1.Clinical analysis of periostinoma of the sinonasal glomangiopericytoma

OBGECTIVE To explore the clinical diagnosis and treatment of sinonasal glomangiopericytoma.METHODS A retrospective analysis was conducted on the clinical data of 6 cases of sinonasal glomangiopericytoma involving the nasal cavity and paranasal sinuses treated in our hospital from October 2010 to December 2024.The clinical features,imaging manifestations,histological characteristics,treatment and prognosis were analyzed in combination with the literature.RESULTS The main symptoms of the 6 patients were nasal congestion accompanied by varying degrees of epistaxis.CT scans showed well-defined masses.The pathological features were perivascular spindle cell proliferation.The positive rate of SMA in immunohistochemistry was 100%,and the nuclear positive rate of β-catenin was 83%.All 6 patients underwent surgical treatment.No recurrence was observed during the 1-year to 10-year postoperative follow-up.CONCLUSION Pericellular tumor of the nasal cavity and paranasal sinuses is a rare tumor with low malignant potential.It lacks clinical specificity.Pathology and immunohistochemistry are the gold standards for diagnosis.Surgical treatment is an effective means of treatment,with a good prognosis,but long-term follow-up is required to monitor recurrence.
2.Exploration of the application of vehicle-mounted 5G remote mobile robotic surgical system in thyroid surgery

Objective:To investigate the feasibility and safety of implementing a domestic vehicle-mounted remote mobile robotic surgical system in thyroid surgery applications, integrated with 5G communication technology.Methods:Using the main system located on the vehicle-mounted mobile robot operating platform of the 960th Hospital of PLA Joint Logistics Support Force and the slave system of Weifang Traditional Chinese Hospital, the remote radical thyroidectomy 5G communication technology, and analyze the clinical and information transmission data of two female patients who underwent remote mobile robot thyroid cancer surgery on October 21, 2024 at Weifang Traditional Chinese Medicine Hospital.Results:The remote radical thyroidectomy was conducted by the robosurgeons utilizing a vehicle-mounted mobile robotic surgical system, and the procedure was successfully completed without necessitating intermediate open surgery. The operation durations for patient 1 and patient 2 were 135 minutes and 108 minutes, respectively, with 7 and 13 lymph nodes dissected, respectively. The average delay in surgical data transmission was recorded at 61.9 milliseconds, with no instances of signal interruption or frame loss. The procedure proceeded smoothly, without any jamming, and the audio and video transmissions were consistently clear. Follow up for 21 days after surgery showed no complications such as hoarseness, skin damage, or lymphatic fistula.Conclusion:The implementation of a vehicle-mounted remote mobile robotic surgery system for thyroid surgery has demonstrated safety and feasibility. Furthermore, the utilization of the 5G network offers rapid data transmission and minimal latency, closely approximating the therapeutic efficacy of traditional robotic thyroidectomy.
3.Application of High-intensity focused ultrasound combined with chemotherapy as neoadjuvant and conversion therapy for advanced pancreatic cancer based on a multidisciplinary treatment model:a report of 4 cases

Pancreatic cancer is highly aggressive and often diagnosed at an advanced stage,leaving most patients ineligible for radical resection.This study retrospectively analyzed four patients with locally advanced or advanced pancreatic cancer to evaluate the clinical efficacy and safety of high-intensity focused ultrasound(HIFU)ablation combined with chemotherapy as a neoadjuvant and conversion therapy.All cases were reviewed and individualized treatment plans were formulated through a multidisciplinary team evaluation.All patients received HIFU plus gemcitabine and nab-paclitaxel chemotherapy,with assessments of tumor volume,vascular involvement,surgical conversion,symptom relief,and adverse events.Three patients achieved marked tumor shrinkage and reduction of vascular invasion,enabling successful R0 resection without recurrence during follow-up.The remaining patient achieved disease stability,significant pain relief,and maintained good quality of life under repeated HIFU therapy.All treatments were well tolerated,and no severe adverse reactions occurred.The combination of HIFU and chemotherapy demonstrated synergistic local and systemic effects,effectively achieving tumor downstaging,improving resectability,and alleviating symptoms.As a safe,noninvasive,and repeatable therapeutic approach,this strategy offers a promising option for patients with advanced pancreatic cancer.Further large-scale prospective studies are warranted to validate its long-term efficacy and elucidate underlying mechanisms.
4.Based on Metabolomics,the Mechanism of Qufengxiaoschonlein Decoction in the Treatment of Henoch-Schonlein Purpura was lnvestigated

