1.Mechanism of negative pressure wound therapy in the auxiliary treatment of bone and soft tissue infection

BACKGROUND:As the population ages at an accelerated rate in China,the incidence of bone and soft tissue infections is likewise on the rise.Bone and soft tissue infections can involve all bone and surrounding soft tissues,including the periosteum,cortex,cancellous bone and bone marrow.Examples of such infections include diabetes foot,osteomyelitis,surgical incision infection,and infection around joint prostheses.Owing to the intricate pathogenesis and challenging treatment,it has become increasingly noteworthy in clinical settings.Negative pressure wound therapy is a modern wound treatment which has been gaining popularity,especially in the area of bone and soft tissue infection. OBJECTIVE:To analyze the global research progress in the utilization of negative pressure wound therapy for treating bone and soft tissue infection in recent years. METHODS:Relevant articles in Chinese and English published in PubMed,Web of Science,and CNKI from 1990 to 2022 were retrieved.Search terms were"negative pressure wound therapy,vacuum assisted closure,negative pressure,osteomyelitis,bone infection"in English and Chinese,separately.A total of 711 articles were initially retrieved,out of which 65 articles were included for further review. RESULTS AND CONCLUSION:Negative pressure wound therapy,as an auxiliary approach to treating wounds,has a range of positive effects such as stabilizing wounds,diminishing edema,decreasing bacterial load,encouraging granulation tissue and angiogenesis,enhancing tissue perfusion,modulating peripheral nerves,modulating biological immunity and promoting the growth and differentiation of osteoblasts.This is a more effective method for managing complex wounds,such as bone and soft tissue infections,than traditional dressings with one-dimensional benefits.Numerous studies,both basic and clinical,have demonstrated the safety and efficacy of negative pressure wound therapy in the auxiliary treatment of bone and soft tissue infections.
2.Status quo of nursing practice and organizational management for reperfusion therapy in ischemic stroke in Beijing

Objective:To investigate the status of nursing practice and organizational management for reperfusion therapy in ischemic stroke across medical institutions in Beijing.Methods:In June 2023, a survey was conducted among nursing managers from 62 medical institutions under the jurisdiction of the Beijing Stroke Treatment Quality Control and Improvement Center. The survey aimed to assess the current nursing practices and organizational management for reperfusion therapy in ischemic stroke.Results:Among the 62 medical institutions, 16.13% (10/62) had dedicated reperfusion therapy nurses for stroke, and 87.10% (54/62) had established specific nursing protocols and procedures for stroke reperfusion therapy within their departments. However, 27.42% (17/62) of these institutions based the development and updates of their protocols on experiential summaries rather than standardized guidelines. In terms of nursing practice, there was room for improvement in emergency identification and triage of suspected stroke patients, assistance with intravenous thrombolysis, patient condition monitoring, early rehabilitation, and related health education. Regarding nursing quality control, significant differences were observed between institutions that regularly conducted quality control of stroke reperfusion therapy nursing and those that organized regular training sessions for nurses in this area ( P<0.05) . Conclusions:While nursing practices and quality for stroke reperfusion therapy in Beijing are progressing, there are disparities in regional development. There is a high demand for specialized stroke nursing and targeted technical training.
3.Expression of secretory protein DKK1 in cervical cancer and its clinical significance

Objective :
To study the expression and clinical significance of Wnt signal secretory protein DKK1 in cervical cancer.
Methods :
Reverse transcription polymerase chain reaction (RT⁃PCR) was used to detect the expression of DKK1 mRNA in 30 pairs of cervical cancer tissues and adjacent normal tissues. The expression of DKK1 protein was detected by immunohistochemistry in 60 cases of cervical cancer and 30 cases of normal cervical tissue , and the relationship between the expression and clinicopathological characteristics of cervical cancer was analyzed.
Results :
The expression of DKK1 in cervical cancer tissues was lower than that in adjacent tissues and normal cervical tissues (P < 0. 05) . The expression of DKK1 was closely related to clinicopathological stage , tissue differentiation , lymph node metastasis and depth of invasion (P < 0. 05) .
Conclusion
The expression of DKK1 is low in cervical cancer and plays an important role in the occurrence and development of cervical cancer.
4.Clinical observation of electrical stimulation biofeedback therapy in treatment of urinary incontinence in children

