1.Hypoxia-inducible factor-1α inhibitor LW6 inhibits myocardial ferroptosis and ameliorates myocardial injury of sepsis in rats

Objective To explore the effect of hypoxia-inducible factor-1α(HIF-1α)inhibitor LW6 on ferroptosis in septic cardiomyopathy rats.Methods Rat septic cardiomyopathy model was prepared using cecal ligation and puncture(CLP)method.Thirty-six specific pathogen-free(SPF)6-8 weeks male SD rats were randomly divided into the sham-operated group,CLP group,CLP+solvent group,LW6 group,ferrostatin-1(Fer-1)group,and LW6+Fer-1 group.The degree of myocardial damage in each group was evaluated through hematoxylin-eosin stai-ning and detection of lactate dehydrogenase and creatine kinase content in cardiac tissue.Myocardial mitochondrial damage was observed by transmission electron microscopy.Ferroptosis level was determined by detecting iron ion concentration,reduced glutathione,malondialdehyde,and reactive oxygen species.Protein expression levels of HIF-1α,solute carrier family 7 member 11(SLC7A11),and glutathione peroxidase 4(GPX4)in cardiac tissue were detected by Western blotting.Results Compared with the CLP group and the CLP+solvent group,the LW6 group could ameliorate myocardial damage,alleviate mitochondrial damage,inhibit ferroptosis-related indicators(all P<0.05),reduce HIF-1α protein expression levels(P<0.05),and enhance SLC7A11 and GPX4 protein expression levels(both P<0.05).Conclusion LW6 decreases HIF-1α expression and ferroptosis levels through the SLC7A11/GPX4 pathway,and ameliorates sepsis-induced cardiomyopathy.
2.Development and testing of validity and reliability in evidence-based health care related competence assessment scale for medical personnel

Objective To develop the Evidence-Based Health Care Related Competence Assessment Scale for Health Professionals(hereinafter referred to as the Scale),and to test its validity and reliability.Methods Based on the JBI evidence-based health care model as the theoretical framework,the initial items of the Scale were formed by reviewing the literature.Through the discussion of the research group,two rounds of Delphi expert consultation and pre-inspection,the items of the Scale were optimized.The convenience sam-pling method was adopted to extract 928 health professionals as the research subjects.The Scale conducted the validity and reliability testing.Results The Scale included the four dimensions of evidence generation,evi-dence synthesis,evidence transfer and evidence implementation,including 47 entries in total.The cumulative variance contribution rate was 59.08%.The confirmatory factor analysis results indicated that the model had good fitness.The convergent validity of all dimensions reached the standard,and the distinguishing validity was good.Finally,the Cronbach's α coefficient of the Scale was 0.971,and the split-half reliability was 0.928.Conclusion The developed Scale possesses good reliability and validity,which can be used to evaluate the competence of health professionals carrying the evidence-based healthcare related link works.
3.Risk factors for chronic critical illness of patients with septic shock in the ICU

Objective To analyze the risk factors for the development of chronic critical illness(CCI)in septic shock patients in the intensive care unit(ICU).Methods A retrospective analysis was conducted on 102 sepsis shock patients admitted to the ICU of the hospital from August 2018 to May 2023.According to their clinical dates,they were divided into CCI group(n=60)and rapid recovery(RR)group(n=42).Multi-ple logistic regression analysis was perform to identify independent risk factors for developing CCI in septic shock patients,and construct their receiver operating characteristic(ROC)curves to evaluate their predictive value for CCI.Results The prolonged duration of hypernatremia(OR=1.770,95%CI:1.147-2.731,P=0.010)and low acute physiology and chronic health(APACHEⅡ)scores(OR=1.212,95%CI:1.025-1.433,P=0.025)were independent risk factors for CCI in ICU septic shock patients.The occurrence of acute gastrointestinal injury(OR=0.060,95%CI:0.011-0.341,P=0.002)and peak blood sodium levels during ICU(OR=0.812,95%CI:0.662-0.995,P=0.044)were protective factors.Conclusion For septic shock pa-tients,special attention should be paid to its related independent risk factors and protective factors to improve the prognosis of septic shock patients.
4.Effects of mNGF combined with rehabilitation training on the expression of BDNF and Caspase-3 in brain tissue of neonatal rats with spastic cerebral palsy

