1.Analysis of short-term efficacy of drug-coated balloon in the treatment of atherosclerotic renal artery stenosis

Objective To explore the feasibility,safety and efficacy of paclitaxel drug-coated balloon(DCB)in the treatment of atherosclerotic renal artery stenosis(ARAS).Methods A total of 28 patients with ARAS were selected.Balloon angioplasty was performed using a paclitaxel DCB at the site of renal artery stenosis(RAS)in these patients.Subsequently,a follow-up study was conducted to monitor various parameters of the patients,including vascular restenosis,blood pressure,the types of antihypertensive medications and renal function.The feasibility,efficacy and safety of balloon angioplasty using DCB in the treatment of ARAS were analyzed.Results Twenty-eight patients underwent 33 DCB balloon angioplasty.In one patient,there was no significant improve-ment in the degree of RAS,and thus further treatment with renal artery stenting was administered.The remaining patients all achieved both anatomical and hemodynamic success,with the degree of vascular stenosis at the lesion site decreasing from(79.74±5.13)%to(8.32±4.67)%,and the surgical success rate was 96.97%.The systolic/diastolic blood pressure of the patients was(179.16±30.65)mmHg/(108.26±20.93)mmHg(1 mmHg=0.133 kPa)at 24 hours postoperatively,(131.11±12.99)mmHg/(80.11±7.12)mmHg at 3 months postoperatively,(134.16±11.37)mmHg/(78.68±4.79)mmHg at 6 months postoperatively,and(133.37±12.71)mmHg/(80.11±4.84)mmHg at 12 months postoperatively.In comparison with the preoperative blood pressure of(184.63±27.64)mmHg/(109.11±22.26)mmHg,there was no significant decrease in blood pressure at 24 hours postoperatively,and the difference was not statistically significant.However,at 3,6,and 12 months postoperatively,the patients'blood pressure was signif-icantly lower than that before the operation,and all the differences were statistically significant.The glomerular filtration rate(GFR)was(36.19±18.32)mL/min at 24 hours postoperatively,(35.96±18.51)mL/min at 3 months postoperatively,(36.23±19.30)mL/min at 6 months postoperatively,and(35.59±18.26)mL//min at 12 months postoperatively,which all elevated compared with the preoperative GFR of(28.31±14.67)mL/min,and the differences were statistically significant(P<0.05).At 3,6,and 12 months postoperatively,the vascular patency rate was 100%as indicated by multifunctional color Doppler ultrasound examination or renal artery computed tomography angiography(CTA).No relevant com-plications and postoperative adverse events,such as renal artery rupture or dissection,renal artery thrombosis and acute renal insuffi-ciency,occurred in all 28 patients.Conclusion The paclitaxel DCB is safe and reliable for the treatment of ARAS and has remarkable curative effects,and it can be used as an effective vascular treatment regimen for ARAS.
2.Clinical application effect of Paris polyphylla var. yunnanensis oral care solution in the prevention of ventilator-associated pneumonia in patients with endotracheal intubation in ICU

Objective:To investigate the clinical effect of Paris polyphylla var. yunnanensis oral care solution in the prevention of ventilator-associated pneumonia (VAP) in patients with endotracheal intubation in ICU, and to provide reference for continuing to explore the best oral care solution of traditional Chinese medicine for preventing VAP. Methods:Randomized controlled experiment was used. Fifty patients with tracheal intubation admitted to the ICU of the Affiliated Hospital of Yunnan University from May 2022 to February 2023 were selected by convenient sampling method. The patients were divided into control group and observation group with 25 cases in each group by random number table method. The control group was treated with Xipayi mouth rinse for oral care, and the observation group was treated with self-made Paris polyphylla var. yunnanensis oral care solution for oral care. The incidence of VAP, clinical efficacy of oral care, leukocyte and procalcitonin after 7 days of intervention were observed, the mechanical ventilation time and ICU hospitalization time were recorded in the two groups. Results:All patients completed the study. There were 19 males and 6 females in the control group with the age of (60.72±19.32) years old. There were 15 males and 10 females in the observation group with the age of (60.24±16.80) years old. The incidence of VAP after 7 days of intervention in the observation group was 12% (3/25), which was lower than that in the control group 36% (9/25). The difference between the two groups was statistically significant ( χ2=3.95, P<0.05). The total clinical effective rate of oral care after 7 days of intervention in the observation group was 88% (22/25), which was higher than that in the control group 68% (17/25). The difference between the two groups was statistically significant ( χ2=9.78, P<0.01). There was no significant difference in the level of the leucocyte and procalcitonin before and after 7 days of intervention between the two groups (all P>0.05). The mechanical ventilation time and ICU hospitalization time in the observation group were (6.92 ±3.89), (10.36±6.17) days respectively, which were lower than those in the control group (9.72±4.76), (12.12±4.87) days. The difference of mechanical ventilation time between the two groups was statistically significant (t=2.28, P<0.05). The ICU hospitalization time in the observation group was lower than that in the control group, but there was no significant difference between the two groups ( P>0.05). Conclusions:Paris polyphylla var. yunnanensis oral care solution can improve the oral condition of patients, reduce the incidence of VAP, shorten the time of mechanical ventilation, which has certain clinical value.
