1.Manganese porphyrin metal-organic framework nanoparticles loaded with DMXAA combined with sonodynamic therapy for the treatment of triple-negative breast cancer mouse xenografts

[摘 要] 目的:构建负载STING激动剂DMXAA的锰卟啉金属有机框架纳米颗粒(DPM),探讨其对三阴性乳腺癌(TNBC)细胞4T1及其小鼠移植瘤的治疗效果。方法:通过物理吸附法制备 DPM 纳米颗粒,利用透射电镜、扫描电镜及纳米粒度电位仪表征其形貌与理化性质。常规培养4T1细胞,细胞实验分为对照组、超声辐照组(US组)、DPM治疗组(DPM组)和DPM治疗联合超声辐照组(DPM + US组),用CCK-8法检测细胞活性,免疫荧光法检测高迁移率族蛋白B1(HMGB1)和钙网蛋白(CRT)的表达,WB法检测STING通路相关蛋白的表达。构建4T1细胞移植瘤小鼠模型,分为四组,处理同细胞实验,测量肿瘤体积,免疫荧光法检测移植瘤组织中Ki-67、HMGB1、CRT和缺氧诱导因子-1ɑ(HIF-1ɑ)蛋白的表达,TUNEL法检测细胞凋亡,流式细胞术检测免疫细胞活化情况,对主要器官进行H-E染色,以评估纳米材料的体内安全性。结果:DPM呈梭形,平均粒径(268 ± 3.302)nm,电位(33.1 ± 0.87)mV。细胞实验中,DPM联合超声辐照可明显抑制4T1细胞的增殖(P < 0.001),提高4T1细胞中ROS水平(P < 0.001),诱导4T1细胞CRT表达上调(P < 0.001),HMGB1从细胞核中移至细胞质,激活STING信号通路[p-STING、p-TBK1、p-IRF3蛋白表达均显著增加(均P < 0.001)]。体内实验中,DPM联合超声辐照可显著抑制4T1细胞移植瘤生长(P < 0.001)并促进免疫细胞表型转化(P < 0.001),抑制移植瘤组织中Ki-67、HIF-1α蛋白表达(均P < 0.01),谷胱甘肽(GSH)产生(P < 0.01),促进CRT、HMGB1蛋白表达、ROS产生(P < 0.001),对主要器官结构无明显影响。结论: DPM联合超声辐照可通过激活STING通路显著抑制4T1细胞及其移植瘤的生长,诱导抗肿瘤免疫应答,且对主要器官无明显毒性。
2.The SMAD-Pathway Mediates HMGB1-Induced Proliferation and Metastatic Progression in Cutaneous Squamous Cell Carcinoma Cells

Background:
High-mobility group box protein 1 (HMGB1) is a chromatin-binding protein involved in arthritis, ischemia, sepsis, atherosclerosis, neurodegenerative disorders, meningitis, and cancer. HMGB1 exhibits dual roles in cancer, acting as either a tumor suppressor or oncoprotein depending on context.
Objective:
This research aimed to elucidate HMGB1’s functional significance in cutaneous squamous cell carcinoma (cSCC).
Methods:
We overexpressed HMGB1 in cSCC cell lines using recombinant adenovirus and examined its effects on cell proliferation, colony formation, and cell migration.
Results:
Immunohistochemical analysis revealed elevated HMGB1 expression levels in cSCC tissue relative to normal epidermis. To assess the influence of HMGB1, we employed recombinant adenoviruses expressing HMGB1 to transduce SCC cell lines (SCC12 and SCC13). Enhanced HMGB1 expression significantly promoted cellular proliferation and colony formation capacity.Notably, HMGB1 overexpression elevated the levels of proliferation regulators, including P63, SOX2, CDK4 and CDK6. Furthermore, HMGB1 overexpression substantially enhanced tumor invasiveness, accompanied by upregulation of epithelial-mesenchymal transition (EMT) biomarkers. Mechanistically, overexpression of HMGB1 enhanced transforming growth factor-β signaling by increasing phosphorylation of SMAD2/3, the key mediators of EMT.
Conclusion
These data imply that HMGB1 acts as a tumor-promoting factor in cSCC.

