1.Comparison of efficacy of high dose tigecycline and polymyxin B in the treatment of pulmonary infection due to carbapenem-resistant organism

Objective To compare the clinical efficacy of high-dose tigecycline(TGC)and polymyxin B(PMB)in the treatment of pulmonary infection due to carbapenem-resistant organism(CRO).Methods Clinical data of pa-tients with CRO pulmonary infection and received PMB or high-dose TGC combined with other antimicrobial treat-ment regimens in Department of Critical Care Medicine of Xiangya Hospital,Central South University from January 2019 to March 2022 were analyzed retrospectively,including basic information,pathogen detection results,antimi-crobial use regimen,clinical efficacy,30-day mortality,bacterial clearance rate,etc.Results A total of 173 pa-tients were included in analysis,with 103 in the TGC group and 70 in the PMB group.Compared with TGC group,PMB group had a higher score of acute physiology and chronic health evaluation Ⅱ(APACHE Ⅱ)(25.0 vs 20.0,P<0.001),but clinical efficacy rates were not statistically different(67.1%vs 52.4%,P=0.054).Stratified analysis revealed that when the APACHE Ⅱ score was ≥15 points,compared with TGC group(n=78),PMB group(n=66)had a higher APACHE Ⅱ score(27.0 vs 22.0,P=0.005)and a higher clinical efficacy rate(66.7%vs 47.4%,P=0.020).After adjusting confounding factors through logistic regression analysis,it was found that PMB treatment was a protective factor for clinical efficacy rate compared with TGC treatment.Conclusion For treating pulmonary infection caused by CRO in patients,PMB-based treatment regimen has a significant protec-tive effect on the clinical efficacy rate compared with the high-dose TGC-based treatment regimen.

3.Research on medical students' humanities quality education under the vision of nationwide reading:curriculum theory and practice of Reading and Thinking

To explore the medical students' humanities quality education practice,taking the teaching practice of reading and thinking as an example from the perspective of nationwide reading.The curriculum plan is set up through teaching objectives,teaching content,teaching methods and evaluation of teaching effect.As a general knowledge course for medical students' humanistic education,the reading and thinking could cultivate the concept that "learning to learn" is more important than "learning knowledge" among medical students,so that to enhance the medical students' humanities quality comprehensive level.Medical students' humanistic quality education is of great practical significance,in which the rich teaching contents and diverse teaching methods could stimulate medical students' interest in reading and improve their thinking ability.What's more,the flexible forms of examination is helpful to realize the curriculumgoal.
4.Construction of academic smart library based on Hall 3D structure

The relationship of the main contents of academic smart library with the time and logic during its con-struction was studied by establishing a 3D (time dimension, logic dimension and professional dimension) structure model of academic smart library according to the Hall 3D structure system thinking in order to provide the theoretical framework for the construction of academic smart library, promote its development by deeper studies on its function.
5.Effect of Anchang Yuyang Decoction on Colon Tissue TFF3,MUC2 and TLR4 Gene Expressions in Rats with Ulcerative Colitis

Background:TLR4 can mediate immune and inflammatory responses,TFF3,MUC2 are the intestinal mucosa protection factor and can maintain the intestinal barrier function. Aims:To investigate the effect of Anchang Yuyang decoction on colon tissue TFF3,MUC2 and TLR4 gene expressions in rats with ulcerative colitis. Methods:TNBS was used to establish ulcerative colitis model in rats. Ninety Wistar rats were randomly divided into normal control group,model group,low,moderate and high dose Anchang Yuyang decoction groups and mesalazine group,and distilled water,different concentrations of Anchang Yuyang decoction and mesalazine were given respectively. All the rats were sacrificed after 21 days. Colonic histopathological score was assessed,and RT-PCR was used to detect gene expressions of colon tissue TFF3, MUC2 and TLR4. Results:Compared with model group,histopathological score and TLR4 expression were significantly decreased in moderate,high dose Anchang Yuyang decoction groups and mesalazine group(P < 0. 05),expressions of TFF3 and MUC2 were significantly increased( P < 0. 05). Compared with moderate dose Anchang Yuyang decoction group,histopathological score in high dose Anchang Yuyang decoction group and mesalazine group was significantly decreased(P < 0. 05),and TFF3 expression was significantly increased( P < 0. 01). Compared with moderate dose Anchang Yuyang decoction group and mesalazine group,MUC2 expression in high dose Anchang Yuyang decoction group was significantly increased(P < 0. 01),and TLR4 expression was significantly decreased( P < 0. 01). Conclusions:Anchang Yuyang decoction can promote the repair of colonic mucosa in rats with ulcerative colitis,and its mechanism may be related to the increase of TFF3 and MUC2 gene expressions and down regulation of TLR4 gene expression.
