Objective To analyze the incidence of prehypertension (PHTN) and hypertension of male workers in a coal mine and investigate the influencing factors. Methods A total of 10 155 male workers in a coal mine in Shanxi Province were selected as the study subjects using the judgment sampling method. Occupational medical examination was carried out to examine the prevalence of abnormal blood pressure (including PHTN and hypertension) and its influencing factors. Results The prevalence of abnormal blood pressure was 69.9%, with a PHTN prevalence of 44.2% and a hypertension prevalence of 25.7%. The results of multinomial logistic regression analysis showed that the risk of PHTN and hypertension increased with age, drinking frequency and body mass index (all P<0.05). Workers with a pulse rate≥100 times/min had a higher risk of PHTN and hypertension than those with a pulse rate of 60-100 times/min (all P<0.05). The risk of PHTN and hypertension in workers exposed to noise, silica dust or other dust was higher than those without such occupational hazard exposure (all P<0.05). The risk of hypertension in workers with abnormal blood glucose was higher than that in workers with normal blood glucose (P<0.05). Exposure to organic solvents was associated with a higher risk of hypertension (P<0.05). Conclusion The prevalence of PHTN and hypertension among male coal miners in this mine is relatively high. The individual factors such as age, alcohol consumption, pulse rate, and body mass index, along with occupational exposure factors such as noise, silica dust, and other dusts, are influencing factors for PHTN and hypertension.
3.Analysis on influencing factors of chronic diseases of male workers in a coal mine

Objective To analyze the prevalence of chronic diseases and its influencing factors of dust-exposed male workers in a coal mine. Methods A total of 9 782 dust-exposed male workers from a coal mine in Shanxi Province were selected as the study subjects using the purposive sampling method. Their occupational health examination results were collected to analyze the prevalence of chronic diseases and its influencing factors. Results The prevalence of dyslipidemia, hyperuricemia, hypertension and diabetes were 40.3%, 30.7%, 23.5% and 5.6%, respectively. The prevalence of chronic diseases was 64.8%. Among them, the prevalence of having one, two, three or more chronic diseases were 36.5%, 21.6% and 6.7%, respectively. The prevalence of comorbid chronic diseases was 28.3%, with the highest prevalence of concurrent dyslipidemia and hyperuricemia of 11.0%. The results of binary logistic regression analysis showed that the risk of chronic disease was higher in workers <40 years old, smoking, overweight, obesity and total working years >20 years (all P<0.05). The results of multinomial logistic regression analysis showed that workers <40 years old, overweight, obesity and total working years >20 years were risk factors for having one chronic disease (all P<0.05). The workers <40 years old, smoking, overweight, obesity and total working years >20 years were risk factors for having two chronic diseases (all P<0.05). The workers <40 years old, smoking, alcohol consumption, overweight, obesity, other types of work, and working years >20 years were risk factors for having three or more chronic diseases (all P<0.05). Conclusion The prevalence of chronic diseases is high and the comorbidity of chronic diseases is common among dust-exposed male workers. The main influencing factors were age, smoking, alcohol consumption, overweight, obesity, type of work, and working year. Workers with more contributing factors have higher risk of chronic comorbidities.
4.Research on the application of PBL case library in integrated teaching of circulation system

To evaluate the advantages of using PBL case library in teaching circulatory system diseases to clinical medicine undergraduate students, a PBL case library was established and applied in teaching practice in the reform of circulatory system teaching. The PBL case library achieved the characteristics of combining theories with cases, morphology with functions, and basic knowledge with clinical knowledge. The PBL case library also realized the informatization, query, and update of cases. Preliminary application showed that the median practical score of students in the case library group was 94.00 points, which was significantly higher than the 92.00 points in the control group ( P=0.005). The average lesson preparation time for teachers in the case library group was (5.00±1.00) hours, which was significantly shorter than the (6.89±0.42) hours in the control group ( P<0.001). The difficulty score for lesson preparation among teachers in the case library group was significantly lower than that of the control group [(1.89±1.05) vs. (3.22±0.44), P<0.001]. However, there were no significant difference in theoretical scores and student satisfaction with teachers and courses. These results suggest that the construction of case library can improve practical teaching effectiveness and enhance the efficiency of lesson preparation for teachers.
5.Arthroscopic minimally invasive reduction for talus posterior process fractures

