Chinese Journal of Experimental Traditional Medical Formulae 2026;32(2):25-33
ObjectiveTo investigate the mechanisms by which Huanglian Jiedutang (HLJDT) modulates microglial (MG) phenotypes through the sialic acid-binding Ig-like lectin 2 (SIGLEC2/CD22)/Src-homology-2-domain-containing protein tyrosine phosphatase-1 (SHP-1)/phosphorylated protein kinase B (p-Akt) signaling pathway, thereby promoting myelin repair and alleviating agitation-like behaviors in vascular dementia (VAD). MethodsSixty C57BL/6J mice were randomly assigned to a sham (normal) group, model group, HLJDT low-, medium-, and high-dose groups (2.5, 5, and 10 g·kg-1·d-1), and a risperidone group (2 mg·kg-1·d-1), with 10 mice per group. VAD was induced by bilateral common carotid artery stenosis (BCAS). From day 42, mice received drug interventions for 2 weeks. Agitation-like behaviors were assessed using the resident-intruder test. After behavioral testing, ventrolateral part of the ventromedial hypothalamus (VMHvl) tissues were collected. Western blot was used to measure protein levels of myelin oligodendrocyte glycoprotein (MOG), myelin basic protein (MBP), proteolipid protein (PLP), inducible nitric oxide synthase (iNOS), arginase-1 (Arg1), CD86, CD206, and CD22, SHP-1, and p-Akt. Immunofluorescence was used to evaluate myelin-associated glycoprotein (MAG) intensity and the proportion of iNOS+/ionized calcium-binding adapter molecule 1 (Iba1)+ cells. ELISA was used to detect tumor necrosis factor-α (TNF-α), interleukin (IL)-6, and IL-1β. ResultsCompared with the normal group, the model group exhibited markedly increased biting and aggressive behaviors and shortened attack latency (P<0.01). MOG, MBP, and PLP protein levels and MAG fluorescence intensity were significantly reduced (P<0.05, P<0.01). INOS and CD86 expression and TNF-α, IL-6, and IL-1β levels were significantly elevated (P<0.01). CD22 and SHP-1 expression increased significantly (P<0.01), whereas p-Akt expression decreased (P<0.01). Compared with the model group, the medium- and high-dose HLJDT groups and the risperidone group showed markedly reduced biting and aggression (P<0.05, P<0.01) and prolonged attack latency (P<0.01). MOG, MBP, and PLP levels and MAG fluorescence intensity were significantly increased (P<0.05, P<0.01). INOS, CD86, TNF-α, IL-6, and IL-1β levels decreased significantly (P<0.05, P<0.01). CD22 and SHP-1 expression decreased, while p-Akt expression increased significantly (P<0.05, P<0.01). ConclusionHLJDT may modulate CD22/SHP-1/p-Akt signaling in the VMHvl, promote the shift of MG toward an anti-inflammatory and phagocytic phenotype, enhance myelin repair, and improve agitation-like behaviors in VAD mice.

OBJECTIVE To investigate the apoptosis-inducing effect of esculetin (Esc) on acute myeloid leukemia (AML) HL-60 cells by regulating the protein kinase B (AKT)/S-phase kinase-associated protein 2 (SKP2)/MutT homolog 1 (MTH1) pathway. METHODS AML HL-60 cells were randomly divided into control group (routine culture), Esc low-concentration group (L-Esc group, 25 μmol/L Esc), Esc medium-concentration group (M-Esc group, 50 μmol/L Esc), Esc high-concentration group (H-Esc group, 100 μmol/L Esc), and high-concentration of Esc+ SC79 (AKT agonist) group (100 μmol/L Esc+5 μmol/L SC79). Cell proliferation in each group was detected by MTT assay and colony formation assay. The level of reactive oxygen species (ROS) in cells was measured by using the CM-H2DCFDA fluorescent probe. Cell apoptosis was analyzed by flow cytometry. Western blot assay was performed to detect the expression levels of apoptosis-related proteins [B-cell lymphoma 2 (Bcl-2), Bcl-2-associated X protein (Bax), cleaved caspase-3], AKT/SKP2/MTH1 pathway-related proteins (p-AKT, AKT, SKP2, MTH1), along with the upstream and downstream proteins of AKT phosphatidylinositol 3-kinase (PI3K), cyclin-dependent kinase inhibitor 1 (P21) and cyclin-dependent kinase inhibitor 1B (P27). RESULTS Compared with control group, the cell viability, colony number, and the phosphorylation levels of AKT and PI3K proteins as well as protein expressions of SKP2, MTH1 and Bcl-2 were significantly decreased (P<0.05), while ROS level, apoptosis rate, and the expression levels of Bax, cleaved caspase-3, P21 and P27 proteins were significantly increased (P<0.05). Moreover, the effects of Esc exhibited concentration-dependence (P<0.05). Compared with H-Esc group, above indexes of high-concentration of Esc+ SC79 group were reversed significantly (P<0.05). CONCLUSIONS Esc may promote massive ROS production and induce activation of apoptosis in HL-60 cells by inhibiting the AKT/SKP2/MTH1 pathway, thus inhibiting the proliferation of HL-60 cells.