3.Paramyotonia congenita in a family with SCN4A gene mutation

Objective:To report the clinical, electrophysiological, and genetic mutation characteristics of a family with paramyotonia congenita, to enhance clinical understanding of ion channelopathies, provide better guidance for clinical diagnosis, and optimize treatment plans.Methods:The clinical data, nerve conduction studies, needle electromyography, long exercise test, short exercise test, and whole-exome sequencing results of the proband with paramyotonia congenita and his family members admitted to Ruijin Hospital, Shanghai Jiao Tong University School of Medicine, in August 2023 were collected and analyzed.Results:The proband was a 15-year-old male who presented with episodic muscle weakness for 1 year, accompanied by elevated creatine kinase during episodes. Between the ages of 3 and 4, he exhibited facial and bilateral hand myotonia triggered by cold exposure. His mother, sister, maternal grandmother, maternal aunt and uncle also displayed similar cold-induced facial and hand myotonia during childhood. Additionally, his cousin exhibited walking difficulties, frequent falls, mild difficulty in opening eyes after closure, and mild chewing and swallowing difficulties at the age of 1. Electromyography of the proband showed widespread myotonic discharges and myopathic impairment, while the electromyography of his mother and sister revealed only widespread myotonic discharges. The proband′s long exercise test results were consistent with periodic paralysis, while the short exercise test results aligned with paramyotonia congenita. His sister′s long exercise test results were within normal range, while the short exercise test results of his mother and sister were consistent with paramyotonia congenita. Genetic testing of the family revealed that all tested patients carried a heterozygous SCN4A mutation, c.4343G>A (R1448H). Conclusions:The R1448H mutation in the SCN4A gene is the genetic cause of this family. Patients carrying the SCN4A R1448H mutation may exhibit clinical features of cold-induced myotonia with or without episodic muscle weakness. In addition to widespread myotonic discharges on electromyography, certain patients may also show myopathic impairment. The long and short exercise tests can further assist in differentiating various types of myotonia and periodic paralysis for clinical diagnosis.
