1.Hypoxia-inducible factor-2α:a novel therapeutic target for intestine-related diseases

The intestine in a hypoxic state is an essential physiological organ,and its primary func-tions include digestion,absorption,excretion,hormone secretion,and providing barrier and immune protec-tion.Hypoxia-inducible factor-2α(HIF-2α)represents one of the important physiologicalregulators for the intestine,partaking in the regulation of iron homeostasis,oxygen homeostasis and energy metabo-lism in the intestinal environment.Recent studies have shown that HIF-2α is closely associated with the onset and progression of various intestinal-related diseases,including iron-relatedblood diseases,inflam-matory bowel disease(IBD),colorectal cancer(CRC),and obesity-related metabolic diseases.Thus,HIF-2α may be a novel target for the treatment.HIF-2α is currently a hot topic in drug development,and numerous studies have revealed that it has therapeutic potential for intestinal-related diseases.HIF-2α is a key transcription factor that regulates intestinal iron absorption and systemic iron homeostasis.Aberrant expression of HIF-2α has been closely linked to various hematological disorders associated with iron metabolism dysregulation.In the pathological hypoxic microenvironment of the intestine,sustained activation of HIF-2α induces inflammatory response and impairs epithelial barrier function,thereby exacerbating the progression of IBD.Within the tumor microenvironment,HIF-2α contributes to CRC progression through multiple mechanisms,including metabolic reprogramming,angiogenesis,enhanced prolifera-tion,migration,and invasion of tumor cells,as well as inflammation,iron accumulation,and immune evasion.Moreover,HIF-2α is involved in the regulation of obesity,insulin resistance,and glucose-lipid metabolism via the gut-liver axis.Although seven HIF-2α modulators have been approved for clinical use,adverse effects such as anemia and thrombosis remain concerns.Therefore,developing next-generation HIF-2α-targeted strategies with improved specificity and safety profiles is critical to future research.This article is an overview of the recent advancements in understanding the role and mecha-nisms of HIF-2α in intestinal health and associated diseases while analyzing the challenges to develop-ment and application of HIF-2α modulators in the future,in hopes of offering novel therapeutic avenues for intestinal-related ailments.
2.Research and challenges of radiopharmaceuticals in the treatment of bone diseases

Both primary (e.g. osteosarcoma and Ewing sarcoma) and secondary (e.g. metastatic) bone cancers cause severe health problems and can lead to death. In recent years, radiopharmaceuticals have shown significant potential in the treatment of refractory bone diseases due to their unique targeting properties and low toxicity. Radiopharmaceutical therapy for bone cancer utilizes bone-seeking radiopharmaceuticals that target lesions through the high affinity of ethylenediaminetetramethylenephosphonic acid (EDTMP) ligand for bone hydroxyapatite. Traditional therapeutic agents such as 89Sr and 153Sm-EDTMP, administered via intravenous injection, accumulate in bone metastases and release beta rays to kill tumor cells. These agents can relieve pain and inhibit bone destruction; however, they have limited ability to shrink tumor volume or slow tumor growth (tumor volume reduction rate < 30%). Furthermore, these agents may cause systemic side effects, and existing evidence does not support their effectiveness in prolonging survival. The development of palliative bone pain therapies based on novel radiopharmaceuticals may overcome these limitations and improve the quality of life for patients during their remaining lifespan. Researchers have investigated the combination of various beta- and alpha-emitters with novel radionuclide carrier systems, significantly enhancing treatment precision in bone tumor models. For instance, 177Lu-EDTMP (beta ray 0.497 MeV, half-life 6.7 days) enabled 66% of patients to completely discontinue analgesics by the twelfth week. 186Re- and 188Re-hydroxyethylidene diphosphonate, by targeting hydroxyapatite, achieved a pain relief rate of up to 90% and significantly prolonged median survival. These agents caused only reversible hematological toxicity, with no Grade III/IV bone marrow suppression events, and significantly reduced tumor volume. Additionally, the alpha nuclide 223RaCl2 induces DNA breaks in tumor cells and inhibits osteoblast-osteoclast imbalance. When combined with best supportive care, it can extend overall survival by 3.6 months (14.9 months vs. 11.3 months), reduce the risk of death by 30%, and lead to significant tumor volume reduction. However, bone marrow toxicity management, cost-effectiveness balance, and the design of multimodal combination regimens remain key challenges.
3.Development of a diagnostic model for severe coronary artery stenosis using resting echocardiography

Objective:To evaluate the diagnostic performance of resting echocardiography in detecting severe coronary artery stenosis.Methods:A total of 136 patients with suspected coronary artery disease(CAD)who presented to Sichuan Provincial People's Hospital between January 2021 and December 2024 were prospectively enrolled. All patients underwent both coronary computed tomography angiography(CCTA)and transthoracic echocardiography within one week. Based on CCTA results,the patients were divided into non-severe stenosis group( n=78)and severe stenosis group( n=58). Echocardiographic parameters including left atrial maximum volume(LAVmax),left ventricular global longitudinal strain(GLS),left ventricular longitudinal strain of endo-myocardium,mid-myocardium,epi-myocardium(LSendo,LSmid,LSepi),early diastolic mitral inflow velocity(E),early diastolic mitral annular velocity of the lateral and septal walls(e'),and E/e' were measured. Predictive factors for severe coronary stenosis were identified using LASSO regression,and a nomogram model was developed via multivariate Logistic regression. Model performance was evaluated using ROC curves,calibration curves,and decision curve analysis. Results:Multivariate Logistic regression analysis revealed LSendo,LAVmax,and E/e' as independent predictors of severe coronary artery stenosis. The nomogram constructed based on these predictors achieved an area under the curve of 0.798(95% CI=0.723-0.873),with sensitivity and specificity of 0.756 and 0.759,respectively. Conclusions:The resting echocardiography-based nomogram model demonstrates good diagnostic efficacy for severe coronary artery stenosis. It may serve as a noninvasive tool to assist in risk stratification and clinical decision-making in patients with suspected CAD.

