1.Association between heatwave and risk of traffic injuries and its disease burden in Yunnan Province

Background Previous studies found that high temperature and heatwave increase the risk of traffic injuries. The complex road conditions in Yunnan Province result in frequent traffic accidents. However, there is limited evidence on the correlation between heatwave and traffic injuries in Yunnan Province. Objective To assess the association between heatwave events and traffic injuries, to estimate its disease burden, and to identify relevant sensitive groups. Methods We collected data on traffic injury cases and concurrent meteorological information from four surveillance sites in Yunnan Province, China: Dali, Lufeng, Zhaoyang, and Qilin from May to September each year from 2015 to 2023. Traffic injury cases refer to patients who visited the outpatient or emergency departments of local surveillance hospitals for the first time due to traffic injuries. Meteorological data were derived from the fifth generation atmosphericreanalysis dataset of the global climate provided by the European Centre for Medium-Range Weather Forecasts. A time-stratified case-crossover design combined with distributed lag non-linear model was used to analyze the association between short-term exposure to heatwave and traffic injuries. ObjectiveTo investigate the triglyceride-glucose (TyG) index and the level of serum homocysteine (Hcy) in association with the incidence of stroke in type 2 diabetes mellitus (T2DM) patients. MethodsBased on the chronic disease risk factor surveillance cohort in Pudong New Area, Shanghai, excluding those with stroke in baseline survey, T2DM patients who joined the cohort from January 2016 to October 2020 were selected as the research subjects. During the follow-up period, a total of 318 new-onset ischemic stroke patients were selected as the case group, and a total of 318 individuals matched by gender without stroke were selected as the control group. The Cox proportional hazards regression model was used to adjust for confounding factors and explore the serum TyG index and the Hcy biochemical indicator in association with the risk of stroke. ResultsThe Cox proportional hazards regression results showed that after adjusting for confounding factors, the risk of stroke in T2DM patients with 10 μmol·L⁻¹
3.Professor YANG Jun's experience in treatment of refractory facial paralysis with refined mode of acupuncture and moxibustion.

The paper summarizes Professor YANG Jun's experience in treatment of refractory facial paralysis with the refined mode of acupuncture and moxibustion, so as to provide new ideas and methods for the disease treated with acupuncture and moxibustion. Professor YANG Jun puts forward the refined mode of diagnosis and treatment of acupuncture and moxibustion for refractory facial paralysis. On the basis of syndrome/pattern differentiation of TCM, combined with meridian theory and modern anatomy, the symptom differentiation and meridian differentiation are performed accurately. By selecting local acupoints (e.g. Touwei [ST8], Sibai [ST2], Cuanzhu [BL2] and Yangbai [GB14]), distal acupoints (e.g. Zusanli [ST36], Hegu [LI4] and Jinsuo [GV8]), specific acupoints (e.g.Qianzheng [EX-HN16] and Touqiaoyin [GB11]) and the acupoints for regulating the mind (e.g. Dazhui [GV14] and Shuigou [GV26]), the prescription is composed. The different needling techniques are delivered at the acupoints (e.g. point-to-point needling with long needle, stuck needling by lifting and dragging, and balance needling on the healthy side). Besides, the facial rehabilitation is considered as the adjuvant treatment. This mode of treatment obtains the precise diagnosis and treatment to achieve the utmost therapeutic effects.