Objective:To evaluate the efficacy of arthroscopic minimally invasive reduction in the treatment of talus posterior process fractures.Methods:The clinical data were retrospectively studied of the 42 patients with talus posterior process fracture who had been admitted to Department of Orthopedics, The Fourth Hospital of Wuhan from January 2010 to June 2021. There were 25 males and 17 females, aged from 21 to 60 years (average, 40.5 years). They were assigned into 2 groups according to their different treatments. In the arthroscopic group of 15 cases, arthroscopic reduction and internal fixation (ARIF) were conducted via the posteromedial and posterolateral approaches; in the open reduction group of 27 cases, open reduction and internal fixation (ORIF) were conducted via the posteromedial para-Achilles approach. The 2 groups were compared in terms of operation time, blood loss, hospital stay, fracture clinical healing time, postoperative complications, and the American Society for Foot and Ankle Surgery (AOFAS) ankle-hindfoot score at one year postoperation.Results:There was no significant difference in the preoperative general data between the 2 groups, showing comparability ( P> 0.05). The arthroscopic group incurred significantly less blood loss [(32.0±11.5) mL], hospital stay [(5.3±1.8) d], and fracture clinical healing time [(4.6±1.0) months], and a significantly lower incidence of postoperative complications [20.0% (3/15)] than the open reduction group did [(80.0±15.2) mL, (8.4±2.4) d, (6.3±2.2) months, and 29.6% (8/27)], but significantly longer operation time [(74.0±8.9) min] than the open reduction group [(62.9±5.1) min] ( P<0.05). The AOFAS ankle-hindfoot scores at one year postoperation in both groups were higher than those before operation. The AOFAS ankle-hindfoot scores in the arthroscopic group [(83.0±13.0) points] were significantly higher than those in the open reduction group [(72.3±16.0) points] ( P<0.05). Conclusion:ARIF is a preferred minimally invasive treatment for talus posterior process fractures, because it leads to a smaller incision, less blood loss, shorter hospital stay, quicker clinical healing, a lower incidence of postoperative complications, and better functional improvement of the ankle and hindfoot than ORIF.
6.Genetic analysis of a Fra(16)(q22) fragile site in a female with secondary infertility.

OBJECTIVE:
To explore the genetic basis for a Fra(16)(q22)/FRA16B fragile site in a female with secondary infertility.
METHODS:
The 28-year-old patient was admitted to Chengdu Women's and Children's Central Hospital on October 5, 2021 due to secondary infertility. Peripheral blood sample was collected for G-banded karyotyping analysis, single nucleotide polymorphism array (SNP-array), quantitative fluorescent polymerase chain reaction (QF-PCR) and fluorescence in situ hybridization (FISH) assays.
RESULTS:
The patient was found to harbor 5 mosaic karyotypes involving chromosome 16 in a total of 126 cells, which yielded a karyotype of mos 46,XX,Fra(16)(q22)[42]/46,XX,del(16)(q22)[4]/47,XX,del(16),+chtb(16)(q22-qter)[4]/46,XX,tr(16)(q22)[2]/46,XX[71]. No obvious abnormality was found by SNP-array, QF-PCR and FISH analysis.
CONCLUSION
A female patient with FRA16B was identified by genetic testing. Above finding has enabled genetic counseling of this patient.

7.Effect of septic shock rapid response group on compliance with hour-1 bundle therapy strategy

Objective:To investigate the effect of septic shock rapid response team (SSRRT) on the compliance and prognosis of hour-1 bundle therapy strategy in emergency department patients with septic shock.Methods:This study was conducted on emergency patients with septic shock who were admitted to Huai’an First Hospital Affiliated to Nanjing Medical University from January 2020 to December 2021. The inclusion criteria were emergency patients with septic shock who met the international guideline for surviving sepsis campaigns (Sepsis 3.0). Exclusion criteria: age<18 years, pregnant patients, patients transferred from another hospital who had received fluid resuscitation and/or vasoactive drugs, patients requiring emergency surgery, patients with emergency detention time<1 h, patients who refused to place central venous catheterization or had contraindications for catheterization, and patients who refused to give informed consent. SSRRT was established in January 2021. According to the establishment of SSRRT, patients were divided into the pre-SSRRT intervention group and the post-SSRRT intervention group. The general clinical data of the enrolled patients were collected, including vital signs, lactate, fluid resuscitation volume, maximum vasoactive drug pumping rate at the diagnosis of septic shock, implementation of hour-1 bundle therapy strategies, and ICU and 28-day mortality. Statistical software SPSS 25.0 was used. Pearson chi-squared test was used to compare categorical variables between groups, and Mann-Whitney U test was used to compare continuous variables between groups.Results:A total of 289 emergency patients met the inclusion criteria, 115 patients were excluded, and 174 patients were eventually included, including 83 patients in the pre-SSRRT group and 91 patients in the post-SSRRT group. Compared with the pre-SSRRT group, the proportion of lactate monitoring (54.2% vs. 100.0%, P<0.001), blood culture (27.7% vs. 93.4%, P<0.001), antibiotics (57.8% vs. 97.8%, P<0.001), fluid resuscitation volume ≥ 30 mL/kg (4.8% vs. 34.1%, P<0.001), and mean arterial pressure ≥ 65 mmHg (49.4% vs. 68.1%, P<0.001) were significantly increased. There was no significant difference in ICU mortality (50.6% vs. 37.4%, P=0.079) or 28-day mortality (53.0% vs. 38.5%, P=0.054) between the two groups. Conclusions:SSRRT can significantly improve the compliance of hour-1 bundle therapy strategy implementation in patients with emergency septic shock, and has a trend of decreasing mortality.
8.Meta analysis of the effect of low microbiological diet in preventing infection in patients with malignant tumors during the agranulocytosis after chemotherapy