Chinese Journal of Experimental Traditional Medical Formulae 2026;32(6):206-216
ObjectiveTo analyze the pharmacodynamic substance basis of Shangkeling Spray and its potential mechanisms in intervening knee osteoarthritis (KOA) using ultra-performance liquid chromatography-tandem mass spectrometry (UPLC-MS), network pharmacology, and molecular docking technology. MethodsUPLC-MS was used to identify the chemical components of Shangkeling Spray. Pharmacokinetic properties were employed to screen potential active ingredients. Network pharmacology methods were utilized to collect potential targets of these ingredients and the pathological gene set of KOA. An "active ingredient-disease" target network was constructed using databases such as STRING. Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) functional enrichment analyses were performed using clusterProfiler. Libraries including NumPy were employed to calculate shortest path lengths to identify dominant pharmacodynamic links. Core gene clusters were identified using MCODE, validated through the Gene Expression Omnibus (GEO) database, and molecular docking was performed between key active ingredients and core targets. ResultsA total of 322 and 314 chemical components were identified under positive and negative ion modes, respectively, with 410 components in total after de-duplication, mainly including flavonoids, coumarins, terpenoids, organic acids, and alkaloids. Analysis of the "active ingredient-disease" network identified "development and regeneration", "cell growth and death", "immune system", and "nervous system" as the dominant pharmacodynamic links of Shangkeling Spray in the treatment of KOA. Molecular docking showed that key active ingredients, such as bletillin A, formononetin, morin, oxymatrine, aconitine, gallic acid, curdione, apigenin, naringenin, and oleanolic acid, tightly bound to functional domains of 10 key targets including Jun proteins(JUN), interleukin-6 (IL-6), protein kinase B1 (Akt1), Caspase-3, nuclear transcription factor-κB subunit p65(RELA), nuclear factor-kappaB1(NF-κB1), Cyclin D1, mammalian target of rapamycin(mTOR), tumor necrosis factor (TNF), and Fos proto-oncogene protein (FOS). These interactions synergistically regulated the phosphatidylinositol 3-kinase (PI3K)/Akt/mTOR-related signaling axis and nervous system-related pathways, mediating cartilage repair, reducing inflammation and pain, and improving KOA. ConclusionThis study preliminarily clarifies the pharmacodynamic substance basis of Shangkeling Spray and suggests that its main active ingredients may improve KOA by synergistically regulating the PI3K/Akt/mTOR-related pathways, providing a reference for subsequent exploration of its substance benchmark and mechanism of action.

Chinese Journal of Tissue Engineering Research 2026;30(4):832-840
BACKGROUND:Orthodontic treatment to move the mandibular second molar in place of the missing first molar is a good method for preserving natural teeth. However, molars often exhibit mesial tipping when using clear aligners. The impact of attachments and overcorrection on the molar mesialization with clear aligners is unclear.OBJECTIVE:To investigate the effects of attachments and overcorrection on displacement and biomechanics of the mandibular second molar mesialization.METHODS:A finite element model with the left mandibular first molar missing was constructed using a volunteer's cone-beam CT and intraoral scan data. The experiment was divided into four groups based on the position of attachments on the left mandibular second molar:without attachment, buccal attachment, lingual attachment, and buccal-lingual attachment, with overcorrection degrees(0°,1°,2°,3°,and 4°)for each group, totaling 20 models. The calculation and analysis of the displacement trends and stress were performed using Abaqus software.RESULTS AND CONCLUSION:(1)Mesial tipping and intrusion of the molar, distal tipping of the canines and permolars, and linguaing of the incisors occurred during molar mesialization.(2)The displacement of molars increased when attachments were used, with the maximum displacement occurring when buccal-lingual attachments were applied.Molars tended to tip towards the side without attachments when attachments were designed unilaterally. However, the inclination of molars did not decrease when attachments were used.(3)Molars exhibited reduced mesial tipping and intrusion, approaching overall movement, and the stress of periodontal ligament was relieved and more evenly distributed during overcorrection treatment, and this effect became more pronounced with increased degrees of overcorrection.(4)Attachments can enhance the effectiveness of overcorrection, with the best performance observed when buccal-lingual attachments are used.Using attachments and overcorrection together helps control molar mesial tipping and intrusion, but overall molar movement is not achieved.
