1.Unveiling the metabolic fate of drugs through metabolic reaction-based molecular networking.

Effective annotation of in vivo drug metabolites using liquid chromatography-mass spectrometry (LC-MS) remains a formidable challenge. Herein, a metabolic reaction-based molecular networking (MRMN) strategy is introduced, which enables the "one-pot" discovery of prototype drugs and their metabolites. MRMN constructs networks by matching metabolic reactions and evaluating MS2 spectral similarity, incorporating innovations and improvements in feature degradation of MS2 spectra, exclusion of endogenous interference, and recognition of redundant nodes. A minimum 75% correlation between structural similarity and MS2 similarity of neighboring metabolites was ensured, mitigating false negatives due to spectral feature degradation. At least 79% of nodes, 49% of edges, and 97% of subnetworks were reduced by an exclusion strategy of endogenous ions compared to the Global Natural Products Social Molecular Networking (GNPS) platform. Furthermore, an approach of redundant ions identification was refined, achieving a 10%-40% recognition rate across different samples. The effectiveness of MRMN was validated through a single compound, plant extract, and mixtures of multiple plant extracts. 2.Development and validation of the Perceived Spouse Support Scale for pregnant women in the second and third trimesters

Objective To develop the Perceived Spouse Support Scale for pregnant women in the second and third trimester of pregnancy and test its reliability and validity,so as to provide an effective tool for assessing the perceived spouse support level of pregnant women.Methods With social support theory as the theoretical framework,the questionnaire item pool was initially constructed through literature review and semi-structured interview,and the initial questionnaire was formed through expert correspondence and cognitive interview.From April to November 2023,570 pregnant women in the second and third trimester of pregnancy were selected by random sampling method for investigation,item analysis and reliability and validity test of the scale.Results Eventually 558 pregnant women completed the suivey.The Perceived Spouse Support Scale for pregnant women in the second and third trimesters contained 22 items,including instrumental support,informational support and emotional support.The Cronbach's α coefficient of the scale was 0.867;the split-half reliability was 0.793;the retest reliability was 0.965;the content validity index was 0.980.The cumulative variance contribution rate of the 3 common factors was 54.138%.Confirmatory factor analysis suggested that the model fit well.Conclusion The Perceived Spouse Support Scale has good reliability and validity,and it can be used as an evaluation tool for spouse support level of pregnant women.
3.Exploration and Practice on Building a Lifelong Training System for Innovative Medical Talents:Taking Zhejiang Cancer Hospital As an Example

Cultivating innovative medical talents is a key strategy for enhancing a hospital's core competitiveness.Taking Zhejiang Cancer Hospital as a case study,this paper constructs a lifelong training system for innovative medical talents based on the"Triple-Phase Cultivation Framework(three stages,four integrations,five mechanisms)".This system delineates the cultivation cycle in-to three stages aligned with talent development trajectories:the"30s Start-up Phase"(young talents under 35),the"40s Growth Phase"(core professionals under 45),and the"50s Breakthrough Phase"(academic leaders under 50).It implements a four-dimensional cultivation model integrating medicine with research,education,engineering/informatics,and industry.Furthermore,a com-prehensive guarantee mechanism is established,encompassing policies & systems,platform re-sources,project drivers,financial investment,and talent teams.Practice demonstrates that this system has significantly enhanced talent development effectiveness.Over the past five years,the hospital has established 2 academician workstations and recruited 10 academician teams.The annual number of postdoctoral researchers has surged from 4 in 2020 to 58 in 2024.A total of 71 innova-tive talents have been selected across three batches,with 69 recognitions in provincial/ministerial-level talent programs,placing the hospital among the leaders within hospitals directly under the Zhejiang Provincial Health Commission.Notably,the hospital achieved a"zero breakthrough"in in-dependently cultivating national-level talents,including recipients of the National Ten-Thousand Talent Program(Young Top Talents)and the Postdoctoral Innovative Talent Support Program.The"Triple-Phase Cultivation Framework"establishes a replicable paradigm for nurturing innovative medical talents through its full-career-cycle approach,multidisciplinary integration,and systematic support mechanisms,offering valuable insights for talent strategies in specialized cancer hospitals.
4.YTHDF1 regulation of Fis1 on the activation and proliferation and migration ability of hepatic stellate cells

