant sites of 125 pathogenic genes, including 8 cases (4.9%, 8/163) by data reanalysis, and the overall positive diagnosis rate was 13.9%. Diagnostic rate was highest in the group of skeletal malformation (31.5%, 39/124) and lowest in that with thoracic malformation (0, 0/32). The gene clusters of fetal edema and intrauterine growth restriction were independent, and were not associated with the major structural malformations. The probability of each parent carrying the same recessive gene variant was 0.03 (39/1146) and 0.08 (4/53) with positive family history.
CONCLUSION
For fetuses with congenital structural abnormalities that are negative for conventional genetic tests, 13.9% of phenotypic associated pathogenic/likely pathogenic genetic variants can be detected by whole exome sequencing technology. Its application value for prenatal diagnosis varies in fetus with different organs involved. Reanalysis of sequencing data for cases with new phenotypes in late pregnancy or after birth can further improve the molecular diagnosis rate. Further investigations are needed to explore the related genetic mechanisms.

4.Analysis of families with fetal congenital abnormalities but negative prenatal diagnosis by whole exome sequencing

Objective:To evaluate the value of whole exome sequencing (WES) in prenatal clinical application.Methods:A total of 1 152 cases of congenital abnormal [including structural malformation, nuchal translucency (NT) thickening and intrauterine growth restriction] with traditional prenatal diagnosis [including G-band karyotype analysis and chromosome microarray analysis (CMA)] negative were analyzed. The congenital abnormal fetuses were divided into retrospective group and prospective group according to the time of WES detection, that is whether the pregnancy termination or not. According to the specific location of fetal malformation and their family history, the cohort was divided into subgroups. The clinical prognosis of all fetuses were followed up, and the effect of WES test results on pregnancy decision-making and clinical intervention were analyzed. According to the follow-up results, the data of fetuses with new phenotypes in the third trimester or after birth were re-analyzed.Results:Among 1 152 families who received WES, 5 families were excluded because of nonbiological parents. Among the remaining 1 147 families, 152 fetuses obtained positive diagnosis (13.3%,152/1 147), including 74 fetuses in the retrospective group (16.1%,74/460) and 78 fetuses in the prospective group (11.4%,78/687). In fetuses with negative CMA and G-band karyotype analysis results but new phenotypes in the third trimester or after birth, the positive rate by WES data re-analysis was 4.9% (8/163). A total of 34 (21.3%, 34/160) fetuses were directly affected by the corresponding positive molecular diagnosis. Among 68 cases of live births with diagnostic variation grade 4, 29 cases (42.7%, 29/68) received appropriate medical intervention through rapid review of WES results.Conclusions:WES could increase the detection rate of abnormal fetuses with negative G-banding karyotype analysis and CMA by 13.3%. Prenatal WES could guide pregnancy decision-making and early clinical intervention. It might be an effective strategy to pay attention to the special follow-up of the third trimester and postnatal fetus and to re-analyze the WES data.
5.Intervention study of Orff rhythm-beat-tap training on positive emotions in elderly patients with mild dementia

Objective:To study effects of Orff rhythm-beat-tap training on positive emotions in elderly patients with mild dementia.Methods:Using the convenient sampling method, a total of 72 elderly patients with mild dementia who were treated in the Third People's Hospital of Huzhou from August 2018 to August 2019 were selected as the research objects and they were divided into two groups using the simple random sampling method. After the start of the study, 2 cases dropped out because of failure to persist in training, 1 case dropped out because of transfer during hospitalization and 1 case dropped out because of aggravation of the disease and changing drugs. A total of 68 cases were included in the study, including 33 cases in the intervention group and 35 cases in the control group. The control group received routine nursing, while the intervention group received 12-week Orff rhythm-beat-tap training on this basis. The scores of Positive and Negative Affect Schedule (PANAS) , Apathy Evaluation Scale (AES) and Dementia Mood Assessment Scale (DMAS) were evaluated before and after the intervention.Results:The score of positive emotion after intervention in the intervention group was (31.58±1.87) , higher than (26.66±2.93) in the control group, and the difference was statistically significant ( t=8.297, P<0.01) . After intervention, the score of negative emotion of the intervention group was (21.97±2.30) , the score of apathy was (44.88±5.22) , and the score of dementia mood was (23.18±4.33) , which were lower than (23.80±3.94) , (51.91±7.49) and (26.09±6.20) of the control group, and there were statistically significant differences in scores between the two groups ( t=-2.356, -4.514, -2.225; P<0.05) . Conclusions:Orff rhythm-beat-tap training is suitable for elderly patients with mild dementia, which can effectively enhance positive emotional experience of patients, improve apathy and optimize mood.
6.Corvis measurement of intraocular pressure and its relationship with corneal biomechanical properties

Background Corneal biomechanical properties is important in the safety assessment of corneal refractive surgery.Corvis is a new device for measuring corneal biomechanics properties.Objective This study was to observe the correlation among corneal thickness, Corvis intraocular pressure and corneal biomechanical properties with Corvis.Methods A prospective observational study was performed.One hundred and fifty eyes of 75 patients with corneal thickness from 501 μm to 590 μm were divided into three groups according to the corneal thickness:low corneal thickness group (corneal thickness range from 501 μm to 530 μm), middle corneal thickness group (corneal thickness range from 531 μm to 560 μm) ,and high corneal thickness group (corneal thickness range from 561 μm to 590 μm);and 50 eyes of 25 patients for each group.The difference of intraocular pressure, corneal thickness and deformation amplitude (DA) among the three groups were analyzed by one-way ANOVA and the correlation among the groups were analyzed by liner regression.Results The DA in the low corneal thickness group and middle corneal thickness group were significantly higher than that in the high corneal thickness group (P < 0.05).The intraocular pressure was statistically different among the 3 groups (F =9.98, P<0.05).DA was negatively correlated with intraocular pressure and corneal thickness (r=-0.84,-0.33;both at P<0.01), with the linear regression DA =1.69-0.04×IOP (F=366.19, t=-19.14,P<0.01).Conclusions Corneal thickness cannot simply represent the corneal biomechanical properties in the safety assessment of corneal refractive surgery,IOP should be considered.
7.Evaluation indexes and screening results for general appraisal of hospital core competence

9.Application on Wechat group for health education in discharged patients with PICC catheter

Objective To investigate the clinical application on Wechat for health education in discharged patients with PICC catheter.Methods Totals of 120 discharged patients with PICC catheter were selected from January to December in 2013,they were divided into the Wechat group (68 cases) and control group (52 cases) by random numerical table method.Before discharging from hospital,both groups received health education of PICC catheter and self-nursing instruction.The control group was given the telephone visits regularly and outpatient PICC consultation in hospital.Besides,the Wechat group was aggregated in a group,in which the self-nursing education and maintenance knowledge of PICC catheter were provided,and the questions from patients were answered timely as well.The awareness of self-nursing of PICC catheter and rate of patients' satisfaction for health education were compared between the two groups.Results The awareness self-nursing of patients in the Wechat group was 95%,while the control group was 77%.There was significant difference between the two groups (x2 =11.73,P < 0.01).The rate of patients' satisfaction in the Wechat group was 97%,while the control group was 75%.There was significant difference between the two groups(x2 =8.23,P<0.01).Conclusions Setting up a Wechat group can increase self-nursing education of the discharged patients with PICC catheter as well as the rate of patients' satisfaction.