Objective To study the regulatory effect of Qufeng Xiaodian Formula on serum differential metabolites of allergic purpura,and provide scientific basis for the diagnosis and treatment of allergic purpura in traditional Chinese medicine.Methods Sixty rats were randomly divided into a blank control group(referred to as the blank group),a model group,a compound glycyrrhizin group,a low-dose Qu Feng Xiao Dian Fang group,a medium dose Qu Feng Xiao Dian Fang group,and a high-dose Qu Feng Xiao Dian Fang group according to a random number table method,with 10 rats in each group.The model group was constructed by combining dried ginger,pepper,and long pepper with ovalbumin to create an allergic purpura rat model.After successful modeling,each treatment group was intervened with corresponding drugs for 4 weeks.After 4 weeks,serum was collected and non targeted metabolomics screening of serum differential metabolites was performed using ultra-high performance liquid chromatography quadrupole time-of-flight tandem mass spectrometer(UPLC-QTOF/MS).Subsequently,data extraction and multivariate statistical analysis will be conducted to identify potential metabolic pathways.Results Compared with the control group,there were 91 possible differential metabolites in the serum of the model group rats,corresponding to 20 metabolic pathways;Compared with the model group,there were a total of 43 possible differential metabolites in the serum of rats in the wind dispelling and disease eliminating group,corresponding to 15 metabolic pathways.Among them,there are a total of 12 metabolic pathways.Inflammatory metabolites such as arachidonic acid and ceramide can damage vascular endothelium.Ten biomarkers,including arachidonic acid and ceramide,were significantly abnormal in the serum of the model group rats compared to the normal group.The Qufeng Xiaodian formula can significantly reverse these metabolites and significantly enrich them in arachidonic acid metabolism and sphingolipid metabolism pathways.Conclusion Qufeng Xiaodian Formula has a certain regulatory effect on metabolites such as arachidonic acid and ceramide that affect vascular endothelial injury.

7.Clinical features and retrospective analysis of non-sebaceous gland lymphoma

OBJECTIVE To explore the clinical diagnosis and treatment strategies of non-sebaceous lymphoma by analyzing the clinical and pathological characteristics.METHODS The clinical data of 5 cases of non-sebaceous gland lymphoma diagnosed and treated in the First Affiliated Hospital,College of Clinical Medicine of Henan University of Science and Technology from January 2015 to October 2023 were retrospectively analyzed,and their clinical features,imaging findings,histological features,treatment and prognosis were analyzed in combination with the literature.RESULTS Among the 5 patients,2 cases occurred in the nasopharynx and 3 cases occurred in the parotid gland,all of which were treated by surgery in our department.No recurrence was observed in 1-9 years of postoperative follow-up.CONCLUSION Non-sebaceous gland lymphoma is a rare benign tumor occurring in nasopharynx.It has certain histological characteristics,and differential diagnosis is particularly important.Surgical resection is an effective means of treatment.
8.Differentiation and Treatment of Interstitial Cystitis/Bladder Pain Syndrome Based on the Theory of Sores and Ulcers

It is believed that the endoscopic manifestations of interstitial cystitis/bladder pain syndrome (IC/BPS) such as extensive bleeding and Hunner's ulcers as well as its recurrent and prolonged condition are in line with the characteristics of sores and ulcers in traditional Chinese medicine (TCM), and the treatment can be referred to TCM theory of sores and ulcers. The mechanism of IC/BPS is characterized by root deficiency and branch excess. The root cause is deficiency of the spleen and kidney qi, while the manifesting factors are dampness-heat and blood stasis. The treatment is based on the principles of promoting and supplementing, combining with resolving therapy, so that the symptoms could be relieved by promoting blood circulation and clearing heat, unblocking the collaterals and relieve pain. In clinical practice, modified Neituo Shengji Powder (内托生肌散) was commonly used to boost qi and invigorate blood, expel swollen sore and express toxin; combining with Sijunzi Decoction (四君子汤) or Suoquan Pill (缩泉丸) based on specific symptoms to strengthen the spleen and kidneys, thereby reinforcing healthy qi; according to concurrent symptoms like blood stasis, dampness heat, prescription with Sanleng (Cyperus rotundus), Ezhu (Curcuma aromatica Salisb) to invigorate blood and dissolve stasis, or Yiyiren (Coix lacryma-jobi), Zexie (Alisma plantago-aquatica) to clear heat and drain dampness.
9.Mechanism of Action of Coptidis Rhizoma and Ophiopogonis Radix in Delaying Diabetic Nephropathy Based on EGFR/PI3K/Akt Signaling Pathway

