1.Automatic recognition and segmentation of brachial plexus in ultrasonic images based on deep learning

Objective:To propose a deep learning(DL)-based ultrasound imaging auxiliary tool for automatic segmentation and recognition of the brachial plexus(BP),and to enhance the accuracy and safety of clinical procedures.Methods:It was a multicenter study that collected 773 healthy subjects from Peking University Third Hospital and its branch campuses,the Third Medical Center of the Chinese PLA General Hospital,and Shanghai Eighth People's Hospital between August 2024 and February 2025. Brachial plexus(BP)images in the interscalene groove were captured used high-frequency ultrasound by senior sonographers,a dataset comprising 1 289 standardized images were constructed and the improved model(CHA-TransUNet)was trained. The test set was input into 6 different models(CHA-TransUNet,R50-Unet,TransUnet,SegFormer,SwinUnet,MISSFormer)for segmentation. Segmentation accuracy was evaluated using metrics including the Dice similarity coefficient(DSC),95% Hausdorff distance(HD95)and mean intersection over union(mIoU),and was compared with the segmentation results of 3 ultrasound physicians with varying experience levels(junior physicians and senior physicians)to validate the model's segmentation efficacy.Results:The CHA-TransUNet model established based on a dataset of 1 289 standardized images achieved segmentation results for the BP with a DSC of 90.15%,mIoU of 91.02%,and HD95 of 8.08. Its accuracy was higher than other mainstream models(DSC:90.15% vs. 87.60%,87.77%,81.35%,84.78%,84.55%),significantly better than junior physicians(DSC:90.15% vs. 68.73%, Z=-127.76, P<0.001),and approached the level of senior physician(DSC:90.15% vs. 86.15%, Z=-31.33, P=0.549). The model demonstrated superior boundary recognition in complex anatomical structures(e.g.,C6/C7 nerve roots)compared to ultrasound physicians(junior and senior)(HD95:8.08 vs. 26.34,17.44,56.80). Conclusions:This study proposes an analysis model for BP ultrasound images,CHA-TransUNet. This model achieves segmentation and recognition of the BP with relatively complex pathways and structures. The model exhibits high accuracy and stability,outperforming current mainstream network models and junior physicians while approaching the performance level of senior physicians. It assists junior physicians or trainees in more accurately identifying and localizing the BP.

Objective:
To compare the carcinogenic abilities of CD133+CD44+ laryngeal cancer stem cells and general laryngeal cancer stem cells and to identify the mechanism underlying the action of miRNAs.
Methods:
Solid tumor-derived laryngeal carcinoma stem cells and Hep-2-derived laryngeal carcinoma stem cells were cultured, and CD133+CD44+ laryngeal cancer stem cells were sorted by flow cytometry. Boden chamber invasion assay, cell migration assay and tumor formation assay were then performed to compare the invasion, migration and tumorigenic abilities of CD133+CD44+ laryngeal cancer stem cells and general laryngeal cancer stem cells. And then, miRNAs isolated from two laryngeal cancer stem cells were detected and analysed with miRNA chip.
Results:

Objective:
To explore the correlation between sleep and laryngopharyngeal reflux disease by epidemiological approaches.
Methods:
From May 1, 2017 to April 30, 2018, data of age, gender, height, weight, smoking, alcohol consumption, constipation and high fat diet in patients in Otorhinolaryngology specialist clinic, the Eighth Medical Center, General Hospital of the Chinese PLA were retrospectively analyzed. Reflux Symptom Index (RSI), Pittsburgh Sleep Quality Index (PSQI) and Hospital Anxiety and Depression Scale (HADS)were filled. According to RSI scores, patients were divided into case group and control group. The differences of the above indicators between the two groups were compared by Stata 12.0 software, and the risk factors of LPRD were analyzed by multivariate Logistic regression.
Results:

OBJECTIVE:
To investigate the expression and clinical significance of phosphated signal transducer and activator of transcription 3 (p-STAT3) and p53 protein in laryngeal squamous cell carcinoma (LSCC), and the relationship between the two genes.
METHOD:
Formalin-fixed and paraffin-embedded tissues, which came from 60 cases of LSCC and 32 samples of normal mucosa over 2.0 cm away from tumor margin in 32 patients with total or subtotal laryngectomy were detected for the expression of p-STAT3 and p53 protein by in SP immunohistochemistry. Micro-image analysis system was used to determine the optical density,and the result was analyzed statistically.
RESULT:
There is over expression of p-STAT3 and p53 protein in LSCC. The expression of p-STAT3 and p53 protein in LSCC was associated with clinical stage and lymph nodal metastases (P < 0.05). There was a positive correlation between the expression of p-STAT3 and P53 protein. The correlation coefficient (r) was 0.6558 (P < 0.01).
CONCLUSION
The expression of p-STAT3 and p53 protein may play an important role in the tumorigenesis,metastases and poor prognosis of LSCC. There was a positive correlation between the over expression of p-STAT3 and p53 protein in LSCC.

