Objective:To investigate the clinical efficacy of transcranial alternating current stimulation (tACS) combined with drug treatment in patients with bipolar Ⅰ disorder (BD Ⅰ).Methods:Forty-two patients with BD Ⅰ who were admitted to the Mental Health Center Affiliated to Anhui Medical University from March 2022 to June 2023 were included in this randomized double-blind study. These patients were randomly divided into a control group and an observation group using an Excel spreadsheet. In the control group, patients received 10 sessions of sham stimulation in addition to drug treatment, while the observation group received 10 sessions of tACS along with drug treatment. All patients were treated for 2 weeks. Each patient received stimulation for 15 minutes on each of the right and left prefrontal lobes once every working day. The Montreal Cognitive Assessment (MoCA) scores and Bech-Rafaelsen Mania Scale (BRMS) scores were compared between the two groups before and after treatment. Eighteen patients from the observation group and nineteen patients from the control group were included in the final analysis.Results:Two weeks after treatment, the MoCA score in the observation group was higher than that in the control group [(27.39 ± 1.88) vs. (24.63 ± 2.39)], and the BRMS score in the observation group was lower than that in the control group [(15.89 ± 3.18) vs. (19.00 ± 3.32)]. These differences were statistically significant ( t = -3.89, 2.91, both P < 0.05). After treatment, the MoCA score in the observation group increased, while the BRMS score decreased ( t = 5.04, -4.14, both P < 0.05). Pearson correlation analysis indicated the change in MoCA score was negatively correlated with BRMS score in both groups ( r = -0.433, P < 0.05). Conclusions:Drug treatment combined with tACS greatly improved clinical cognitive symptoms and reduced manic symptoms in patients with BD Ⅰ. The combined therapy exhibited better efficacy than monotherapy.

[Objective] To analyze the clinical curative effect of 20G and 23G vitrectomy for posterior segment intraocular foreign bodies,to explore the differences of their efficacy and safety.[Methods] This was a retrospective case study.Select 71 patients (71 eyes) who suffered from posterior segment intraocular foreign bodies and underwent different ways of vitrectomy,according to the way of vitrectomy,the patients were enrolled into 20G vitrectomy group (20G group,37 patients,37 eyes) and 23G vitrectomy group (23G group,34 patients,34 eyes).All patients were given wound suture,and patients complicated traumatic cataract should underwent cataract surgery;then were given 20G/23G vitrectomy and extraction of intraction foreign bodies,and (or) be given retinal laser photocoagulation,cryocoagulation,and endotamponade during the procedure.Extraction of intraction foreign bodies and the location of retinal,surgical time,postoperative inflammation and stimulus syndrome,length of hospital stay,the best corrected acuity (BCVA) and other complications after surgery were registered.Minimum follow-up was 6 months.[Results] The rate of extraction of intraction foreign bodies and early retinal reattachment rate were 100％ of the two groups.Comparing the complications after surgery between the two groups,the differences had no statistical significance (all P ＞ 0.05) except the incidence of postoperative ocular hypotenison.The significant difference was found in the comparison of surgical time,average hospitalizcd days,postoperative inflammation score and stimulus syndrome between the two groups,and there were significant differences when the vision distribution before and after surgery in two groups were self-compared (P ＜ 0.05).At lastest follow up,the differences had no statistical significance when comparing the cases of unplanned surgical reoperation,the location of retinal and the vision distribution between the two groups (P ＞ 0.05).[Conclusion] 20G and 23G vitrectomy are both safely and effectively performed in patients with posterior segment intraocular foreign bodies,the efficacy and safety of them are comparable.There is higher incidence of postoperative ocular hypotension in 23G group,and it has an ascendant than 20G group in shorter surgical time and length of hospital stay,lighter postoperative inflamnation and stimulus syndrome.

Objective To explore the effect of combined gastrointestinal internal and external nutritional support in the treatment of elderly patients with lung cancer.Methods According to the random number table method,86 patients with advanced non-small cell lung cancer(NSCLC) were divided into the control group and observation group,43 cases in each group.The observation group was given nutritional intervention on the basis of treatment in the control group.The 12-month survival rate,median progression time and median survival time were evaluated.The clinical efficacy of the two groups was evaluated.The quality of life before and after treatment was evaluated by the KPS score.Results The 12-month survival rate of the observation group was 39.53％,which was significantly higher than 18.60％ of the control group(x2 =4.56,P =0.03).The median survival time and median survival of the observation group were (7.85 ± 1.27) months,(9.98 ± 1.71) months,respectively,which were significantly higher than (6.07 ±0.98)months,(7.99 ± 1.38)months of the control group(t =7.27,5.93,P =0.00,0.00).The total effective rate of the observation group was 88.37％,which was significantly higher than 55.81％ of the control group (x2 =11.32,P =0.00).The KPS scores of the observation group and the control group after treatment were (73.17 ± 2.75)points,(59.68 ±2.52) points,respectively,which were higher than (52.75 ± 2.02) points,(53.01 ± 1.98) points before treatment,and the KPS scores of the observation group were significantly higher than those of the control group (t =39.24,13.64,P =0.00,0.00).The incidence rate of adverse reaction in the observation group was 48.84％,which in the control group was 55.81％,the difference was not statistically significant (x2 =0.41,P =0.51).The CD4+ cells,CD4+/CD8+ and NK cells in the control group were significantly lower than those before treatment (t =6.14,8.44,7.48,P =0.00,0.00,0.00),which in the observation group were significantly higher than before treatment (t =4.89,5.17,6.33,P =0.00,0.00,0.00),the differences were statistically significant compared with the control group after treatment(t =8.11,4.26,5.88,P =0.00,0.00,0.00).The levels of hemoglobin and albumin in the observation group were significantly higher than those in the control group(t =3.88,6.39,P =0.00,0.00).Conclusion The combination of chemotherapy combined with nutritional intervention in the treatment of locally advanced NSCLC is beneficial to improve the immune function and nutritional status of patients,improve clinical efficacy and survival of patients,better improve their quality of life,and will not increase adverse reactions.
