1.Progress in minimally invasive surgical treatment of esophageal cancer

Carcinoma of the esophagus is one of the common malignant tumors, and surgical treatment is still the main method for the treatment of esophageal cancer. With the development of thoracoscopic and laparoscopic instruments, minimally invasive esophagectomy is more and more widely used in the treatment of esophageal cancer. At present, minimally invasive esophagectomy mainly includes thoracoscopic and laparoscopic esophagectomy, mediastinoscopic esophagectomy, robot assisted esophagectomy, etc. At present, there are many studies on the comparison between minimally invasive esophagectomy and open esophagectomy. Although it is not clear which minimally invasive surgical method is more superior, each method may be superior to open esophagectomy in terms of blood loss, pulmonary complications and hospital stay. No matter what method is used, surgeons must be trained in a structured system to improve the level of experience required for independent operation. Ultimately, with the implementation of experienced doctors, the minimally invasive method in esophagectomy is superior to open esophagectomy in technically feasible cancer cases.
2.Advantages and disadvantages of trauma effects during robot-assisted total knee arthroplasty

BACKGROUND:The application of robot-assisted technology for total knee arthroplasty is one of the current research hotspots.Since the 1980s,robot-assisted technology has been introduced into total knee arthroplasty outside China to achieve accurate osteotomy and good recovery of lower limb alignment.After decades of use,the robot has continuously improved its performance with new iterations,but has been criticized for increasing perioperative time and surgical trauma. OBJECTIVE:To summarize the advantages and disadvantages of current orthopedic surgical robots in total knee arthroplasty. METHODS:PubMed database and CNKI were searched to analyze the advantages and disadvantages of robot-assisted total knee arthroplasty in surgical trauma.English search terms were"arthroplasty,replacement,knee,knee replacement arthroplasty,procedure,robotic surgical,total knee arthroplasty,arthroplasty,replacement,knee,robotic-assisted".The Chinese search terms were"robot-assisted,robotic arm,knee osteoarthritis,arthritis".After the initial screening of all articles according to the inclusion and exclusion criteria,62 articles with high quality and relevance were reviewed. RESULTS AND CONCLUSION:(1)Robot-assisted total knee arthroplasty did not increase the degree of surgical trauma in patients,and showed a lower trauma effect than conventional manual total knee arthroplasty.(2)Robot-assisted total knee arthroplasty has the advantages of accurate auxiliary osteotomy,individualized prosthesis implantation,better protection of soft tissue around the knee joint,reduction of analgesic drug use,reduction of postoperative inflammatory index changes,and shortening of hospital stay.However,there are also shortcomings such as prolonged operation time,increased complications,and increased medical costs.(3)It is concluded that preliminary clinical application studies have shown that robot-assisted total knee arthroplasty can reduce surgical trauma,but it is necessary to be alert to potential risks.Simultaneously,its exact advantages compared with conventional manual total knee arthroplasty need to be verified by large-sample randomized controlled studies and long-term follow-up.
3.Relationship between psychological quality and non-suicidal self-injury behavior in middle school students: path analysis of coping style

