1.Drying kinetics of Salviae Miltiorrhizae Radix et Rhizoma and dynamics of active components in drying process.

This study explored the drying kinetics of Salviae Miltiorrhizae Radix et Rhizoma(SM), established the suitable models simulating the drying kinetics, and then analyzed the dynamic changes of active components during the drying processes with different methods, aiming to provide a basis for the establishment of suitable drying methods and the quality control of SM. The drying kinetics were studied based on the drying curve, drying rate, moisture effective diffusion coefficient, and drying activation energy, and the appropriate drying kinetics model of SM was established. The drying performance of different methods, such as hot air drying, infrared drying, and microwave drying of SM was evaluated, and the changes in the content of 10 salvianolic acids and 6 tanshinones during drying were analyzed by UPLC-TQ-MS. The Technique for Order Preference by Similarity to an Ideal Solution(TOPSIS) was employed to evaluate the quality of SM dried with different methods. The results showed that the drying rate and moisture effective diffusion coefficient of SM increased with the rise in drying temperature, and the maximum drying rates of different methods were in the order of microwave drying > infrared drying > hot air drying, slice > whole root. The drying rate decreased with the rise in temperature and the extension of drying time. The activation energy of hot air drying was higher than that of infrared drying in SM. The most suitable model for simulating the drying process of SM was the Page model. The TOPSIS results suggested infrared drying at 50 ℃ was the optimal drying method for SM. During the drying process, the content of salvianolic acids increased in different degrees with the loss of moisture, among which salvianolic acid B showed the largest increase of 44 times compared with that in the fresh medicinal material. Tanshinones also existed in the fresh herb of SM, and the content of tanshinone Ⅱ_A increased by 3 times after drying. The results provided a basis for the establishment of suitable drying methods and the quality control of SM.

2.Direct anterior craniocervical junction fenestration decompression and bone graft for the treatment of early and middle stage osteonecrosis of the femoral head: a 3-year follow-up.

OBJECTIVE:
To assess the mid-term clinical efficacy of the direct anterior approach for window decompression and bone grafting surgery in the treatment of early to mid-stage osteonecrosis of the femoral head (ONFH).
METHODS:
A retrospective analysis was performed on 40 patients (40 hips) diagnosed with osteonecrosis of the femoral head (ONFH), classified as types L1 and L2 according to the China-Japan Friendship Hospital (CJFH) classification system, and at stagesⅡ, ⅢA, and ⅢB based on the Association Research Circulation Osseous (ARCO) staging system. All patients underwent head-neck junction fenestration decompression and bone grafting via the direct anterior approach between January 2015 and May 2022, with complete follow-up data available for a minimum of three years. The ages of the patients ranged from 35 to 69 years old, with a mean of (49.13±6.14 ) years old;their body mass index (BMI) ranged from 20.02 to 27.94 kg·m-2, with a mean of (23.65±1.69) kg·m-1;the duration of the disease ranged from 13 to 36 months, with a mean of (24.55±4.14) months. Preoperative and 3-year postoperative X-ray parameters were collected, including the anterior preserved angle(APA), lateral preserved angle (LPA), and combined preserved angle (CPA). Additionally, hip joint disability and osteoarthritis outcome scores (HOOS) and Harris hip scores (HHS) were recorded.
RESULTS:
Forty patients were followed up for a period ranging from 36 to 59 months, with a mean duration of (47.18±6.18) months. At 3 years postoperative, none of the patients underwent hip replacement surgery. The APA (73.15±19.35)°, LPA (75.35 ±21.48)°, and CPA (136.25±26.78)° at the 3-year postoperative significantly improved compared to preoperative measurements (61.93±20.54)°, (59.46±22.67)°, and (116.58±32.47)°, with statistical significance (P<0.05). The HOOS (20.37±1.39) and HHS (89.74±3.28) scores at the 3-year postoperative were significantly improved from preoperative scores (12.36±1.58) and (50.27±6.15), respectively, with statistical significance (P<0.05).
CONCLUSION
The direct anterior approach for window decompression and bone grafting surgery can relieve joint pain, improve joint function, and enhance X-ray preserved angles, effectively preventing femoral head collapse, making it an effective surgical method for treating ONFH classified as L1, L2 according to CJFH and stagesⅡ, ⅢA, ⅢB according to ARCO.

