Objective To systematically evaluate the incidence rate of low-level viremia(LLV)in chronic hepatitis B(CHB)patients and related influencing factors, and to provide evidence-based medicine evidence for effective intervention and prevention of LLV in clinical practice. Methods This study was conducted according to the PRISMA guideline, with a PROSPERO registration number of CRD42023455304.CNKI,Wanfang Data,VIP,SinoMed,PubMed,Embase,Web of Science, and the Cochrane library were searched for observational studies on LLV and related influencing factors in CHB patients published up to July 21,2023.Stata 16.0 software was used to perform the meta-analysis.Results A total of 12 articles were included, with a total sample size of 3408 cases, among whom there were 1181 patients with LLV.The meta-analysis showed that the incidence rate of LLV was 32.8%(95%confidence interval[CI]:27.6%—38.3%)in treatment-experienced CHB patients.High HBsAg quantification(odds ratio[OR]=2.107,95%CI:1.782—2.491,P<0.001),positive HBeAg(OR=3.258,95%CI:2.629—4.038,P<0.001),high HBV DNA level at baseline(OR=1.286,95%CI:1.157—1.430,P<0.001),and history of entecavir treatment(OR=3.089,95%CI:1.880—5.074,P<0.001)were risk factors for LLV;duration of antiviral therapy≥3 years(OR=0.175,95%CI:0.093—0.331,P<0.001)and high alanine aminotransferase level at baseline(OR=0.985,95%CI:0.978—0.992,P<0.001)were protective factors against LLV.The sensitivity analysis showed no significant change in effective value, suggesting that the results of the meta-analysis were relatively stable. The funnel plot of the studies included was basically symmetrical, and the results of the Egger's test and the Begg's test suggested that there was no obvious publication bias in the articles included. Conclusion Clinicians should guide decision making based on the influencing factors for LLV and related clinical evidence,so as to reduce long-term clinical risks and avoid adverse outcomes.

Objective:To evaluate the effect of sodium sivelestat on the expression of specialized pro-resolving mediators (SPMs) synthesis enzymes in mice with lipopolysaccharide (LPS)-induced acute lung injury (ALI).Methods:Eighteen SPF healthy male C57BL/6 mice, aged 6-8 weeks, weighing 20-25 g, were divided into 3 groups ( n=6 each) using a random number table method: control group (C group), LPS-induced ALI group (ALI group), and LPS-induced ALI + sodium sivelestat group (ALI+ SV group). ALI was induced by intravenous injection of LPS 15 mg/kg through the tail vein. Sodium sivelestat 50 mg/kg was intraperitoneally injected at 1 h after LPS administration. At 12 h after LPS administration, blood samples were collected from the eyeballs for routine blood tests, and the remaining blood was processed for serum extraction. The mice were sacrificed after anesthesia, and lung tissues were collected to determine the wet/dry weight (W/D) ratio, serum concentrations of interleukin-1beta (IL-1β) and IL-10 (by enzyme-linked immunosorbent assay), expression of neutrophil elastase (NE) and SPMs synthesis enzymes 5-lipoxygenase (5-LOX), 12-lipoxygenase (12-LOX), and 15-lipoxygenase (15-LOX) in lung tissues (by Western blot) and to examine the pathological changes of lung tissues which were scored. Results:Compared with C group, the lung injury scores, W/D ratio, white blood cell counts, percentage of neutrophil, and serum IL-1β and IL-10 concentrations were significantly increased, the expression of NE was up-regulated, and the expression of 5-LOX, 12-LOX and 15-LOX was down-regulated in ALI group ( P<0.05). Compared with ALI group, the lung injury scores, W/D ratio, white blood cell counts, percentage of neutrophil, and serum IL-1β concentration were significantly decreased, the serum IL-10 concentration was increased, the expression of NE was down-regulated, and the expression of 5-LOX, 12-LOX and 15-LOX was up-regulated in ALI+ SV group ( P<0.05). Conclusions:The mechanism by which sodium sivelestat alleviates LPS-induced ALI may be related to up-regulating the expression of SPMs synthesis enzyme and promoting the resolution of pulmonary inflammation in mice.

Objective To explore the mechanism of Sihuang powder(Radix et Rhizoma Rhei,Cortex Phellodendri,Radix Scutellariae and Fructus Gardeniae)in the treatment of knee synovitis by network pharmacology. Methods The active ingredients and corresponding targets of Sihuang powder were obtained from TCMSP database.A network diagram including medicines, active ingredients and targets was drawn by Cytoscape 3.9.1 software. The core targets were obtained by intersecting with the disease targets obtained from OMIM and Gene Cars databases.A target interaction network was constructed based on STRING database, and the GO/KEGG(gene ontology/Kyoto encyclopedia of genes and genomes)visualization analysis was performed on the core targets through DAVID database. The main chemical components and core target information were queried in TCMSP database and PDB database, and the results were imported into Autodock Tools.Autodock Vina was used for molecular docking analysis, and PyMol was used to draw 3D maps. Results A total of 68 active ingredients and 244 drug targets of Sihuang powder were obtained, including 18 core targets, such as tumor necrosis factor(TNF),interleukin-6(IL-6),protein kinase B1(AKT1)and tumor protein p53(TP53).A total of 118 signal pathways were obtained by KEGG enrichment analysis. The molecular docking results showed that quercetin, kaempferol, baicalein and wogonin in Sihuang powder had strong affinity with key targets. Conclusion Sihuang powder has the characteristics of multi-component,multi-target,and multi-pathway in the treatment of knee synovitis. It lays a foundation for further basic experimental research, and provides a reference for the exploration of the mechanism of this prescription in the treatment of knee synovitis.
