Objective:To explore the efficacy of tracheotomy and nasal intubation in airways management in treatment of patients with respiratory failure caused by severe tetanus.Methods:A respective case series study was conducted to analyze the data of 92 patients with respiratory failure caused by severe tetanus admitted to Jiangxi Chest Hospital from January 2012 to December 2019. There were 60 males and 32 females, aged 23-81 years [(47.5±14.1)years]. Overall 43 patients underwent tracheotomy (tracheotomy group), and 49 patients underwent nasal intubation (nasal intubation group). The blood gas related indexes (PaO 2, PaCO 2), catheter retention time, incidence of complications such as airway bleeding, nasosinusitis, catheter blockage and ventilator-associated pneumonia (VAP), and clinical outcome were compared between the two groups. Results:After mechanical ventilation, the PaO 2 and PaCO 2 in both groups were significantly improved ( P<0.01), with no significant difference between groups ( P>0.05). Both groups were similar regarding the catheter indwelling time, catheter blockage, airway bleeding and nasosinusitis ( P>0.05). The incidences of VAP and catheter blockage after extubation in tracheotomy group [12%(5/43), 12%(5/43)] were significantly different from those in nasal intubation group [0%(0/49), 31%(15/49)] ( P<0.05). There were 3 deaths in each group, with the mortality rate of 7% (3/43) in tracheotomy group and of 6% (3/49) in nasal intubation group ( P>0.05). Both groups of live tetanus patients were cured. After 3-month follow-up, the tetanus was cured in the two groups. Conclusions:The tracheotomy and transnasal tracheal intubation are optional for severe tetanus patients with respiratory failure. The former has airway and tissue trauma, but it is convenient for mechanical ventilation management, especially indicated for patients with severe convulsions, angular arch reflexes, poor sedation, and continuous application of muscle relaxants. Nasal intubation is relatively non-invasive, simple and fast method, but the prevention and monitoring of VAP requires higher requirements.
3.Application of "2+1" PAD class based on online course in medical statistics undergraduate teaching

Objective:To explore the effects of "2+1" PAD class based on online course in medical statistics undergraduate teaching.Methods:A total of 326 clinical undergraduates who took medical statistics in the spring term of 2017 were randomly divided into experimental group ( n=186) and control group ( n=140). Students in the experimental group were taught by "2+1" teaching method, which included self-study on line, classroom teaching, discussion and so on. While students in the control group were taught traditionally. Feedbacks about the teaching progress and effects from the students in the two groups were compared. SAS 9.4 was used for quantitative data description, t-test and rank sum test. Results:according to the students, the online-based "2+1" teaching method paid more attention to interdisciplinarity than traditional method ( P<0.01), and the new teaching method was more beneficial for the improvement of students' capacity ( P=0.008) and more conducive for students to engaging in scientific research ( P=0.012). Students in the experimental group had higher overall satisfaction scores ( P<0.01) and better ability for solving comprehensive problems ( P<0.01). Conclusion:The "2+1" PAD class teaching model based on online courses has more advantages than traditional teaching methods does, which should be recommended and adopted in undergraduate teaching.
4.Efficacyof hydromorphone hydrochloride injection for treatment of chronic pain : a meta-analysis

Objective To systematically review the efficacy of hydromorphone hydrochloride injection for treatment of chronic pain.Methods Web of Science Proceedings and PubMed were searched for clinical trials involving the efficacy of hydromorphone for treatment of chronic pain, with no language or time limit.Evaluation indexes included visual analogue scale (VAS) score and the rate of pain control or relief.The studies were screened independently, and the data were extracted by two researchers.Meta-analysis was conducted using the Stata 10 software.Results Eleven studies involving 452 patients were included in our meta-analysis.VAS score was significantly decreased after treatment compared with that before treatment.For the patients with cancer pain, VAS score was significantly decreased after treatment with hydromorphone hydrochoride injection, and the rate of pain control or relief was increased when compared with the other opioid analgesics.Conclusion Hydromorphone hydrochloride injection can treat chronic pain, and it may provide better therapeutic effect than the other opioid analgesics for the patients with cancer pain.
5.Isolation and characterization of promoter of ADS from Artemisia annua.

