Atypical lipomatous tumor(ALT)is a rare soft tissue sarcoma originating from adipocytic tissue. The clinical manifestations, imaging findings, and pathological examinations of one patient with ALT in the lower extremity were reported to provide reference for the clinical diagnosis and treatment of this disease. The patient was a 64-year-old male who was admitted due to a mass in the left thigh with swelling and pain for 5 years. The physical examination results showed a subcutaneous mass was palpable on the medial side of the left thigh, soft in texture, demonstrating good mobility and positive tenderness. The magnetic resonance imaging(MRI)non-contrast scan showed a well-defined lesion with an intact capsule. The lesion presented high signal intensity on T1-weighted image(T1WI),similar to subcutaneous fat signal;on T2-weighted image(T2WI)-Ideal in-phase images, the lesion showed high signal, while water images showed low signal, with multiple high-signal foci inside. On T2WI fat-suppression sequence,a low-signal mass was observed with multiple patchy high signals inside.Three days after admission, the patient underwent lesion resection.Intraoperatively, the tumor was located within the fascia, between the medial quadriceps muscles, presenting as lipomatous tissue with an intact capsule, mildly adherent to surrounding tissue, and with partial muscle invasion. The mass and invaded muscle were completely excised. The postoperative pathological examination results revealed a gray-yellow nodule measuring 16.0 cm.× 10.0 cm×4.0 cm,with a smooth surface and intact capsule. The cut surface was gray-yellow,fatty-like,and soft in texture.Additional gray-yellow tissue fragments were found, with a total volume of 5.0 cm×4.0 cm×1.2 cm,exhibiting a gray-brown cut surface with moderate texture.Under microscope, the lesion consisted mainly of relatively mature adipose tissue.Enlarged and hyperchromatic nuclei were observed, with scattered mononuclear or multinuclear atypical stromal cells.Fibrous septa contained variable numbers of univacuolated or multivacuolated lipoblasts. The immunohistochemistry results showed positivity for cyclin-dependent kinase 4(CDK4)and murine double minute 2(MDM2).The fluorescence in situ hybridization(FISH)results demonstrated MDM2 gene amplification in tumor cells. The pathological diagnosis was ALT of the left lower extremity. At 6-month follow-up after operation,no tumor recurrence was observed. The preoperative MRI detection may provide effective evidence for the diagnosis of ALT,while postoperative pathological examination can confirm the diagnosis and help evaluate the prognosis of the patients.

Abstract
Modern western medicine typically focuses on treating specific symptoms or diseases, and traditional Chinese medicine (TCM) emphasizes the interconnections of the body’s various systems under external environment and takes a holistic approach to preventing and treating diseases. Phenomics was initially introduced to the field of TCM in 2008 as a new discipline that studies the laws of integrated and dynamic changes of human clinical phenomes under the scope of the theories and practices of TCM based on phenomics. While TCM Phenomics 1.0 has initially established a clinical phenomic system centered on Zhenghou (a TCM definition of clinical phenome), bottlenecks remain in data standardization, mechanistic interpretation, and precision intervention. Here, we systematically elaborates on the theoretical foundations, technical pathways, and future challenges of integrating digital medicine with TCM phenomics under the framework of “TCM phenomics 2.0”, which is supported by digital medicine technologies such as artificial intelligence, wearable devices, medical digital twins, and multi-omics integration. This framework aims to construct a closed-loop system of “Zhenghou–Phenome–Mechanism–Intervention” and to enable the digitization, standardization, and precision of disease diagnosis and treatment. The integration of digital medicine and TCM phenomics not only promotes the modernization and scientific transformation of TCM theory and practice but also offers new paradigms for precision medicine. In practice, digital tools facilitate multi-source clinical data acquisition and standardization, while AI and big data algorithms help reveal the correlations between clinical Zhenghou phenomes and molecular mechanisms, thereby improving scientific rigor in diagnosis, efficacy evaluation, and personalized intervention. Nevertheless, challenges persist, including data quality and standardization issues, shortage of interdisciplinary talents, and insufficiency of ethical and legal regulations. Future development requires establishing national data-sharing platforms, strengthening international collaboration, fostering interdisciplinary professionals, and improving ethical and legal frameworks. Ultimately, this approach seeks to build a new disease identification and classification system centered on phenomes and to achieve the inheritance, innovation, and modernization of TCM diagnostic and therapeutic patterns.