This paper has summarized current studies on retinitis pigmentosa (RP) from two perspectives, which were disease mechanism insights from western medicine, and etiology, pathogenesis and therapeutic principles in traditional Chinese medicine (TCM). From the perspective of western medicine, the understanding of RP has evolved from an early single-factor explanation focusing on pathogenic gene mutations to a more comprehensive, multifactorial model involving genetic abnormalities, photoreceptor cell degeneration, retinal pigment epithelium dysfunction, oxidative stress, inflammatory responses, microenvironmental imbalance, and vascular insufficiency. In TCM, RP is considered to be a disorder manifested in the eyes but rooted in dysfunction of the zang-fu (脏腑) organs. Its pathogenesis is primarily associated with congenital insufficiency, liver-kidney depletion, impaired spleen function of transportation and transformation, and stasis obstructing the meridians. The core pathological mechanism can be summarized as "deficiency complicated by stasis", emphasizing holistic imbalance, dynamic disease progression, and syndrome differentiation and treatment. The differences and complementarities between the two medical systems in understanding disease progression are further discussed. RP is proposed to be more appropriately regarded as a complex disease characterized by long-term progression, stage-dependent evolution, and persistent decompensation. Western medicine contributes to elucidating the molecular mechanisms underlying RP and provides the foundation for targeted therapeutic interventions, whereas TCM offers a holistic perspective for understanding disease evolution through the assessment of overall functional status, disease-stage stratification, and long-term syndrome-based regulation.

5.A study on the practical effect of critical value management based on the goal and key result approach

Objective To explore the effectiveness of the OKR method in improving the efficiency of the whole process of critical value management,and to provide new ideas for the implementation of medical quality improvement in other medical insti-tutions.Methods A hospital in Tianjin was selected as the study object,which used the OKR method to reform the management of critical value since January 2024.The core goal of"improving the effectiveness of critical value management"was set,and the goal was broken down into quantitative key results such as"the rate of receiving the critical value system is over 95%"and"the rate of completing the standardised writing of medical records is up to 85%".Results After the reform,several quantitative key results,such as the completion rate of critical care medical record writing,the rate of standardised medical record writing,and the rate of overtime acceptance,were all better than before.Conclusion Through the OKR method to unify the whole hospital's strategic objectives,and the dynamic adjustment of the program based on data review,the hospital's critical value management efficiency has been significantly improved,effectively guaranteeing the safety of patients,and providing new perspectives and methods for the management of critical value in other medical institutions.

The molecular mechanisms underlying the develop-ment and metastasis of prostate cancer remain elusive.This comprehensive review delves into the intricate role of cofilin,an actin-binding protein,in the pathogenesis and progression of prostate cancer.Cofilin is a significant protein in cytoskeletal dynamics,and any dysregulation may result in the morphological changes in normal cells and the invasion and metastasis of tumor cells.Research has revealed that the activity of cofilin is regula-ted by various mechanisms,including phosphorylation/dephos-phorylation and interactions with other molecules.Moreover,this review discusses promising therapeutic interventions,such as co-filin inhibitors and gene therapy,which have demonstrated effica-cy in preclinical models.The challenge of clinically preventing the transition to castration-resistant prostate cancer and tumor metastasis is widely recognized,necessitating the development of precise drug treatments and biomarker identification.As a key regulatory protein,cofilin provides a more comprehensive refer-ence for the prevention and treatment of prostate diseases.
7.Application effect of the bispectral index in ventilator weaning after congenital heart surgery in infants

Objective:To explore the application and guiding value of bispectral index(BIS)in postoperative ventilator weaning in infants with congenital heart disease.Methods:A retrospective analysis was conducted on 81 cases of infants with congenital heart disease treated at Xuzhou Children's Hospital affiliated with Xuzhou Medical University from January 2022 to November 2023. The infants were divided into the successful weaning group(62 cases)and the failed weaning group(19 cases)based on the success of ventilator withdrawal. Univariate and multivariate Logistic analyses were performed on the clinical data of the two groups of infants,and ROC curves were plotted to analyze the predictive value of BIS for postoperative ventilator withdrawal failure in infants with congenital heart disease.Results:The mechanical ventilation time,ICU stay time,hospitalization time,and BIS values at 1 hour,2 hours,3 hours,4 hours,and 5 hours after sedation in the two groups of infants showed statistically significant differences( P<0.05). The results of the multivariate Logistic regression analysis indicated that BIS values at 1 hour,2 hours,3 hours,4 hours,and 5 hours after sedation are factors affecting the failure of postoperative ventilator weaning in infants with congenital heart disease( P<0.05). The ROC curve analysis revealed that the AUCs for predicting postoperative ventilator weaning failure in infants with congenital heart disease were 0.886,0.877,0.873,0.907,0.925,and 0.954 for BIS at 1 hour,2 hours,3 hours,4 hours,and 5 hours after sedation,and the combined prediction,respectively. The sensitivities and specificities were as follows:for BIS at 1 hour after sedation,94.7% and 67.7%;for BIS at 2 hours after sedation,84.2% and 93.5%;for BIS at 3 hours after sedation,84.2% and 83.9%;for BIS at 4 hours after sedation,89.5% and 79.0%;for BIS at 5 hours after sedation,84.2% and 85.5%;and for the combined prediction,100.0% and 90.3%. Conclusion:The use of BIS monitoring during the weaning of ventilators in infants after congenital heart disease surgery has certain predictive value for the success of the weaning process.
8.The accuracy of virtual surgical planning assisted management for L-shaped reduction malarplasty