3.CT-assisted 3D printing template-guided implantation of 125I particles for the treatment of head and neck malignancies:a clinical study

Objective To evaluate the short-term clinical effect(including curative efficacy,pain relief,and quality of life)of CT-assisted 3D printing template-guided implantation of 125I particles in treating head and neck malignancies.Methods A total of 12 patients with head and neck malignant tumors complicated by moderate to severe cancerous pain,who were admitted to Yichang Municipal second People's Hospital to receive treatment from September 2019 to July 2024,were enrolled in this study.All patients received CT-assisted 3D individualized template-guided implantation of 125I particles therapy.The short-term curative efficacy,pain relief,and quality of life of patients were evaluated.Results Three months after implantation of 125I particles,complete response(CR)was obtained in 3 patients,partial remission(PR)in 5 patients,and stable disease(SD)in 4 patients,with an objective response rate(ORR)of 66.67%and a disease control rate(DCR)of 100%.The preoperative mean NRS score was(5.58±0.79)points,and the postoperative 3-month mean NRS score was(2.08±0.67)points.The postoperative 3-month physiological well-being(PWB),emotional well-being(EWB),functional well-being(FWB),and the total score were significantly better than their preoperative values,the differences were statistically significant(P<0.05).No statistically significant difference in social/family well-being(SWB)existed between its postoperative 3-month value and its preoperative value.Adverse reactions occurred in 2 patients,including local mucosal ulceration(n=1)and skin redness with swelling(n=1),which were improved after treatment.No serious complications occurred.Conclusion For the treatment of patients with head and neck malignant tumors complicated by moderate to severe cancerous pain,radioactive 125I particle implantation has a significant short-term effect,it can effectively relieve cancerous pain and improve quality of life of patients with reliable clinical safety.
4.Effect of vitamin D on NF-κB / CBS-H2 S signaling pathway in rats with chronic sciatic nerve injury

Objective:
To investigate the ameliorative effect of vitamin D on sciatic nerve chronic compression injury(CCI) rats and its regulatory effect on nuclear factor κB(NF-κB)/cystathione-β-synthase(CBS)-hydrogen sulfide(H2S) system.
Methods:
Fifty rats were randomly divided into five groups: control group,model group,vitamin D group,activator group,and activator + vitamin D group,with 10 rats in each group. Except for the control group,the rats in the other groups underwent sciatic nerve ligation to establish the CCI model. The vitamin D group received intraperitoneal injection of 500 mg/kg vitamin D,the activator group received intraperitoneal injection of10 mg/kg NF-κB activator lipopolysaccharide,and the activator + vitamin D group received intraperitoneal injection of 500 mg/kg vitamin D along with 10 mg/kg lipopolysaccharide. The control and model groups received intraperitoneal injection of an equal volume of saline,once per day for 2 weeks. The following parameters were compared among the groups: paw withdraw thermal latency(PWTL),mechanical withdrawal threshold(MWT),serum 25-hydroxyvitamin D3[25(OH)D3]levels,H2S content,calcitonin gene-related peptide(CGRP),prostaglandin E2(PGE2) levels,and the expression of vitamin D receptor(VDR),NF-κBp65,and CBS proteins in the L4-L6 dorsal root ganglia of rats.
Results:
Compared with the control group,the model group showed decreased PWTL,MWT,25( OH) D3 levels and VDR protein expression,while H2 S content,CGRP and PGE2 levels,and NF-κBp65 and CBS protein expression significantly increased ( P<0. 05) .Compared with the model group,the vi- tamin D group exhibited increased PWTL,MWT,25 ( OH) D3 levels and VDR protein expression ( P <0. 05) , while H2 S content,CGRP and PGE2 levels,and NF-κBp65 and CBS protein expression significantly decreased ( P < 0. 05) .Compared with the activator group,the activator + Vitamin D group showed increased PWTL,MWT, 25( OH) D3 levels and VDR protein expression,and decreased H2 S content,CGRP and PGE2 levels and decreased NF-κBp65,CBS protein expression ( P<0. 05) .Compared with the vitamin D group,the activator + vitamin D group showed decreased PWTL,MWT,25( OH) D3 levels and VDR protein expression,while H2 S content,CGRP and PGE2 levels,and NF-κBp65 and CBS protein expression significantly increased ( P <0. 05) .
Conclusion
Exogenous supplementation of vitamin D can relieve nerve pain and reduce pain sensitivity in CCI rats,possibly by inhibiting NF-κB / CBS-H2 S signaling pathway.
5.Weifuchun Alleviates Gastric Precancerous Lesions by Inhibiting Pyroptosis via NF-κB/GSDME Pathway

