1.Meteorological factor-driven prediction of high-use days of budesonide: construction and comparison of ensemble learning models

OBJECTIVE To construct ensemble learning models for predicting high-use days of budesonide based on meteorological factors, thereby providing reference for hospital pharmacy management. METHODS Meteorological data for 2024 and outpatient budesonide usage data from the jurisdiction of Sanming Hospital of Integrated Traditional Chinese and Western Medicine were collected. High-use days were defined as the 75th percentile of outpatient budesonide usage, and a corresponding dataset was established. The prediction task was formulated as a classification problem, and three ensemble learning models were developed: Random Forest, Extreme Gradient Boosting (XGBoost), and Histogram-based Gradient Boosting Classifier. Model performance was evaluated using accuracy, precision, recall, F1-score, and log-loss. Model interpretability was analyzed using Shapley Additive Explanations (SHAP). RESULTS The Histogram-based Gradient Boosting Classifier achieved the best performance (accuracy=0.75, F1-score=0.48), followed by XGBoost (accuracy=0.74, F1-score=0.43) and Random Forest (accuracy=0.72, F1-score=0.22). SHAP results suggested that the prediction results of the last two models have the highest correction. CONCLUSIONS Ensemble learning models can effectively predict high-use days of budesonide, with the Histogram- based Gradient Boosting Classifier demonstrating the best predictive performance. Low temperature, high humidity, and low atmospheric pressure show significant positive impacts on the prediction of daily budesonide usage.
2.Clinical diagnostic value of whole-body bone imaging combined with serum N-Osrteoc and VEGFR2 detection for bone metastasis in elderly patients with lung cancer

Objective To analyze the clinical diagnostic value of whole-body bone imaging combined with serum N-terminal osteo-calcin(N-Osrteoc)and vascular endothelial growth factor receptor 2(VEGFR2)for evaluating bone metastasis in elderly patients with lung cancer.Methods General data of 100 elderly patients with lung cancer diagnozed for the first time at Tianjin Hospital between December 2021 and December 2023 were retrospectively collected.Based on the pathological results,the patients were separated into a lung cancer bone metastasis group of 42 cases and a lung cancer non-bone metastasis group of 58 cases.Serum N-Osrteoc and VEGFR2 levels were detected by using an enzyme-linked immunosorbent assay(ELISA).All patients underwent whole-body bone imaging using SPECT diagnostic equipment.Receiver operator characteristic(ROC)curve analysis was performed to determine the diagnostic value of serum N-Osrteoc,VEGFR2,and whole-body bone imaging for lung cancer bone metastasis.Furthermore,Kappa test was performed to analyze the consistency between different examination methods for diagnozing lung cancer bone metastasis and pathological diagnosis results.Results The serum N-Osrteoc and VEGFR2 levels in the bone metastasis group were significantly higher than those in the non-bone metastasis group(P<0.05).The area under the curve(AUC)for serum N-Osrteoc and VEGFR2 levels,whole-body bone imaging,and their combination for diagnozing lung cancer bone metastasis were 0.847,0.846,0.907,and 0.956,respectively.Furthermore,com-pared with the pathological results,the numbers of false-positive cases were 14,19,8,and 1,those of false-negative cases were 7,7,2,and 3,and the Kappa values were 0.579,0.487,0.799,and 0.917,respectively(P<0.05).The specificity of whole-body bone imaging combined with serum N-Osrteoc and VEGFR2 in diagnozing lung cancer bone metastasis was significantly higher than alone(P<0.05).Conclusion The combination of whole-body bone imaging and serum N-Osrteoc and VEGFR2 levels is of great significance for early diagnosis of bone metastasis in elderly patients with lung cancer.This combined diagnosis has high sensitivity and specificity and can be widely used in clinical practice.
3.Advances in the study of common complications and prevention after arthroscopic rotator cuff injury repair

Rotator cuff injury is a shoulder disease most commonly seen with the elderly, and as China enters an aging society, the number of patients with rotator cuff injury is rapidly increasing. For patients with poor results of conservative treatment and serious conditions, arthroscopic rotator cuff repair has become the first choice due to less trauma and better efficacy. However, the rehabilitation exercise time required for postoperative functional recovery is long, and at the same time, the existence of a series of complications seriously affects patients' postoperative recovery and quality of life. The purpose of this article is to summarize the complications and preventive measures after arthroscopic rotator cuff injury repair, and to provide help for the diagnosis and treatment of postoperative complications and postoperative rehabilitation.
4.Application of a novel auxiliary sleeve in arthroscopic repair of large rotator cuff tear

