1.The Impact of Visual Impairment on Gait Characteristics in Hemiplegic Patients

Objective To explore the impact of vision impairment(VI)on the gait of hemiplegic patients,assess their walking ability and fall risks,and provide a basis for developing effective rehabilitation strategies.Methods Thirty hemiplegic patients were enrolled and stratified by the severity of visual acuity impairment into three groups(unimpaired,mildly impaired,and severely impaired).The gait data of patients under uncorrected vision were collected using the Qualisys motion capture system and the Kistler three-dimensional force platform,and the balance ability of patients was assessed simultaneously.Subsequently,the gait and assessment data were statistically analyzed to compare inter-group differences.Results Compared with the visually unimpaired group,significant differences in step length,symmetry,and walking speed were observed in hemiplegic patients of the mild visual impairment group and severe visual impairment group.As VI increased,gait abnormalities became more pronounced,with a longer double-limb support phase,a longer swing phase of the affected limb,and a shorter single-limb support phase of the affected limb in the gait cycle.Compared with the visually unimpaired group,significant differences in center of pressure(COP)and COP symmetry were found between the mild visual impairment group and severe visual impairment group,with gait abnormalities intensifying.The Berg balance scale(BBS)scores showed that there was a significant difference between the visually unimpaired group and severe visual impairment group,indicating that the group with visual impairment had poorer balance ability.Conclusions VI has a significant negative impact on the gait and walking ability of hemiplegic patients.This study emphasizes the importance of focusing on the impact of VI in the rehabilitation of hemiplegic patients,with regular vision assessments and personalized interventions being conducted,which are of great significance in enhancing patients' walking quality.

3.Risk factors for postoperative hypoxemia after whole lung lavage in patients with pneumoconiosis and the predictive value of ultrasound indicators

Objective To analyze the influencing factors of postoperative hypoxemia after whole lung lavage (WLL) in patients with pneumoconiosis and to evaluate the predictive value of lung ultrasound scores. Methods A total of 757 patients with pneumoconiosis who underwent unilateral large-volume WLL were selected as the research subjects by continuous sampling method. The patients with hypoxemia after large-volume WLL were assigned to the observation group (82 cases), and those without hypoxemia were assigned to the control group (675 cases). Patients′ basic information and clinical data of the two groups were collected using a retrospective investigation method. The binary logistic regression analysis was used to analyze the influencing factors of hypoxemia after large-volume WLL in patients with pneumoconiosis, and receiver operating characteristic (ROC) curve analysis was conducted to assess the predictive value of ultrasound indicators. Results The combined rate of postoperative hypoxemia in the patients was 10.8% (82/757). The multivariate logistic regression analysis showed that the risk of hypoxemia in patients aged ≥65 years was higher than that in patients aged <65 years (P<0.05). The risk of hypoxemia was higher in patients with increased pre-extubation lung ultrasound score than those with lower pre-extubation lung ultrasound score (P<0.01). Patients with a higher partial pressure of carbon dioxide in arterial blood (PaCO2) had a higher risk of concurrent hypoxemia than those with a lower pressure (P<0.01), while patients with lower arterial partial pressure of oxygen (PaO2) had a higher risk of concurrent hypoxemia than those with higher PaO2 (P<0.01). Pearson correlation analysis showed that lung ultrasound scores were negatively correlated with PaO2 and positively correlated with PaCO2 (both P<0.01). The ROC curve analysis showed that the area under the curve was 0.855, the 95% confidence interval was 0.828 to 0.879, the diagnostic sensitivity was 84.2%, and the specificity was 75.1%. Conclusion Age, pre-extubation lung ultrasound score, and PaCO₂ are risk factors for hypoxemia after WLL in patients with pneumoconiosis, while PaO₂ is a protective factor. Pre-extubation lung ultrasound scoring demonstrates good accuracy in predicting postoperative hypoxemia among pneumoconiosis patients after WLL.
4.Immediate Impact of Acute Visual Acuity Decline on the Gait of Hemiplegic Patients

Objective To explore the immediate impact of acute vision changes in hemiplegic patients on their gait.Methods Thirty visually normal hemiplegic patients were recruited.Plane mirrors(0°),concave lenses(+150°,+450°)were selected to simulate normal vision,moderate myopia,and high myopia scenarios for straight line walking tests.The Qualisys three-dimensional(3D)motion capture system and Kistler 3D force platform were employed to collect kinematic and dynamic parameters of the patients,and the differences in related indicators before and after visual intervention were compared.Results Visual intervention affected the step length,walking speed,and joint angles of hemiplegic patients.Especially after acute vision changes,there was a significant difference in the stride length and ankle joint angles on the patient's affected side.Under high myopia,the step length and step length symmetry were better than those under moderate myopia,but at the expense of gait speed.Visual intervention led to asymmetric trends in step the length symmetry and joint angle symmetry.There were very significant differences in center of pressure(COP)and COP symmetry between normal vision and high myopia.Conclusions Acute vision changes can affect the gait of hemiplegic patients,especially in individuals with high myopia presenting both a notable decrease in walking speed and an increased ankle range of motion,and an increase in walking speed with moderate myopia.Meanwhile,the symmetry of the COP decreases,indicating that acute vision changes lead to a higher risk of falls for patients.Reasonable vision assessment and corresponding intervention measures are expected to improve walking ability and life quality of the patients.

