1.Association of triglyceride-glucose index and cardiovascular disease in a community-based Chinese cohort

Objective:To investigate the association between the triglyceride-glucose(TyG)index and the incidence and mortality of cardiovascular disease(CVD)in a large population-based cohort.Methods:Participants aged 40-79 years without a history of CVD at baseline were drawn from the CHi-nese Electronic health Records Research in Yinzhou(CHERRY)study between January 1,2010,and May 31,2020.The TyG index was calculated using baseline triglyceride and fasting blood glucose.Cox proportional hazards models were used to assess the association between the TyG index and the composite outcome of CVD(incidence and mortality),adjusting for age,gender,education,region,smoking sta-tus,body mass index,systolic blood pressure,and total cholesterol.Hazard ratios(HR)and 95%confi-dence intervals(CI)were calculated.Nonlinear associations between the TyG index and CVD were fur-ther evaluated using restricted cubic splines,and subgroup analyses by gender and age were conducted to explore potential differences.Results:A total of 226 406 individuals were included,with a mean age of(55.0±9.7)years at baseline,46.8%of whom were men,and a median TyG index of 8.68.Over a median follow-up of 7.99 years,9 815(4.34%)participants experienced CVD incidence or mortality.After adjusting for age,gender,education,region,smoking status,body mass index,systolic blood pressure and total cholesterol,the risk of CVD increased with higher TyG index levels(P<0.001).The risk in the highest TyG quartile(TyG>9.10)was 42%higher than in the lowest quartile(TyG ≤8.32)(HR=1.42,95%CI:1.34-1.51).Individuals under 60 years had a higher HR for CVD compared with those aged 60 years and above(HR:1.71 vs.1.27,P<0.05).Restricted cubic spline analysis revealed a reverse L-shaped association between the TyG index and CVD risk in the overall population(P<0.001 for nonlinear trend),with risk increasing after the TyG index exceeded 8.67.However,the threshold varied by gender,with a lower threshold in women(8.51)than in men(8.67).Conclusion:A significant nonlinear relationship was revealed between the TyG index and CVD risk,with a threshold effect.The risk of CVD increased once the TyG index surpassed a certain threshold,with a lower threshold in women than in men.These findings suggest that cardiovascular risk prediction and interven-tions based on the TyG index should be gender-stratified,and early intervention for individuals under 60 years old might have important public health implications.

3.Uyghur Medicine Yangxin Dawayimixike Honey Paste (养心达瓦依米西克蜜膏) in Treatment of 279 Cases Stable Angina Pectoris Patients with Qi Stagnation and Blood Stasis Syndrome:A Multi-center,Double-blind,Positive-controlled Randomized Clinical Trial

ObjectiveTo observe the efficacy and safety of Uyghur medicine Yangxin Dawayimixike Honey Paste (养心达瓦依米西克蜜膏, YDMHP) in the treatment of stable angina pectoris (SAP) of qi stagnation and blood stasis syndrome. MethodsA randomized , double-blind, positive-controlled,multi-center clinical trial was conducted, in which 370 patients with SAP of qi stagnation and blood stasis syndrome were randomly divided into treatment group(279 cases)and control group(91cases)at a ratio of 3∶1. The treatment group was orally administered with YDMHP, 3 g each time, and placebo of Xuefu Zhuyu Capsule (血府逐瘀胶囊), 2.4 g each time, while the control group was treated with Xuefu Zhuyu Capsule, 2.4 g each time, and placebo of YDMHP, 3 g each time, both twice a day for a course of 12 weeks. The primary outcome was the effect of angina pectoris symptom. The secondary outcomes include single angina symptom scores such as number of attacks, duration of attacks, pain intensity and usae of nitroglycerin scores, the total angina symptom score before and after the treatment, the usage of nitroglycerin, the exercise duration in treadmill exercise test (TET) and the Duck treadmill score among patients,the scores of Seattle Angina Questionnaire (SAQ) on five dimensions including physical limitations, anginal stability, anginal frequency, treatment satisfaction, and disease perception, and efficacy of TCM syndrome and of each single TCM symptom after treatment. The safety were evaluated by examine blood routine, urine routine, liver and kidney function, fasting blood sugar, electrocardiogram, adverse events. ResultsThe total effective rate of angina symptom in the treatment group was 71.69% (200/279), significantly higher than 51.64% (47/91) in the control group (P<0.01). The curative and markedly effective rate of TCM syndrome in the treatment group was 53.05% (148/279), which was significantly higher than 25.27% (23/91) in the control group (P<0.01). After treatment, scores of the number as well as duration of angina attacks and pain severity, the total score of angina symptoms, and the usage of nitroglycerin significantly decreased in both groups, and more changes were seen in the treatment group than in the control group; the scores of physical limitations, anginal stability, anginal frequency, treatment satisfaction, and disease perception in both groups significantly increased, and more improvement were shown in the experimental group regarding the anginal stability, anginal frequency and treatment satisfaction (P<0.05 or P<0.01). The effects of chest pain, chest tightness, palpitation, shortness of breath and fatigue in experimental group were significantly higher than those in control group (P<0.05 or P<0.01). There was no significant difference in the exercise duration of treadmill test and Duke score among patients between the two groups either before or after treatment (P>0.05). Adverse events occurred in 66 cases (23.66%) of the experimental group and 16 cases (17.58%) of the control group, with no statistical significance between the two groups (P>0.05). ConclusionThe Uyghur medicine YDMHP can effectively improve symptoms of angina pectoris, reduce the number, duration, and intensity of attacks, decrease the dosage of nitrogly-cerin and improve the individual TCM symptoms and has good safety in the treatment of SAP patients of qi stagnation and blood stasis.
4.Blood flow restriction combined with resistance training attenuates renal fibrosis in type 2 diabetic rats by inhibiting TGF-β1/Smad3 pathway

