Background:
and Purpose Limited evidence exists on the effectiveness of endovascular treatment (EVT) for acute posterior circulation tandem lesion (PCTL). This study aimed to explore the role of extracranial vertebral artery (VA) stenting in patients with PCTL stroke undergoing EVT.
Methods:
Individual patient data were pooled from the BASILAR (EVT for Acute Basilar Artery Occlusion Study) and PERSIST (Posterior Circulation Ischemic Stroke) registries. Patients with PCTLs who underwent EVT were included in the present cohort and divided into the stenting and nonstenting groups based on the placement of extracranial VA stents. The primary efficacy outcome was the modified Rankin Scale (mRS) scores at 90 days and 1 year. Safety outcomes included 24-hour symptomatic intracranial hemorrhage (sICH) and all-cause mortality at 90 days and 1 year post-surgery.
Results:
A combined dataset of 1,320 patients with posterior circulation artery occlusion, including 263 (19.9%) with tandem lesions, of whom 217 (median age, 65 years; 82.9% male) met the inclusion criteria for the analysis. The stenting group had 84 (38.7%) patients, while the non-stenting group had 133 (61.3%). After adjustment for the potential confounders, extracranial VA stenting was associated with favorable shifts in mRS scores at both 90 days (adjusted common odds ratio [OR], 2.30; 95% confidence interval [CI], 1.23–4.28; P<0.01) and 1 year (adjusted OR [aOR], 2.04; 95% CI [1.05–3.97]; P=0.04), along with lower rate of mortality at both 90 days (aOR, 0.45; 95% CI [0.21–0.93]; P=0.01) and 1 year (aOR, 0.36; 95% CI [0.16–0.79]; P=0.01), with no significant difference in sICH incidence (aOR, 0.35; 95% CI [0.06–1.98]; P=0.24).
Conclusion
Extracranial VA stenting during EVT may improve functional outcomes and reduce mortality in patients with PCTL strokes.

Objectives:To propose a method based on machine learning to predict the neurological function-al status of patients with traumatic cervical spinal cord injury(TCSCI).Methods:The clinical data of 180 pa-tients with TCSCI admitted to Shanghai Changzheng Hospital were retrospectively analyzed, including cervical spine MRI images and American Spinal Injury Association(ASIA)scores within 24 hours after injury and the ASIA scores at 1-year follow-up after injury. The 180 patients were randomly divided into a training set of 144 patients and a test set of 36 patients in a ratio of 8∶2.Overall,a new clinical-imaging prediction method was proposed using the two-stage integration concept, which used the ASIA scores and MRI images within 24 hours after TCSCI to achieve a full-feature prediction of the patient's sensory and motor function one year after injury. In the first stage, models such as GradentBoosting,GaussianNB,KNeighbors,Decision-Tree,RandomForest,and support vector classifier were used to independently predict the sensory-motor func-tion recovery of 132 skin nodes and muscle nodes. In the second stage, the optimal model for each feature prediction was screened out through horizontal and vertical comparison of performance indicators,so as to fi-nally achieve the best prognostic prediction of neurological function at 56 light touch and 56 acupuncture skin nodes, and 20 key muscle nodes. After the constructed prediction model is trained and verified, the pre-diction of the test set is evaluated using accuracy, precision, recall, average precision and F1 score. Results:In terms of the overall performance of this prediction model in predicting sensory-motor function in TCSCI patients 1 year after injury, all models in the test set achieved accuracy ≥0.886,recall ≥ 0.845,precision ≥0.875,average precision ≥0.853,and F1 score≥0.859,demonstrating that the correct prediction ability of each model and the quality and completeness of the actual prediction results were relatively high. Moreover, the two-stage prediction model can optimize the prediction effect of each model on each feature, and the prediction performance is better. Conclusions:This sensory-motor full-feature prediction method can effective-ly predict the neurological function recovery of TCSCI patients at 1-year follow-up after injury. Its predictive ability is significantly higher than that of a single model, and is expected to provide a useful reference for the personalized diagnosis, treatment and rehabilitation of TCSCI patients.

Objective:To quantitatively evaluate the level of the three medical linkage in China from 2009 to 2022,explore the influencing factors and driving paths of the three medical linkage in China, and provide a new perspective for promoting the development of the three medical linkage. Methods:An optimized coupling coordination degree model was used to calculate the coupling coordination degree between the trinity healthcare systems and different binary systems within the systems in 31 provinces of China(excluding Hong Kong,Macao and Taiwan),and the Fuzzy-set Qualitative Comparative Analysis method was used to explore the condition configurations of multi-factor-driven three medical linkage. Results:From 2009 to 2022,the coupling coordination degree between the trinity healthcare systems in each province of China generally showed an increasing trend year by year.Among the binary systems, the overall coordinated development situation between the medical and medical insurance systems was the best and the regional development was the most balanced. The coupling coordination degree gap between the trinity healthcare system and the internal binary systems among provinces gradually widened, and the multi-polarization trend intensified. The paths to promote high-level three medical linkage can be summarized into two types:internal and external balanced development type(H1)and government-led type(H2,H3),among which the H1 path with per capita GDP and health expenditure as core conditions was the most common. Conclusion:It is suggested to enhance institutional and technological innovation, and integrate resources through a cross-departmental collaboration mechanism and digital technology.Provinces should select high-level optimization paths by leveraging regional endowments to narrow the regional development gap.Meanwhile, under the impetus of high-level policies, the protection and supervision system continues to improve, thereby promoting the three medical linkage.

Objective::There is limited evidence regarding the choice of P2Y 12 receptor inhibitors as a component of dual antiplatelet therapy in patients with left main (LM) disease undergoing percutaneous coronary intervention (PCI). This study aimed to evaluate long-term clinical outcomes of ticagrelor- vs. clopidogrel-based dual antiplatelet therapy strategy in acute coronary syndrome (ACS) patients undergoing LM PCI. Methods::This is a post-hoc analysis from a prospective, single-center, real-world PCI registry. A total of 1,163 patients discharged post-ACS who underwent LM PCI and received ticagrelor or clopidogrel between March 2016 and March 2019 were included in the study. The primary endpoint was ischemic events at 12 months, including cardiac death, myocardial infarction, or stroke. Secondary outcomes included all-cause death and Bleeding Academic Research Consortium types 2, 3, and 5, and types 3 and 5 bleeding. Propensity score matching was used to adjust for bias due to confounders between the 2 groups.Results::The ticagrelor and clopidogrel groups comprised 529 (45.49%) and 634 (54.51%) patients, respectively. During the follow-up period, the rate of ischemic events was significantly lower with ticagrelor than with clopidogrel before (1.32% (7/529) vs. 3.63% (23/634), P = 0.013,6) and after propensity score matching (1.41% (6/425) vs. 4.00% (17/425), P = 0.020,1). The rates of all-cause death, Bleeding Academic Research Consortium-defined type 2, 3, and 5 bleeding, and type 3 and 5 bleeding were similar between the ticagrelor group and clopidogrel group before or after propensity score matching adjustment (all P > 0.05). Conclusion::Among patients with ACS undergoing LM PCI, ticagrelor use was associated with ischemic events benefit without excessive risk of bleeding at 12 months compared with clopidogrel.