8.Effect of Yiqi Huoxue Prescription on cAMP/Epac1/Rap1 Signaling Pathway in Coronary Heart Disease Rats with Qi Deficiency and Blood Stasis Syndrome

ObjectiveBased on cyclic adenosine monophosphate (cAMP)/cAMP-regulated guanine nucleotide exchange factor 1 (Epac1)/Ras-homologous protein 1 (Rap1) signaling pathway to explore the myocardial protective mechanism of Yiqi Huoxue prescription on coronary heart disease (CHD) rats with Qi deficiency and blood stasis syndrome. MethodEighty-eight specific-pathogen-free (SPF) grade male Sprague-Dawley (SD) rats were divided into a sham operation group (n=12) and an experimental group (n=76) according to the random number table. The experimental group underwent a restricted diet and exhaustive swimming combined with left anterior descending (LAD) coronary artery ligation to construct a model of CHD with Qi deficiency and blood stasis syndrome, and electrocardiograms (ECGs) of rats before and after the LAD operation were collected. After the model was successfully established, the rats were randomly divided into model group, Yiqi Huoxue prescription low-dose group (4.28 g·kg-1), medium-dose group (8.55 g·kg-1), high-dose group (17.1 g·kg-1), and Western medicine group (isosorbide mononitrate tablets, 3.6 mg·kg-1). Rats were intragastrically administered assigned drugs for 4 weeks consecutively, while the sham operation group and the model group were administered with equal volumes of double distilled water. Twenty-four hours after the final administration, the rats were anesthetized with isoflurane to detect the left ventricular end-diastolic diameter (LVEDD), left ventricular end-systolic diameter (LVESD), left ventricular fractional shortening (FS, %), and ejection fraction (EF, %) by echocardiography. Blood samples were collected from the abdominal aorta for hemorheological measurements, and plasma cAMP levels were determined using enzyme-linked immunosorbent assay (ELISA). Myocardial tissue was collected for hematoxylin-eosin (HE) staining and Masson staining to observe myocardial pathological damage, and a transmission electron microscope was used to observe ultrastructural changes of myocardial tissue, fluorescent quantitative real-time polymerase chain reaction (Real-time PCR) was used to detect the expressions of myocardial Epac1, Rap1 GTPase activating protein (Rap1GAP) and Rap1 mRNA, and Western blot was used to detect the expressions of myocardial Epac1, Rap1GAP and Rap1 protein. ResultCompared with those in the sham operation group, LVEDD and LVISD of rats in the model group significantly increased (P<0.01), and the ratios of EF and FS significantly decreased (P<0.01), indicating symptoms of heart function decline, referred to as "heart Qi deficiency". The viscosity of whole blood and plasma significantly increased (P<0.01), and the content of cAMP significant increased (P<0.01). In addition, there was a significant proliferation of collagen fibers in myocardial tissue (P<0.01), and the ultrastructure of the myocardial tissue was severely damaged, indicating pathological changes consistent with "blood stasis". Real-time PCR results showed that Epac1 and Rap1 mRNA levels in the model group were significantly reduced (P<0.01), while Rap1GAP mRNA levels were significantly increased (P<0.01). Western blot analysis showed a significant decrease in Epac1 protein expression (P<0.01) and a significant increase in Rap1GAP protein expression (P<0.05). Compared with the model group, Yiqi Huoxue prescription improved cardiac function, reduced blood viscosity, lowered plasma cAMP levels, decreased collagen fiber proliferation, and improved myocardial ultrastructure damage in CHD rats with Qi deficiency and blood stasis syndrome. The high-dose group showed the most significant effects. In the high-dose group, Epac1 mRNA and protein expression levels significantly increased (P<0.01), Rap1 mRNA expression significantly increased (P<0.01), and Rap1GAP mRNA and Rap1GAP/Rap1 protein expression levels significantly decreased (P<0.05, P<0.01). ConclusionYiqi Huoxue prescription can improve cardiac function, reduce blood viscosity and plasma cAMP levels, improve myocardial damage, and reduce collagen fiber proliferation in CHD rats with Qi deficiency and blood stasis syndrome. The myocardial protection mechanism may be related to the regulation of the cAMP/Epac1/Rap1 signaling pathway.
9.Analysis of the characteristics of newly-diagnosed occupational diseases among female workers in Foshan City, 2013-2023

