1.Application of a hybrid artificial intelligence model integrating view detection and structural segmentation in evaluating cardiac function of anemic fetuses

Objective:To compare the cardiac size,morphology,and function between anemic and normal fetuses using a hybrid artificial intelligence(AI)model,and to evaluate the utility of AI in quantitatively assessing fetal cardiac function in cases of anemia.Methods:A retrospective study was conducted by collecting data from 2018 to 2024 at the Seventh Affiliated Hospital of Sun Yat-sen University,including 15 cases of anemic fetuses(anemia group)diagnosed through umbilical venous puncture and 32 cases of normal fetuses(control group). Four-chamber fetal cardiac ultrasound videos and left/right ventricular segments were included,with 44 videos and 1 056 segments in the anemia group,and 46 videos and 1 104 segments in the control group. Based on dynamic four-chamber heart images,the hybrid AI model was employed to extract heart measurement parameters,including basal-apical length(BAL),transverse width(TW),global sphericity index(GSI),end-diastolic area(EDA),24-segment left and right ventricular end-diastolic diameter(LVEDD,RVEDD),segmental sphericity index(LVSI,RVSI),global longitudinal strain(LVGLS,RVGLS),fractional area change(LVFAC,RVFAC),segmental fractional shortening(LVFS,RVFS),along with their corresponding Z-scores. The differences in cardiac size,morphology,and function parameters between the two groups were compared. Pearson correlation analysis was performed for the parameters of the control group(BAL,TW,EDA,GLS,LVGLS,RVGLS,LVFAC,and RVFAC)against gestational age. The measurement consistencies of AI technology and fetal HQ technology in normal and anemia groups were evaluated.Results:No significant differences were found in BAL,TW,EDA,or GSI between groups(all P>0.05). RVEDD in segments 3-24 was significantly larger in the anemia group(all P<0.05),with significantly higher Z-score abnormality rates for LVEDD and RVEDD across 24 segments(both P<0.001). LVSI in segments 7-10,12,14-15 and RVSI in segments 1-23 were lower in the anemia group(all P<0.05),with significantly higher Z-score abnormality rates for LVSI and RVSI across 24 segments(both P<0.001). The absolute values of LVGLS and LVFAC were significantly reduced in the anemia group(both P<0.05),while the absolute values of RVGLS and RVFAC showed no significant differences(both P>0.05). Segmental LVFS values were significantly lower in the anemia group for segments 2,5-8,11-13(all P<0.05). In the control group,BAL,TW,and EDA positively correlated with gestational age( r=0.913,0.947,0.907;all P<0.001),while GSI,LVGLS,RVGLS,LVFAC and RVFAC showed no or weak correlations( r=-0.221,0.353,0.515,-0.409,-0.425). The intraclass correlation coefficient(ICC)between AI-based and conventional fetal HQ evaluations were 0.788 for the control group and 0.837 for the anemia group,indicating good consistency. Conclusions:AI offers a reliable approach for quantitatively evaluating fetal cardiac size,shape,and systolic function. Fetal anemia primarily affects right ventricular morphology and left ventricular systolic performance,characterized by spherical remodeling of the right ventricle and reductions in LVGLS,LVFAC,and segmental LVFS. The hybrid AI model holds potential value in fetal cardiac function assessment.
2.Risk factors and pregnancy outcomes of hyperglycemia in pregnancy complicated with hypertensive disorders of pregnancy