ObjectiveTo explore the role and molecular mechanism of Weifuchun (WFC) in inhibiting inflammation and alleviating gastric precancerous lesions (GPL). MethodHuman gastric mucosal epithelial cells (GES-1) were stimulated with N-methyl-N′-nitro-N-nitrosoguanidine (MNNG) for the modeling of GPL (MC cells), with Caspase-3 inhibition by Z-DEVD-FMK. MC cells were divided into control (20% blank serum), WFC (15% and 20% WFC-containing serum), and caspase-3 inhibitor groups. The cell counting kit-8 (CCK-8) was used to examine the viability of GES-1 cells or MC cells. The Transwell assay and 5-acetylidene-2′-deoxyuridine (EdU) staining were employed to examine cell invasion and proliferation, respectively. Flow cytometry was employed to determine the level of reactive oxygen species. Real-time PCR was conducted to determine the mRNA levels of interleukin (IL)-1, IL-6, and tumor necrosis factor (TNF)-α. Gene Expression Omnibus (GEO) was used to analyze the role of pyroptosis in gastric cancer progression. Western blotting was employed to determine the protein levels of nuclear factor-κB (NF-κB) p65, gasdermin E (GSDME), and Caspase-3. Immunofluorescence staining was employed to detect the NF-κB p65 protein level and nuclear translocation. Hematoxylin-eosin staining was carried out to observe the pathological changes in the gastric mucosa before and after WFC treatment in the patients. ResultCompared with the control group, MC cells presented enhanced proliferation and invasion energy (P<0.01). Compared with the blank serum group, WFC-containing serum inhibited the proliferation and invasion of MC cells (P<0.01), down-regulated the mRNA levels of IL-1, IL-6, and TNF-α, and lowered the level of reactive oxygen species (P<0.05, P<0.01). The transcriptome data at different stages of gastric cancer showed that pyroptosis was involved in gastric cancer progression, and the GSDME level was significantly higher in GPL patients than in the normal group. Compared with the blank serum, WFC-containing serum lowered the level of NF-κB and inhibited the nuclear translocation of NF-κB (P<0.05), and it inhibited pyroptosis by suppressing the cleavage of Caspase-3 on GSDME (P<0.05, P<0.01). The analysis of patient specimens further demonstrated that WFC treatment down-regulated the NF-κB level and GSDME cleavage (P<0.01), inhibited pyroptosis, and alleviated gastric mucosal inflammation and intestinal epithelial metaplasia. ConclusionPyroptosis is involved in the progression of gastric cancer, and WFC inhibits pyroptosis via the NF-κB/GSDME pathway, thereby alleviating gastric mucosal inflammation in GPL.

Professor ZHOU Peng has deeply discussed the pathological characteristics of psoriasis vulgaris,emphasizing that the disease is usually manifested deficiency interweaved with excess,leading to frequent recurrence and persistent refractory,which may lead to psychological and emotional problems of patients.This paper further expounds the effect of blood stasis on the pathogenesis,progression and prognosis of psoriasis,and puts forward a new method of combining Lingnan fire needling and filiform needling acupuncture technique to treat psoriasis vulgaris with blood stasis syndrome.Professor ZHOU Peng believes that the treatment principle of this disease is"regulating the mind first,rectifying blood as a base,syndrome differentiating and eliminating pathogenic factors",aiming at comprehensively considering the etiology and symptoms,in order to achieve more effective treatment results.Combined with the analysis of dermoscopic signs,it provides a possible improvement direction for the treatment of psoriasis vulgaris from a new perspective.
7.Clinical Observation of Warming-Needle Moxibustion Combined with Cross Acupoint Selection Method in Treating Knee Osteoarthritis

Objective To observe the clinical efficacy of warming-needle moxibustion combined with cross acupoint selection method in the treatment of knee osteoarthritis(KOA).Methods Fifty patients with KOA were randomly divided into observation group and control group,25 patients in each group.The control group was treated with warming-needle moxibustion on local of the knee joint,and the observation group was treated with elbow cross acupoint selection on the basis of the treatment of the control group.The course of treatment covered five days,with an interval of one day between courses of treatment,and a total of two courses of treatment.After two courses of treatment,the changes in Visual Analogue Scale(VAS)scores and Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC)scores in the two groups before and after treatment were observed.Changes in step frequency,step speed,step length,stride,and gait period between the two groups of patients before and after treatment were compared.Results(1)During the research,there was one case in the observation group and four cases in the control group lost to follow-up.Finally,24 cases in the observation group and 21 cases in the control group were included in the efficacy statistics.(2)After treatment,the scores of VAS and WOMAC of patients in the two groups decreased significantly(P<0.01),and the improvement in the observation group was significantly superior to that in the control group,and the differences were statistically significant(P<0.05).(3)After treatment,the step frequency and step speed of patients in the two groups were significantly improved compared with those before treatment,and the difference was statistically significant(P<0.05),and the parameters of step length and stride were slightly improved compared with those before treatment,but the difference was not statistically significant(P>0.05).After treatment,the observation group was slightly superior to that in the control group in improving the patients'step frequency,step length,step speed and stride,but the difference was not statistically significant(P>0.05).(4)After treatment,the gait period of patients in both groups were improved significantly(P<0.05),and the improvement in the observation group was significantly superior to that in the control group,but the difference was not statistically significant(P>0.05).Conclusion The efficacy of warming-needle moxibustion combined with cross acupoint selection method in the treatment of KOA is precise,with good analgesic effect,moreover,it can significantly improve the daily joint function of the patients,and improve the step frequency and step speed of patients with KOA and shorten the gait period,so as to improve the quality of life of patients,with remarkable therapeutic effect.