BackgroundNon-suicidal self-injury (NSSI) behavior is highly prevalent in middle school students, which poses a significant risk to the physical and mental health of middle school students, so finding ways to improve the NSSI behavior among middle school students is of great significance for promoting their healthy growth. ObjectiveTo investigate the relationship between psychological quality and NSSI behavior among middle school students in western Anhui, and to examine the pathway of coping style in the above relationship. MethodsFrom November to December 2020, 22 872 middle school students in western Anhui were selected using stratified cluster random sampling method, and were subjected to complete the assessment of Adolescent Non-suicidal Self-injury Assessment Questionnaire (ANSAQ), Adolescent's Psychological Suzhi Scale-Simplified Version (APSS-SV) and Coping Style Scale for Middle School Students (CSSMSS). Spearman correlation analysis was adopted to examine the correlation among scores of scales and the frequency of NSSI behavior in middle school students. Amos 23.0 was utilized to test the pathway of coping style in the relationship between psychological quality and NSSI behavior. ResultsA total of 21 718 (94.95%) middle school students completed the effective questionnaire survey. NSSI behavior was detected in 7 798 middle school students (35.91%). ANSAQ total score and CSSMSS positive coping style score of middle school students were negatively correlated with the frequency of NSSI behavior (r=-0.219, -0.179, P<0.01). CSSMSS negative coping style score was positively correlated with the frequency of NSSI behavior (r=0.093, P<0.01). The direct effect value of psychological quality on the frequency of NSSI behavior was -0.136, and the indirect effect value of coping style on the relationship between psychological quality and the frequency of NSSI behavior was -0.084. Among them, the effect value of positive coping style was -0.122, accounting for 55.45% of the total effect, while the effect value of negative coping style was 0.038, accounting for 17.27% of the total effect. ConclusionCoping style may mediate the relationship between psychological quality and NSSI behavior in middle school students, and the positive coping style and negative coping style play separate roles in the pathway of psychological quality on NSSI behavior.[Funded by 2020 Natural Science Research Project of Anhui Provincial Education Department (number, KJ2020B006)]
4.Clinical status and research progress of primary pulmonary mucinous adenocarcinoma

Primary pulmonary mucinous adenocarcinoma is a subtype of lung adenocarcinoma, and its epidemiology is similar to other pulmonary adenocarcinoma. Because of its low incidence rate, the survival data of patients with pulmonary mucinous adenocarcinoma are few and often contradictory. KRAS mutations often occur in pulmonary mucinous adenocarcinoma, but EGFR mutations are rare. The expression of PD-L1 in pulmonary mucinous adenocarcinoma is very low. Patients with early pulmonary mucinous adenocarcinoma can benefit from surgery. Lobectomy is still the standard operation at present, but sub lobectomy may also be effective for early pulmonary mucinous adenocarcinoma. Other treatment options include platinum based dual drug chemotherapy, targeted therapy targeting driving genes, and the recent rise of immunotherapy. Future new targets and corresponding treatments should require more research to confirm.
5.Comparison of early clinical outcomes between domestic robot-assisted total knee arthroplasty and traditional surgery: Meta analysis of randomized controlled trials

Objective:To compare the early clinical outcomes between domestic robot-assisted total knee arthroplasty (RA-TKA) and conventional manual total knee arthroplasty (CM-TKA) for patients with primary knee osteoarthritis.Methods:Embase, Pubmed, Web of Science, Zhi.com and Wanfang databases from January 2015 to April 2023 were searched for clinical controlled trials (RCTs) comparing the clinical outcomes between RA-TKA and CM-TKA. After literature screening, quality evaluation and data extraction according to the criteria required, Revman 5.3 software was applied to perform a Meta-analysis of the literature data. The operation time, intraoperative bleeding, hip-knee-ankle angle (HKA), HKA bias value, frontal femoral component (FFC), frontal tibia component (FTC), lateral femoral component (LFC), lateral tibia component (LTC), Knee Society Score (KSS), visual analogue scale (VAS), Western Ontario and McMaster University Osteoarthritis Index (WOMAC), knee mobility, Hospital for Specialty Surgery (HSS) knee score, length of hospital stay, and rate of complications were compared between the RA-TKA and CM-TKA patients.Results:Eight RCTs and 449 patients were included, with 221 patients in the RA-TKA group and 228 ones in the CM-TKA group. The Meta-analysis showed that the RA-TKA group had significantly longer operation time ( MD=18.41, 95% CI: 11.28 to 25.23, P<0.001), significantly better HKA ( MD=0.41, 95% CI: 0.06 to 0.76, P=0.020), significantly better HKA bias value ( MD=-0.92, 95% CI: -1.25 to -0.60, P<0.001), significantly better FTC ( MD=0.38, 95% CI: 0.08 to 0.67, P=0.010), significantly better LTC ( MD=1.71, 95% CI: 0.94 to 2.48, P<0.001), and significantly better knee mobility ( MD=-2.23, 95% CI: -4.18~-0.27, P=0.030) than the CM-TKA group. However, the differences were not statistically significant between the 2 groups in the intraoperative bleeding, FFC, LFC, KSS, VAS, WOMAC, HSS, length of hospital stay, or rate of complications ( P>0.05). Conclusion:Use of a domestic robot to assist conventional manual TKA can significantly improve the accuracy of prosthesis fixation and reconstruct the alignment of lower limb better, showing potential advantages in promoting functional recovery of the knee for the patients.
6.Anatomical morphology of the aortic valve in Chinese aortic stenosis patients and clinical results after downsize strategy of transcatheter aortic valve replacement