8.A multi-center, randomized, controlled, double blind and double dummy clinical trial of antofloxacin hydrochloride tablet versus levofloxacin tablet for the treatment of acute bacterial infections

Objective To evaluate the clinical efficacy and safety of antofloxacin hydrochloride tablet for the treatment of acute bacterial infections. Methods A multi-center randomized control, double blind and double dummy clinical trial was conducted; levofloxacin tablet was chosed as controlled drug. The duration of treatment was 7-14 days in both groups. Results A total of 719 patients were enrolled in the study, in which 359 patients treated with antofloxacin and 360 patients treated with levofloxacin were included. Three hundred and thirty and 337 patients completed the study and met with all the criteria for perprotocol analysis, respectively. By the end of chemotherapy, the cured rates in per protocol set (PPS)population were 79.7% and 77.4%, the effective rates were 95.2% and 96. 7%, and the bacterial clearance were 96. 7% and 97. 5% for the treating and control group, respectively. The clinical and bacterial efficacy of antofloxacin and levofloxacin was comparable by the analysis of infectious sites. Three hundred and fifty-seven and 356 patients in antofloxacin and levofloxacin groups were evaluated the safety.The drug adverse events occurred both in 10. 1%, and drug adverse reactions accurred in 7. 8% and 7.9%patients in the two groups. The most common drug adverse reactions were mild gastroenteric symptoms. No QTc prologation was detected in all the patients. One patient in each group had mild blood glucose increase at the end of therapy, but the glucose returned to normal level without any intervention. No statistic significant difference between the two groups in clinical efficacy and safety was detected (P>0.05).Conclusions Antofloxacin hydrochloride tablet was effective and safe for the treatment of acute bacterial infections.
9.The relationship of MMP-9, VEGF and PCNA expressions and their clinical significance in gastric adenocarcinoma

Objective To investigate abnormal protein expression of matrix metalloproteinase-9 (MMP-9), vascular endothelial growth factor (VEGF) and proliferating cell nuclear antigen (PCNA) in human gastric adenoearcinoma, and further reveal the clinical significance. Method The MMP-9, VEGF and PCNA proteins expression was determined by immunohistochemistry staining in 45 gastric adenocarcinoma tissues, 45 adjacent specimens and 10 normal gastric mucosa tissues via tissue arrays accordingly. The relationship of these protein expression with differentiation degree, development and progression of gastric adenocarcinoma were also analyzed. Results Positive rates of MMP-9, VEGF and PCNA in gastric adenocarcinoma, adjacent specimens and gastric normal mucosa were as follows: MMP-9, 82.2%(37/45), 64.4% (29/45), 30.0% (3/10) (P=0.019); VEGF, 73.3% (33/45), 62.2% (28/45), 30. 0% (3/10) (P=0.029); PCNA,84.4% (38/45), 71.1% (32/45), 10.0% (1/10) ,there were statistically significant difference (P = 0. 001). The positive rates of MMP-9, VEGF and PCNA in well-differentiated adenocarcinoma, moderately differentiated adenocarcinoma and poorly differentiated adenocarcinoma were as follows: MMP-9,70.0%(7/10), 80. 0% (8/10), 88.0%(22/25), there were statistically significant difference (P=0.015);VEGF, 50.0%(5/10), 60.0% (6/10), 88.0% (22/25), there were statistically significant difference (P =0.000);PCNA, 60.0% (6/10) ,90.0% (9/10) ,92.0% (23/25) ,the difference is significant statistically (P = 0.004). The expression of MMP-9, VEGF and PCNA showed positive relationship with each other by rank correlation analysis (P < 0. 05). Conclusion Tissue arrays technology is effective tool to analyze the expression of cancer related proteins in gastric adenocarcinoma. The expression of MMP-9, VEGF and PCNA proteins participates in the tumorigenesis and development process of gastric adenocarcinoma, and these can be used as indexes to evaluate prognosis in clinical.
10.Lower eyelid flap in double-eyelid blepharoplasty of epicanthic folds

Objective To investigate the possibility of plastic and cosmetic operation incorporating epicanthoplasty with the double-eyelid procedure in one stage to achieve a more subtle scar in epicanthus patients with single eyelid. Methods After designing incision line of the epicanthic flap, double-eyelid line and the lower eyelid incision curved line, we cut the medial canthus skin transversely to reach a new medial canthus point, incised the part of front angle of medial palpebral ligamentum longitudinally and fixed the end point of front angle of medial palpebral ligamentum to nasal dorsum fascia firmly. Double eyelid operation was routinely performed to remove redundant small triangle skin as well as some patchy of orbicularis on the new medial canthus point. Along with eyelid edge about 1 to 2 mm to eyelash, the temporal side was cut with curved line till its disappearance by eyelid's "cat ear". The lower eyelid flap was separated downward and the superfluous flap and some patchy of orbicularis cut. The skin was su-tured to make postoperative scar hidden, blepharophimosis increased and the fold disappeared. Results 46 eyes (23 cases) were operated and satisfactory aesthetic results were obtained. Palpebral fissure was enlarged to 2 to 4 mm with epicanthic scar disappearance and formation of double-eyelid. Conclusions This is a simple and effective procedure with hidden epicanthic scar and the double-eyelid blepharoplasty could be performed simultaneously. Most patients receive satisfactory results during the 0.5 to 2 years' follow-up period except 2 cases with mild proliferation of epicanthus in half a year. It is especially suitable to correct the severe epicanthus palpebralis or epieanthus tarsalis.
