1.Effects of Modified shaoyao gancao decoction on intestinal transit function,intestinal flora and metabolite content in slow transit constipation rat

OBJECTIVE To observe the effects of Modified shaoyao gancao decoction on intestinal transit function, intestinal flora and the contents of metabolites [γ aminobutyric acid (GABA) and 5-hydroxytryptamine (5-HT)] in slow transit constipation (STC) rats. METHODS SD rats were randomly divided into blank group (10 rats) and modeling group (30 rats), with half male and half female. The STC model was established by intragastric administration of Compound diphenoxylate tablets in the modeling group. The successfully modeled rats were randomly divided into model group, Modified shaoyao gancao decoction group [56 g/(kg·d), calculated by crude drug] and positive control group [lactulose 2.09 g/(kg·d)], with 10 rats in each group. Each administration group was given relevant medicine intragastrically, the blank group and model group received an equivalent volume of normal saline, once a day, for 14 consecutive days. During the experiment, the general situation of rats was observed in each group. After the last medication, the body weight was measured, and the Bristol score was used to evaluate the fecal characteristics. The fecal moisture content, intestinal propulsion rate, and the contents of GABA and 5-HT in intestinal content were detected; the diversity of intestinal flora in intestinal contents was investigated, and the correlation between the contents of GABA, 5-HT and relative abundance of microbiota was analyzed. RESULTS Compared with the model group, general conditions such as small body shape, sparse and rough fur, and slow movement were all improved in Modified shaoyao gancao decoction body weight, Bristol score, fecal moisture content,intestinal propulsion rate, 5-HT content, Chao1 index and Shannon index were increased significantly, while GABA content and Simpson index were decreased significantly (P<0.05). The intestinal flora of rats in the Modified shaoyao gancao decoction group could be classified as the same as the blank group, but was far from the model group; the relative abundances of Desulfobacterota, Firmicutes and Bacteroidota in this group showed a tendency of pull back, but the differences were not statistically significant compared to model group (P>0.05). Desulfobacterota was an intergroup differential factor (P<0.05). The content of GABA was negatively correlated with the relative abundance of Bacteroidota, Cyanobacteria, Patescibacteria and Actinobacteriota (P<0.05). The content of 5-HT was positively correlated with the relative abundance of Campilobacterota (P<0.05). CONCLUSIONS Modified shaoyao gancao decoction can improve the fecal properties and intestinal motility of STC rats. Its mechanism may be related to improving intestinal flora and then affecting the contents of GABA and 5-HT in intestinal contents. In addition, the contents of GABA and 5-HT may be significantly correlated with the relative abundance of specific bacterial phyla such as Bacteroidota and Campilobacterota.

3.Targeting ceramide-induced microglial pyroptosis: Icariin is a promising therapy for Alzheimer's disease.

Alzheimer's disease (AD), a progressive dementia, is one of the most common neurodegenerative diseases. Clinical trial results of amyloid-β (Aβ) and tau regulators based on the pretext of straightforward amyloid and tau immunotherapy were disappointing. There are currently no effective strategies for slowing the progression of AD. Herein, we spotlight the dysregulation of lipid metabolism, particularly the elevation of ceramides (Cers), as a critical yet underexplored facet of AD pathogenesis. Our study delineates the role of Cers in promoting microglial pyroptosis, a form of programmed cell death distinct from apoptosis and necroptosis, characterized by cellular swelling, and membrane rupture mediated by the NLRP3 inflammasome pathway. Utilizing both in vivo experiments with amyloid precursor protein (APP)/presenilin 1 (PS1) transgenic mice and in vitro assays with BV-2 microglial cells, we investigate the activation of microglial pyroptosis by Cers and its inhibition by icariin (ICA), a flavonoid with known antioxidant and anti-inflammatory properties. Our findings reveal a significant increase in Cers levels and pyroptosis markers (NOD-like receptor family, pyrin domain containing 3 (NLRP3), apoptosis-associated speck-like protein containing a caspase recruitment domain, caspase-1, gasdermin D (gasdermin D (GSDMD)), and interleukin-18 (IL-18)) in the brains of AD model mice, indicating a direct involvement of Cers in AD pathology through the induction of microglial pyroptosis. Conversely, ICA treatment effectively reduces these pyroptotic markers and Cer levels, thereby attenuating microglial pyroptosis and suggesting a novel therapeutic mechanism of action against AD. This study not only advances our understanding of the pathogenic role of Cers in AD but also introduces ICA as a promising candidate for AD therapy, capable of mitigating neuroinflammation and pyroptosis through the cyclooxygenase-2 (COX-2)-NLRP3 inflammasome-gasdermin D (GSDMD) axis. Our results pave the way for further exploration of Cer metabolism disorders in neurodegenerative diseases and highlight the therapeutic potential of targeting microglial pyroptosis in AD.
4.Expression and prognostic value of serum VitD,FRT and HB-EGF in sep-sis patients