Objective:
To explore the effect of YTH domain family protein 1(YTHDF1) on the activation, proliferation and migration of hepatic stellate cells(HSCs) by regulating mitochondrial fission mediated by mitochondrial fission protein 1(Fis1).
Methods:
The mouse hepatic stellate cell line JS-1 was treated with 5 ng/ml TGF-β1 for 24 h to induce its activation and proliferation, andYTHDF1-siRNA was used to construct aYTHDF1silencing model.The experiment was divided into Control group, TGF-β1 group, TGF-β1+si-NC group and TGF-β1+si-YTHDF1 group.Expression changes ofYTHDF1,Fis1and key indicators of fibrosis, type Ⅰ collagen(CollagenⅠ) and α-smooth muscle actin(α-SMA) were detected through reverse transcription quantitative polymerase chain reaction(RT-qPCR) and Western blot; CCK-8 was used to detect cell proliferation ability; Transwell migration assay and cell scratch assay were used to detect cell migration ability; immunofluorescence staining experiment was used to detect the effect ofYTHDF1onFis1-mediated mitochondrial fission; finally, JC-1 staining was used to experimentally detect the effect ofYTHDF1on mitochondrial membrane potential.
Results:
Compared with the Control group, RT-qPCR and Western blot experimental results showed that the expression ofYTHDF1andFis1increased in the TGF-β1 group(P<0.05,P<0.01;P<0.000 1), as well as the fibrosis markersCollagenⅠand the expression level of α-SMA increased(P<0.01;P<0.001,P<0.000 1); while adding CCK-8, the experimental results showed that the proliferation ability of HSCs in the TGF-β1 group was enhanced(P<0.000 1); Transwell experimental results showed that the migration ability of HSCs in the TGF-β1 group was enhanced(P<0.01); the cell scratch experiment results showed that the migration ability of HSCs in the TGF-β1 group was enhanced(P<0.000 1); the immunofluorescence experiment results showed that the TGF-β1 group Mito-Tracker Red staining andFis1co-localization signal increased(P<0.05); JC-1 staining experiment results showed that the mitochondrial membrane potential increased in the TGF-β1 group(P<0.01). Compared with the TGF-β1+si-NC group, RT-qPCR and Western blot experimental results showed that the expression ofYTHDF1andFis1in the TGF-β1+si-YTHDF1 group was reduced(P<0.01;P<0.001), and fibrosis markers the levels ofCollagenⅠandα-SMAwere reduced(P<0.01;P<0.001,P<0.01).CCK-8 experimental results showed that the proliferation ability of HSCs in the TGF-β1+si-YTHDF1 group was weakened(P<0.000 1); Transwell experiment results showed that the migration ability of HSCs in the TGF-β1+si-YTHDF1 group was weakened(P<0.001); cell scratch experiment results showed that the migration ability of HSCs in the TGF-β1+si-YTHDF1 group was weakened(P<0.000 1); immunofluorescence experiment results showed that the Mito-Tracker Red staining andFis1co-localization signal decreased in the TGF-β1+si-YTHDF1 group(P<0.01); JC-1 staining experiment results showed that mitochondrial membrane potential decreased in the TGF-β1+si-YTHDF1 group(P<0.05).
Conclusion
YTHDF1promotes the activation, proliferation and migration capabilities of HSCs by positively regulatingFis1-mediated mitochondrial fission. This suggests thatYTHDF1may be a key gene involved in regulating the activation, proliferation and migration of HSCs.

7.Prognostic value of atherogenic index of plasma in elderly patients with acute ST-segment elevation myocardial infarction