8.Double lactate threshold exercise training:development context, basic connotation, application effect and mechanism of action
Chinese Journal of Tissue Engineering Research 2026;30(4):964-974
BACKGROUND:The lactate threshold is a crucial physiological indicator for assessing the aerobic metabolic capacity and training level of endurance athletes, representing the intensity point at which lactate production and clearance reach a dynamic equilibrium within the organism. The traditional lactate threshold training, which is a prolonged and stable-intensity training, aims to enhance the aerobic endurance and lactate clearance efficiency of athletes. In recent years, with the advancement of exercise physiology, double lactate threshold training has emerged as an innovative training modality, drawing extensive attention from both the academic circle and practitioners.Nevertheless, current systematic research on double lactate threshold training remains limited, particularly in aspects such as its physiological mechanisms, optimal implementation plans, and long-term effects.OBJECTIVE:To systematically sort out the development course of double lactate threshold training, deeply analyze its fundamental connotations, objectively assess its application effects, and investigate its physiological action mechanisms, with the expectation of providing scientific basis and guidance for the improvement and optimization of endurance sports training theories and practices.METHODS:With the search terms of"lactate threshold training, double lactate threshold training, lactate training, threshold training, endurance training,Norwegian training method,Norwegian training mode,Norwegian training experience,"a systematic search was conducted in databases, including PubMed,Web of Science,Embase,Medline,Cochrane Library,CNKI,WanFang, and VIP.After screening,8articles met the requirements, involving 8 training cases, and 14 top international athletes, and sports events include 800 m,1 500 m,5 000 m,10 000 m,and cross-country running(9.5 km for men and 4.5 km for women).RESULTS AND CONCLUSION:(1)For the first time, double lactate threshold training was defined as a training strategy aimed at improving aerobic and speed capabilities, involving separate aerobic and anaerobic threshold training sessions within a single training day(aerobic training load intensity requirement of 0.7-2.0 mmol/L,anaerobic training load intensity requirement of 2.0-4.5 mmol/L),with a training frequency of>2 times per week and a training volume of 120-160 km per week.(2)Double lactate threshold training has positive effects on muscle adaptability and plasticity(dimension and elasticity),increasing aerobic and anaerobic capabilities, and alleviating central neural fatigue and peripheral fatigue.(3)The main mechanisms by which double lactate threshold training yields effects may include hormone secretion, protein synthesis and inhibitory regulation, muscle fiber recruitment and cell swelling, mitochondrial biosynthesis and improvement in the chain of respiratory function, and neurotransmitter regulation.(4)In the practical application of double lactate threshold training, factors such as individualized training plans, load intensity design and monitoring for threshold training days, and applicable sports events should be fully considered. For the systematic improvement of double lactate threshold training, further exploration is still needed in terms of application subjects, training plans, action mechanisms, effect evaluation, and safe application.