The paper reports a 32-year-old female acute myeloid leukemia patient who developed graft-versus-host disease after paternal hematopoietic stem cell transplantation, which subsequently led to renal thrombotic microangiopathy. She subsequently required a kidney transplant from the same donor 5 years later due to renal failure. Considering that both the bone marrow and kidney were from the same donor and the recovery of renal function was favorable, immunosuppressive therapy was discontinued after a short course of anti-rejection treatment, with maintained stable kidney function. This case suggests that under the condition of high chimerism, allogeneic hematopoietic stem cell transplantation and kidney transplantation from the same donor can achieve immune tolerance, potentially improving solid organ transplantation success rate. The findings provide a novel therapeutic approach for solid organ transplantation following allogeneic hematopoietic stem cell transplantation.

AIM: To explore the clinical features, diagnosis and treatment experience and the distribution characteristics of pathogenic microorganisms of primary canaliculitis, and provide reference for its diagnosis and treatment. METHODS: Retrospective clinical study. A total of 119 cases(120 eyes)diagnosed as primary canaliculitis in the department of ophthalmology of Wuxi No.2 People's Hospital from June 2019 to February 2023 were included. The treatment methods were mainly divided into conservative treatment(removing canaliculus stones through lacrimal punctum combined with injecting antibiotic eye ointment into the tube)and surgical treatment. The inspection methods of pathogenic microorganisms included secretion smear microscopy and microbial culture.RESULTS: Primary canaliculitis was more common in middle-aged and older female, mainly manifested by long-term red eye and increased secretion; however, the majority was not accompanied by tearing. Totally, 118 cases(99.2%)had monocular disease, while 63 cases(63 eyes; 52.5%)had inferior lacrimal canaliculus disease. Laboratory examination: Among 119 cases(120 eyes), 4 cases(4 eyes)did not undergo laboratory examination, and the other 115 cases(116 eyes)were as follows: Gram staining microscopy of secretion smear showed that Actinomyces were detected in 102 cases(103 eyes; 88.8%), while no fungus was detected; Microbial culture: 85 cases(86 eyes; 74.1%)were positive for bacterial culture. A total of 111 bacterial strains were cultured, which contained 26 types of bacteria. Among them, 32 strains were aerobic(28.8%); 26 strains were anaerobic(23.4%); and 53 strains were facultative anaerobic(47.7%). The most common bacteria were streptococcus(20 strains), staphylococcus(13 strains), Propionibacterium(10 strains), and capnocytophaga(10 strains). Only 4 cases(4 eyes; 3.4%)of microbial cultures were positive for Actinomyces. Fungus was negative in all microbial cultures. Treatment: Of the 119 cases(120 eyes), 114 cases(115 eyes; 95.8%)were cured by conservative treatment of removing lacrimal canaliculus stones through lacrimal punctum and intracanalicular ointment infiltration(IOI), while 5 cases(5 eyes)were not effective in conservative treatment; however, all of them were cured after surgical treatment, and the cure rate for primary canaliculitis was 100.0%.CONCLUSION: The incidence of primary canaliculitis is low, and it is prevalent in middle-aged and older female. Single lacrimal canaliculus is more common, which could be missed and misdiagnosed in clinic. Actinomyces is the major pathogen observed mostly in mixed infections, with heterogeneous strains, mainly anaerobic or facultative anaerobic bacteria. Streptococcus and Staphylococcus are the most common whereas fungal canaliculitis is rare. The cure rate of primary canaliculitis is high after diagnosis, and IOI method is recommended as the initial treatment of canaliculitis.