Dental treatments for children and adolescents have unique clinical characteristics that differ from dental care for adults in terms of children's physiology, psychology, and behavior. These differences impose specific requirements on the application of local anesthesia in pediatric dental procedures. This article presents expert consensus on the principles of local anesthesia techniques in pediatric dental therapies, including the use of common anesthetic drugs and dosage control, safety and efficacy evaluation, and prevention and management of complications. The aim is to improve the safety and quality of pediatric dental treatments and offer guidance for clinical application by dentists.

7.Parallel acquisition technique combined with deep learning reconstruction for shoulder joint MRI

Objective To observe the value of parallel acquisition technique(PAT)combined with deep learning reconstruction(DLR)for shoulder joint MRI.Methods Totally 74 subjects who underwent shoulder joint MR scanning were prospectively enrolled.Shoulder joint MRI were acquired using standard sequence(group A),PAT accelerated sequence(group B)and PAT accelerated sequence combined with DLR(group C),respectively.Subjective evaluation of overall image quality,artifacts and clarity of anatomical structures(including articular cartilages,ligaments and bone marrow)were performed using a 5-point scale,and signal-to-noise ratio(SNR)and contrast-to-noise ratio(CNR)of articular cartilages were calculated for objective evaluation,which were then compared among groups.Results No significant difference of the subjective scores of overall image quality,artifacts nor clarity of anatomical structures was found between group A and C(all P>0.05),which were all higher than those in group B(all P<0.05).SNR and CNR of articular cartilages of shoulder joint MRI in group C,A,B decreased in order(all P<0.05).Conclusion PAT combined with DLR could shorten scanning time of shoulder joint MRI,reduce image noise and improve image quality.
8.Functional magnetic resonance imaging study about repetitive transcranial magnetic stimulation for dysfunction after stroke: a scoping review

ObjectiveTo explore the brain mechanism of repetitive transcranial magnetic stimulation (rTMS) on dysfunction after stroke using functional magnetic resonance imaging (fMRI). MethodsLiteratures about the functional magnetic resonance imaging study about repetitive transcranial magnetic stimulation for dysfunction after stroke were retrieved in PubMed, Web of Science, CNKI and Wanfang data from establishment to June 1st, 2021. The quality of the literature was evaluated with Physiotherapy Evidence Database (PEDro) scale. Literature screening, and data extraction were performed by two researchers. ResultsA total of 14 randomized controlled trials were finally enrolled. They were of high or very high quality. They mainly involved the therapeutic effect and imaging mechanisms of rTMS on dysfunction after stroke. ConclusionrTMS could change the excitability of the cerebral cortex and the effective connections between brain regions after stroke, promote the reorganization of brain function, and achieve the recovery of post-stroke dysfunction.
9.Research and Practice of Vehicle-mounted Mobile CT Based on Emergency Medical Rescue.

OBJECTIVE:
In order to solve the problem of inadequate CT screening ability in emergency medical rescue in remote mountainous areas, high-altitude areas, other public health events and sudden natural disasters, a vehicle-mounted mobile CT suitable for emergency medical rescue is studied.
METHODS:
A vehicle chassis system suitable for long-distance transportation and a cabin system suitable for epidemic prevention and control was designed. A domestic 32-slice CT with small volume, light weight and high stability was selected to design a vehicle-mounted mobile CT suitable for emergency medical rescue.
RESULTS:
The high-performance vehicle-mounted mobile CT can provide rapid support, and provide large-scale screening, emergency medical rescue, a supplement to daily CT scanning in peacetime and wartime.
CONCLUSIONS
The vehicle CT has high stability, fast scanning speed and high social and economic value.
10.Summary of the best evidence for nutritional management in patients with chronic heart failure

Objective:To retrieve evidence on nutritional management in patients with chronic heart failure and summarize the best evidence.Methods:Based on the search terms "heart failure" and "nutrition", the evidence on nutritional management in patients with chronic heart failure including guidelines, evidence summary, expert consensus, recommended practices, systematic reviews, and original studies closely related to the topic were searched through computer on British Medical Journal (BMJ) Best Practice, UpToDate, Joanna Briggs Institute (JBI) Evidence-Based Health Care Center Database in Australia, Scottish Intercollegiate Guidelines Network, Guidelines International Network, Cochrane Library, PubMed, China National Knowledge Infrastructure, and other websites and databases. The search period was from the establishment of the database to February 25, 2022. Two researchers evaluated the quality of the included literature and extracted evidence from the literature that met the quality standards.Results:A total of 15 articles were included, including 5 guidelines, 5 expert consensus, and 5 systematic reviews. Finally, 24 best pieces of evidence were summarized, covering 7 aspects, including multidisciplinary team building, nutritional screening and evaluation, nutritional needs, nutrient recommendation, liquid intake management, weight management, and nutritional intervention.Conclusions:This study summarizes the best evidence for nutritional management in patients with chronic heart failure from 7 aspects. Clinical medical and nursing staff can implement evidence conversion applications based on the patient's wishes and clinical actual situation.