AIM:To investigate the improvement effect of blood flow-limited resistance training on renal fibro-sis in type 2 diabetes mellitus(T2DM)rats and its potential mechanism to attenuate renal fibrosis by inhibiting the trans-forming growth factor β1(TGF-β1)/Smad3 signaling pathway.METHODS:The T2DM model was prepared by combining a high-fat diet and streptozotocin(STZ),and after successful modeling,the rats were randomly divided into a T2DM con-trol group,a low-load resistance training group,a high-load resistance training group,a blood flow restriction group and a blood flow restriction combined with resistance training group for 8 weeks of exercise.The renal index,fasting blood glu-cose(FBG),serum creatinine(SCr),and blood urea nitrogen(BNU)were recorded in each group.The morphological changes of the kidneys were observed by hematoxylin and eosin(HE)and Masson's trichrome staining,and the collagen volume fraction was calculated.The mRNA expression levels of renal Klotho,TGF-β1,and α-smooth muscle actin(α-SMA)were detected by RT-qPCR.The protein expression levels of renal Klotho,TGF-β1,Smad3,phosphorylated Smad3(p-Smad3),α-SMA and connective tissue growth factor(CTGF)were detected using Western blot.RESULTS:Compared with the other groups,FBG,SCr,BNU,and renal collagen volume fraction were significantly decreased in the blood flow restriction combined with resistance training group of rats(P<0.05),Klotho expression was significantly in-creased(P<0.05),and the expression of TGF-β1,p-Smad3,CTGF and α-SMA was significantly decreased(P<0.05),and there was no significant change in the expression level of Smad3(P>0.05).CONCLUSION:Blood flow restriction combined with resistance training attenuates renal fibrosis in T2DM rats,the mechanism of which may be related to the up-regulation of Klotho expression,disruption of the TGF-β1/Smad3 signaling pathway,and inhibition of the deposition of epi-thelial-mesenchymal transformation.
5.Comparison of initiation of antihypertensive therapy strategies for primary preven-tion of cardiovascular diseases in Chinese population:A decision-analytic Markov modelling study