Objective:To analyze the effect of mouse nerve growth factor(mNGF)combined with rehabilitation training on the expression of BDNF and Caspase-3 in brain tissue of newborn rats with spastic cerebral palsy(SCP).Method:A total of 150 clean-grade SD rats aged 7days were randomly divided into the control group(group NS,n=40)and the model group(group T,n=110)using a random number table.Group T received directional injection of absolute ethanol(1μl/g)into the left intracranial pyramidal tract at the age of 7 days,while the NS group received an equal volume of saline.24 hours after injection(at 8 days old),the spastic cerebral palsy rat model was determined by modified Li Xiaojie et al.'s criteria combined with Longa scoring method.Ninety successfully modeled SD rats in group T were randomly divided into group A1,A2 and A3,with 30 each.The intervention began on the 1st day of the experiment(8 days old).Group A1 was given intramuscu-lar injection of mNGF(30ng/g),group A2 was given intramuscular injection of an equal dose of mNGF plus rehabilitation training,and group A3 was given intramuscular injection of an equal volume of saline(2.27ml/kg).The group NS was given intramuscular injection of the equal volume of normal saline.Intervention lasted until 21 days of age.At 8,14 and 21 days of age,HE staining was used to observe the dynamic changes in the morphology of cells in the internal capsule area of brain,TUNEL staining to observe the dynamic changes in the apoptosis rate of cells in the internal capsule area of brain,and Western Blot to monitor dy-namic changes in the expression of BDNF and Caspase-3 proteins in the hippocampus.Result:Model preparation showed that a total of 97 spastic cerebral palsy litters were identified in group T at 8 days old,while no rats with cerebral palsy in groupNS.TUNELstaining revealed that the apoptosis rates of the rats in group A1,A2 and A3 were significantly higher than in group NS at each age(P<0.01),and were significantly higher in group A3 than in group A1 and A2 at 14 and 21 days of age(P<0.01),with group A1 significantly higher than group A2(P<0.01).Western Blot showed that the relative expression of BDNF and Caspase-3 protein in group A1,A2 and A3 at 14 days old were higher than that in group NS(P<0.05).The rel-ative expression of BDNF protein in group A1 and A2 was higher than that in group A3(P<0.05),with group A2 was higher than that in group A1(P<0.05).There was no significant difference between group A2 and A1 at 21 days old(P>0.05).The relative expression of Caspase-3 protein in group A1 and A3 was higher than that in group A2(P<0.05).and group A3 was significantly higher than that in A1 group(P<0.05).Conclusion:It is speculated that both mNGF and combined rehabilitation training can inhibit neuronal apoptosis by promoting BDNF protein expression and reducing the activity of Caspase-3.The effect of mNGF combined with rehabilitation training is better than that of mNGF alone.
5.Dynamic network analysis of postoperative core symptoms in hepatocellular carcinoma patients and nursing insights

Objective This study conducted a dynamic network analysis of the symptoms in patients with hepatocellular carcinoma(HCC)at different postoperative time points,aiming to explore evolving core symptoms and provide a basis for developing precise clinical symptom management programs.Methods A longitudinal study was carried out involving 370 HCC patients who underwent surgical treatment at a hospital in Kunming city between February and December 2024.Questionnaires were administered on postoperative day 2(T1)and day 7(T2),using the Chinese version of the Memory Symptom Assessment Scale.Symptom networks of 18 symptoms were constructed for both time points using R software to explore core symptoms and their interconnections.A network comparison test was also performed to assess differences between the 2 time points.Results All 370 patients completed the questionnaire at T1,and 357 valid responses were collected at T2,yielding a response rate of 96.49%.At T1,"feeling nervous"had the highest centrality(rs=1.29),while at T2,"feeling sad"was most central(rs=1.11).Overall strength of the symptom networks showed that notable changes were observed in the edge weights between symptoms(P=0.01).Conclusion While the overall structure of early postoperative symptoms in HCC patients remained relatively stable over time,the core symptoms shifted."Feeling nervous"emerged as the central symptom at T1,whereas"feeling sad"became predominant.Digestive-related symptoms are progressively more closely linked over time.Healthcare professionals should dynamically assess patients' early postoperative psychological symptoms and digestive system related symptoms,and focus on intervening with symptoms of tension and sadness.