Objective:To systematically evaluate the effects of a low-microbiological diet on the incidence of infection in cancer patients with agranulocytosis.Methods:The Cochrane Library, PubMed, Embase, Web of Science, CNKI, China Biology Medicine disc (CBM) , Wanfang Database and VIP database were searched by computer. After two researchers independently carried out literature screening, data extraction and evidence quality evaluation, RevMan 5.3 software was used for Meta-analysis.Results:A total of 7 studies were included, including 5 RCT studies and 2 retrospective case-control trials. Meta-analysis results showed that in retrospective controlled studies [ RR=1.29, 95% CI (1.10, 1.50) , P=0.002], and between different subjects (transplanted and non-transplanted patients) [ RR=1.21, 95% CI (1.08, 1.36) , P=0.001], the incidence of infection in the routine diet group was lower than that in the low-microbial diet group. In the prospective randomized controlled study, there was no statistically significant difference in the infection rate between the two groups [ RR=1.08, 95% CI (0.92, 1.28) , P=0.340]. In terms of infection-related mortality, there was no statistically significant difference between the two groups [ RR=1.06, 95% CI (0.76, 1.47) , P=0.750]. Conclusions:Compared with patients with unrestricted diet, the low-microbial diet does reduce the infection rate and infection-related mortality of patients. Dietary restrictions for patients with granulocytic cancer may be appropriately relaxed, and a focus on safe food purchase and handling and good hand hygiene may provide better infection prevention for patients with agranulocytosis.
9.Primary hepatic adenosquamous carcinoma: report of 5 cases

Objective:To explore the clinicopathological characteristics of the primary hepatic adenosquamous carcinoma(ASC).Methods:A retrospective analysis was performed on the clinical data of 5 ASC patients admitted to the First Affiliated Hospital of Anhui Medical University from 2006 to 2019 who underwent surgical resection and were pathologically confirmed.Results:Among the 5 ASC cases, there were 4 males and 1 female. The age ranged from 48 to 73 years. As for the initial symptoms, there were 5 cases complaining upper abdominal pain, 2 cases presenting fever, 1 case presenting weight loss and 1 case presenting jaundice. CA19-9 was significantly higher than normal in 4 cases, while AFP was normal in all. None had definite preoperative diagnosis.All the 5 patients underwent surgical resection with pathology proved primary hepatic ASC. Lymph node metastasis was found in 4 cases and nerve invasion in 2 cases. There were 4 cases at TNM stage ⅣA, one at stage ⅠB. The median disease-free survival (DFS) was 5 months and the overall survival (OS) was 9 months.Conclusions:Primary hepatic adenosquamous carcinoma is a rare type of liver malignant tumor with an extremely poor prognosis. Surgical resection helps little in improving the prognosis.
10.Effects of clinical pathway in cancer pain management among cancer patients

Objective? To explore the effects of clinical pathway in cancer pain management among cancer patients. Methods? We selected 114 cancer patients with pain and opioids pain management in Radiotherapy and Chemotherapy Center at Ningbo First Hospital of Zhejiang Province. A total of 56 cancer patients with pain from March to December 2018 were in observation group with nursing based on self-design clinical pathway; a total of 58 cancer patients with pain from May 2017 to February 2018 were in control group with routine nursing. The Numerical Rating Scale (NRS) was used to compare the effects of pain management of patients between two groups. We also investigated the medication compliance and pain management satisfaction of patients between two groups. Results? As time progressed, the NRS scores of cancer pain patients in the two groups decreased, and the interaction, time and intervention differences between the two groups were statistically significant (P< 0.01). One day before discharge, the score of NRS in breakout pain attack of patients of observation group was (4.78±0.59) lower than that (5.56±0.74) of control group; times of breakout pain of patients of observation group was (2.70±0.71) times per day less than (3.21±0.84) times per day of control group; the differences were statistical (P<0.01). The medication compliance of patients of observation group was (6.69±0.97), higher than (6.02±1.10) of control group; patients' satisfaction with pain management of observation group was (87.84±7.21), higher than (75.93±6.95) of control group;the differences were also statistically significant (P< 0.01). Conclusions? Application of clinical pathway in pain management and nursing for cancer patients with pain can effectively management patients pain and improves patients' medication compliance and satisfaction with nursing.