Background::The study aimed to describe the aortic valve morphology in Chinese patients underwent transcatheter aortic valve replacement (TAVR) for symptomatic severe aortic stenosis (AS), and the impact of sizing strategies and related procedural outcomes.Methods::Patients with severe AS who underwent TAVR were consecutively enrolled from 2012 to 2019. The anatomy and morphology of the aortic root were assessed. "Downsize" strategy was preformed when patients had complex morphology. The clinical outcomes of patients who performed downsize strategy were compared with those received annular sizing strategy. The primary outcome was device success rate, and secondary outcomes included Valve Academic Research Consortium-3 clinical outcomes variables based on 1-year follow-up.Results::A total of 293 patients were enrolled. Among them, 95 patients (32.4%) had bicuspid aortic valve. The calcium volume (Hounsfield Unit-850) of aortic root was 449.90 (243.15-782.15) mm 3. Calcium is distributed mostly on the leaflet level. Downsize strategy was performed in 204 patients (69.6%). Compared with the patients who performed annular sizing strategy, those received downsize strategy achieved a similar device success rate (82.0% [73] vs. 83.3% [170], P= 0.79). Aortic valve gradients (downsize strategy group vs. annular sizing group, 11.28 mmHg vs. 11.88 mmHg, P = 0.64) and percentages of patients with moderate or severe paravalvular regurgitation 2.0% (4/204) vs. 4.5% (4/89), P = 0.21) were similar in the two groups at 30 days after TAVR. These echocardiographic results were sustainable for one year. Conclusions::Chinese TAVR patients have more prevalent bicuspid morphology and large calcium volume of aortic root. Calcium is distributed mostly on the leaflet level. Compare with annular sizing strategy, downsize strategy provided a non-inferior device success rate and transcatheter heart valve hemodynamic performance in self-expanding TAVR procedure.
7.Impact of fluence smoothing on the dosimetry of cervical cancer radiotherapy