Objective To explore and analyze the expression and prognostic value of serum vitamin D(VitD),fer-ritin(FRT)and heparin-binding epidermal growth factor(HB-EGF)in sepsis patients.Methods 86 sepsis patients who were admitted to the intensive care unit(ICU)of a hospital from January 2021 to January 2022 were selected as the case group,and 60 non-sepsis patients in the ICU were selected as the control group.According to the prognosis of sepsis patients one month later,patients were divided into survival group and death group.Patient's serum upon admission was taken,levels of serum VitD,FRT and HB-EGF were detected,the correlation with the prognosis of sepsis patients was analyzed,and the prognostic value was evaluated by area under curve(AUC)of receiver opera-ting characteristic curve.Results Levels of white blood cell count(WBC),C-reactive protein(CRP),procalcitonin(PCT),tumor necrosis factor-α(TNF-α),interleukin(IL)-6,IL-1β and FRT of case group were all higher than those of non-sepsis patients in the control group,while the VitD and HB-EGF levels were lower than those of non-sepsis patients in the control group,differences were all statistically significant(all P<0.05).One month follow-up on the prognosis of sepsis patients showed that 55 patients survived and 31 died.Acute physiology and chronic health evaluationⅡ(APACHE IⅡ)score,sequential organ failure assessment(SOFA)score,PCT,TNF-α,L-1βand FRT score in patients in the death group were all higher than those in patients in the survival group,while VitD and HB-EGF were lower than patients in the survival group,differences were all statistically significant(all P<0.05).The Pearson correlation analysis results showed that VitD were negatively correlated with APACHE Ⅱ score,SOFA score,WBC,CRP,PCT and TNF-α(all P<0.05),HB-EGF was negatively correlected with APACHE Ⅱ,score,CRP,PCT,TNF-α,IL-6 and IL-1β(all P<0.05);while FRT was positively correlated with APACHE Ⅱscores,CRP,PCT,TNF-α,IL-6 and IL-1β(all P<0.05).The AUC,sensitivity,and specificity of combined de-tection of serum VitD,FRT and HB-EGF in predicting the prognosis of sepsis patients were 0.82(95%CI:0.72-0.86),84.39%,and 69.35%,respectively.Conclusion Serum levels of VitD and HB-EGF are lower and FRT is higher in sepsis patients,their expression levels are closely related to patient prognosis,and have good predictive value for predicting the prognosis of sepsis patients.
5.The mediating effect of psychological resilience and empathy on the relationship between job stress and job burnout among clinical medical graduate students

ObjectiveTo explore the mediating effect of psychological resilience and empathy on the relationship between job stress and job burnout in clinical medical graduate students. Methods A total of 437 graduated students of clinical medicine or related majors were selected as the research subjects using a stratified sampling method. The Connor-Davidson Resilience Scale, Interpersonal Reactivity Index, Perceived Stress Scale, and Maslach Burnout Inventory were used to measure their psychological resilience, empathy, job stress, and job burnout. Results The average scores for psychological resilience, empathy, job stress, and job burnout among the research subjects were (90.2±13.4), (93.8±10.1), (53.7±12.2) and (47.3±12.6), respectively. The detection rate of job burnout was 37.5%. The results of the structural equation model showed that psychological resilience and empathy partially mediated the relationship between job stress and job burnout, with psychological resilience accounting for 13.6% and empathy accounting for 41.3% of the mediating effect. Conclusion Psychological resilience and empathy play a significant mediating role in the relationship between job stress and job burnout among clinical medical graduate students.
6.Progress in intrathecal therapy for solid tumor meningeal metastasis

