1.Predictive value of TyG-BMI,AIP,and postprandial glucose excursion for sarcopenia in patients with type 2 diabetes mellitus

Objective To evaluate the predictive value of triglyceride-glucose body mass index(TyG-BMI),atherogenic index of plasma(AIP),and postprandial glucose excursion(PPGE)for sarcopenia in patients with type 2 diabetes mellitus(T2DM).Methods An observational cohort trial was conducted on 590 hospitalized T2DM patients(322 males and 268 females)admitted to Department of Endocrinology of the First Affiliated Hospital of Chongqing Medical University between January 2023 and December 2024.General demographic data and key metabolic indicators,including fasting plasma glucose(FPG),postprandial 2-hour plasma glucose(PPG),triglyceride(TG)were collected,and then TyG-BMI,AIP,and PPGE were calculated.Based on the Asian Working Group for Sarcopenia(AWGS)2019 criteria,they were divided into a sarcopenia group(n=140)and a non-sarcopenia group(n=450).After 1∶1 propensity score matching(PSM),102 patients with sarcopenia were matched with 102 without.Differences in metabolic indicators FPG,PPG,TyG-BMI,AIP and PPGE were compared between the 2 groups.Spearman correlation analysis was conducted to assess correlations of these indicators with sarcopenia.Conditional logistic regression analysis was performed to identify independent risk factors,and receiver operating characteristic(ROC)curves were plotted to assess predictive performance.An external validation cohort consisting of 192 T2DM patients from Department of Endocrinology of Jiangbei Branch of First Affiliated Hospital of Army Medical University between January 2023 and December 2024 was included to validate the prediction model.Results After PSM,the baseline data of the 2 groups was balanced(P>0.05).The sarcopenia group showed higher levels of PPG,AIP,and PPGE,but lower TyG-BMI value than the non-sarcopenia group(all P=0.001).Spearman correlation analysis revealed that the TyG-BMI was negatively correlated(r=-0.404,P=0.001),whereas AIP and PPGE were positively correlated with concomitant sarcopenia(r=0.280,P=0.001;r=0.372,P=0.001)in the T2DM patients.Conditional logistic regression analysis identified AIP(OR=14.367)and PPGE(OR=1.245)as independent risk factors,while TyG-BMI(OR=0.966)as independent protective factors.ROC curve analysis revealed that the area under the curve(AUC)of the combined model of TyG-BMI,AIP and PPGE in predicting sarcopenia was 0.918,and the AUC value was 0.954 in external validation,with good sensitivity and specificity.Conclusion TyG-BMI,AIP,and PPGE are important metabolic predictors in T2DM patients with sarcopenia.Their combination has good screening value for sarcopenia,and can be applied in external prediction for T2DM patients.
2.Clinical characteristics of patients with brucellosis complicated with epididymo-orchitis

Objective:To explore the clinical characteristics of patients with brucellosis complicated with epididymo-orchitis (Brucellar epididymo-orchitis, BEO), so as to provide reference for clinical diagnosis and treatment of BEO.Methods:General and clinical data of 293 male patients with acute brucellosis admitted to the Beidahuang Industry Group General Hospital from January 2023 to December 2024 were retrospectively collected. They were divided into a BEO group (30 cases) and a non-BEO group (263 cases) based on the presence or absence of epididymo-orchitis. Epidemic characteristics, clinical manifestations and laboratory examination results were compared and analyzed.Results:Among 293 male patients with acute brucellosis, the incidence of BEO was 10.24% (30/293). Their age was mainly concentrated in 45 - < 60 years old (53.33%, 16/30), and occupation was mostly farmers (63.33%, 19/30). There were no statistically significant differences in the distribution of age, occupation, exposure history and onset season between the BEO group and the non-BEO group ( P > 0.05). The distribution of abdominal pain and urinary frequency/urgency/pain symptoms was compared, and the differences were statistically significant ( P < 0.05). White blood cell count (WBC), neutrophil count (NEUT), and C-reactive protein (CRP) levels in the BEO group were higher than those in the non-BEO group ( P < 0.05), while the levels of cytokines interleukin (IL)-6 and interferon-γ (IFN-γ) were lower than those in the non-BEO group ( P < 0.05). After 6 - 8 weeks of hospitalization, the levels of WBC, NEUT, CRP, albumin, alanine aminotransferase, aspartate aminotransferase, γ-glutamyl transpeptidase, alkaline phosphatase, α-hydroxybutyrate dehydrogenase, lactate dehydrogenase, IL-4, IL-6, IL-10, IL-17, tumor necrosis factor-α, and IFN-γ in patients of the BEO group were significantly different from before treatment ( P < 0.05). Twenty-eight patients were cured, 1 patient underwent orchiectomy, and 1 patient experienced recurrence. Conclusions:Middle-aged male patients with brucellosis are more prone to BEO. Clinical manifestations and laboratory tests have certain diagnostic value for BEO. Suspected BEO patients should be diagnosed and treated as early as possible to reduce the occurrence of adverse prognosis.
3.Clinical characteristics of 22 patients with neurobrucellosis