Objective To explore the dosimetric differences of radiotherapy plan for cervical cancer with 4 different fluence smoothing (FS) parameters using Monaco treatment planning system (Monaco TPS). Methods Fifteen patients with ⅠB2 stage cervical cancer in our hospital were enrolled in this study. And a 2 Volumetric Modulated Arc Therapy (VMAT) plan for each patient were completed by Monaco 5.11 TPS according to the X-Ray Voxel Monte Carlo (XVMC) method. For each plan was optimized by FS function, with the level of Off, Low, Medium and High. To compare the difference of plan optimization time, conformity index (CI), Homogeneity index (HI), Dmean, Dmin, D2% of PTV,dose to the organ at risk (OAR),the number of Segments# and MU#,estimated total delivery time (ETDT), quantum Efficiency (QE) of the plans, the formation of Segments# with the same angle and verification of inserting 729 two-dimensional matrix into PTW octavius 4D module of different FS function levels, with the precondition of the Prescription isodose curve covering 95% of the target area. The data was analysed by multivariate factor analysis with the application of SPSS, and P < 0.05 was considered as statistically significant. And the Planned revenue score of different FS levels was also calculated. Results Except for the Dmin of PTV (the lowest value is (32.09 ± 0.26) Gy for the Off group, and the highest value is (35.98 ± 0.42) Gy for the High group), V40 of the rectum (the lowest value in the Medium group is 55.88% ± 2.02%, and the highest value in the High group was 61.90% ± 2.98%) and bladder (the lowest value was 45.01% ± 2.08% in the Medium group, and the highest value is 50.45% ± 1.98% in the High group), the V20 (the lowest value High group was 49.05% ± 1.98%, the highest value Off group was 56.52% ± 1.75%) of femoral head (P < 0.05), there was no significant difference of the dose assessment results for PTV and OARs in 4 different FS function levels. In the High level, the ETDT, QE and MU# were showed better than other groups evidently, however, the number of Segments# showed no significant difference. The plan validation results was increased with the improvement of FS function level, and the level of High was considered to be the optimal. To compare the score of overall benefits of the plan, the level of Medium (−17.18 ± 0.05) got the highest score, and the Low group (−17.58 ± 0.05) and the High group (−17.42 ± 0.06) have similar scores, and Off group (−18.81 ± 0.08) has the lowest score. Conclusion Different FS levels of the Monaco 5.11 TPS can optimize the radiotherapy plan for cervical cancer, but the level of Medium is considered to be the most applicable.
8.Impact of statistical uncertainty on esophagus cancer plan for dose to water and dose to medium

Objective To study the dosimetry effect of Dw and Dm middle and lower esophageal cancer in Monaco treatment planning system (TPS). Methods 30 patients with T3N0M0StageⅡa middle and lower esophageal cancer were selected for experiment. For each patient, optimize the plan using dose to water (Dw) and dose to medium (Dm) dose calculation mode, then rescale prescription dose to 95% volume of PTV. Compare the difference in the two mode, conformity index (CI), Homogeneity index (HI), Mean dose (Dmean), Minimum dose (Dmin), Maximum dose (D2), Dose to Organ at risk (OAR), MU, Optimization time, photon usage, and QA results of MatriXX and Arc Check. Use SPSS for multivariate analysis. Results In the dose evaluation of the middle and lower esophageal cancer cases under different dose calculation methods, the spinal cord, trachea, V20 of the whole lung, and D2 of the liver have significant dosimetric differences, the dose value, the sequential dose results were compared as (37.92 ± 1.11)/(35.85 ± 1.08), (59.91 ± 1.43)/(60.25 ± 0.98), (22.52 ± 1.75)/(21.38 ± 2.01), (42.89 ± 0.52)/(41.73 ± 0.58). In the comparison of dose cloud distribution, the difference is mainly located in the cavity and the inner wall of the lung in the target area, the dose in the target cavity in the Dw group is higher than that in the Dm group. The dose in the inner and outer walls of the lung cavity in the Dw group are slightly adducted than that in the Dm group, especially in the central area.Dose QA of MartiXX (3%-3 mm) and Arc Check (2%-2 mm) with different dose calculation methods of 60 plans of 30 cases have all passed clinical requirements. Dm Group is better than Dw group. Conclusion It is recommended to use Dm dose calculation method for Monaco 5.11 TPS in the condition of treatment planning for middle and lower esophageal cancer.
9.Endoscopic sinuvertebral nerves neurotomy for the treatment of discogenic low back pain

