1.Longitudinal qualitative study of supportive care needs on heart transplant patients

Objective To explore the supportive care needs of heart transplant patients at different stages.Methods Purposive sampling method was used to select 15 heart transplant patients who were hospitalized in a tertiary A hospital in Guangdong Province from July 2023 to February 2024 as research subjects.According to the"Timing It Right"theory,5 semi-structured interviews were conducted with patients.Interpretive phenomenology was used to sort out and analyze the data.Results A total of 5 themes and 18 subthemes were extracted,including urgent desire for transplant information and psychological support during diagnosis,strong physiological and emotional needs during hospitalization,significant demand for health education in the preparation period for discharge,expectation of family support during the adjustment period,increasing demand for social support during the adaptation period.Conclusion The supportive care needs of heart transplant patients at different stages are dynamic.Medical staff should adopt the concept of dynamic and continuous care,and provide personalized care in stages,plans and continuance,in order to improve the quality of life of heart transplant patients.

3.CT features of inflammatory pseudotumor like follicular dendritic cell sarcoma of the spleen

Objective:To explore the CT features of inflammatory pseudotumor like follicular dendritic cell sarcoma (FDCS) of the spleen.Methods:The clinical data of 12 patients with splenic inflammatory pseudotumor like FDCS admitted to 3 central hospitals including Yongjia People's Hospital in Zhejiang Province from January 2015 to December 2022 were retrospectively analyzed, including 4 males and 8 females, with a median age of 60 years old. The number, shape, size and CT features of the lesions were analyzed based on patient's CT image data.Results:CT scans of 12 patients showed 15 lesions, including 10 single lesions and 2 multiple lesions. The lesions were circular in 5 cases, elliptical in 4 cases, and irregular in 3 cases. The median maximum diameter of the mass is 6.5 cm. On plain scan, all 12 tumors showed low density or slightly low density. The CT value is (41.3±7.2) HU; 8 cases had uneven density and 4 cases had uniform density. There were 8 cases with clear tumor boundaries and 4 cases with unclear boundaries. There were 8 cases with tumor necrosis and cystic transformation, and 5 cases showed patchy bleeding lesions in the center of the tumor. Enhancement: the arterial phase shows small patches or flocculent enhancement at the edges or parenchymal parts of the tumor, with CT value of (56.0±3.8) HU. Among them, there were 7 cases of mild enhancement, 4 cases of moderate enhancement, and 1 case of significant enhancement. During the portal phase, there was mild to moderate persistent small patchy uneven enhancement, with CT value of (62.0±4.3) HU. Among them, there were 8 cases of mild enhancement and 4 cases of moderate enhancement. The delayed phase showed a slow withdrawal of enhancement, with CT value of (45.0±8.2) HU. All 12 cases underwent complete resection and were diagnosed with FDCS through pathological examination.Conclusion:FDCS plain scan shows circular or elliptical uneven low-density masses, with small patches or flocculent light to moderate uneven enhancement in the arterial phase, continuous enhancement in the portal phase, and slow withdrawal in the delayed phase as the main characteristics.
4.Relationship between pilots' physiological data of anterior eye segments and flying time

Objective To observe the relationship between pilots' physiological data of anterior eye segments and flying time in order to provide a reference basis for better ophthalmological health care.Methods The ophthalmological data of the fighter pilots who were in the physical examination for transformation in General Hospital of Air Force or in annual examination but hospitalized due to non-ophthalmological reasons.Pilots were divided into <30 yr group (n=132) and ≥ 30 yr group (n=28).They were also divided into <1 000 h group (n=137) and >1 000 h group (n=23).Statistical analysis was done with SPSS 19.0 software.Results Pilots of different age groups had no statistic differences in visual acuity,axial length and mean corneal curvature (P>0.05).The ratio of astigmatism with the rule and astigmatism against the rule between age groups also showed insignificant difference (x2 =0.013,P>0.05).Pilots in <30 yr group had bigger spherical equivalent degree (t'=2.074,P<0.05).The visual acuity,spherical equivalent degree,type of astigmatism,axial length,anterior chamber depth and mean corneal curvature were found no statistical difference between two age groups.No correlation existed between axial length in anterior chamber depth for the pilot in same group.A weak positive relationship was observed between visual acuity and spherical equivalent degree of the pilots in <30 yr group (r=0.287,P<0.01) but in ≥ 30 yr group (P>0.05).Axial length negatively correlated to mean corneal curvature in all pilots (r=-0.813).Conclusions Pilots' physiological data of anterior eye segments are mainly correlated with age but with flying time.