4.Correlations of serum levels of soluble vascular cell adhesion molecule-1,homocysteine and D-dimer with inner ear microcirculation disorders in patients with sudden sensorineural hearing loss

Objective To investigate the correlations of serum levels of soluble vascular cell ad-hesion molecule-1(sVCAM-1),homocysteine(Hcy)and D-dimer(D-D)with inner ear microcircu-lation disorders in patients with sudden sensorineural hearing loss.Methods Eighty-four patients with sudden deafness were selected as subjects.Patients were divided into mild(17 cases),moderate(31 cases)and severe(36 cases)groups based on the degree of hearing loss.According to the types of audiogram maps,they were divided into low-frequency group(22 cases),high-frequency group(33 cases)and full-frequency group(29 cases).Forty-two healthy individuals undergoing physical examination during the same period served as healthy group.The levels of sVCAM-1,Hcy as well as D-D and inner ear microcirculation arterial diameters were compared among different audiogram types and degrees of hearing loss.Pearson correlation analysis and linear regression analysis were em-ployed to verify the correlations between the aforementioned serum indicators and arterial diameters in inner ear microcirculation of affected patients.Results Compared with the healthy group,the diameters of the internal auditory artery,basilar artery,and anterior inferior cerebellar artery in the inner ear microcirculation were significantly reduced in the low-frequency type,high-frequency type and full-frequency type groups,with the high-frequency and full-frequency types being significantly smaller than the low-frequency type,and the full-frequency type being significantly smaller than the high-frequency type(P<0.05).Serum levels of sVCAM-1,Hcy and D-D were significantly higher in the low-frequency type,high-frequency type and full-frequency type groups compared with the healthy group,with the full-frequency type being significantly higher than the low-frequency and high-frequency types(P<0.05).Serum levels of sVCAM-1,Hcy and D-D were significantly high-er in the mild,moderate and severe groups compared with the healthy group,with the severe group being significantly higher than the mild and moderate groups(P<0.05).The diameters of the in-ternal auditory artery,basilar artery and anterior inferior cerebellar artery in the inner ear microcir-culation were significantly smaller in the mild,moderate and severe groups compared with the healthy group(P<0.05).Compared with the mild group,the diameters of the internal auditory ar-tery,basilar artery and anterior inferior cerebellar artery were significantly reduced in the moderate and severe groups,with the severe group being significantly smaller than the moderate group(P<0.05).Serum levels of sVCAM-1,Hcy and D-D showed negative correlation with the diameters of the internal auditory artery,basilar artery and anterior inferior cerebellar artery(P<0.05).Arterial di-ameters of the internal auditory artery,basilar artery and anterior inferior cerebellar artery were inde-pendently correlated with serum levels of sVCAM-1,Hcy and D-D(P<0.05).Conclusion Serum levels of sVCAM-1,Hcy and D-D are closely related to inner ear microcirculation disorders in pa-tients with sudden deafness.As serum levels of sVCAM-1,Hcy and D-D increase,the severity of hearing loss and inner ear microcirculation disorder intensifies.
5.Narrow band imaging-guided microscopic CO2 laser surgery versus conventional microscopic CO2 laser surgery in treatment of vocal cord atypical hyperplasia

Objective To compare the clinical efficacy of narrow band imaging(NBI)-guided microscopic CO2 laser surgery and conventional microscopic CO2 laser surgery in treatment of patients with vocal cord atypical hyperplasia.Methods A total of 90 patients with vocal cord atypical hyper-plasia were enrolled as study subjects.Based on different surgical approaches,they were divided into conventional group(43 cases,undergoing conventional microscopic CO2 laser surgery)and NBI-guided group(47 cases,undergoing NBI-guided microscopic CO2 laser surgery).The negative mar-gin rate,complication rate,recurrence rate,and the levels of voice-related parameters(jitter,shim-mer,noise-to-harmonic ratio)and inflammatory factors[C-reactive protein(CRP),interleukin-6(IL-6)]before and after surgery were compared between the two groups.Results The negative mar-gin rate of the first-time biopsy after laser surgery in the NBI-guided group was 97.87%,which was higher than 86.05%in the conventional group,with a statistically significant difference(P<0.05).After surgery,the jitter,shimmer and noise-to-harmonic ratio in both groups were lower than those before surgery,with statistically significant differences(P<0.05),but there was no statistically sig-nificant difference between the two groups(P>0.05).After surgery,the levels of CRP and IL-6 in both groups were lower than those before surgery,with statistically significant differences(P<0.05),but there was no statistically significant difference between the two groups(P>0.05).The compli-cation rate was 4.26%in the NBI-guided group,and 9.30%in the conventional group,with no statistically significant between-group difference(P>0.05).During a follow-up of 6 months to 2 years,the recurrence rate in the NBI-guided group was 2.13%and 6.98%in the conventional group,with no statistically significant between-group difference(P>0.05).Conclusion Com-pared with conventional microscopic CO2 laser surgery,NBI-guided microscopic CO2 laser surgery can more precisely determine the range of the lesion and the surrounding mucosal margins and re-duce the number of intraoperative biopsies in the treatment of vocal cord atypical hyperplasia.How-ever,the two surgical methods have similar impacts on postoperative voice-related parameters,in-flammatory factors,complications and recurrence rates.