Objective:To evaluate the accuracy of L-shaped reduction malarplasty under the guidance of virtual surgical planning (VSP).Methods:The data of adult female patients who were diagnosed with zygomatic protrusion or hypertrophy at Department of Orthognathic and Temporo-mandibular Joint Surgery, West China Hospital of Stomatology, Sichuan University, from January 2018 to December 2020 were analyzed retrospectively. L-shaped reduction malarplasty with or without bone resection and with the mortice and tenon joint structure on the zygomatic arch was conducted either by digital procedures comprising VSP and three-dimensional printing titanium templates (digtal surgery group) or by conventional method (control group). The incidence of postoperative complications and the patient postoperative satisfaction [using a Likert scale with a score ranging from 1 to 5, representing very dissatisfied, dissatisfied, average, satisfied and very satisfied, satisfaction rate = (very satisfied + satisfied)/ total number of patients × 100%] were statistically analyzed in the two groups. The differences in the postoperative symmetry of the zygomatic complex between the digital group and the control group were analyzed by three-dimensional cephalometry. The accuracy of VSP in L-shaped reduction malarplasty was evaluated by comparing the preoperative design model with the actual postoperative model in the digital group. The statistical analyses were conducted using SPSS 24.0 software. The chi-square test was used in the comparison of surgical complications and patient satisfaction rates. The symmetry of bilateral landmarks in the three-dimensional direction between the two groups was evaluated using the independent t-test, and the pre- and post-operative measurements in the digital group were compared using paired t-test. Results:A total of 78 patients were included, with 36 in the digital group, aged (25.2±3.6) years, and 42 in the control group, aged (24.3±2.8) years. Satisfactory reduction of zygomatic protrusion or hypertrophy was recognized among all patients. Compared with the control group, the digital group had lower percentage of complications [25% (9/36) vs. 55% (23/42)] and higher postoperative satisfaction [78% (28/36) vs. 48% (20/42)], both of which were statistically significant (all P<0.01). Regarding the symmetry of bilateral zygomatic complexes, the average deviations of ΔZb (bottom point of zygoma) in the digital group in the horizontal, vertical, and sagittal directions [(1.05±0.24), (1.05±0.24), (1.00±0.88) mm] were significantly smaller than those in the control group [(2.03±0.58), (1.32±0.68), (1.47±0.47) mm], with statistically significant differences (all P<0.05). The bone segment movements of virtual plans and actual result in the digital surgery group were measured and showed no obvious difference for the inward movement [(5.42±0.98) mm vs. (5.33±0.93) mm] and the sagittal overlap [(4.87±1.21) mm vs. (4.77±1.32) mm] at the zygoma roots, along with the step length at the long-arm of the L-shaped osteotomy line [(2.43±1.11) mm vs. (2.39±0.89) mm] (all P>0.05). The mean differences of facial width and protrusion measurements between virtual simulations and actual result in the digital group ranged from (1.13±0.47) mm to (2.07±0.88) mm, with no significant differences( P>0.05). Meanwhile, the high resemblance between virtual plans and actual result was depicted via superimposition models, with a deviation controlled within ±0.5 mm. Conclusion:The application of VSP in reduction malarplasty significantly improved surgical accuracy and reduced difficulties in the operation, which would improve patients’ postoperative satisfaction.
9.Exploring the Construction of Key miRNA mRNA Networks in TS Model Rats Based on Transcriptomics and Potential Traditional Chinese Medicine Intervention