Objective:To explore the clinical effects of endoscopic sinuvertebral nerves neurotomy for discogenic low back pain.Methods:Based on the anatomical research of sinuvertebral nerves, a total of 40 patients, including 9 males and 21 females aged 35±10 (24-55) years, with single-segment discogenic low back pain were treated with endoscopic sinuvertebral nerves neurotomy in our hospital from July 2018 to February 2019. The operating section included 4 cases of L 3,4 (10.0%, 4/40), 31 cases of L 4, 5 (77.5%, 31/40), and 5 cases of L 5S 1 (12.5%, 5/40). The preoperative visual analogue scale (VAS) score was 4.5±0.9 with the preoperative Oswestry disability index (ODI) score 49.7%±14.0%. For diagnostic nerves block, lidocaine (0.1-0.3 ml of 0.05 g/L) was successfully injected into the intersection of the lateral edge of the bilateral pedicle projection and the upper edge of the intervertebral disc projection. The initial segment of the sinuvertebral nerves was destroyed by a radiofrequency blade or a nerve dissector after bilateral percutaneous transforaminal endoscopic. All cases were followed up at 1, 3, 6 and 12 months after surgery, observing the changes in VAS and ODI. Results:Filamentous lumbar sinuvertebral nerve was observed under endoscope with its main trunk tranversed into the spinal canal against the intervertebral disc. The deputy trunk crossed at the posterolateral edge of the intervertebral disc and entered the intervertebral disc or the posterior edge of the vertebral body. By moving along with postcentral branches of spinal artery, the main trunk of sinuvertebral nerve was with tension and was capable of moving with the nerve root. In spite of moving the working channel along the main trunk of the sinuvertebral nerve laterally, the starting point of the sinuvertebral nerve at the ventral ganglion could be observed. All 40 patients successfully completed the sinuvertebral nerve destruction. The VAS was reduced to 1.7±0.9, 1.3±0.9, 1.2±0.8, 1.3±0.7 at 1, 3, 6 and 12 months after sugery respectively, which were significantly lower than those at pre-operation ( F=116.7, P=0.00). The improvement rate of VAS in 40 cases was 68.9%± 17.1% (33.3%-100.0%) at 12 months after operation. The VAS score in 6 cases was higher at 12 months after surgery than that preoperatively ( t=4.2, P=0.48), namely 1 case of L 3, 4, 2 cases of L 4, 5, and 3 cases of L 5S 1. In all cases, the ODI was reduced to 18.3%±5.2%, 14.5%±4.3%, 13.6%±3.7%, 12.8%±3.0% points at 1, 3, 6 and 12 months after surgery respectively, which were significantly lower than those before surgery ( F=237.7, P=0.00). The improvement rate of ODI was 72.0%±11.6% (33.3%-88.9%) at 12 months after surgery in all cases. Conclusion:The destruction of sinuvertebral nerve after transforaminal endoscope could improve the pain and function in patients with discogenic low back pain at L 3,4 and L 4, 5 segments within 12 months. For patients with discogenic low back pain at L 5S 1 segment, the clinical effects could be better within 6 months.
10.Olfactory electroencephalogram signal recognition based on wavelet energy moment.

Studying the ability of the brain to recognize different odors is of great significance in the assessment and diagnosis of olfactory dysfunction. The wavelet energy moment (WEM) was proposed as a feature of olfactory electroencephalogram (EEG) signal and used for odor classification. Firstly, the olfactory evoked EEG data of 13 odors were collected by an experiment. Secondly, the WEM was extracted from olfactory evoked EEG data as the signal feature, and the power spectrum density (PSD), approximate entropy, sample entropy and wavelet entropy were used as the contrast features. Finally, -nearest neighbor ( -NN), support vector machine (SVM), random forest (RF) and decision tree classifier were used to identify different odors. The results showed that using the above four classifiers, the classification accuracy of WEM feature was higher than other features, and the -NN classifier combined with WEM feature had the highest classification accuracy (91.07%). This paper further explored the characteristics of different EEG frequency bands, and found that most of the classification accuracy based on the features of γ band was better than that of the full band and other bands, among which the WEM feature of the γ band combined with the -NN classifier had the highest classification accuracy (93.89 %). The research results of this paper could provide a new objective basis for the evaluation of olfactory function. On the other hand, it could also provide new ideas for the study of olfactory-induced emotions.