Objective:To investigate the association between small vulnerable newborn (SVN) phenotypes and the risk of neurodevelopmental delay at the age of 1 year. Methods:A prospective cohort study was conducted. A total of 25 860 singleton infants from "The Born in Guangzhou Cohort Study" who completed the Gesell developmental scale assessment at 1 year of age between January 2013 and June 2025 were included. Maternal sociodemographic characteristics, and other information were collected using a self-administered questionnaire, and maternal pregnancy-related information and neonatal birth data were extracted from medical records. Global developmental delay (GDD) was defined as a developmental quotient below 86 in ≥3 domains of the Gesell developmental scale, which assesses the adaptive, gross motor, fine motor, language, and personal-social domains. The random forest algorithm was employed for missing data imputation. Based on prematurity, small for gestational age (SGA), and low birth weight (LBW), newborns were categorized into 6 phenotypes: preterm-SGA-LBW, preterm-appropriate for gestational age (AGA)-LBW, preterm-AGA-nonLBW, term-SGA-LBW, term-LBW-only or term-SGA-only, and term-AGA-nonLBW phenotype. Among these, the first 5 were classified as SVN phenotypes, and the last one served as the reference group. Inter-group comparisons were performed using analysis of variance (ANOVA), χ2 tests, or Kruskal-Wallis test, as appropriate.?? Multivariable robust Poisson regression models were applied to analyze the association of different SVN phenotypes with the risks of GDD and developmental delays in specific domains, with stratified analyses by sex. Results:Among the 25 860 infants, 13 719 (53.1%) were male and 12 141 (46.9%) were female. The gestational age at birth was 39.4 (38.6, 40.0) weeks. The overall detection rate of GDD at 1 year of age was 3.7% (962/25 860). The rates of delay across developmental domains, in descending order, language in 8 134 cases (31.5%), gross motor in 4 488 cases (17.4%), personal-social in 1 271 cases (4.9%), adaptive in 1 262 cases (4.9%), and fine motor in 621 cases (2.4%). Compared with the reference group, preterm-AGA-LBW, preterm-SGA-LBW, preterm-AGA-noneLBW, and term-SGA-LBW phenotypes were all associated with an increased risk of GDD, with the adjusted RR (95% CI) of 6.07(5.01-7.35), 4.81(3.11-7.46), 2.10(1.54-2.88) and 1.89(1.29-2.76) respectively. The preterm-AGA-noneLBW phenotype was all associated with an increased risk of delay in gross motor, language and personal-social functional domains (all P<0.05). The term-SGA-LBW phenotype was associated with an increased risk of delay in gross motor, fine motor and personal-social functional domains (all P<0.01). Whereas the term-LBW-only or term-SGA-only phenotype showed no statistically association with developmental delay in any functional domain (all P≥0.05). Conclusion:The combined classification based on gestational age and birth weight helps identify infants at high risk for neurodevelopmental delay at 1 year of age, suggesting that it may offer a reference for the rational allocation of clinical resources.

Chinese Journal of Medical Genetics 2026;43(4):241-247
OBJECTIVE:
To investigate the current status of clinical genetics specialization development and the diagnostic and therapeutic capabilities for hereditary diseases across medical institutions in Shanghai, and to assess the necessity and feasibility of establishing training bases for clinical genetics specialists.
METHODS:
By employing a cross-sectional survey design, the Clinical Genetics Committee of Shanghai Medical Association has conducted questionnaire surveys from March to April 2025 across 54 healthcare institutions in Shanghai (including 33 tertiary hospitals and 21 secondary hospitals). The survey involved administrative departments and medical personnel from 15 clinical specialties. The survey has covered current genetic disease diagnosis and treatment practices, relevant and specialised disease types, genetic department establishment, testing capabilities, personnel teams, and training requirements.
RESULTS:
The results revealed that 78.0% of clinical departments surveyed had treated patients with hereditary disorders. Shanghai possesses diagnostic and therapeutic expertise for over 95% of hereditary diseases listed in its rare disease catalogue, reflecting both the practical clinical demand for such conditions and the city's overall diagnostic and therapeutic strengths in this field. Nevertheless, significant disparities exist in the development of genetics departments across different tiers of healthcare institutions. Resources for genetic testing capabilities (including molecular, cellular, and biochemical testing) are also unevenly distributed across different tiers of hospitals. The survey further revealed that only 26.0% of departments believe that their current physician structure fully meets the diagnostic and treatment demands. Over 90% of departments consider standard training for clinical genetic specialists necessary, with 74.0% expressing willingness to participate in establishing training bases. Based on above findings and thorough deliberation, the Clinical Genetics Committee of the Shanghai Medical Association proposes advancing specialist training and discipline development through establishing a standard training system. The committee has drafted a three-year training protocol featuring a "joint training"-centered model, recommending a pilot-first, dynamically optimized strategy for steadily advancing training base development.
CONCLUSION
Shanghai faces substantial demand for genetic disease diagnosis and treatment, yet exhibits shortcomings in clinical genetics specialization development, resource allocation, and talent pipeline cultivation. To establish a standard training system holds significant practical importance and is underpinned by a broad demand.