OBJECTIVE To explore the distribution and drug resistance of pathogens isolated from the children with recurrent respiratory tract infection(RRI)and analyze the expressions of Toll-like receptors(TLRs)in peripheral blood mononuclear cells(PBMCs)so as to provide bases for the diagnosis.METHODS A total of 52 children with RRI who were treated in Hainan Modern Women and Children's Hospital from Oct.2021 to Oct.2023 were re-cruited as the case group,meanwhile,61 healthy children who received physical examination were chosen as the healthy group.The distribution and drug resistance of the pathogens isolated from the case group were statistically analyzed.The levels of PBMCs TLR2 and TLR4 were compared between the two groups.The values of TLR2 and TLR4 in diagnosis of the children with RRI were analyzed by means of receiver operating characteristic(ROC)curves.RESULTS Totally 311 strains of pathogens were isolated from the 52 children with RRI,among which gram-negative bacteria were dominant;Haemophilus influenzae was the major species of gram-negative bacteria,and Staphylococcus aureus was the predominant species of gram-positive bacteria.The H.influenzae strains were mainly resistant to ceftazidime,cefepime and gentamycin,but were sensitive to imipenem and meropenem.The S.aureus strains were highly resistant to penicillin and ampicillin,but were sensitive to vancomycin and nitrofuran-toin.There were significant differences in the levels of PBMCs TLR2 and TLR4 between the case group and the healthy group(P<0.05).The area under the curve(AUC)value of the joint detection of TLR2 and TLR4 was higher than that of the single detection in diagnosis of the children with RRI(P<0.05),the sensitivity of the joint detection was 76.92%,with the specificity 95.08%.CONCLUSIONS The gram-negative bacteria are dominant a-mong the pathogens isolated from the children with RRI.Imipenem and meropenem are recommended for the treatment of H.influenzae infection;vancomycin and nitrofurantoin are recommended for treatment of S.aureus infection.The children with RRI show the abnormal expressions of TLRs in PBMCs.The joint detection of TLR2 and TLR4 can raise the diagnostic value of the RRI children.
7.Identification of the first cluster infection of Brucella in Nujiang Lisu Autonomous Prefecture, Yunnan Province

Objective:To identify the serotype and genotype characteristics of the first cluster infection of Brucella in Nujiang Lisu Autonomous Prefecture of Yunnan Province. Methods:The information of seven suspected cluster brucellosis cases reported in Lanping County, Nujiang Lisu Autonomous Prefecture in April 2023 were collected, blood samples were collected, and the strains were isolated and cultured. Serological methods were employed for brucellosis diagnosis, bacterial species identification was performed using BCSP31-PCR and AMOS-PCR. Multilocus locus variable-number tandem repeat analysis (MLVA) was conducted for strain genotyping, followed by phylogenetic analysis comparing the strains with those from other regions of China.Results:All seven cases were diagnosed with brucellosis, they were all villagers from the same village and had daily contact with sheeps. Four suspected Brucella strains were isolated, identified as Brucella melitensis biotype by BCSP31-PCR and AMOS-PCR. The four isolated strains exhibited identical MLVA-16 genotype, with MLVA-8 genotype 42 and MLVA-11 genotype 180, belonging to the Eastern Mediterranean lineage. Compared with the MLVA-16 of the isolated strains from other regions of China, the four isolated strains formed an independent cluster. Conclusion:The first cluster infection of Brucella in Nujiang Lisu Autonomous Prefecture of Yunnan Province is caused by Brucella melitensis biotype, and the four isolated strains have showed unique MLVA genotype.

In July 2024,the Diagnosis Related Groups(DRG)2.0 is released based on the Notice from the National Healthcare Security Administration on Issuing the DRG 2.0 and Deepening the Relevant Work.Compared with DRG 1.1,version 2.0 was established based on a wider range of suggestions regarding the Adjacent Diagnosis Related Groups(ADRG),Major Comorbidity or Complication(MCC),and Comorbidity or Complication(CC)from various institutions.A list of disease diagnoses and surgical operations that are not used as grouping rules was compiled,and grouping efficacy was further improved by upgrading the algorithms for MCC and CC with the help of AI.Meanwhile,it is necessary to pay more attention to the number of cases of ADRG,the better methods to list the MCC/CC,the suggestions of various doctors and continuously standardize the data and update the grouping scheme of DRG.