Objective To investigate the prognostic value of atherogenic index of plasma(AIP)for the occurrence of major adverse cardiovascular events(MACE)in elderly patients with acute ST-segment elevation myocardial infarction(STEMI).Methods A total of 355 elderly patients with acute STEMI who received coronary interventional therapy in Department of Cardiology,Affilia-ted Hospital of Hebei University from January to May 2023 were recruited retrospectively,and fi-nally 343 of them with complete telephone follow-up data were included in this study.According to their AIP quartile level,they were divided into A1 group(<0.212,84 cases),A2 group(0.212-0.339,87 cases),A3 group(0.339-0.434,86 cases)and A4 group(≥0.434,86 cases).The incidences of cardiac death,nonfatal myocardial infarction,ischemia-driven target vessel re-modeling and heart failure re-hospitalization were observed during 1-year follow-up.Kaplan-Meier survival curve was plotted to compare the incidence of MACE in the 4 groups.ROC curve analysis was employed to determine the predictive value of AIP.Results During 1-year follow-up,signifi-cant differences were observed in the proportions of ischemia-driven target vessel revasculariza-tion,heart failure re-hospitalization and non-fatal acute myocardial infarction among the 4 groups(P<0.05,P<0.01),and such difference was also seen in the cumulative survival rate among them(log rankx2=8.528,P=0.036).Multivariate Cox proportional hazards regression analysis showed that gender,hypertension,atrial fibrillation,multi-vessel disease,left main artery disease,number of stents,SYNTAX score,Killip grade,BNP,HbA1c,TC,LDL-C and HDL-C levels,and AIP were independent predictors of MACE.The AUC value of AIP in predicting MACE in elderly patients with acute STEMI was 0.855(95%CI:0.776-0.933),with a sensitivity of 66.7%and a specificity of 93.0%.When the above indicators combined together,the AUC value was 0.907(95%CI:0.954-0.987),and the sensitivity and specificity was 100.0%and 90.7%,respectively.The AUC value of combined prediction was significantly better than that of single indicator(P<0.05).Conclusion AIP is a powerful biomarker,and can be used to predict the prognosis of elderly acute STEMI after coronary interventional therapy,and it combined with Killip grade,SYNTAX score,HbA1c,and number of stents shows better predictive efficacy.

9.Meta-synthesis of experience of heart failure patients using mobile medical App

Objective:To systematically analyze and integrate the psychological experience of heart failure patients using mobile medical applications (App) for self-management.Methods:Qualitative research on the self-management experience of heart failure patients using mobile medical App was retrieved through computer on PubMed, Web of Science, Embase, Cochrane Library, CINAHL, PsycINFO, Australian Joanna Briggs Evidence-Based Health Care Center Database, China National Knowledge Infrastructure, WanFang Data, VIP, and China Biomedical Literature Database. The search period was from the establishment of the database to December 30, 2022. The quality of literature was evaluated in accordance with the quality evaluation criteria for qualitative research of the Joanna Briggs Institute Evidence-Based Health Care Center (2016) . The results were integrated using the aggregative integration method.Results:A total of 14 articles were included, 32 research results were extracted, and 7 new categories were summarized. Finally, three integrated results were obtained (positive experience of heart failure patients using mobile medical App, risks and challenges of heart failure patients using mobile medical App, and preferences and expectations of heart failure patients towards mobile medical App) .Conclusions:The mobile medical App provides new ideas for self-management of heart failure patients, and its effectiveness is recognized by most heart failure patients, who perceive significant benefits. Future research should enrich the functionality and participation forms of the App based on the personalized needs of patients and their families, explore remote health management intervention models at home, and promote the promotion and application of mobile medical App.
10.Clinical Study on the Occlusal Change of Skeletal Class Ⅰ Malocclusion Treated with Invisible Appliance and Fixed Appliance

Objective To compare the clinical efficacy of invisible appliance and fixed appliance in the treatment of skeletal class I malocclusion.Methods A prospective cohort study with a minimum sample size of no less than 19 subjects in each group was used,and 46 adult patients with bone type I were eventually enrolled.According to the type of appliance,they were divided into the invisible appliance group(group A)with 25 patients and the fixed appliance group(group B)with 21 patients.Data were analyzed before orthodontic treatment(T0)after(T1)between invisible and traditional fixed orthodontics.Results The static occlusion index and PAR score were improved after treatment,and the differences were statistically significant(P<0.01).The occlusal dynamic parameters including OT,OFAT,OFPT,AOF and DT were significantly improved after treatment(P<0.01).There were significant differences in AOF,forward and lateral DT between the two groups(P<0.01),the invisible group was better than the fixed group.Conclusion Both the dynamic and static occlusal changes can be greatly improved with invisible and fixed appliance in the treatment of patients with class I malocclusion.Invisible appliance is better than fixed appliance in balancing occlusion,eliminating and reducing occlusal interference.