Objective:To learn about the clinical characteristics of patients with neurobrucellosis (NB) and provide references for clinical diagnosis and treatment of NB.Methods:The clinical data of 22 NB patients diagnosed and treated at Beidahuang Industry Group General Hospital from January 2018 to November 2024 and 178 healthy individuals undergoing physical examinations during the same period were retrospectively collected. The epidemiological characteristics, clinical manifestations, laboratory tests, treatment and prognosis of NB patients were analyzed.Results:Among the 22 NB patients, 12 were males (54.55%) and 10 were females (45.45%). The age was (51.77 ± 12.75) years old, ranging from 27 to 80 years old. Most of the patients were farmers (95.45%, 21/22), and 16 cases (72.73%) had contacted with cattle/sheep. The onset seasons were mainly in summer (40.91%, 9/22) and spring (31.82%, 7/22). Among all NB patients, there were 10 cases of encephalitis/meningoencephalitis, 9 cases of myelitis, and 3 cases of meningitis. The general symptoms were mainly fever (68.18%, 15/22), the neurological symptoms were mainly nausea and vomiting (36.36%, 8/22), and the physical signs were mainly muscle weakness (50.00%, 11/22) and pathological signs (45.45%, 10/22). The laboratory test results showed that the levels of hemoglobin, hematocrit, C-reactive protein, total protein, albumin, γ-glutamyl transpeptidase, α-hydroxybutyric acid dehydrogenase, lactate dehydrogenase, and glutathione reductase in NB patients were significantly different from those in healthy individuals ( P < 0.001). Brain magnetic resonance imaging revealed ischemic changes in 5 cases (22.73%), abnormal brain signals in 2 cases (9.09%), and demyelinating lesions in white matter in 1 case (4.55%). After treatment, 18 NB patients were followed up and showed good prognosis, with only 2 cases exhibiting varying degrees of sequelae (walking disorders or memory impairment). Conclusions:The clinical manifestations of NB patients are diverse. A comprehensive judgment should be made by combining epidemiological characteristics, clinical manifestations, laboratory tests and imaging examinations.
4.Dynamic colonization of Enterobacteriaceae in the gastrointestinal tract of tumor pa-tients and bloodstream infections caused by carbapenem-resistant Enterobacteriaceae

Objective:To actively screen the dynamic colonization of Enterobacteriaceae in the gastrointestinal tract of tumor patients and explore the homology between carbapenem-resistant Enterobacteriaceae(CRE)colonized in the gastrointestinal tracts of these patients and the bacteria causing bloodstream infections,to provide meaningful reference data for diagnosis and treatment by clinicians.Methods:A total of 32 strains of gram-negative bacilli were isolated from 353 intestinal specimens of tumor patients at The First Affiliated Hospital of Henan University of Science and Technology from December 2021 to November 2022.The bacterial identification instruments VITEK 2 Com-pact,a carbapenemase detection kit using colloidal gold immunochromatography,and polymerase chain reaction(PCR)were used to de-tectdrug resistance genes and phenotypes.Pulsed-field gel electrophoresis(PFGE)was used to analyze and compare the homology between CRE strains in the intestinal tract of patients and pathogenic microorganisms causing bloodstream infections.Results:Among the 32 strains of Gram-negative bacilli screened from the gastrointestinal tract of cancer patients,6 strains(1.6%,6/353)were carbapenem-resistant Kleb-siella pneumoniae(CRKp).All six CRKp strains harbored blaKPC gene.The proportion of CRE is 3.3%(12/353).Bloodstream infection oc-curred in 2 patients with intestinal colonization of Klebsiella pneumoniae,which was caused by the same CRKp strain as intestinal coloniza-tion.Conclusions:The proportion of CRE intestinal colonization in tumor patients was not high;however,under certain circumstances,CRE colonized in the intestines of tumor patients can cause ectopic infections else where in the body,including the bloodstream,which should be closely monitored.The findings indicate the importance of increased attention to critically ill patients who have undergone multiple hospital-izations with long hospitalization times,as well as early screening for intestine-colonizing microorganisms and rational regulation of the anti-biotic usage.