Objective:To explore the clinical risk factors and pregnancy outcomes in women with hyperglycemia in pregnancy (HIP) complicated with hypertensive disorders of pregnancy (HDP).Methods:This retrospective cohort study included 43 973 singleton live births delivered at the Obstetrics and Gynecology Hospital of Fudan University between September 2017 and December 2022. Participants were categorized into four groups: HH group (HIP with HDP, n=1 011), HIP group ( n=5 469), HDP group ( n=3 486), and control group ( n=34 007). Baseline characteristics and perinatal outcomes were compared using the Chi-square test (or Fisher's exact test). Logistic regression identified risk factors and adverse outcome risks and analyze the impact of HH on neonatal weight. Results:HH accounted for 15.6% (1 011/6 480) of HIP cases, with an overall incidence of 2.3% (1 011/43 973). HIP and HDP were strongly correlated [ OR=1.803, 95% CI: 1.672-1.945]. Advanced maternal age (≥35 years at estimated due date), primiparity, and pre-pregnancy overweight/obesity (body mass index ≥24 kg/m2) were independent risk factors for HH [ OR (95% CI): 1.305 (1.113-1.529), 1.845 (1.545-2.203), and 2.316 (1.981-2.718), respectively]. Compared to the HIP group, the HH group had significantly higher risks of preterm birth [10.3% (104/1 011) vs. 6.3% (344/5 469), OR=1.627 (95% CI:1.280-2.068)], cesarean delivery [57.0% (576/1 011) vs. 41.9% (2 289/5 469), OR=1.701 (95% CI:1.474-1.963)], and neonatal birth weight < P10 [13.9% (141/1 011) vs. 9.0% (494/5 469), OR=1.668 (95% CI:1.336-2.083)]. Stratified analysis revealed a 73.4% increased risk of birth weight < P10 in the gestational diabetes mellitus A1 with HDP subgroup ( aOR=1.734, 95% CI: 1.416-2.125). Conclusions:Advanced age, pre-pregnancy overweight/obesity, and primiparity are risk factors for HH. Compared to isolated HIP, HH is associated with elevated risks of preterm birth, cesarean delivery, and abnormal neonatal birth weight, though the impact on birth weight may vary by HIP subtype.
3.Construction and phenotypic analysis of p2rx2 knockout zebrafish lines.

OBJECTIVES:
The purinergic receptor P2X2 (P2RX2) encodes an ATP-gated ion channel permeable to Na+, K+, and especially Ca²⁺. Loss-of-function mutations in P2RX2 are known to cause autosomal dominant nonsyndromic deafness 41 (DFNA41), which manifests as high-frequency hearing loss, accelerated presbycusis, and increased susceptibility to noise-induced damage. Zebrafish, owing to their small size, rapid development, high fecundity, transparent embryos, and high gene conservation with humans, provide an ideal model for studying human diseases and developmental mechanisms. This study aims to generate a p2rx2 knockout zebrafish model using CRISPR/Cas9 gene editing system to investigate the effect of p2rx2 deficiency on the auditory system, providing a basis for understanding P2RX2-related hearing loss and developing gene therapy strategies.
METHODS:
Two CRISPR targets (sgRNA1 and sgRNA2) spaced 47 bp apart were designed within the zebrafish p2rx2 gene. Synthesized sgRNAs and Cas9 protein were microinjected into single-cell stage Tübingen (TU)-strain zebrafish embryos. PCR and gel electrophoresis verified editing efficiency at 36 hours post-fertilization (hpf). Surviving embryos were raised to adulthood (F0), tail-clipped, genotyped, and screened for positive mosaics. F1 heterozygotes were generated by outcrossing, and F2 homozygous mutants were obtained by intercrossing. Polymerase chain reaction (PCR) combined with sequencing verified mutation type and heritability. At 5 days post-fertilization (dpf), YO-PRO-1 staining was used to examine hair cell morphology and count in lateral line neuromasts and the otolith region. Auditory evoked potential (AEP) thresholds at 600, 800, 1 000, and 2 000 Hz were measured in nine 4-month-old wild type and mutant zebrafish per group.
RESULTS:
A stable p2rx2 knockout zebrafish line was successfully established. Sequencing revealed a 66 bp insertion at the first target site introducing a premature stop codon (TAA), leading to early termination of protein translation and loss of function. Embryos developed normally with no gross malformations. At 5 dpf, mutants exhibited significantly reduced hair cell density in the otolith region compared with wild type, although lateral line neuromasts were unaffected. AEP testing showed significantly elevated auditory thresholds at all 4 frequencies in homozygous mutants compared with wild type (all P<0.001), indicating reduced hearing sensitivity.
CONCLUSIONS
We successfully generated a p2rx2 loss-of-function zebrafish model using CRISPR/Cas9 technology. p2rx2 deficiency caused hair cell defects in the otolith region and increased auditory thresholds across frequencies, indicating its key role in maintaining zebrafish auditory hair cell function and hearing perception. The phenotype's restriction to the otolith region suggests tissue-specific roles of p2rx2 in sensory organs. This model provides a valuable tool for elucidating the molecular mechanisms of P2RX2-related hearing loss and for screening otoprotective drugs and developing gene therapies.