OBJECTIVETo try to find the ways to enhance the expression of ADS gene encoding amorpha-4,11-diene synthase, a key enzyme in artemisinin biosynthesis pathway catalyzing the formation of amorpha-4,11-diene from farnesyl diphosphate, and accelerate the artemisinin synthesis, the promoter of ADS was isolated and characterized.
METHOD5' untranslated regions of ADS were isolated from Artemisia annua with PCR. For functional characterization, the isolated fragment was fused with GUS reporter gene and introduced into Nicotiana tabacum by Agrobacterium-mediated transformation. The GUS expression regulated by 5' untranslated regions of ADS in transgenic N. tabacum under the normal or stressed conditions were detected by histochemical staining and quantitative spectrophotometry assay.
RESULTThe 2 448 bp DNA fragment upstream of ADS coding sequence was isolated from A. annua and introduced into N. tabacum. Histochemical staining showed that the isolated fragment conferred stable GUS expression in transgenic plants. The quantitative results showed that the GUS activity in transgenic tobacco plants treated by low-temperature (4 degrees C) and ultraviolet irradiation were 1. 6 and 2.2 folds higher than that in the controls.
CONCLUSIONIt was suggested that the isolated fragment had promoter activity and maybe responsive to adverse environmental stresses.

6.Tongbiling for interleukin 2 and its receptor o-chain in vivo and in vitro

BACKGROUND: Tongbiling is the modified guizhi shaoyao zhimu tang (Decoction of Cinnamon Twig, Peony and Anemarrhena) recorded in Synopsis of Prescriptions of the Golden Chamber. It has been used for rheumatoid arthritis(RA) for more than ten years. Here is the experimental study on effects of tongbiling on interleukin 2(IL-2) and its α-chain, IL-2Rα(CD25), to explain the mechanism of tongbiling in regulating the cellular immunity and resisting RA From the aspects of in vivo and in vitro.OBJECTIVE: To study the effect of tongbiling on IL-2 in collagen introduced arthritis of rats and the influence of total base of tongbiling on expression of IL2-Rα (CD25).DESIGN: A totally randomized and controlled experimental study based on the experimental animalsSETTING: Department of Jinkui of a university and the center of organ transplantation and immunity of another university.MATERIALS: The experiment was carried out from April to July 2001 in the Central Laboratory of the First Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine. The 36 Wistar rats, clean degree, male,bought from the Animal Experimental Center of Medical College of Sun Yat-sen University, certificate No. 2000A050, weighted(110 ± 20) g, were divided into 6 groups with 6 in each: normal group, model group, methotrexate (MTX) high dosage group, MTX low dosage group, tongbiling high dosage group, and tongbiling small dosage group.METHODS: The experiment in vivo was done with the modeled rats of collagen-induced arthritis. The foot swelling was measured with volume method, the cells of primary synovium cultured, and the upper clear fluid of culture fluid tested with radioimmunoassay to observe the effect of tongbiling on synovial IL-2. The experiment in vitro was done in the way of separating the lymphocytes, giving extraneous stimulation with phorbol 12, 13-dibutyrate (PDB) or concanavalin A(ConA), using bicolor immunofluorescence labeling, testing with flow cytometer to observe the influence of total base of tongbiling on the expression of CD3 +, CD25 +.MAIN OUTCOME MEASURES: The influence of tongbiling on the foot swelling and synovial IL-2 of rats with collagen-induced arthritis.RESULTS: In the experiment in vivo, the high dosage of tongbiling could relieve obviously the foot swelling, compared with MTX, there was no remarkable difference ( P > 0.05) . Both high and low dosage of tongbiling could inhibit the synthesis of synovial IL-2 ( P < 0.01 ). In the experiment in vitro, the total base of tongbiling could extremely inhibit the expression of CD3 +, CD25 + stimulated by ConA, but not effective for that stimulated by PDB plus ionomycin.CONCLUSION: Tongbiling can inhibit the IL-2 synthesis and the signal transduction of IL-2Rα (CD25) activated by ConA, relieving the inflammation of local joint. It provides an experimental evidence for the application of tongbiling in the treatment of RA.
7.Cloning and Sequencing of Artemisia annua Squalene Synthase Gene and Its cDNA

【Objective】 To clone the squalene synthase gene of Artemisia annua L. for improving the quality and production of Artemisia annua L. by genetic engineering. 【Methods】 PCR amplification, RT-PCR amplification, ligation of the target fragment with a T-vector and sequence analysis of the interested gene were performed. 【Results】 An expected 3590 bp fragment was amplified by PCR and an expected 1257 bp fragment was amplified by RT-PCR. The two cloned fragments were identified by PCR and restriction enzyme digestion respectively. The preliminary sequence data indicated that the results obtained were similar to that from GenBank, and the difference was only found in several base pairs. 【Conclusion】 The squalene synthase gene and cDNA of Artemisia annua L. were successfully cloned and sequenced.
8.Expression of epidermal growth factor and epidermal growth factor receptor in rat periodontal tissues during orthodontic tooth movement.