Objective:To explore the therapeutic effect of electrical stimulation biofeedback in children with different types of urinary incontinence.Methods:A total of 20 children with stress urinary incontinence admitted to the Department of Pediatric Surgery, the First Affiliated Hospital of Xinxiang Medical University from October 2010 and June 2019 were enrolled in this study and treated by electrical stimulation biofeedback twice a week, with 5 weeks as a course of treatment.There were 12 males and 8 females aged 2.5-7.0.Nine cases developed urinary incontinence after spina bifida operation, 6 cases developed urinary incontinence after high anal atresia operation and 5 cases were non-neurogenic urinary incontinence.The frequency of involuntary urination, urinary volume and the voltage of pelvic floor muscles were compared before and after treatment.Results:After 1 course of treatment, 10 cases (50.00%) of urinary incontinence were cured, 4 cases (20.00%) were effective, and 6 cases (30.00%) were ineffective.Among the 6 patients developing urinary incontinence after high anal atresia operation, 5 cases (83.33%) were cured and 1 case (16.67%) was effective.Of 9 cases developing urinary incontinence after spina bifida operation, 3 cases (33.33%) were effective.All 5 cases with non-neurogenic urinary incontinence (100.00%) were cured.The voltage of pelvic floor muscles was increased from (20.36±3.92) μV before electrical stimulation biofeedback therapy to (36.44±7.57) μV after treatment, and the difference was statistical significance ( t=10.68, P<0.05). Conclusions:Electrical stimulation biofeedback therapy is effective for children with urinary incontinence.
5.Current status of the training model of full-time professional postgraduate students in public health in Australia and Japan and their reference significance

With the increasing frequency and impact of public health incidents in the world's medical and health services in recent years,it has become extremely necessary to train public health talents suitable for the medical environment in China.This article mainly discusses the training model of professional postgraduate students in public health in Australian universities (Sydney University and Melbourne University) and Japanese universities (Tokyo University and Hokkaido University).Both countries attach importance to the education of practical ability.Australian universities focus on the training of allround and comprehensive public health talents,while Japanese universities emphasizes the training of professional public health talents.At present,there is a lack of a clear understanding of public health and detailed standards in China,which is the main problem of public health education in China.A detailed and efficient training model of public health talents should be established based on the unique features of our country.

Novel biologics that redirect cytotoxic T lymphocytes (CTLs) to kill tumor cells bearing a tumor associated antigen hold great promise in the clinic. However, the ability to safely and potently target CD3 on CTL toward tumor associated antigens (TAA) expressed on tumor cells remains a challenge of both technology and biology. Herein we describe the use of a Half DVD-Ig format that can redirect CTL to kill tumor cells. Notably, Half DVD-Ig molecules that are monovalent for each specificity demonstrated reduced non-specific CTL activation and conditional CTL activation upon binding to TAA compared to intact tetravalent DVD-Ig molecules that are bivalent for each specificity, while maintaining good drug like properties and appropriate PK properties.

7.MiR-22-3p suppresses cell proliferation by targeting AEG-1 in NSCLC

Objective To investigate the expression and effect of miR-22-3p in non-small cell lung cancer (NSCLC). Methods The miR-22-3p expression level in seventy-six NSCLC tissues and para-cancer tissues was detected by qRT-PCR. The relationship between the expression of miR-22-3p and gender,age,tumor size,histolo-gy grade,pathological type and lymph node metastasis was analyzed. The function of miR-22-3p on the prolifera-tion of NSCLC cells was tested by growth curve assay. Target genes of miR-22-3p were predicted by online software Targetscan. Luciferase reporter assay and qRT-PCR was used to certificate the prediction. Results The expression of miR-22-3p was increased in NSCLC tissues than the para-cancer tissues and was correlated to lymph node metas-tasis. Overexpression of miR-22-3p could suppress the proliferation of A549 cells. Astrocyte-Elevated Gene-1(AEG-1) was predicted to be a target of miR-22-3p. MiR-22-3p was revealed to bind to AEG-13′UTR by luciferase report-er assay. Overexpression of miR-22-3p could inhibit the expression of AEG-1 in A549 cells. Suppression of miR-22-3p could increase AEG-1 expression. Conclusion MiR-22-3p could inhibit the proliferation of NSCLC by tar-geting AEG-1.
8.Significance of detecting miR-145 hypermethylation in non-small cell lung cancer

