1.Anatomical Importance Between Neural Structure and Bony Landmark in Neuroventral Decompression for Posterior Endoscopic Cervical Discectomy

Objective:
This study aims to investigate the anatomical relationship among the nerve roots, intervertebral space, pedicles, and intradural rootlets of the cervical spine for improving operative outcomes and exploring neuroventral decompression approach in posterior endoscopic cervical discectomy (PECD).
Methods:
Cervical computed tomography myelography imaging data from January 2021 to May 2023 were collected, and the RadiAnt DICOM Viewer Software was employed to conduct multiplane reconstruction. The following parameters were recorded: width of nerve root (WN), nerve root-superior pedicle distance (NSPD), nerve root-inferior pedicle distance (NIPD), and the relationship between the intervertebral space and the nerve root (shoulder, anterior, and axillary). Additionally, the descending angles between the spinal cord and the ventral (VRA) and dorsal (DRA) rootlets were measured.
Results:
The WN showed a gradual increase from C4 to C7, with measurements notably larger in men compared to women. The NSPD decreased gradually from the C2–3 to the C5–6 levels. However, the NIPD showed an opposite level-related change, notably larger than the NSPD at the C4–5, C5–6, and C7–T1 levels. Furthermore, significant differences in NIPD were observed between different age groups and genders. The incidence of the anterior type exhibited a gradual decrease from the C2–3 to the C5–6 levels. Conversely, the axillary type exhibited an opposite level-related change. Additionally, the VRA and DRA decreased as the level descended, with measurements significantly larger in females.
Conclusion
A prediction of the positional relationship between the intervertebral space and the nerve root is essential for the direct neuroventral decompression in PECD to avoid damaging the neural structures. The axillary route of the nerve root offers a safer and more effective pathway for performing direct neuroventral decompression compared to the shoulder approach.

Sarin(GB)is a typical representative of nerve agents with high toxicity,and very low amount can cause death.GB can cause water and atmospheric environment poisoning,so the detection of GB in water and air is of great significance.In this work,a colorimetric sensor array(CSA)based on GB inhibition of cholinesterase activity was constructed to detect GB with high selectivity.A 4×4 colorimetric array was constructed using acetylcholinesterase(AChE),butyryl cholinesterase(BuChE)and the corresponding substrate acetylthiocholine iodide(S-ACh),butyryl thiocholine iodide(S-BCh),acetylcholine chloride(ACh),butyryl choline chloride(BCh)and 2,6-dichloroindophenol ethyl ester(DCIE).The linear curve of the sensor was Y=131.3×lgC+271.6(R2=0.997),where Y was the array response Euclidean distance,C was the concentration of GB(mg/L),the linear range was 0.03?0.32 mg/L,and the detection limit was 27.6 μg/L.The method could effectively distinguish chemical warfare agents(CWA)such as VX,Soman(GD),mustard gas(HD),Louie reagent(L),and had high anti-interference ability,sensitivity and good repeatability.It was successfully applied to the detection of GB in simulated water and simulated air samples,and the sample recovery rate was 97.2% ?100.9%.This method would be potentially applied to the field rapid detection of nerve agents.
5.Development and application of information management system for occupational health technical service institutions

With the vigorous development of computers and internet, the construction of the information management system for Occupational Health Technical Service (OHTS) institutions in China has achieved impressive progress. But for the management of OHTS institutions, there are relatively few systems that can fully explore and utilize OHTS information. Base on this background, in light of the actual situation of the OHTS institution in Henan Province, an OHTS Information Management System was developed under the Java Spring Boot framework, with a MySQL database and a B/S multi-tier architecture. The platform integrates a vertical three-level network of ″provincial-municipal-county/district″ and a horizontal network involving health commissions, disease prevention and control bureaus, Centers for Disease Control and Prevention (occupational disease prevention and treatment institutes), and OHTS institutions. The system includes five core modules: dynamic management of institutional and personnel qualifications, full-process project supervision (including five categories of technical services such as pre-evaluation and control-effectiveness evaluation), multidimensional decision analysis (including eight statistical indicators of institutional distribution, equipment allocation, and occupational hazard factors), rapid generation and automated submission of various reports, and early warning and intelligent supervision. The system has been implemented in 61 OHTS institutions in Henan Province, improving the ″off-site supervision rate″ of supervision department and promoting the standardization and digital transformation of occupational health services.
6.Application of Remimazolam in Pediatric Adenoidectomy and Tonsillectomy Anesthesia and Its Effect on Emergence Agitation

