OBJECTIVE To investigate the improvement effects and potential mechanisms of Sangbaipi decoction (SBP) on mice with second-hand smoke-induced pulmonary fibrosis (PF). METHODS Using network pharmacology methods, the common targets between the active components of SBP and PF-specific targets were screened, the protein-protein interaction network was constructed, the pathway enrichment analysis was performed using the Kyoto Encyclopedia of Genes and Genomes, and the molecular docking was conducted. Based on these results, the male Kunming mice were divided into a normal control group, a model control group, an SBP group (1.08 g/kg, calculated by crude drugs), and a pirfenidone group (positive control, 90 mg/kg), with eight mice in each group. Except for the normal control group, all other groups were exposed to second-hand smoke via the static respiratory tract smoking method (30 min of smoking twice daily for 20 consecutive days) to establish a mouse model of PF induced by second-hand smoke. Each drug group was administered the corresponding drug solution via gavage 0.5 h after the second daily smoke exposure; the normal control and model control groups were simultaneously administered an equal volume of water via gavage, once daily for 20 consecutive days. Twenty-four hours after the final administration, pathological changes in lung tissue were observed, and the levels of pro-inflammatory factors, injury markers, and the expression of proteins in related signaling pathways in lung tissue were detected. RESULTS A total of 2 448 common targets were identified, with core targets including protein kinase B1 (AKT1), tumor necrosis factor-α (TNF-α), and interleukin-6 (IL-6), which were enriched in signaling pathways such as phosphoinositide 3-kinase (PI3K)/AKT. The bin ding energies of AKT1, PI3K, TNF-α, and IL-6 to the active compound chaksine were -11.0, -7.7, -9.0, and -9.4 kcal/mol, respectively. Compared with the model control group, pathological changes such as inflammatory cell infiltration and collagen deposition in lung tissue were alleviated in the administration groups. The expression of collagen Ⅲ in lung tissue was significantly downregulated, and the levels of IL-6, TNF-α, tissue inhibitor of metalloproteinase 1, and pulmonary surfactant-associated protein D, as well as the percentage of positive areas with pulmonary fibrosis and protein phosphorylation levels of PI3K and AKT, were all significantly downregulated or reduced ( P <0.05). CONCLUSIONS The role of SBP in improving PF induced by second-hand smoke may be related to the regulation of multiple core targets, including AKT1, and the inhibition of the PI3K/AKT signaling pathway, thereby alleviating pulmonary inflammation and collagen deposition.

To investigate the effects of genistein (Gen) on the biosynthesis of N-glycolylneuraminic acid (Neu5Gc) in rats, 80 4-week-old male SD rats were randomly equally into the control and genistein groups. The rats of control and genistein groups were fed 5% ethanol and 300 mg/(kg·d) genistein respectively by gavage. The contents of Neu5Gc in hind leg muscle, kidney and liver tissues of rats were measured by using high performance liquid chromatography coupled with fluorescence detector (HPLC/FLD), and the mechanism of inhibition of Neu5Gc synthesis was investigated by using the molecular docking of Gen and sialyltransferase. On the 15th day, the content of Neu5Gc in hind leg muscle and liver tissues decreased 13.77% and 15.45%, respectively, and there was no significant change in the content of Neu5Gc in kidney tissues. On the 30th day, the content of Neu5Gc in liver tissues decreased 13.35%, however, there was no significant change in the content of Neu5Gc in kidney tissues and Neu5Gc was not detected in hind leg muscle. The content of Neu5Gc in hind leg muscle, kidney and liver tissues decreased respectively 32.65%, 32.78%, 16.80% and 12.72%, 11.42%, 12.30% while rats fed on the 45th and the 60th days. Genistein has formed the hydrogen bond with sialyltransferase activity site residues His319, Ser151, Gly293, Thr328 and formed a hydrophobic interactions with the residues His302, His301, Trp300, Ser271, Phe292, Thr328, Ser325 and Ile274. The results of molecular docking indicated that the weak intermolecular interaction was the main cause of genistein inhibiting sialyltransferase activity. The research results provided an experimental basis for the subsequent reduction of Neu5Gc in red meat before slaughter.

Objective To discuss the influence of family function and social support on insulin treatment compliance in patients with diabetes mellitus. Methods A total of 368 diabetic patients diagnosed using insulin treatment were selected between March 2013 and May 2014 in outpatient department of endocrinology department. Situations of Insulin treatment compliance, family function, social support were investigated by questionnaire. Results Insulin treatment compliance were good in 202 cases (54. 9%), poor in 120 cases (32. 6%) and very poor in 46 cases (12. 5%). The cases of different levels of family function were 183 (49. 7%) with a high level, 82 (22. 3%) with a general level and 103 (28. 0%) with dereliction of duty level. The cases of low, a general and a high level of social support were 119 (32. 3%), 108 (29. 3%) and 141 (38. 3%). Multiple regression analysis showed that the main factors influencing the compliance of insulin treatment were age, education degree, marital status, occupation, monthly income, payment methods, disease duration, family function and social support. Conclusions When the scores of family function and social support are higher, the treatment compliance is higher too. The nursing staff should improve patient's family function and social support in order to improve the patients' insulin treatment compliance, so as to improve patients'quality of life.