Objective:To evaluate the health benefits and intervention efficiency of different strategies of initiating antihypertensive therapy for the primary prevention of cardiovascular diseases in a community-based Chinese population from the Chinese electronic health records research in Yinzhou(CHERRY)study.Methods:A decision-analytic Markov model was used to simulate and compare different antihy-pertensive initiation strategies,including:Strategy 1,initiation of antihypertensive therapy for Chinese adults with systolic blood pressure(SBP)≥140 mmHg(2020 Chinese guideline on the primary preven-tion of cardiovascular diseases);Strategy 2,initiation of antihypertensive therapy for Chinese adults with SBP≥130 mmHg;Strategy 3,initiation of antihypertensive therapy for Chinese adults with SBP ≥140 mmHg,or with SBP between 130 and 140 mmHg and at high risk of cardiovascular diseases(2017 American College of Cardiology/American Heart Association guideline for the prevention,detection,evaluation,and management of high blood pressure in adults);Strategy 4,initiation of antihypertensive therapy for Chinese adults with SBP≥ 160 mmHg,or with SBP between 140 and 160 mmHg and at high risk of car-diovascular diseases(2019 United Kingdom National Institute for Health and Care Excellence guideline for the hypertension in adults:Diagnosis and management).The high 10-year cardiovascular risk was de-fined as the predicted risk over 10%based on the 2019 World Health Organization cardiovascular disease risk charts.Different strategies were simulated by the Markov model for ten years(cycles),with parame-ters mainly from the CHERRY study or published literature.After ten cycles of simulation,the numbers of quality-adjusted life years(QALY),cardiovascular events and all-cause deaths were calculated to evaluate the health benefits of each strategy,and the numbers needed to treat(NNT)for each cardiovas-cular event or all-cause death could be prevented were calculated to assess the intervention efficiency.One-way sensitivity analysis on the uncertainty of incidence rates of cardiovascular disease and probabilis-tic sensitivity analysis on the uncertainty of hazard ratios of interventions were conducted.Results:A to-tal of 213 987 Chinese adults aged 35-79 years without cardiovascular diseases were included.Com-pared with strategy 1,the number of cardiovascular events that could be prevented in strategy 2 increased by 666(95%UI:334-975),while the NNT per cardiovascular event prevented increased by 10(95%UI:7-20).In contrast to strategy 1,the number of cardiovascular events that could be prevented in strategy 3 increased by 388(95%UI:194-569),and the NNT per cardiovascular event prevented decreased by 6(95%UI:4-12),suggesting that strategy 3 had better health benefits and intervention efficiency.Compared to strategy 1,although the number of cardiovascular events that could be prevented decreased by 193(95%UI:98-281)in strategy 4,the NNT per cardiovascular event prevented decreased by 18(95%UI:13-37)with better efficiency.The results were consistent in the sensitivity analyses.Conclusion:When initiating antihypertensive therapy in an economically developed area of China,the strategy combined with cardiovascular risk assessment is more efficient than those purely based on the SBP threshold.The cardiovascular risk assessment strategy with different SBP thresholds is suggested to balance health benefits and intervention efficiency in diverse populations.
6.Status quo of pain catastrophizing in patients with diabetic peripheral neuropathic pain and influencing factors analysis

Objective To investigate the status quo of pain catastrophizing(PC)in the patients with di-abetic peripheral neuropathic pain(DPNP),and to analyze the influencing factors to provide reference for for-mulating clinical preventive intervention strategies.Methods A total of 206 patients with DPNP admitted and treated in the People's Hospital of Guangxi Zhuang Autonomous Region were selected as the research sub-jects by convenience sampling method.The general data questionnaire,Numerical Rating Scale(NRS),Pain Catastrophizing scale(PCS),Perceived Social Support Scale(PSSS)and diabetes distress scale(DDS)were used to conduct the investigation.Results The incidence rate of PC in 206 cases of DPNP patients was 44.66%(92/206),and the total score of PCS was(30.10±5.16)points.The results of multiple linear regres-sion analysis showed that the gender,duration of diabetes(≥10 years),multiple drug use,number of compli-cations(>5),NRS score,PSSS score and scores of DDS dimensions were the main influencing factors of PC(all P<0.05),which could explain 92.3%of the total variation of PC.Conclusion The PC incidence rate in the patients with DPNP is high.Clinical healthcare workers should pay attention to the evaluation of PC in these patients,and formulate the scientific and effective targeted intervention measures according to the main influen-cing factors to help the patients to reduce the pain burden in order to reduce the level of PC.
7.Practical exploration of innovative hospitals to build a joint research and development platform between hospitals and ienterprises: Taking Beijing Tiantan Hospital as an example

Objective:To explore the organizational and operational models of collaborative construction of Research and Development (R&D) platforms by innovative hospitals and enterprises, aiming to provide support and a guarantee for accelerating scientific and technological innovation and the transformation of achievements.Methods:Taking the co-construction of a joint R&D platform by Beijing Tiantan Hospital with enterprise as a case study, this study discussed the necessity of platform construction, and from the perspective of management practices, focusing on analyzing construction models, operational management, achievement attribution, and profits distribution, as well as other critical issues in platform construction.Results:Hospitals and enterprises agreed on the cooperation and co-construction model by signing an agreement. The management committee system and the centre director responsibility system were important operation and management mechanisms. The intellectual property rights of the outcomes generated by the cooperative R&D based on the joint R&D platform were co-owned by the cooperative parties. Data security, close communication, and high-quality research output were the critical issues for the sustainable cooperation between hospitals and enterprises. At the same time, this article proposed optimized management measures to address existing problems in the cooperation of the hospital-enterprise joint R&D platform.Conclusions:The hospital-enterprise joint R&D platform, as a new model of innovative collaboration between hospitals and businesses, held significant importance for tackling ″bottleneck″ technologies in medical science, the implementation of medical scientific and technological outcomes, and the cultivation of interdisciplinary talents.
8.Research on the driving factors and countermeasures for the transformation of scientific and technological achievements in Beijing municipal tertiary comprehensive hospitals