6.Effect of Longmu Piyan Prescription on Oxidative Stress in Atopic Dermatitis Mice

ObjectiveThis study observes the intervention effect of Longmu Piyan prescription on oxidative stress in BALB/c mice with atopic dermatitis (AD) induced by 2,4-dinitrochlorobenzene (DNCB) and explores its mechanism. MethodThe AD model was established using the method of DNCB sensitization on the back skin of BALB/c mice. Forty male BALB/c mice were randomly divided into a blank group, a model group, a vitamin C control group (0.5×10-3 mg·kg-1), and a Longmu Piyan prescription group (26 g·kg-1). Except for the blank group, other groups were sensitized with different concentrations of DNCB on the back to induce AD, and the blank group was treated with matrix coating. The gastric administration was started on the seventh day after sensitization with 2% DNCB and on the 24th day after sensitization with 0.2% DNCB continuously for 21 days. The changes in skin lesions of each group were directly observed after the experiment. Enzyme-linked immunosorbent assay (ELISA) was used to detect the levels of interleukin (IL)-4, tumor necrosis factor (TNF)-α, immunoglobulin E (IgE), and reactive oxygen species (ROS) in the serum of each group. The total antioxidant capacity determination kit-trace method (ABTS method) was used to measure the level of total antioxidant capacity (TAOC) in serum. The Hematoxylin eosin (HE) staining method was used to observe the pathological and morphological changes of the skin lesion site. The immunohistochemical method was used to detect the expression of thymic stromal lymphopoietin (TSLP) in the skin lesion site. Western blot was used to detect the expression of filaggrin (FLG) in the dorsal skin lesions. ResultThe results showed that compared with the blank group, the skin lesion score of the model group mice was significantly increased (P<0.01), and HE staining showed characteristic pathological changes of AD in the skin lesion site. At the same time, the expression of TSLP in the skin lesion was significantly increased, and that of FLG was reduced (P<0.05). The levels of TNF-α, IL-4, IgE, and ROS in serum increased, while the activity of TAOC decreased (P<0.01). Compared with the model group, the Longmu Piyan prescription group showed a significant decrease in skin lesion scores and a significant improvement in skin lesion pathology. At the same time, the expression of TSLP decreased, and the expression of FLG increased in the skin lesions (P<0.05). In addition, compared with the model group, the serum levels of TNF-α, IL-4, IgE, and ROS also decreased to varying degrees (P<0.05,P<0.01), and TAOC activity increased in the Longmu Piyan prescription group (P<0.01). ConclusionThere is a significant correlation among the degree of oxidative stress, the severity of skin lesions in AD, and the levels of inflammatory cytokines. Longmu Piyandu prescription can improve AD-like skin lesions in BALB/c mice by promoting ROS clearance, enhancing TAOC, and inhibiting oxidative stress, thus protecting the skin barrier and reducing inflammation.
7.Application of customized dental-bone supported osteotomy guide plate in bilateral sagittal split osteotomy

Objective:To investigate the effects of customized dental-bone supported osteotomy guide plate in bilateral sagittal split osteotomy.Methods:24 patients(48 sides)with maxillofacial deformity underwent BSSO were included.The maxillofacial region of all patients was scanned by CT,the plaster dental models were scanned using laser surface scanner,and the 3D models were established.The osteotomy guide plates of the inner horizontal and anterior sagittal bone incision of mandible ramus were manufactured by digital technology.All splits underwent operation with(27 side)and without(21 sides)the osteotomy guide plate respectively by the same doc-tor,and the time for the inner horizontal and anterior sagittal bone incision of mandible ramus was recorded.Postoperative CT scan was performed to evaluate the surgical effects according to the lingual split scale(LSS)classification.Results:The wound in all patients healed well and no serious complication was observed.The time for the inner horizontal and anterior sagittal bone incision of mandible ramus in plate group and no plate group was(125.67±2.23)s and(141.15±3.69)s respectively(P<0.05).The probability of mandi-ble splitting according to Hunsuck standard osteotomy line increased from 42.86%to 66.67%,and the probability of osteotomy line passing through mandibular nerve canal decreased from 33.33%to 7.41%,by using osteotomy guide plate.In addition,LSS4 type of osteotomy line was avoided by using osteotomy guide plates.Data analysis showed that the split pattern of sagittal split osteotomy of the mandibular ramus was influenced by the application of osteotomy guide plates(P<0.05).Conclusion:The customized dental-bone supported osteotomy guide plate is effective in the completion of the operation and reducing time consuming and surgical complication in BSSO.
8.One case of Rosai-Dorfman disease misdiagnosed as facial inflammation

Rosai-Dorfman disease(RDD)is a rare non-Langerhans cell histiocytosis.The etiology and pathogenesis of RDD have not been fully explained,and its occurrence is even rarer in the maxillofacial region.This paper reported a patient who was first misdiagnosed as facial inflammation caused by oral disease and finally diagnosed as RDD.Clinical features,diagnosis,treatment,and prognosis were discussed based on the literature.This work provides reference for fu-ture diagnosis and differentiation of RDD in cheek.
9.Protective effect and mechanism of RSR13 on lung blast injury in rats at high altitude