The paper summarizes Professor YANG Jun's thoughts on clinical treatment with acupuncture and moxibustion. Professor YANG Jun puts forward the "refined mode for diagnosis and treatment of diseases with acupuncture and moxibustion", aiming to improve the capacity of diagnosis and treatment in clinical practice. He advocates that the diagnosis and treatment should be guided by the identification of etiologies, syndromes and meridians; in accordance with regulating the shape/form, balancing yin and yang, and harmonizing the mind; and by means of skillful techniques of acupuncture and moxibustion, simplified selection of acupoints and delicate manipulations. Objective:To explore the clinical efficacy of modified Chaihu Jia Longgu Muli Decoction in patients with liver and kidney Yin deficiency syndrome of Parkinson's disease (PD) and the effects on phosphatase and tensin homolog-induced putative kinase 1 (PINK1) /E3 ubiquitin ligase (Parkin) signaling pathway.Methods:A randomized controlled trial study was conducted. A total of 120 PD patients from the Department of Encephalopathy of Xi'an Hospital of Traditional Chinese Medicine from May 2020 to May 2024 were selected as the observation objects and divided into 2 groups through random number table method, with 60 cases in each group. The control group took dopashydrazine tablets orally, while the combined group took Chaihu Jia Longgu Muli Decoction on the basis of the control group. Two groups were treated continuously for 2 months. The levels of glial cell-derived neurotrophic factor (GDNF), nerve growth factor (NGF), brain-derived neurotrophic factor (BDNF), and basic fibroblast growth factor 2 (FGF2) were detected by ELISA. The expression levels of PINK1, Parkin and chelate 1 (SQSTM1) mRNA were detected by real-time fluorescence quantitative PCR. The adverse reactions during the treatment were observed and recorded to evaluate the clinical efficacy.Results:The total effective rate was 95.00% (57/60) in the combined group and 83.33% (50/60) in the control group, with statistical significance ( χ2=4.23, P=0.040). After treatment, the levels of serum GDNF [(510.78±31.57) ng/L vs. (431.89±30.45) ng/L, t=13.93], BDNF [(36.47±3.72) ng/L vs. (27.84±3.55) ng/L, t=13.00], NGF [(57.24±4.17) ng/L vs. (50.72±4.11) ng/L, t=8.63] and FGF2 [(26.21±3.71) ng/L vs. (21.63±3.54) ng/L, t=6.92] in the combined group were higher than those in the control group ( P<0.001). After treatment, the mRNA expression levels of PINK1 (2.38±0.07 vs. 2.19±0.05, t=17.11), Parkin (1.89±0.03 vs. 1.74±0.05, t=19.93), and SQSTM1 (1.82±0.07 vs. 1.61±0.05, t=18.91) in the combined group were higher than those in the control group ( P<0.001). During the treatment period, the incidence of adverse reactions was 6.67% (4/60) in the combined group and 10.00% (6/60) in the control group, without statistical significance ( χ2=0.44, P=0.509). Conclusion:Modified Chaihu Jia Longgu Muli Decoction can increase the levels of neurotrophic factors in patients with PD of liver and kidney yin deficiency syndrome, regulate the PINK1/Parkin pathway, protect neurons, improve mitochondrial autophagy function, and enhance clinical efficacy.

This article systematically summarizes the clinical experience of Professor Yang Jun,a nationally renowned traditional Chinese medicine(TCM)physician,in applying refined direct moxibustion(applying a moxibustion pen made by Chinese medical extract)to treat functional dyspepsia(FD)of the stuffiness-fullness type.Based on decades of clinical practice,Professor Yang innovatively established a moxibustion therapy system for FD,which centers on TCM syndrome differentiation and treatment.The system emphasizes the refined identification of epigastric stuffiness and fullness syndrome,particularly focusing on the relative significance of abdominal distension and poor appetite.Its therapeutic features lie in establishing the principle of"prioritizing mind regulation while holistically harmonizing body and spirit",combined with personalized moxibustion dosage control and a unique refined direct moxibustion technique.By optimizing the configuration of each step in moxibustion therapy,the system maximizes therapeutic efficacy,providing novel theoretical foundations and clinical strategies for moxibustion treatment of stuffiness-fullness type of FD.