9.Rapid identification and differential markers of Curcumae Radix decoction pieces of different sources based on Heracles Neo ultra-fast gas phase electronic nose.

Since Curcumae Radix decoction pieces have multiple sources, it is difficult to distinguish depending on traditional cha-racters, and the mixed use of multi-source Curcumae Radix will affect its clinical efficacy. Heracles Neo ultra-fast gas phase electronic nose was used in this study to quickly identify and analyze the odor components of 40 batches of Curcumae Radix samples from Sichuan, Zhejiang, and Guangxi. Based on the odor fingerprints established for Curcumae Radix decoction pieces of multiple sources, the odor components was identified and analyzed, and the chromatographic peaks were processed and analyzed to establish a rapid identification method. Principal component analysis(PCA), discriminant factor analysis(DFA), and soft independent modeling cluster analysis(SIMCA) were constructed for verification. At the same time, one-way analysis of variance(ANOVA) combined with variable importance in projection(VIP) was employed to screen out the odor components with P<0.05 and VIP>1, and 13 odor components such as β-caryophyllene and limonene were hypothesized as the odor differential markers of Curcumae Radix decoction pieces of diffe-rent sources. The results showed that Heracles Neo ultra-fast gas phase electronic nose can well analyze the odor characteristics and rapidly and accurately discriminate Curcumae Radix decoction pieces of different sources. It can be applied to the quality control(e.g., online detection) in the production of Curcumae Radix decoction pieces. This study provides a new method and idea for the rapid identification and quality control of Curcumae Radix decoction pieces.

10.Automatic determination of mandibular landmarks based on three-dimensional mandibular average model.

OBJECTIVE:
To explore an efficient and automatic method for determining the anatomical landmarks of three-dimensional(3D) mandibular data, and to preliminarily evaluate the performance of the method.
METHODS:
The CT data of 40 patients with normal craniofacial morphology were collected (among them, 30 cases were used to establish the 3D mandibular average model, and 10 cases were used as test datasets to validate the performance of this method in determining the mandibular landmarks), and the 3D mandibular data were reconstructed in Mimics software. Among the 40 cases of mandibular data after the 3D reconstruction, 30 cases that were more similar to the mean value of Chinese mandibular features were selected, and the size of the mandibular data of 30 cases was normalized based on the Procrustes analysis algorithm in MATLAB software. Then, in the Geomagic Wrap software, the 3D mandibular average shape model of the above 30 mandibular data was constructed. Through symmetry processing, curvature sampling, index marking and other processing procedures, a 3D mandible structured template with 18 996 semi-landmarks and 19 indexed mandibular anatomical landmarks were constructed. The open source non-rigid registration algorithm program Meshmonk was used to match the 3D mandible template constructed above with the tested patient's 3D mandible data through non-rigid deformation, and 19 anatomical landmark positions of the patient's 3D mandible data were obtained. The accuracy of the research method was evaluated by comparing the distance error of the landmarks manually marked by stomatological experts with the landmarks marked by the method of this research.
RESULTS:
The method of this study was applied to the data of 10 patients with normal mandibular morphology. The average distance error of 19 landmarks was 1.42 mm, of which the minimum errors were the apex of the coracoid process [right: (1.01±0.44) mm; left: (0.56±0.14) mm] and maximum errors were the anterior edge of the lowest point of anterior ramus [right: (2.52±0.95) mm; left: (2.57±1.10) mm], the average distance error of the midline landmarks was (1.15±0.60) mm, and the average distance error of the bilateral landmarks was (1.51±0.67) mm.
CONCLUSION
The automatic determination method of 3D mandibular anatomical landmarks based on 3D mandibular average shape model and non-rigid registration algorithm established in this study can effectively improve the efficiency of automatic labeling of 3D mandibular data features. The automatic determination of anatomical landmarks can basically meet the needs of oral clinical applications, and the labeling effect of deformed mandible data needs to be further tested.