ObjectiveTo observe the effect of Coptidis Rhizoma and Ophiopogonis Radix on renal tissue injury and epidermal growth factor receptor (EGFR)/phosphatidylinositol 3-kinase (PI3K)/protein kinase B (Akt) signaling pathway in rats with diabetic nephropathy (DN) and explore its possible mechanism of delaying DN. MethodThirty-six male Wistar rats were randomly divided into a normal group (6 rats) and a model group (30 rats). The model group was fed with a high-fat and high-sugar diet combined with streptozotocin (STZ) to establish a rat model of type 2 diabetes. After the successful preparation of the model, the rats were randomly divided into the model group, low, medium, and high dose groups of Coptidis Rhizoma and Ophiopogonis Radix (100, 200, 400 mg·kg-1), and metformin group (200 mg·kg-1). After administration, the levels of fasting blood glucose (FBG), 24 h urine protein (24 h-UTP), creatinine (SCr), urea nitrogen (BUN), and uric acid (UA) were detected. Hematoxylin-eosin (HE) staining and Masson staining were used to observe the pathological changes of renal tissue in rats. Western blot and Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR) were used to detect the related protein expression of EGFR, PI3K, and Akt and their mRNA expression levels in the renal tissue of rats in each group. ResultsCompared with the normal group, the levels of FBG, SCr, BUN, UA, 24 h-UTP, and kidney index in the model group were significantly increased (P<0.01), most renal tubular epithelial cells were necrotic, and the content of collagen in glomeruli was significantly increased (P<0.01). Compared with the model group, the above indexes of rats in each administration group were improved to varying degrees. The FBG, SCr, BUN, UA, 24 h-UTP, and kidney index of rats in each dose group and metformin group were significantly decreased (P<0.01, P<0.05). The necrosis degree of renal tubular epithelial cells was reduced, and the fibrosis area was decreased (P<0.01). There related protein and mRNA expressions of EGFR, PI3K, and Akt were significantly increased (P<0.05, P<0.01). ConclusionCoptidis Rhizoma and Ophiopogonis Radix can alleviate renal tissue injury in rats with DN, and their mechanism may be related to the regulation of the EGFR/PI3K/Akt signaling pathway.
10.Acupuncture for overactive bladder in adults: Study protocol for a randomized controlled trial

Background:
Overactive bladder (OAB) is a prevalent condition that substantially degrades patient quality of life. Acupuncture is recognized as an effective therapeutic approach for various urological diseases. However, there is limited evidence validating the effectiveness and safety of acupuncture for OABs.
Objective:
To assess the therapeutic efficacy of acupuncture for OAB and to investigate the potential mechanisms by analyzing its effects on relevant urinary biomarkers.
Methods:
This is a randomized, participants and outcome assessors blinded, sham acupuncture controlled trial. A total of 110 patients with OABs will be randomly divided in a 1:1 ratio between the acupuncture and sham acupuncture groups. Participants in the acupuncture group will undergo 30-min authentic acupuncture, while their counterparts in the sham acupuncture group will undergo sham acupuncture needling non-acupoints superficially three times weekly for a duration of 8 weeks. The two co-primary outcomes will be the change in the mean number of micturitions per 24 h from baseline to the end of the 8-week treatment and 20-week follow-up. The secondary outcomes will encompass the change in Overactive Bladder Symptom Score, Overactive Bladder Questionnaire Short Form, and average 24 h values of urgency, daytime micturition, nocturia, and mean volume voided per micturition from baseline to weeks 8 and 20. Urinary nerve growth factor, brain-derived neurotrophic factor, and monocyte chemoattractant protein-1 levels will be measured at baseline and week 8. Adverse events will also be documented.
Discussion
The results of this trial will provide evidence for the effectiveness and safety of acupuncture in the management of OAB.