Objective:To investigate the miRNA-mRNA regulatory network in a rat model of Tourette syndrome(TS)using transcriptomic technology and to screen key signaling pathways and potential traditional Chinese medicine(TCM)candidates for intervention.Methods:A TS rat model was established using iminodipropionitrile(IDPN).RNA sequencing was performed to identify differentially expressed miRNAs and mRNAs in the brain tissues of TS rats.Bioinformatics analysis was applied to construct interaction networks,and network pharmacology was further employed to screen potential TCM compounds.Results:After 7 days of IDPN modeling,the model group exhibited motor and stereotypical behavioral changes,with behavioral scores greater than 3 points.Hema toxylin-eosin(HE)staining revealed irregular neuronal nuclear morphology,uneven chromatin distribution,nuclear pyknosis,and increased glial cell density.KEGG enrichment analysis identified key pathways:calcium signaling pathway,neuroactive ligand-receptor interaction,p53 signaling pathway,ECM-receptor interaction,and TGF-β signaling pathway.miR-125a-3p,miR-106-3p,and miR-760-3p were identified as pivotal miRNAs.Potential TCM candidates included Ajuga decumbens,Acanthopanax bark,Codonopsis pilosula,Stephania japonica,Os Draconis,Notopterygium root,Siraitia grosvenorii,Zanthoxylum nitidum root,Morinda officinalis,and Corydalis yanhusuo.Conclusion:The miRNAs miR-106-3p,miR-125a-3p,and miR-760-3p may mediate TS pathogenesis by altering critical signaling networks,including the calcium signaling pathway,neuroactive ligand-receptor interaction,and ECM-receptor interaction pathways,leading to neuroimmune inflammation and dopaminergic system dysregulation.TCM compounds such as Corydalis yanhusuo and Ajuga decumbens may exert therapeutic effects through multi-component synergistic regulation of these miRNAs and downstream pathways.
10.Effect of circular RNA circ_0004535 on type 2 diabetes mellitus combined with metabolism-related fatty liver disease model mice

Objective To explore the influence of hsa_circ_0004535 on type 2 diabetes(T2DM)combined with metabolic-associated fatty liver disease(MAFLD)model mice.Methods Forty-eight healthy SPF grade Balb/c mice were selected for modeling and divided into the following groups(n=6 per group):Control group:normal feed;T2DM group:diabetes model induced by high-glucose and high-fat diet;T2DM combined MAFLD group:non-alcoholic fatty liver high-glucose and high-fat diet-induced diabetes combined with fatty liver model;T2DM combined MAFLD+hsa_circ_NC group:after 4 weeks of modeling,10 nmol hsa_circ_NC injected into the tail vein;T2DM combined MAFLD+hsa_circ_0004535 group:after 4 weeks of modeling,10 nmol circ_0004535 injected into the tail vein;T2DM combined MAFLD+miRNA_NC group:after 4 weeks of modeling,10 nmol miRNA blank control injected into the tail vein;T2DM combined MAFLD+miR-1827 agomir group:after 4 weeks of modeling,10 nmol miR-1827 agomir injected into the tail vein;and T2DM combined MAFLD+miR-1827 antagomir group:after 4 weeks of modeling,10 nmol miR-1827 antagomir injected into the tail vein.Mouse body weight was measured after the interventions and recorded weekly.Glucose and insulin tolerance tests were performed,blood lipids and liver function were measured,the liver and insulin resistance indexes were calculated,and pathological tests(hematoxylin/eosin(HE),oil red O,and Masson staining,immunohistochemistry,terminal deoxynucleotidyl transferase dUTP nick end labeling(TUNEL))were performed to measure the degree of hepatic inflammation,fat deposition,and fibrosis.Results(1)Body weight,liver weight,liver index,insulin resistance index,and biochemical indexes were all significantly lower in the hsa_circ_0004535 injection group compared with the hsa_circ_NC injection group and the T2DM combined MAFLD group(both P<0.05).(2)Steatosis vacuoles were reduced and smaller and inflammatory cell infiltration was reduced in the T2DM combined MAFLD+circ_0004535 group,as shown by HE and oil red staining.(3)TUNEL-positive cells were significantly reduced in the T2DM combined MAFLD+hsa_circ_0004535 group(P<0.05).(4)Collagen fiber deposition was significantly reduced in the T2DM combined MAFLD+hsa_circ_0004535 group,as shown by Masson staining(P<0.05).Conclusions The expression of hsa_circ_0004535 and miRNA-1827 play important roles in regulating lipid metabolism,insulin sensitivity,inflammatory pathways,hepatocyte apoptosis,and hepatic fibrosis-related pathways in an animal model of T2DM combined with MAFLD.