Objective To evaluate the application of different anesthetic drugs—remimazolam(RM)and propofol(PPF)—in anesthesia for pediatric adenoidectomy and tonsillectomy and its effect on emergence agitation.Methods A total of 120 children undergoing elective adenoidectomy and tonsillectomy under general anesthesia in Xinhua Hospital,Huainan Xinhua Medical Group between December 2022 and August 2024 were enrolled.With a random number table,they were assigned to a PPF group receiving PPF and remifentanil and a RM group receiving RM and remifentanil,with 60 cases in each group.The primary and secondary outcome indicators of the two groups were compared.The primary outcome indicator was the incidence of emergence agitation.Secondary outcome indicators included anesthesia-related parameters,changes in mean arterial pressure(MAP)and heart rate(HR)at different time points during surgery,changes in pediatric anesthesia emergence delirium(PAED)scores at different time points after recovery,changes in Ramsay sedation scores and Face,Legs,Activity,Cry,Consolability(FLACC)pain scores at 2,4,12,and 24 hours after surgery,incidence of negative postoperative behavioral changes(NPOBCs)at 7 and 14 days,and adverse events during anesthesia.Results The incidence rate of emergence agitation was lower in the RM group than that of the PPF group(5.00%vs.18.33%,P<0.05).The RM group also demonstrated significantly shorter anesthesia time,extubation time,wake-up time,and postanesthesia care unit(PACU)than those in the PPF group(P<0.05).HR and MAP in the RM group were higher than those in the PPF group at 3 min after induction,at the time of tracheal intubation,during tonsillectomy,and at the end of surgery,with the difference being statistically significant(P<0.05).The PAED scores at different time points after recovery,Ramsay sedation scores and FLACC scores at 2,4 and 12 hours after surgery,the incidence of NPOBCs at 7 days after surgery,and the incidence of adverse reactions during anesthesia were lower in the RM group than those in the PPF group,with the difference being statistically significant(P<0.05).Conclusion RM improves the anesthesia effect and recovery quality of children undergoing adenoidectomy and tonsillectomy,relieves the severity of emergence agitation,reduces the incidence of agitation,and demonstrates good safety.

Clinically,the incidence of portal vein thrombosis(PVT)in patients with cirrhosis can be up to 10%-23%.When PVT is not treated promptly,it may develop to cavernous transformation of the portal vein(CTPV).CTPV can aggravate portal hypertension,accelerate the progression of esophagogastric varices bleeding,refractory ascites,refractory peritonitis,biliary tract diseases,and hepatic insufficiency.At present,noninvasive imaging techniques such as portal vein reconstruction,enhanced CT and ultrasound are mostly used to make the diagnosis and evaluation of CTPV.It is rather difficult to perform portosystemic shunt surgery in patients with CTPV complicated by portal hypertension,which was once regarded as a contraindication for interventional portosystemic shunt procedures.With the improvement of related technologies and surgical instruments,the transjugular intrahepatic portosystemic shunt(TIPS)has become an important treatment for CTPV.This paper aims to make a comprehensive review about the relevant researches concerning the portosystemic shunt surgery in patients with CTPV so as to clarify the importance of TIPS in the treatment of CTPV.

Acute lateral ankle sprain (ALAS) is one of the most common sport injuries, with high incidence, recurrence and disability rates. Currently, exercise rehabilitation-based non-surgical treatment is the primary management approach for ALAS. However, there remain improper practices such as excessive immobilization or uncontrolled activity, which contribute to recurrent sprains and chronic ankle instability, significantly impairing patients′ athletic function and quality of life. To standardize the non-surgical management of ALAS, improve the cure rates, and reduce the recurrence and disability rates, Chinese Sports Rehabilitation Medicine Training Project of Chinese Medical Association, Foot and Ankle Basics and Orthopedics Group, Orthopedic Branch of Chinese Medical Doctor Association, and Sports Medicine Branch of Jiangsu Medical Association organized relevant experts to formulate Expert consensus on non-surgical treatment for acute lateral ankle sprain ( version 2025), following the principles of scientific vigor, practicality, and innovation. Thirteen recommendations were proposed for standardized treatment protocols across different healing phases, aiming to provide references for standard management of ALAS and improve the therapeutic outcomes.