Objective To analyze the characteristics of newly-diagnosed occupational diseases among female workers in Foshan City from 2013 to 2023. MethodsClinical data of newly-diagnosed occupational diseases among female workers in Foshan City from 2013 to 2023 were collected from the "Occupational Disease and Hazardous Detection Information System" subsystem of the "China Disease Prevention and Control Information System", and their distribution characteristics were analyzed using descriptive epidemiology. Results A total of 218 cases of newly-diagnosed occupational diseases among female workers in Foshan City were reported from 2013 to 2023, categorized into seven categories and 17 types. The median age and median length of exposure to occupational hazards among female workers were 45.1 and 7.8 years, respectively. The top three categories of newly-diagnosed cases were occupational diseases of the ear, nose, throat, and oral cavity (90 cases, all related to occupational noise-induced hearing loss), occupational chemical poisoning (61 cases), and occupational pneumoconiosis and other respiratory diseases (51 cases, including 50 cases with occupational pneumoconiosis). The newly-diagnosed cases were concentrated in Nanhai District, Shunde District, and Gaoming District, accounting for 75.2%. The newly-diagnosed cases were found in non-metallic mineral products manufacturing industry, metal products industry, and electrical machinery and equipment manufacturing industry, accounting for 50.9%. Enterprises of newly-diagnosed cases were mainly distributed in private enterprises and foreign-invested enterprises, accounting for 80.3%. The scale of enterprises of newly-diagnosed cases was mainly small- and medium-sized, accounting for 79.4%. Among the 218 cases, there were 21 cluster cases, involving 16 cases of occupational silicosis and five cases of occupational acute n-hexane poisoning, involving six manufacture industries, with 81.3% cases worked in sanitary ceramic products industry. Conclusion Occupational noise-induced deafness should be listed as a key occupational disease for female workers in Foshan City. Attention should be paid to the prevention of occupational diseases among middle-aged female workers in the non-metallic mineral products industry and small and medium-sized enterprises, and the cluster outbreaks of occupational pneumoconiosis.
10.Evaluation of the correlation between diabetic retinopathy and diabetic ne-phropathy by emission computed tomography and clinical testing data via convolutional neural network

Objective To evaluate the relationship between diabetic nephropathy(DN)and diabetic retinopathy(DR)in patients with type 2 diabetes mellitus(T2DM)based on imaging and clinical testing data.Methods Totally 600 T2DM patients who visited the First People's Hospital of Ziyang from March 2021 to December 2022 were included.The fundus photography and fundus fluorescein angiography were performed on all these patients and their age,gender,T2DM duration,cardiovascular diseases,cerebrovascular disease,hypertension,smoking history,drinking history,body mass in-dex,systolic blood pressure,diastolic blood pressure and other clinical data were collected.The levels of fasting blood glu-cose(FPG),triglyceride(TG),total cholesterol(TC),high-density lipoprotein cholesterol(HDL-C),low-density lipo-protein cholesterol(LDL-C),glycosylated hemoglobin(HbA1c),24 h urinary albumin(UAlb),urinary albumin to creati-nine ratio(ACR),serum creatinine(Scr)and blood urea nitrogen(BUN)were measured.Logistic regression was used to analyze the risk factors associated with DR.DR staging was performed according to fundus images,and the convolutional neural network(CNN)algorithm was used as an image analysis method to explore the correlation between DR and DN based on emission computed tomography(ECT)and clinical testing data.Results The average lesion area rates of DR and DN detected by the CNN in the non-DR,mild-non-proliferative DR(NPDR),moderate-NPDR,severe-NPDR and pro-liferative DR(PDR)groups were higher than those obtained by the traditional algorithm(TCM).As DR worsened,the Scr,BUN,24 h UAlb and ACR gradually increased.Besides,the incidence of DN in the non-DR,mild-NPDR,moderate-NPDR,severe-NPDR and PDR groups was 1.67%,8.83%,16.16%,22.16%and 30.83%,respectively.Logistic regression analysis showed that the duration of T2DM,smoking history,HbA1c,TC,TG,HDL-C,LDL-C,24 h UAlb,Scr,BUN,ACR and glomerular filtration rate(GFR)were independent risk factors for DR.Renal dynamic ECT analysis demonstrated that with the aggravation of DR,renal blood flow perfusion gradually decreased,resulting in diminished renal filtration.Conclusion The application of CCN in the early stage DR and DN image analysis of T2DM patients will improve the diag-nosis accuracy of DR and DN lesion area.The DN is worsening as the aggravation of DR.