This article introduces Professor Yang Jun's clinical experience in treating bronchial asthma using warming needle moxibustion via the governor vessel-unblocking and conception vessel-regulating method.Professor Yang posits that asthma pathogenesis-whether triggered by internal imbalances or external pathogens-ultimately stems from yin-yang disharmony leading to rebellious lung qi and impaired diffusion/descending functions.Thus,restoring dynamic yin-yang balance constitutes the core therapeutic principle.As the governor and conception vessels govern the body's yin-yang regulation,Professor Yang's decades of clinical practice substantiate that"harmonizing these vessels determines life's vitality".His protocol combines warming needle moxibustion with press needles to activate governor-conception vessel functions,achieving five therapeutic effects:(1)yin-yang harmonization,(2)qi movement regulation,(3)meridian unblocking,(4)visceral stabilization,and(5)pathogen elimination,demonstrating remarkable efficacy.
9.Association Between Low Birth Weight and Dementia Risk:A Large-scale Prospective Study

Objective To investigate the association between birth weight and dementia risk and the mediating roles of chronic diseases,and to assess potential biological pathways underlying the birth weight-associated dementia risk based on large-scale proteomics.Methods We used data from 279 743 participants aged 40 to 69 years enrolled in the UK Biobank.Birth weight was categorized into low birth weight(≤2 500 g),normal birth weight(2 500-3 999 g),and macrosomia(≥4 000 g).Multivariable Cox proportional hazards regression models were used to assess the associations between birth weight categories and all-cause dementia and its subtypes(Alzheimer's disease and vascular dementia).Proteomics analyses were conducted to identify proteins and the potential pathways involved.Results Low birth weight was associated with higher risks for all-cause dementia and its subtypes.The hazard ratios were 1.18(95%CI,1.08-1.30)for all-cause dementia,1.14(95%CI,1.00-1.31)for Alzheimer's disease,and 1.22(95%CI,1.01-1.48)for vascular dementia.A non-linear relationship was observed between birth weight and dementia risk(P for nonlinearity<0.001).Certain cardiometabolic diseases in middle-aged adults,such as diabetes,stroke,hypertension,and dyslipidemia,played a significant mediating role in the relationship between low birth weight and dementia risk,with the mediation proportion being 6.3%to 15.8%.Proteomic analyses identified 21 proteins linked to both low birth weight and all-cause dementia risk,which were significantly enriched in the pathways for viral protein interaction with cytokines and cytokine receptors,adipocytokine signaling,and cytokine-cytokine receptor interaction.Conclusion Low birth weight is positively associated with dementia risk.Cardiometabolic diseases in middle-aged adults may mediate the relationship between low birth weight and dementia risk.A number of proteins and the associated pathways underscore the relationship between low birth weight and dementia risk.
10.Association of long working hours, anxiety symptoms and insomnia symptoms among footwear industry workers

Objective To explore the association between long working hours, anxiety symptoms, and insomnia symptoms among footwear industry workers, and to examine the mediating effect of anxiety symptoms between long working hours and insomnia symptoms. Methods A total of 710 workers from 15 footwear manufacturing enterprises in Guangdong Province were selected as study subjects using a stratified random sampling method. Long working hours, anxiety symptoms, and insomnia symptoms among the workers were assessed using the National Key Population Occupational Health Literacy Monitoring Questionnaire, the Chinese version of the Generalized Anxiety Disorder Self-Rating Scale, and the Self-Rating Sleep Questionnaire. Binary logistic regression was applied to analyze the associations of long working hours and anxiety symptoms with insomnia symptoms, and a mediation effect model was used to examine the mediating role of anxiety symptoms. Results The workers with long working hours accounted for 69.6% of participants. The detection rates of anxiety symptoms and insomnia symptoms were 17.0% and 20.7% in these workers, respectively. Binary logistic regression analysis showed that both long working hours and anxiety symptoms were influencing factors for insomnia symptoms (both P<0.01). Mediation analysis indicated that long working hours had a direct effect on insomnia symptoms, accounting for 67.9% of the total effect. Anxietysymptoms played a partial mediating role between long working hours and insomnia symptoms, with an effect value of 0.18 (95% confidence interval 0.07-0.30, P<0.01), accounting for 32.1% of the total effect. Conclusion Both long working hours and anxiety symptoms are influencing factors for insomnia symptoms among the footwear industry workers, and anxiety symptoms partially mediate the impact of long working hours on insomnia symptoms.