Objective:This study aims to identify and enhance management strategies for fostering the conversion of scientific and technological advancements in tertiary general hospitals in Beijing.Methods:A questionnaire survey was conducted to pinpoint and organize the areas needing improvement in the transformation process of scientific and technological achievements at Beijing Tiantan Hospital. This involved aspects such as organization, leadership, investment, incentives, and service guarantees. The data were then analyzed and summarized, taking into account the hospital′s management, research activities, and the specific circumstances of the institution.Results:The primary drivers for successful achievement transformation in Beijing′s tertiary general hospitals were identified across six key indicators: proficiency of professional and technical managers, allocation of special funds, access to market and industry resources, criteria for job title evaluation and promotion, a culture promoting diligence and fault tolerance, and comprehensive transformation training programs.Conclusions:The study concludes that implementing effective incentives and refining the institutional environment for innovation and transformation are crucial. Additionally, it′s vital to adjust work focus and enhance management in weaker areas, in line with the hospital′s actual situation. Moreover, boosting service efficiency and providing full life cycle services have emerged as pivotal measures to augment the efficacy of hospital innovation and transformation efforts.
9.The effects of acupuncture of the tongue on the cortical activity of stroke survivors with dysphagia: A functional near-infrared spectroscopy study

Objective:To seek any correlation between cortical activity and the swallowing of dysphagia patients with infratentorial stroke, and to observe any effect of three-needle acupuncture of the tongue on such activity.Methods:Thirty infratentorial stroke survivors with dysphagia were randomly divided into a tongue three-needle group and a sham acupuncture group, each of 15. Functional near-infrared spectroscopy was used to monitor changes in the concentration of oxygenated hemoglobin (ΔHbO 2) at rest, during acupuncture (or sham acupuncture), during real or sham electro-acupuncture, and at rest after the acupuncture or sham acupuncture treatment. The Modified Rankin Scale, a Penetration-Aspiration Scale (PAS), and the Functional Oral Intake Scale were employed to assess overall functional disability and the swallowing of both groups. Results:At rest the average ΔHbO 2 concentrations recorded in the left primary motor cortex, the left dorsolateral prefrontal cortex and the left premotor cortex in both groups were positively correlated with the PAS scores. During the acupuncture ΔHbO 2 concentration in the right inferior frontal gyrus and the left middle temporal gyrus increased significantly in the tongue three-needle group. It decreased significantly in the left somatosensory cortex and the left primary motor cortex. Conclusion:Three-needle acupuncture of the tongue induces changes in cortical activity in infratentorial stroke survivors with dysphagia, suggesting a potential technique for improving disordered swallowing.
10.Methamphetamine Use Disorder Severity Scale:development and validation

Objective:The aims of this study were to develop a simple and easy-to-use scale, named Methamphetamine Use Disorder (MUD) Severity Scale (MUDSS), and to complete the reliability and validity tests, so as to provide a new tool for the measurement of substance use.Methods:Participants with MUD ( n=286) were recruited from 2 compulsory isolated detoxification centers in Changsha, Hunan Province from April to September 2019. The item pool of MUDSS ( n=19) was obtained according to diagnostic items of stimulant use disorder in the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5) and the opinions of experts in the field of addiction. The scale was applied to participants with MUD. Items of the scale were first screened according to the results of item analysis, and then the formal scale was formed. Exploratory factor analysis was used to construct the scale dimension, and the reliability of the total scale and each dimension were tested by Cronbach α coefficient. Pearson correlation analysis was used to verify criterion validity and DSM-5 was used as criterion. Results:The effective information of 257 subjects was collected. According to the results of item analysis, the fourth item from the preliminary MUDSS was deleted, and the remaining 18 items were included in the formal scale, including four dimensions: adverse consequences, tolerance, priority, and craving. The internal consistency coefficient of the total scale and the four dimensions ranged from 0.646 to 0.909, and the test-retest reliability ranged from 0.592 to 0.724. The correlation between MUDSS total score and DSM-5 was 0.654.Conclusions:The reliability and validity of MUDSS are good and have reached the standard of psychometrics, therefore it can be further applied in a larger sample of subjects with MUD.