Objective To explore the significance of detecting miR-145 hypermethylation in non-small cell lung cancer (NSCLC). Methods Seventy-five NSCLC tissues and adjacent tissues was collected from May 2012 to January 2015. The methylation status of miR-145 promoter was tested by bisulfite sequencing PCR (BSP) and the expression of miR-145 evaluated by ISH. The correlation between miR-145 methylation and clinical parameters , and the relationship between miR-145 methylation and expression were statistically analyzed. Results The hypermethy-lation ratio of miR-145 in NSCLC tissues was higher than that in the adjacent tissues. Hypermethylation of miR-145 was correlated with NSCLC differentiation stage, but not with gender and age. In NSCLC, miR-145 hypermethyla-tion was negatively related with its expression. Conclusion MiR-145 hypermethylation is closely related with dif-ferentiation stage in NSCLC tissues. MiR-145 hypermethylation may be a potential biomarker for estimating NSCLC differentiation stage.
9.Effect of mechanical thrombectomy with Solitaire stent in the treatment of acute intracranial large artery occlusion

Objective To evaluate the safety and effectiveness of mechanical thrombectomy with Solitaire AB stent in the treatment of large intracranial artery occlusions.Methods The calinical data of 15 patients with acute stroke(more than 3.5 h intravenous thrombolysis time window) who were carried out arterial embolectomy with Solitaire AB stent in the No.264th Hospital of the Chinese People's Liberation Army were retrospective analyzed.There were 11 cases of middle cerebral artery(MCA),2 cases of internal carotid artery (ICA),1 case of vertebral artery(VA) pluse basilar artery(BA),and 1 case of anterior artery(AA).The recanalization occluded situation and surgical complications were analyzed,NIHSS score of preoperatively and at discharge were compared.Results In 15 cases,there were 14 cases of totally recanalization,1 case of partial recanalization.Two case with MCA stenosis and 1 case with VA stenosis after the recanalization were accepted stent angioplasty,2 csase died.NIHSS score of 13 cases survival patients increased from (22.85±4.75) scores on admission to (4.39 ±3.67) scores out of hospital,and the difference was statistically sinificant(t=2.752,P <0.01).Conclusion The mechanical thrombectomy with Solitaire AB stent can get high recanalization rate,fewer complications and good clinical outcome on patients with large intracranial artery occlusions.For more than venous or arterial thrombolysis time window,the mechanical thrombectomy can be considered within the 8 h after comprehensive evaluation.
10.Investigation and analysis of the delay situation of thrombolysis treatment in hospital in patients with acute ischemic stroke

Objective To know the situation of time delay from the patients with ischemic stroke arrived the emergency to accepted the thrombolysis treatment, and analyze the related factors in order to put forward effective process optimization measures. Methods Totally 122 patients with acute ischemic stroke admitted into a Beijing grade A tertiary hospital who needed receive thrombolysis treatment from June 2014 to June 2015 were selected as the research object according to the purpose sampling method. Then we recorded the time in the whole process which were devided into 4 steps ( arrived at the emergency department- obtained consultation of stroke physicians;obtained consultation of stroke physicians- arrived in CT chamber; arrived in CT chamber- got CT scan results; got CT scan results- accepted thrombolysis therapy ) . Results The time delayed in hospital of intravenous thrombolysis group and artery thrombolysis group were (114.95±29.48)min and (171.22±50.10)min. The step of biggest delta-T between the actual time and standard time of two groups were both from getting a CT scan results to accepting thrombolysis therapy.Conclusions The delay after getting a CT scan results is the most important reason of delay in hospital. Shorten the time of blood test, optimize the hospital management and perfect the construction of the green channel, can promote the situation of the delay in hospital, and, improve the efficiency of thrombolysis treatment.
