1.Construction of A Conceptual Framework for the Integration of Traditional Chinese and Western Medicine in Evolutionary Syndrome Differentiation and Treatment Across Full-cycle of Parkinson's Disease

Parkinson's disease (PD) is a complex neurodegenerative disease involving multiple systems and neurotransmitters. Due to the high clinical heterogeneity of PD,it is urgent to establish a comprehensive and long-term traditional Chinese medicine (TCM) management model. In this paper,the conceptual framework of full-cycle management of PD is preliminarily constructed:based on the evolution of the pathophysiological mechanisms of protein deposition and neurotransmitter disorder in PD,the three-stage syndrome characteristics of the prodromal stage (predominant healthy Qi with subtle pathogenic factors),the early clinical stage (declining healthy Qi with growing pathogenic factors) and the middle and late stages (overwhelming pathogenic factors with deficient healthy Qi) are longitudinally described. Through the syndrome differentiation of visceral manifestations,the etiology and pathogenesis of PD motor and non-motor symptoms were comprehensively analyzed,while the matching treatment methods and prescriptions were inferred,and the modular scheme of the combining main symptoms,accompanying symptoms and secondary symptoms was proposed. The conceptual gap of TCM regarding motor complications ('variable syndrome') and PD-related hyperpyrexia syndrome ('critical syndrome') was explained. This framework reflects the characteristics of combination of disease and syndrome and overall constant motion,and provides new theories and research ideas for individualized and whole-process management of PD in TCM.

3.Protective mechanism of nitrooleic acid on submandibular gland cell radiation injury in rats

BACKGROUND:Recent studies have found that Nrf2/ARE signaling pathway activators have the characteristics of low toxicity and control,and have a protective effect against radiation tissue damage.OBJECTIVE:To investigate whether nitrooleic acid can protect submandibular gland epithelial cells from radiation injury by regulating the Nrf2/ARE signaling pathway.METHODS:Rat submandibular gland epithelial cells were cultured in vitro and CCK-8 assay was used to screen the optimal concentration and time of nitrooleic acid administration.Submandibular gland epithelial cells were divided into non-radiation group,radiation control group,nitrooleic acid group,nitrooleic acid+ML385(Nrf2/ARE signaling pathway specific inhibitor)group,and ML385 group.Submandibular gland cells were pretreated with nitrooleic acid and ML385 for 24 hous according to the experimental groups,and then irradiated with 5 Gy radiation to establish the models.At 48 hours after irradiation,CCK-8 assay was used to detect the cell proliferation rate.Real-time quantitative PCR was used to detect the expression of Nrf2,HO-1,and NQO1 mRNA in the cells.Real-time quantitative PCR and enzyme-linked immunosorbent assay were used to detect the cell secretion function and the expression of inflammatory factors.DCFH-DA fluorescent probe kit was used to detect the level of intracellular reactive oxygen species.RESULTS AND CONCLUSION:(1)Compared with the radiation control group,the proliferation rate of submandibular gland epithelial cells and the expression levels of secretion function related factors aquaporin 5 and α-amylase in the nitrooleic acid group of rats increased(P<0.05),and the expression levels of Nrf2,HO-1,and NQO1 mRNA increased(P<0.05),while the expression levels of inflammatory factors interleukin-1β,interleukin-6,and tumor necrosis factor-αdecreased(P<0.05),and reactive oxygen species generation reduced(P<0.01).(2)Compared with the nitrooleic acid group,the addition of nitrooleic acid and ML385 group resulted in a decrease in cell proliferation rate and expression levels of secretion function related factors aquaporin 5 and α-amylase(P<0.05),and mRNA expressions of Nrf2,HO-1,and NQO1 were all decreased(P<0.05),while the expression levels of inflammatory factors interleukin-1β,interleukin-6,and tumor necrosis factor-α increased(P<0.05),and generation of reactive oxygen species increased(P<0.05).(3)Results indicated that in the radiation environment,nitrooleic acid has a certain protective effect on the proliferation ability and secretion function of rat submandibular gland epithelial cells,reduces the expression of inflammatory factors,lowers intracellular reactive oxygen species levels,and alleviates the damage of rat submandibular gland epithelial cells caused by radiation.This function may be related to the activation of Nrf2/ARE signaling pathway.