OBJECTIVETo detect the expression and distribution of epidermal growth factor (EGF) and epidermal growth factor receptor (EGFR) in periodontal tissues, and analyze the role of EGF in orthodontic tooth movement.
METHODSAccording to Kings methods, 40 g mesial force was applied to pull the left maxillary first molar in the rat. Using immunohistochemical method (HI-SABC method) to localize and examine the expression of EGF and EGFR in decalcified alveodental connective tissues at 24 hours and 168 hours of tooth movement.
RESULTSEGF and EGFR were stained at some of periodontal ligament of furcation and radical regions in control group. These expressions of EGF and EGFR increased in periodontal tissues (P < 0.01), with the expressions at 168 hours higher than those at 24 hours (P < 0.01). And levels of EGF and EGFR at tension side were higher than those at pressure side at the same time (P < 0.01).
CONCLUSIONSEpidermal growth factor participated in the tissues remodeling during orthodontic tooth movement and especially played a more important role in orthodontic bone formation.

9.Studies on Artemisinin Sensitizing Compound in Inhibiting Tumor Cell Proliferation and Suppressing Transplanted Tumor Growth in Tumor-bearing Nude Mice

Objective To enhance the anti-tumor efficiency of artemisinin and to explore the anti-tumor and sensitivity-enhancing mechanism of Artemisinin Sensitizing Compound(ASC).Methods The growth-inhibition rate of HepG2 cells was determined by methyl thiazolyl tetrazolium(MTT) method.Tumor-bearing nude mice were given inoculation of HepG2 cells subcutaneously.After treatment for 15 days,the tumor-inhibition rate was calculated according to the tumor mass weight.Results The growth-inhibition rate of tumor cells in ASC group(artesunate combined with diethyl maleate,mercaptosuccinic acid and aminotriazole),was increased 43.84%,and the tumor-inhibition rate was three times as much as that in artesunate alone group with the same titer.The amount of antioxidant agents and the activity of antioxidant enzymes were higher in the control group than those in the artesunate alone group and ASC group,while were lower in ASC group than those in the artesunate alone group.Conclusion ASC has anti-tumor and sensitivity-enhancing effect through decreasing the amount of anti-oxidant agents and inhibiting activities of anti-oxidant enzymes.
10.Tissue Culture and Plant Regeneration of Aristolochia contorta Bge

[Objective] To explore the culture conditions of inducing callus from tissue of Aristolochia contorta Bge (ACB). [Methods] The leaves of ACB were used as the explants. Basic medium (including 1/2 MS medium, MS medium and modified MS medium) containing corresponding phyto-hormones was applied for the induction of ACB callus. The influerices of different culture conditions such as three kinds of basic medium, different pH values and addition of different kinds of phyto-hormones at different concentrations into the basic medium respectively or simultaneously, on the callus induction of ACB were observed. [Results] (1) After the three kinds of basic medium were added with the phytohormones of 0.1mg/L kinetin (KT) and 0.2mg/L ?-naphthalene acetic acid (NAA) , a large amount of ACB calluses were induced in the MS medium and modified MS medium, while ACB calluses did not occur in the 1/2 MS medium. (2) When the pH value of the culture medium composed of modified MS medium and 0.1mg/L KT and 0.2mg/L NAA was at 5.8, 7.0 and 8.0, a large amount of ACB calluses were induced in the medium with pH value being 7.0 and 8.0 while a few calluses occurred in the medium with pH value being 5.8. (3) ACB calluses were induced in modified MS medium with NAA added , but calluses did not occur in modified MS medium with KT added. When the modified MS medium was added with different kinds of phyto-hormones at different concentrations, ACB calluses were loose in the medium with high-concentration NAA and this did not benefit to the differentiation of buds for too fast cell division, and the callus induction rate was low in the medium with low-concentration NAA. The optimized culture medium was modified MS medium with 1mg/L KT and 0.5mg/L NAA added. [Conclusion] The optimum culture conditions of inducing callus from ACB tissues are: MS medium or modified MS medium with 1mg/L KT and 0.5 mg/L NAA added being the culture medium, and pH value being 7.0-8.0.
