1.Early stratification of in-hospital mortality risk in adult septic shock patients and development of a simplified scoring system

Objective To analyze the independent factors impacting the in-hospital prognosis of patients with septic shock,and to construct a simplified scoring system and evaluate its predictive value.Methods A retrospective analysis was carried out on 247 patients with septic shock admitted to the People's Hospital of Xinjiang Uygur Autonomous Region from January 2021 to July 2022,among whom 122 patients survived and 125 died.Univariate analysis and multivariate Cox proportional hazard regression model were used to screen the independent factors affecting in-hospital mortality of septic shock patients.The best cut-off value was ob-tained by using the receiver operating characteristics(ROC)curve,and the continuous variables were conver-ted into binary variables and assigned.Finally,a simplified scoring system was established,and its predictive efficacy for hospital death in septic shock patients was verified.Results The results of multivariate Cox pro-portional hazard regression model showed that the Glasgow coma scale(GCS)score(HR=0.929,95%CI:0.875-0.985,P=0.014),quick sequential organ failure assessment(qSOFA)score(HR=1.475,95%CI:1.094-1.989,P=0.011),lactate level(HR=1.096,95%CI:1.049-1.145,P<0.001),procalcitonin level(HR=1.009,95%CI:1.000-1.018,P=0.048),and albumin level(HR=0.958,95%CI:0.922-0.996,P=0.029)were identified as independent influencing factors for in-hospital mortality in patients with septic shock.The ROC curve showed that the simplified scoring system,based on GCS score,qSOFA score,lactate,procalcitonin,and albumin levels,exhibited an area under the curve and 95%CI of 0.866(0.822-0.910),with an optimal cutoff value of 2.5.The sensitivity and specificity were 80.0%and 78.7%,respectively.Con-clusion The simplified scoring system,based on early assessments of GCS score,qSOFA score,lactate,pro-calcitonin,and albumin levels,demonstrates substantial predictive value for in-hospital mortality in patients with septic shock.
2.Analysis of Factors Influencing Cognitive Dysfunction in Sepsis Survivors

Objective To investigate the current incidence of cognitive dysfunction in sepsis survivors and analyze the factors influ-encing cognitive function in these patients.Methods Patients treated for sepsis and discharged from the Emergency Center of the People's Hospital of the Xinjiang Uygur Autonomous Region between January 2022 and December 2023 were selected retrospectively.Based on the Changsha version of the Montreal cognitive assessment(MoCA),these patients were categorized into groups with and without cognitive dysfunction,to analyze the independent influencing factors of cognitive dysfunction in sepsis survivors.Results A total of 190 patients were included in the study,of which 106 patients were male(55.79%),and 91(47.9%)patients were identified with cognitive dys-function.Univariate analysis showed that there were significant differences in age,ICU length of stay,qSOFA score,SOFA score,A-PACHE Ⅱ score,WBC,lactate,BNP,D dimer and education level between the two groups(P<0.05).Multivariate Logistic regression analysis revealed that age(OR=1.037,95%CI:1.004-1.072),length of intensive care unit(ICU)stay(OR=1.074,95%CI:1.001-1.149),blood ammonia levels(OR=1.023,95%CI:1.007-1.039),qSOFA score(OR=1.781,95%CI:1.113-2.851)and educational level(junior high school:OR=0.190,95%Cl:0.050-0.721;high school and vocational school:OR=0.105,95%CI:0.023-0.484;college and above:OR=0.083,95%CI:0.017-0.408)were independent factors influencing cogni-tive function in sepsis survivors.Conclusion Age,length of ICU stay,blood ammonia levels,and qSOFA score were identified as risk factors for cognitive dysfunction in sepsis survivors,whereas educational level served as a protective factor.

4. Role of brain-specific angiogenesis inhibitor 1 in synapse and cognitive impairment

Adhesion G protein-coupled receptors(aGPCRs) play a significant role in cognitive impairment related diseases. As an important member of aGPCRs, brain-specific angiogenesis inhibitor 1(BAI1) has a prominent impact on anti-angiogenesis, anti-tumor and participating in immune phagocytosis. Recent research found out that BAI1 exerts a great influence on synaptogenesis and synaptic plasticity, but few studying concerning BAI1 in nervous system. Nowadays, the aging of population aggravates the occurrence of cognitive impairment. The pathogenesis of Alzheimer's disease and vascular cognitive impairment remains elusive, and identification of cognitive impairment at an early stage faces challenges. In the stage of mild cognitive impairment, synaptic damage is evident. BAI1 can regulate the function of postsynaptic membrane, synaptogenesis, synaptic signal transmission and the morphological development of dendritic spines. Therefore, it may potentially act as an early-warning index and intervention target for cognitive impairment.
5.IL-37 protects hepatocyte injury against hypoxia/reoxygenation by promoting polarization of M2-type macrophages

Objective To investigate the protective effect of IL-37 on hepatocyte injury against hypoxia/reoxygenation (H/R) by promoting polarization of M2-type macrophages and its molecular mechanisms.Methods Real-time fluorescent quantitative PCR (qRT-PCR) and Western blot were used to detect the levels of IL-37 mRNA and protein in human monocyte-macrophage THP-1 cells with different polarizations.The lentivirus with IL-37 gene was infected into THP-1 cells.The levels of CD206,CD86,ARG1 and iNOS mRNA was detected by qRT-PCR.The levels of CD163 and CD86 protein was detected by flow cytometric analysis.THP-1 cells and L02 cells were co-cultured by Transwell and treated with H/R.The survival rate and apoptotic rate of L02 cells were detected.The levels of alanine transaminase (ALT) and aspartate aminotransferase (AST) in culture medium were measured.The levels of STAT6 and its phosphorylation in THP-1 cells were detected by Western blot.Results The levels of IL-37 mRNA and protein were up-regulated in M2-type macrophages.IL-37 promoted the polarization of M2-type macropahges.M2-type macrophages induced by IL-37 were cocultured with L02 cells,the survival rate was significantly increased by H/R treatment (P =0.015),while the apoptotic rate,ALT level and AST level were significantly decreased (P<0.001).The level of phosphorylated STAT6 in THP-1 cells overexpressing IL-37 was up-regulated (P < 0.01).Conclusions IL-37 can induce polarization of M2-type macrophages and protect hepatocyte injury against H/R.Its mechanism may be related to STAT6 signal pathways.