Objective:To explore the clinical efficacy of suture-bridge techniques assisted by our self-designed novel auxiliary sleeve in arthroscopic repair of large rotator cuff tear.Methods:A retrospective analysis was conducted of the clinical data of 35 patients with large rotator cuff tear who had been treated by the arthroscopic suture-bridge technique assisted by our novel auxiliary sleeve at Department of Orthopaedics, The People’s Hospital of Shanxi Province from July 2022 to January 2024. The 35 patients were set as an observation group [14 males and 21 females with an age of (58.3±10.3) years]. According to a 1:1 ratio, another 35 patients with large rotator cuff tear who had been treated by simple arthroscopic suture-bridge technique from January 2021 to June 2022 were selected as a control group [15 males and 20 females with an age of (55.3±10.5) years]. The perioperative efficacy between the 2 groups was evaluated by comparing operation time, total length of incision, and disparity in hemoglobin level between preoperation and postoperative day 1. The American Shoulder and Elbow Surgeons (ASES) score, University of California, Los Angeles (UCLA) score, and visual analogue scale (VAS) pain score at 3 months after operation were compared between the 2 groups to evaluate the functional recovery and shoulder pain relief, and comparisons of the 3 scores were also made between preoperation and 3 months after operation in each group.Results:There were no statistically significant differences in the general preoperative data between the 2 groups, indicating comparability ( P > 0.05). The 70 patients were followed up for 5.7 (5.0, 7.0) months after operation. No such complications as recurrent rotator cuff tear, poor wound healing, infection, or anchor withdrawal occurred in either group after operation. The operation time in the observation group was 133.3 (120.0, 140.0) min, significantly shorter than 171.7 (140.0, 200.0) min in the control group ( P < 0.05). In the control group, the ASES and UCLA scores increased significantly from 29.1 (27.5, 31.4) points and 11.8 (10.0, 13.0) points before operation to 56.0 (51.9, 59.5) points and 24.6 (22.0, 27.0) points at 3-month follow-up, respectively, and the VAS pain score decreased significantly from preoperative 8.1 (7.0, 10.0) points to 2.0 (2.0, 2.0) points at 3-month follow-up ( P < 0.05). In the observation group, the ASES and UCLA scores increased significantly from 29.9 (29.1, 31.2) points and 12.6 (11.0, 14.0) points before operation to 58.8 (53.1, 62.2) points and 26.7 (25.0, 29.0) points at 3-month follow-up, respectively, and the VAS pain score decreased significantly from preoperative 8.0 (7.0, 9.0) points to 1.9 (2.0, 2.0) points at 3-month follow-up ( P < 0.05). At 3 months after operation, the UCLA score for the observation group was significantly better than that for the control group ( P < 0.05). There were no significant differences between the 2 groups in total length of incision, hemoglobin level disparity, ASES score or VAS pain score at 3 months after operation ( P > 0.05). Conclusion:In arthroscopic repair of large rotator cuff tear with suture-bridge techniques, application of our novel auxiliary sleeve can shorten operation time, simplify surgical procedures, and achieve good shoulder joint function and efficacy without increasing the length of surgical incision.
5.Meta-integration of real disease experience in young and middle-aged patients with end-stage renal disease

Objective:To systematically evaluate the disease experience and inner needs of young and middle-aged patients with end-stage renal disease, so as to provide evidence for medical personnel to pay more attention to the mental health of this population and give personalized care.Methods:The qualitative studies on the real experience, psychological feeling and experience of young and middle-aged patients with end-stage renal disease were searched by PsycInfo, PubMed, Embase, CINAHL, Web of Science, China National Knowledge Network, Wanfang, VIP and China Biomedical Literature Database. The retrieval period was from the establishment of the database to April 20, 2024. Joanna Briggs Institute Evidence-based Health Care Center Evaluation criteria (2016 edition) were used to evaluate the literature quality, and the results were summarized and integrated by aggregative meta-integration method.Results:A total of 10 studies were included, and 47 clear findings were extracted and classified into 10 new categories, which were combined into 4 integrated results: the phased changes of patients′ real experience at all levels of body, mind and society, impairment of their own and family role functions, desire for support and lack of autonomy in decision-making, and reconstruction of positive attitude and future outlook in the face of disease.Conclusions:Nursing staff should pay attention to and understand the psychosocial conditions of young and middle-aged patients with end-stage renal disease, timely provide targeted psychological support and help to enhance their confidence in treatment, better return to work and society, and improve their quality of life.

10.Prediction of characteristic chromatogram for Abri Herba based on network pharmacology and molecular docking

Objective To predict the potential Q-markers of Abri Herba based on network pharmacology and molecular docking and establish a quality control characteristic.Methods The network relationship of"Abri Herba-component-target-pathway"was constructed by using a variety of databases and the method of network pharmacology.The potential Q-markers of Abri Herba were predicted and then the characteristic Chromatogram of Abri Herba was established by high performance liquid chromatography Results Through the network pharmacological prediction,it was found that the components of abrine hypaphorine,schaftoside in Abri Herba were closely associated with the main targets,such as AKT1,STAT3,HIF1A,GRB2,MMP9,which could act on HIF-1,PI3K-Akt,JAK-STAT and other signaling pathways and have good pharmacological activities to be potential Q-markers of Abri Herba.Then HPLC was used to establish the characteristic according to retention time.Conclusion Through network pharmacology and molecular dock-prediction combined with HPLC detection,the characteristic chromatogram was established with the components of abrine hypaphorine,schaftoside as Q-markers,which could control the quality of Abri Herba by combining the components and pharmacological activities.