5.Effect of emotional imagery desensitization therapy on post-traumatic stress disorder among school-age children with fracture

Objective:To explore the effect of emotional imagery desensitization therapy on posttraumatic stress disorder among school- age children with fracture, to alleviate the adverse psychological state of the children.Methods:By a randomized controlled trials, a total of 78 school- age children with fracture at the Affiliated Hospital of Jining Medical University from July 2021 to July 2023 were divided into experimental group and control group according to the admission time, with 39 cases in each group. Both groups carried out routine care, the control group was given routine psychological intervention, the experimental group implemented emotional imagery desensitization therapy. Before and after 3 months of intervention, the effects was assessed by Post-Traumatic Stress Disorder Self-rating Scale (PTSD-SS) and Medical Coping Modes Questionnaire (MCMQ), respectively.Results:During the research process, there were 2 cases of dropout in the control group due to disconnection, and 4 cases in the experimental group due to disconnection and voluntary withdrawal. 37 children in the control group [20 males and 17 females, age (9.27 ± 1.62) years], and 35 children in the experimental group [21 males and 14 females, age (9.57 ± 1.63) years] completed the intervention. Before intervention, the difference of PTSD-SS and MCMQ scores between two groups was no statistically significant (all P>0.05). After intervention, the subjective assessment, re-experiencing, avoidance symptoms, hypervigilance, functional impairment factor scores and total PTSD-SS scores were (1.51 ± 1.06), (10.74 ± 3.30), (10.86 ± 2.93), (11.94 ± 3.18), (2.31 ± 1.30), (37.37 ± 6.37) in the experimental group, lower than in the control group (2.08 ± 0.80), (12.92 ± 3.63), (12.73 ± 2.99), (14.65 ± 4.23), (2.97 ± 0.87), (45.35 ± 5.86), the differences were statistically significant ( t=2.53-5.54, all P<0.05). After intervention, the confrontation subscale scores were (23.49 ± 2.48), higher than in the control group (21.89 ± 2.38), the avoidance, acceptance- resignation subscale were (10.31 ± 1.89) and (6.83 ± 1.98), lower than in the control group (11.57 ± 1.79) and (7.86 ± 2.12), the differences were statistically significant ( t=2.79, 2.89, 2.14, all P<0.05). Conclusions:Emotional imagery desensitization therapy can effectively alleviate post- traumatic stress disorder and promote positive coping mode in school-age children with fracture.
6.Application of typical case combined with practice teaching in pediatric clinical teaching

Objective:To explore the application of typical case combined with practice teaching in the pediatric clinical teaching.Methods:A total of 80 clinical interns majoring in pediatrics who practiced in the Affiliated Hospital of Jining Medical University between September 2019 and January 2020 were selected as the research subjects, and they were randomly divided into observation group ( n=41) and control group ( n=39). The control group adopted conventional teaching while the observation group were given typical case combined with practice teaching model. After 16 weeks of training, the theoretical knowledge, practical operation [direct observation of procedural skill (DOPS) evaluation, and mini-clinical evaluation exercise (Mini-CEX)] and overall performance were analyzed in the two groups of students. SPSS 19.0 was used for t test, chi-square test and rank sum test. Results:After the internship, the theoretical scores of common pediatric diseases, clinical disease characteristics, etiopathogenesis, treatment regimen and complications were enhanced in the two groups, and the above scores in observation group were higher than those in control group ( P<0.05). Besides, the DOPS scores and Mini-CEX scores of the two groups were increased, and the scores were higher in observation group than in control group ( P<0.05). Furthermore, the overall excellent rate of the observation group was 87.80% (36/41), which was higher than 69.23% in control group (27/39). Survey found that the number of interns who believed that the teaching regimen could deepen the knowledge mastery degree, increase the internship interest and improve the comprehensive ability, internship efficiency and clinical thinking ability was more in observation group than in control group. Conclusion:Typical case combined with practice teaching can effectively deepen the interns' understanding of theoretical knowledge of clinical pediatrics, improve the practical operation ability and arouse the interest in learning.
7.Clinical effect of hyperbaric oxygen combined with fructose diphosphate sodium on child patients with myocardial injury from acute carbon monoxide poisoning

