1.The construction of a hierarchical training curriculum system for neonatal nurses based on core competencies

Objective:To construct a hierarchical training course system for neonatal nurses based on core competencies, to provide a reference for meeting the training needs of neonatal nurses under the new situation.Methods:Through literature review, questionnaire survey on training needs, and focus group interviews, a preliminary hierarchical training curriculum system for neonatal nurses was developed. Two rounds of Delphi correspondence were conducted with 19 domestic experts to finalize the system.Results:The effective questionnaire recovery rates of the two rounds of expert consultation were 95.00% and 100.00%, and the expert authority coefficient was 0.916, the Kendall harmony coefficient of the first round of expert opinions was 0.351 ( P<0.001), and the Kendall harmony coefficient of the second round of expert opinions was 0.463 ( P<0.001). The hierarchical training course structure and course training content are formed, including N0: 3 first-level items, 9 second-level items, 80 third-level items, N1: 3 first-level items, 9 second-level items, 91 third-level items, N2: 3 first-level items, 9 second-level items, 86 third-level items, N3: 3 first-level items, 10 second-level items, 81 third-level items, N4: 3 first-level items, 10 second-level items, 76 third-level items. Conclusions:The hierarchical training course system for neonatal nurses based on the core competence of nurses is scientific and practical, which can provide a reference for the hierarchical training of neonatal nurses.

3.Family functioning among preterm infants in the NICU from the perspective of social ecosystem theory: a qualitative study

Objective:To investigate the impact of the birth of preterm infants on their family functioning, and to provide a reference for the formulation of targeted intervention programs.Methods:Based on the socio ecosystem theory, phenomenological research methods and objective sampling were used to conduct semi-structured interviews with 16 preterm infants in the neonatal intensive care unit (NICU) of Children′s Hospital of Soochow University from September 2023 to January 2024, who meet the inclusion and exclusion criteria, and the Colaizzi 7-step analysis method was used to analyze the interview data.Results:All 16 caregivers of the NICU preterm infants who participated in the interviews were the infants′ mothers, aged from 26 to 34 years. A total of 3 themes and 10 sub-themes were summarized. The micro system: physical and mental exhaustion coexisted with role growth of caregivers (physical and mental health was affected, positive experience after psychological adjustment, and adaptation to role change). The intermediate system: family relationship mode change (family satisfaction decreased, family difficulties affected family harmony, family communication content changed, family harmony and conflict coexisted, and family conflict resolution was actively resolved). The outer system: social support system needed to be improved (benefited from social support and not yet perceived external support).Conclusions:Medical staff can improve the multiple support system and provide continuous and personalized family support, so as to improve the family adjustment ability and improve the family functioning of preterm infants in the NICU.
4.Rescue and nursing care of a neonate with severe laryngomalacia complicated with breath-holding attack

To summarize the rescue and care of a case of breath-holding attack caused by congenital contractures of limbs and face,hypotonia,and developmental delay(CLIFAHDD)syndrome.Key points of nursing:early identification and intervention of breath-holding attack in CLIFAHDD syndrome,and implementation of rapid reactive rescue;implementing individualized developmental care measures to avoid stimuli that induce breath-holding attack;paying attention to the assessment of oral feeding ability,strengthening the exercise of oral feeding ability and nutrition management;strengthening perioperative diversified airway management to prevent postoperative complications;the monitoring of early neurological and motor system development,and early start of rehabilitation care;doing a good job in health education for caregivers to help them master the first aid treatment and growth and development assessment methods in the event of a breath-holding attack.After 53 days of hospitalization,the patient was discharged.After being discharged from the hospital for more than 1 month,the patient had breath-holding attack 2~4 times a day.After being discharged from the hospital for more than 2 months,the patient had no breath-holding attack,and only had occasional convulsive seizures during sleep,which could be relieved spontaneously.