9.Quality evaluation of Jingtian granule based on fingerprint combined with chemical pattern recognition

OBJECTIVE To establish the ultra-high performance liquid chromatography(UPLC)fingerprint of Jingtian granule,and to evaluate its quality by chemical pattern recognition.METHODS Luna? Omega Polar C18 column(150 mmX2.1 mm,1.6 μm)was used as the chromatographic column,and acetonitrile-0.2%phosphoric acid solution was used as the mobile phase for gradient elution.The flow rate was 0.2 mL/min,the column temperature was 30 ℃,and the detection wavelength was 265 nm.With peak 16 as the reference peak,the UPLC fingerprint of Jingtian granule was established by the Similarity Evaluation System of Chromatographic Fingerprint of Traditional Chinese Medicine(2012 edition).The common peaks were identified,the similarity evaluation was carried out,and the ownership of each common peak was confirmed.Hierarchical cluster analysis(HCA)and principal component analysis(PCA)in chemical pattern recognition methods were used to classify 13 batches of samples(S1-S13),and orthogonal partial least squares-discriminant analysis(OPLS-DA)was used to identify the key components of the differences between different batches of samples.RESULTS RSDs of precision,repeatability and stability of the UPLC method were not more than 4.4%.A total of 25 common peaks were identified in the fingerprints of 13 batches of Jingtian granules.By comparing with the reference substance fingerprint,10 common peaks were identified,namely peak 3(5-hydroxymethyl-2-furaldehyde),peak 5(salidroside),peak 8(chlorogenic acid),peak 15(cinnamic acid),peak 19(aloe-emodin),peak 20(ammonium glycyrrhizinate),peak 21(rhein),peak 23(emodin),peak 24(glycyrrhetinic acid),peak 25(chrysophanol).The similarities of fingerprints of 13 batches of samples were 0.955-0.996.The results of HCA showed that 13 batches of samples could be divided into three categories,among which samples S1,S5,S7,S11-S13 were clustered in one category,S4 and S6 were clustered in one category,S2,S3 and S8-S10 were clustered in one category.PCA results showed that the cumulative variance contribution rate of principal components 1-7 was 92.666%.OPLS-DA further identified 13 differential components,which were mainly derived from Polygonati Rhizoma with wine steaming,Rhodiolae Crenulatae Radix Et Rhizoma,prepared Rhei Radix Et Rhizoma and Glycyrrhizae Radix Et Rhizome Praeparata Cum Melle.CONCLUSIONS The established UPLC fingerprint of Jingtian granule is simple,stable and reproducible.Combined with the chemical pattern recognition method,it can effectively reveal the overall quality difference between different batches of Jingtian granule.The quality of Polygonati Rhizoma with wine steaming,Rhodiolae Crenulatae Radix Et Rhizoma,prepared Rhei Radix Et Rhizoma,Dioscoreae Nipponicae Rhizoma,Polyporus,Cinnamomi Ramulus,Glycyrrhizae Radix Et Rhizome Praeparata Cum Melle is the key to the overall quality of Jingtian granule.