Objective:To investigate the clinical effect of hyperbaric oxygen (HBO) combined with fructose diphosphate sodium on child patients with myocardial injury induced by acute carbon monoxide (CO) poisoning.Methods:A total of 96 cases of acute CO poisoning child patients admitted to the Department of Pediatrics of the Affiliated Hospital of Jining Medical University from December 2012 to December 2019 were selected and randomly divided into control group and observation group according to the random number table method, with 48 cases in each group. The control group was given conventional symptomatic and supportive treatment combined with HBO, while the observation group was given fructose diphosphate sodium on the basis of the treatments in the control group. The clinical effect, the Glasgow coma scale (GCS) score, the levels of serum creatine kinase-MB (CK-MB), cardiac troponin I (cTnⅠ), brain natriuretic peptide (BNP), myoglobin (MYO), creatine kinase (CK), and α-hydroxybutyrate dehydrogenase (α-HBDH) of the two groups were observed.Results:After treatment, the GCS scores and the levels of serum CK-MB, cTnⅠ, MYO, BNP, CK, and α-HBDH of the two groups were significantly improved compared with those before treatment, all with statistically significant differences ( P<0.05 or P<0.01). The total effective rate of the observation group (97.92%) was significantly higher than that of the control group (91.67%), with statistically significant difference ( P<0.05). After treatment, the GCS score of the observation group was significantly higher than that of the control group; the serum expression levels of CK-MB, cTnⅠ, MYO, BNP, CK, and α-HBDH in the observation group were significantly lower than those in the control group; the total effective rate of electrocardiogram improvement in the observation group (93.75%) was significantly higher than that in the control group (85.42%), all the above differences were statistically significant ( P<0.05 or P<0.01). Conclusion:HBO combined with fructose diphosphate sodium can enhance the clinical curative effect on child patients with acute CO poisoning by promoting their regain of consciousness and improving the myocardial injury.

9. Evaluation of left ventricular systolic function in patients with chronic heart failure using vector flow mapping

Objective:
To observe the hemodynamic change characteristics of left ventricle and evaluate left ventricular systolic function in patients with chronic heart failure (CHF) via vector flow mapping(VFM).
Methods:
Sixty-two patients with CHF(CHF group) were selected as case group and were divided into three groups (B, C, D) according to the American College of Cardiology Foundation and the American Heart Association (ACC/AHA ) recommended stages.Sixty healthy volunteers were selected as control group. The left ventricular circulation parameters (vortex quantity, vortex area, circulation) and energy loss (EL) of the apex, mid, and basal segments in different groups were compared in all the systolic phases. Left ventricular ejection fraction (LVEF) was calculated by biplane Simpson method, and the correlation was analyzed with the parameters of circulation and EL.

Objective:
To investigate a mass incident of bromadiolone poisoning and analyze related clinical data.
Methods:
An investigation was performed for a mass incident of bromadiolone poisoning in a place in Shandong, China in December 2015, and related clinical data were analyzed and summarized.
Results:
This incident was a mass incident of bromadiolone poisoning caused by spreading poison. The poisoned patients had major clinical manifestations of bleeding and coagulation disorder and all of them were cured after comprehensive rescue, especially after intravenous drip of vitamin K1.
Conclusion
Bromadiolone poisoning can cause severe visceral hemorrhage and coagulation disorder, and intravenous drip of vitamin K1 has a good therapeutic effect.