8.Epidemiological and clinical characteristics of 34 children with brucellosis

Objective:To learn about the epidemiological and clinical characteristics of children with brucellosis, in order to provide reference for clinical diagnosis and treatment of brucellosis in children.Methods:Clinical data of children with brucellosis (aged ≤14 years) who visited the Department of Infectious Diseases at Beidahuang Industry Group General Hospital from December 2020 to December 2022 were retrospectively collected, and their epidemiological characteristics, clinical characteristics, laboratory tests, treatment and outcome were summarized and analyzed.Results:A total of 34 children with brucellosis were included, including 25 males (73.53%) and 9 females (26.47%), with a gender ratio of 2.78 ∶ 1.00. The median age was 8 years and 1 month, mainly in the age group of 6 to 14 years (19 cases, 55.88%). Epidemiological investigation showed that most of the affected children were rural residents (25 cases, 73.53%), with more contact with cattle/sheep (26 cases, 76.47%). The onset time was mainly concentrated in summer (15 cases, 44.12%) and spring (13 cases, 38.24%). The clinical symptoms were mainly fever (97.06%, 33/34) and arthralgia (64.71%, 22/34). In the laboratory tests, 25 cases (73.53%) had positive blood cultures, and the white blood cell count (WBC) of 30 cases (88.24%) was (4 - 10) × 10 9/L. Among the abnormalities of liver function, aspartate aminotransferase (AST) increased in 19 cases (55.88%), alanine aminotransferase (ALT) increased in 14 cases (41.18%) and γ-glutamyl transpeptidase (GGT) increased in 6 cases (17.65%). Among the myocardial enzymatic abnormalities, α-hydroxybutyrate dehydrogenase (HBDH) increased in 29 cases (85.29%), lactate dehydrogenase (LDH) increased in 27 cases (79.41%), and creatine kinase isoenzyme (CK-MB) increased in 8 cases (23.53%). After treatment, 25 children with positive blood culture turned negative. Conclusions:Children with brucellosis are mainly male, older and rural residents. The clinical manifestations are mainly fever and arthralgia. Doctors in relevant departments should conduct detailed epidemiological investigations and laboratory tests for such children in clinical work, in order to achieve early detection, diagnosis and treatment of pediatric brucellosis.
9.Observation on the therapeutic effect of cryotherapy in the removal of granulation tissue under electronic bronchoscope

Objective To explore and analyze the therapeutic effect of cryotherapy in the removal of granulation tissue under electronic bronchoscope,and find an efficient and safe auxiliary treatment technique for patients undergoing granulation tissue removal surgery.Method A clinical practice study was conducted on 41 patients who underwent granulation tissue removal surgery from June 2021 to June 2022.Patients were divided into groups using single and double numbers,with 21 patients assigned to single numbers included in the control group and 20 patients assigned to double numbers included in the observation group.Both groups underwent electronic bronchoscope granulation tissue removal surgery.During the surgery,the control group received argon-plasma coagulation(APC),while the observation group received cryotherapy.The levels of immune function indicators(including CD4+,CD8+and CD4+/CD8+),postoperative recovery related indicators,and effective rate of the two groups of patients were compared before and after treatment.Result Before treatment,there was no statistically significant difference in CD4+and CD4+/CD8+between the two groups of patients(P>0.05).After treatment,CD4+and CD4+/CD8+in the observation group were significantly higher than those in the control group,and the differences were statistically significant(P<0.05);Before treatment,there was no statistically significant difference in CD8+between the two groups of patients(P>0.05).After treatment,the observation group was significantly lower than the control group,and the difference was statistically significant(P<0.05);The effective rate of the observation group was 95.00%,higher than 80.95%of the control group.The postoperative pain visual analogue scale(VAS)of the observation group was(2.14±0.18)points,significantly lower than the control group's(6.09±0.95)points,and the differences were statistically significant(P<0.05);The first postoperative feeding time of the observation group was(6.08±0.76)hours,and the hospital stay was(5.12±0.68)days,which were shorter than the control group's(15.39±2.97)hours and(7.08±0.93)days;The treatment cost of the observation group was(10 500.60±80.70)yuan,which was lower than the control group's(19 800.00±126.00)yuan,and the differences were statistically significant(P<0.05);The incidence of postoperative complications in the observation group was 5.00%,lower than the control group's 23.81%,and the difference was statistically significant(P<0.05).Conclusion The application of cryotherapy in the removal of granulation tissue under electronic bronchoscope has a significant therapeutic effect.It can not only improve the surgical treatment effect of patients,improve their immune index levels,but also alleviate their postoperative pain,reduce the incidence of postoperative complications,and shorten their postoperative pre-and post cycle.Moreover,the treatment cost is low,and it is worthy of clinical application and promotion.
10.Exploring the Pathogenesis of T2DM based on the Correlation between “Qi-fire Imbalance” and Ferroptosis

Ferroptosis is a novel iron-dependent mode of programmed cell death characterized by iron deposition and accumulation of lipid peroxidation. More and more studies have found that ferroptosis is closely related to the pathogenesis of type 2 diabetes mellitus (T2DM). The yin-fire theory is an important part of LI Gao's spleen-stomach theory, and it is believed that qi-fire imblance and yin-fire internal generation is the main pathogenesis of T2DM. Abnormal iron metabolism may be an important prerequisite for T2DM yin-fire internal generation, while oxidative stress is the specific manifestation of T2DM qi-fire imbalance. Reactive oxygen species (ROS) is the end product of qi-fire imbalance, and lipid peroxide is the pathological products of T2DM yin-fire internal generation. This study intends to explore the pathological mechanism of qi-fire imbalance and yin-fire internal generation from the perspectives of iron metabolism, oxidative stress and lipid peroxidation, enriching the modern connotation of yin-fire theory, and benefiting traditional Chinese medicine to target against ferroptosis, and prevent and treat T2DM precisely.