10.Interaction between fibroblasts and keratinocytes in the wound edge skin tissue of a diabetic foot patient and the mechanism

Objective:To investigate the interaction between fibroblasts (Fb) and keratinocytes (KC) in the wound edge skin tissue of a diabetic foot patient and the mechanism.Methods:This was an experimental research. The wound edge skin tissue from a diabetic foot patient (male and 33 years old) admitted to the Department of Wound Repair of Liyuan Hospital Affiliated to Tongji Medical College of Huazhong University of Science and Technology in August 2021 and from an acute foot injury patient (male and 50 years old) admitted to the Department of Hand Surgery of the hospital in September 2021 was collected. The single-cell transcriptome sequencing was performed to analyze the interaction between chemokine ligands of Fb subgroup and chemokine receptors of KC subgroup. The supernatant was collected after human foreskin fibroblast (HFF) was cultured routinely and with high concentration of glucose for 7 days as normal conditioned medium (CM) and high glucose CM, respectively. HaCaT cells were collected and divided into normal CM group cultured with normal CM and high glucose CM group cultured with high glucose CM, the scratch test was performed to calculate the cell migration rates at 24 and 48 h after scratch ( n=3). The content of cytokines in the two kinds of CM was detected by liquid suspension chip ( n=5). HFF was collected and divided into normal group cultured routinely and high glucose group cultured with high concentration of glucose for 7 days, and the mRNA expressions of C-X-C motif chemokine ligand 1 (CXCL1), CXCL2, CXCL8, and CXCL12 were detected by real-time fluorescence quantitative reverse transcription polymerase chain reaction ( n=6). HaCaT cells in normal CM group and high glucose CM group were collected to detect the protein expressions of C-X-C motif chemokine receptor 4 (CXCR4) in cells cultured for 48 h by Western blotting ( n=3). HaCaT cells were collected and divided into normal CM group, high glucose CM group, normal CM+CXCL12 group, and high glucose CM+CXCL12 group. The first two groups of cells were treated as before, and the latter two groups of cells were cultured with normal CM and high glucose CM containing recombinant human CXCL12, respectively. Scratch test was performed, and cell migration rates were calculated at 24 and 48 h after scratch ( n=3); the protein expression of CXCR4 in cells cultured for 48 h was detected by Western blotting ( n=3). Results:Compared with those in the wound edge skin tissue of acute foot injury, the interactions between chemokine ligands (CXCL1, CXCL2, CXCL3, CXCL8, and CXCL12) of Fb subgroup and chemokine receptors (CXCR2 and CXCR4) of KC subgroup were significantly weakened in the wound edge skin tissue of diabetic foot. At 24 and 48 h after scratch, the migration rates of HaCaT cells in high glucose CM group were significantly lower than those in normal CM group (with t values of 23.50 and 15.65, respectively, P<0.05). Compared with that in normal CM, the content of CXCL1 in high glucose CM was significantly increased ( P<0.05), and the content of CXCL12 was significantly decreased ( P<0.05). After 7 days of culture, compared with those in normal group, the mRNA expressions of CXCL1, CXCL2, and CXCL8 in HFF in high glucose group were significantly increased (with t values of 4.25, 4.98, and 10.04, respectively, P<0.05), while the mRNA expression of CXCL12 was significantly decreased ( t=4.10, P<0.05). After 48 h of culture, the CXCR4 protein expression in HaCaT cells in high glucose CM group was significantly lower than that in normal CM group ( t= 5.13, P<0.05). At 24 and 48 h after scratch, the migration rates of HaCaT cells in high glucose CM group were significantly lower than those in normal CM group and high glucose CM+CXCL12 group (with P values all <0.05); at 24 h after scratch, the migration rate of HaCaT cells in normal CM+CXCL12 group was significantly lower than that in normal CM group ( P<0.05); at 48 h after scratch, the migration rate of HaCaT cells in normal CM+CXCL12 group was significantly higher than that in high glucose CM+CXCL12 group ( P<0.05). At 48 h of culture, the CXCR4 protein expression of HaCaT cells in high glucose CM+CXCL12 group was 0.446±0.050, which was significantly higher than 0.247±0.010 in high glucose CM group ( P<0.05) and similar to 0.522±0.082 in normal CM+CXCL12 group ( P>0.05); the CXCR4 protein expression in HaCaT cells in normal CM group was 0.509±0.055, which was significantly higher than that in high glucose CM group ( P<0.05). Conclusions:The interactions between chemokine ligands of Fb subgroup and chemokine receptors of KC subgroup were significantly weakened in the wound edge skin tissue of diabetic foot. High glucose can inhibit CXCL12 secretion of HFF, and the stimulation of its cell culture supernatant can decrease HaCaT cell migration ability and CXCR4 expression. Exogenous CXCL12 protein can increase the CXCR4 protein expression in HaCaT cells and enhance the cell migration ability.