Solid tumor meningeal metastasis is a catastrophic complication in the late stage of tumors,with a poor prognosis and currently lacking effective treatment methods.As a treatment method for meningeal metastasis,intrathecal chemotherapy has been recommended by many guidelines or expert consensus,and is often used in clinical practice.However,the efficacy of intrathecal chemotherapy has always lacked high-quality research evidence to support it.The efficacy of commonly used intrathecal chemotherapy drugs such as methotrexate,cytarabine,and thiotepa is not ideal.This article summarizes the usage,efficacy,and safety of new intrathecal injection drugs.
7.Two cases of neonatal Legionella pneumonia

Patient 1,a 12-day-old female infant,presented with fever,cough,dyspnea,and elevated infection markers,requiring respiratory support.Metagenomic next-generation sequencing(mNGS)of blood and bronchoalveolar lavage fluid revealed Legionella pneumophila(LP),leading to diagnoses of LP pneumonia and LP sepsis.The patient was treated with erythromycin for 15 days and azithromycin for 5 days,resulting in recovery and discharge.Patient 2,an 11-day-old female infant,presented with dyspnea,fever,elevated infection markers,and multiple organ dysfunction,requiring mechanical ventilation.mNGS of blood and cerebrospinal fluid indicated LP,leading to diagnoses of LP pneumonia,LP sepsis,and LP intracranial infection.The patient was treated with erythromycin for 19 days and was discharged after recovery.Neonatal LP pneumonia lacks specific clinical symptoms,and azithromycin is the preferred antimicrobial agent.The use of mNGS can provide early and definitive diagnosis for severe neonatal pneumonia of unknown origin.

The paper introduces professor ZHUANG Li-xing's clinical experience in treatment of dyskinesia of Parkinson's disease with acupuncture at triple-acupoint prescription. In pathogenesis, dyskinesia of Parkinson's disease refers to yang deficiency and disturbing wind. In treatment, acupuncture focuses on warming yang, promoting the circulation of the governor vessel, regulating the spirit and stopping trembling; and Baihui (GV 20), Suliao (GV 25) and Dingchanxue (Extra) are selected to be "trembling relief needling". In combination with Jin's three needling, named "three-trembling needling" "three-governor-vessel needling" and "three-spasm needling", the triple-acupoint prescription is composed. To ensure the favorable therapeutic effect, this prescription is modified according to the symptoms and the specific techniques of acupuncture are combined such as conducting qi, harmonizing yin and yang, and manipulating gently for reinforcing and reducing.

9.Research progress on chemical constituents and pharmacological activities of Viola plants.

There are 500 species of Viola(Violaceae) worldwide, among which 111 species are widely distributed in China and have a long medicinal history and wide varieties. According to the authors' statistics, a total of 410 compounds have been isolated and identified from plants of this genus, including flavonoids, terpenoids, phenylpropanoids, organic acids, nitrogenous compounds, sterols, saccharides and their derivatives, volatile oils and cyclotides. The medicinal materials from these plants boast anti-microbial, anti-viral, anti-oxidant and anti-tumor activities. This study systematically reviewed the chemical constituents and pharmacological activities of Viola plants to provide a basis for further research and clinical application.

10.The effects of nucleotide-binding oligomerization domain-like receptor 3 inflammasome on obstructive sleep apnea and its complications.

Obstructive sleep apnea (OSA) is a sleep disorder with a high incidence and severe impact on the human body, which can induce systemic chronic inflammatory responses. Chronic inflammation is an important cause of exacerbation of OSA and its associated complications. Nucleotide-binding oligomerization domain-like receptor 3 (NLRP3) is an inflammasome that is widely found in epithelial cells and immune cells and plays an important role in inflammatory diseases as an important component of innate immunity. Research evidence suggests that the activation of NLRP3 inflammasomes can exacerbate the damage to neurons, endothelial cells, lung and kidney caused by OSA, and these effects can be eliminated by genetic or pharmacological deletion of NLRP3. Targeting inhibition of NLRP3 inflammasome may serve as a co-therapeutic strategy for OSA-induced related complications. This article reviews NLRP3 inflammasome and its mechanism in OSA-related concurrent diseases, which can provide scientific basis for prevention and intervention of OSA and its related complications.