ObjectiveBased on cyclic adenosine monophosphate (cAMP)/cAMP-regulated guanine nucleotide exchange factor 1 (Epac1)/Ras-homologous protein 1 (Rap1) signaling pathway to explore the myocardial protective mechanism of Yiqi Huoxue prescription on coronary heart disease (CHD) rats with Qi deficiency and blood stasis syndrome. MethodEighty-eight specific-pathogen-free (SPF) grade male Sprague-Dawley (SD) rats were divided into a sham operation group (n=12) and an experimental group (n=76) according to the random number table. The experimental group underwent a restricted diet and exhaustive swimming combined with left anterior descending (LAD) coronary artery ligation to construct a model of CHD with Qi deficiency and blood stasis syndrome, and electrocardiograms (ECGs) of rats before and after the LAD operation were collected. After the model was successfully established, the rats were randomly divided into model group, Yiqi Huoxue prescription low-dose group (4.28 g·kg-1), medium-dose group (8.55 g·kg-1), high-dose group (17.1 g·kg-1), and Western medicine group (isosorbide mononitrate tablets, 3.6 mg·kg-1). Rats were intragastrically administered assigned drugs for 4 weeks consecutively, while the sham operation group and the model group were administered with equal volumes of double distilled water. Twenty-four hours after the final administration, the rats were anesthetized with isoflurane to detect the left ventricular end-diastolic diameter (LVEDD), left ventricular end-systolic diameter (LVESD), left ventricular fractional shortening (FS, %), and ejection fraction (EF, %) by echocardiography. Blood samples were collected from the abdominal aorta for hemorheological measurements, and plasma cAMP levels were determined using enzyme-linked immunosorbent assay (ELISA). Myocardial tissue was collected for hematoxylin-eosin (HE) staining and Masson staining to observe myocardial pathological damage, and a transmission electron microscope was used to observe ultrastructural changes of myocardial tissue, fluorescent quantitative real-time polymerase chain reaction (Real-time PCR) was used to detect the expressions of myocardial Epac1, Rap1 GTPase activating protein (Rap1GAP) and Rap1 mRNA, and Western blot was used to detect the expressions of myocardial Epac1, Rap1GAP and Rap1 protein. ResultCompared with those in the sham operation group, LVEDD and LVISD of rats in the model group significantly increased (P<0.01), and the ratios of EF and FS significantly decreased (P<0.01), indicating symptoms of heart function decline, referred to as "heart Qi deficiency". The viscosity of whole blood and plasma significantly increased (P<0.01), and the content of cAMP significant increased (P<0.01). In addition, there was a significant proliferation of collagen fibers in myocardial tissue (P<0.01), and the ultrastructure of the myocardial tissue was severely damaged, indicating pathological changes consistent with "blood stasis". Real-time PCR results showed that Epac1 and Rap1 mRNA levels in the model group were significantly reduced (P<0.01), while Rap1GAP mRNA levels were significantly increased (P<0.01). Western blot analysis showed a significant decrease in Epac1 protein expression (P<0.01) and a significant increase in Rap1GAP protein expression (P<0.05). Compared with the model group, Yiqi Huoxue prescription improved cardiac function, reduced blood viscosity, lowered plasma cAMP levels, decreased collagen fiber proliferation, and improved myocardial ultrastructure damage in CHD rats with Qi deficiency and blood stasis syndrome. The high-dose group showed the most significant effects. In the high-dose group, Epac1 mRNA and protein expression levels significantly increased (P<0.01), Rap1 mRNA expression significantly increased (P<0.01), and Rap1GAP mRNA and Rap1GAP/Rap1 protein expression levels significantly decreased (P<0.05, P<0.01). ConclusionYiqi Huoxue prescription can improve cardiac function, reduce blood viscosity and plasma cAMP levels, improve myocardial damage, and reduce collagen fiber proliferation in CHD rats with Qi deficiency and blood stasis syndrome. The myocardial protection mechanism may be related to the regulation of the cAMP/Epac1/Rap1 signaling pathway.

Objective To build a dataset encompassing a large number of stained tongue coating images and process it using deep learning to automatically recognize stained tongue coating images. Methods A total of 1 001 images of stained tongue coating from healthy students at Hunan University of Chinese Medicine and 1 007 images of pathological(non-stained)tongue coat-ing from hospitalized patients at The First Hospital of Hunan University of Chinese Medicine with lung cancer, diabetes, and hypertension were collected. The tongue images were randomi-zed into the training, validation, and testing datasets in a 7:2:1 ratio.A deep learning model was constructed using the ResNet50 for recognizing stained tongue coating in the training and validation datasets. The training period was 90 epochs. The model's performance was evaluated by its accuracy, loss curve,recall,F1 score, confusion matrix, receiver operating characteristic(ROC)curve, and precision-recall(PR)curve in the tasks of predicting stained tongue coating images in the testing dataset. The accuracy of the deep learning model was compared with that of attending physicians of traditional Chinese medicine(TCM). Results The training results showed that after 90 epochs, the model presented an excellent classification performance. The loss curve and accuracy were stable, showing no signs of overfitting. The model achieved an accuracy, recall, and F1 score of 92%,91%,and 92%,re-spectively.The confusion matrix revealed an accuracy of 92%for the model and 69%for TCM practitioners. The areas under the ROC and PR curves were 0.97 and 0.95,respectively. Conclusion The deep learning model constructed using ResNet50 can effectively recognize stained coating images with greater accuracy than visual inspection of TCM practitioners. This model has the potential to assist doctors in identifying false tongue coating and prevent-ing misdiagnosis.