Objective To investigate the impact and mechanism of efaproxiral (RSR13),a hemoglobin allosteric agent,on lung injury in rats caused by explosion-induced shock waves in plateau areas. Methods Eighty-two healthy male SD rats (8-week-old,transferred from an altitude of 2 880 m to 4700 m within 6 h)were randomly divided into blast injury group and RSR13+blast injury group (intraperitoneal injection of 150 mg/kg RSR132 h before explosion).Sixty rats were positioned at 5 m from the explosion source and divided into 5-m blast injury group (n=30)and 5-m RSR13+blast injury group (n=30). Additionally,16 rats were positioned at 6 m from the explosion source and then assigned into 6-m blast injury group (n=8)and 6-m RSR13+blast injury group (n=8).The left 6 rats served as control (n=6).Survival outcomes of each rat group positioned 5 m from the explosion source were observed over a 24-hour period.HE staining was used to evaluate the pathological score of the surviving rats positioned at 6 m from the explosion source in 24 h after explosion,along with arterial blood gas analysis.The contents of glutathione (GSH),malondialdehyde (MDA ) and superoxide dismutase (SOD ) in the lung tissues were determined by colorimetry.Western blotting was conducted to measure the expression levels of cleaved caspase-3 and occludin in the lung tissue.Results RSR13 pretreatment increased the survival rate immediately after explosion (93.3% vs 46.7%,P<0.01 )and at 1 h after explosion (86.7% vs 46.7%,P<0.01 )in plateau areas of 5 m from the explosion source.At high altitude,RSR13 pretreatment reduced the pathological score of lung injury in rats 6 m away from the explosion source (8.27±0.93 vs 13.70±0.78,P<0.01 ),but had no significant effect on the results of arterial blood gas analysis in rats with lung blast injury (P>0.05 ).In addition,RSR13 pretreatment also increased GSH content (40.27±12.47 vs 22.62±10.88 μg/g,P<0.05),but showed no obvious effect on MDA content and SOD activity (P>0.05 ),decreased the protein level of cleaved caspase-3 (P<0.01 )and increased that of occludin (P<0.05 )in the lung tissues.Conclusion RSR13 exerts significant protective effect on lung injury in rats caused by explosion-induced shock waves in high-altitude environment,which may be related to its increasing antioxidant capacity,reducing cell apoptosis and decreasing barrier permeability of lung ventilation.
10.Effect of home-based early intensive behavioral intervention using VB-MAPP on verbal ability in children with autism spectrum disorder

BackgroundHome-based intervention occupies a prominent place in the treatment of autism spectrum disorder (ASD) in children, whereas previous studies on the effect of home-based early intensive behavioral intervention on verbal ability of children with ASD are somewhat inadequate. ObjectiveTo study the effects of intensive family behavioral of intervention based on Verbal Behavior Milestones Assessment and Placement Program (VB-MAPP) on the language ability of children with ASD, so as to provide references for the development of family intervention strategies for children with ASD. MethodsChildren aged 2 to 3 years old who attended the Children's Rehabilitation Department of the Second Affiliated Hospital of Shantou University Medical College from September 2020 to December 2021 and met the Diagnostic and Statistical Manual of Mental Disorders, fifth edition (DSM-5) diagnostic criteria for ASD were selected as the study objects. A non-randomized concurrent control trial was conducted to compare a study group (n=24) receiving home-based early intensive behavioral intervention using VB-MAPP with a control group (n=14) receiving training from other special institutions or early childhood education institutions. The intervention lasted for 6 months in both groups. Before and after the intervention, the VB-MAPP milestone assessment was performed in the two groups, and the VB-MAPP milestone score, mand, trac and listener responds were selected as the study indicators. Then the intervention effect was compared between two groups, and the multiple linear regression was performed to screen the related influencing factors. ResultsAfter intervention, the total milestone assessment score, mand, tact and listener responds scores of study group were higher than those of control group, with statistical difference (Z=-4.339~-2.195, P<0.05 or 0.01). Multiple linear regression analysis denoted that the average weekly hours of home-based intervention in the first three months had certain effect on listener responds (β=1.029, P<0.05). ConclusionApplication of home-based early intensive behavior intervention using VB-MAPP may contribute to the improvement of verbal abilities such as mand, tact and listener responds in children with ASD.
