BACKGROUND:
Immune checkpoint inhibitors (ICIs) have been included in various neoadjuvant therapy (NAT) regimens for non-small cell lung cancer (NSCLC). However, due to the relatively short period for the use of ICIs in NAT, patients' clinical outcomes with different regimens are uncertain. Our study aims to examine the efficacy of neoadjuvant immunotherapy (NAIT) for NSCLC patients and compare the overall survival (OS) and event-free survival (EFS) of patients receiving different NAT regimens.
METHODS:
This study retrospectively included 308 NSCLC patients treated with different NAT regimens and subsequent surgery in National Cancer Center between August 1, 2016 and July 31, 2022. Kaplan-Meier survival analysis and Cox proportional hazards regression analysis were conducted to evaluate the prognosis of patients.
RESULTS:
With a median follow-up of 27.5 months, the 1-year OS rates were 98.8% and 96.2%, and the 2-year OS rates were 96.6% and 85.8% in patients of the NAIT and neoadjuvant chemotherapy (NACT) group, respectively (hazard ratio [HR], 0.339; 95% confidence interval [CI], 0.160-0.720; P = 0.003). The 1-year EFS rates were 96.0% and 88.0%, and the 2-year EFS rates were 92.0% and 77.7% for patients in the NAIT and NACT groups, respectively (HR, 0.438; 95% CI, 0.276-0.846; P = 0.010). For patients who did not achieve pathological complete response (pCR), significantly longer OS ( P = 0.012) and EFS ( P = 0.019) were observed in patients receiving NAIT than those receiving NACT. Different NAT regimens had little effect on surgery and the postoperative length of stay (6 [4, 7] days vs . 6 [4, 7] days, Z = -0.227, P = 0.820).
CONCLUSIONS
NAIT exhibited superior efficacy to NACT for NSCLC, resulting in longer OS and EFS. The OS and EFS benefits were also observed among patients in the NAIT group who did not achieve pCR.

Objective:To compare the efficacy of body posture combined with pharmacotherapy and pharmacotherapy alone in the treatment of chronic subdural hematoma(CSDH).Methods:Firstly, retrospective case series study was conducted. Thirty cases of CSDH that had received body posture combined with pharmacotherapy at Department of Neurosurgery, Huashan Hospital Affiliated to Fudan University from December 2016 to October 2020 were studied retrospectively. Twenty-seven patients were male, and 3 patients were female. The age of patients ( M(IQR)) was 66(16) years (range:28 to 84). Nineteen patients had unilateral hematoma, and 11 patients had bilateral hematoma. All patients received pharmacotherapy and body posture therapy that was to raise their lower limbs 20 to 30 cm with leg lift pad and get abdominal compressed with customized abdominal belt in supine position. Patients were required to maintain the body posture as much as possible, with the maximum to 16 to 18 hours per day. Patients with unilateral hematoma should tilt the head to the affected side and avoid tilting it to the opposite side. For patients with bilateral hematoma, there was no need for head lateralization. Patient were treated with oral dexamethasone and atorvastatin simultaneously. The preliminary efficacy of body posture combined with pharmacotherapy was determined by hematoma improvement rate which was analyzed by Clopper-Pearson method. Then, the multi-center, prospective, randomized controlled trial had carried out in 9 medical centers from August 2020 to November 2021. The stratified block randomization method was adopted. Patients were randomized in a ratio of 1∶1 to either receive pharmacotherapy alone(the control group) or body posture combined with pharmacotherapy(the experiment group) for 3 months and followed up for 6 months. Effective treatment was defined as complete absorption of hematoma, or the hematoma volume decreased by more than 10 ml and Markwalder grading scale score had improved by more than 1 point compared to the baseline. The efficacy rate and surgery conversion rate at 3 months and recurrence at 6 months were observed. Comparison between groups was performed with paired sample t test, Mann-Whitney U test, χ2 test, corrected χ2 test, or Fisher exact probability method. Logistic regression was used to compare the effective rate and operation rate between the two groups. Results:In the respective study, 30 patients completed follow-up 13 to 353 days after treatment. At the last follow-up, the incidence of almost complete absorption or significantly absorption of hematoma (hematoma volume was significantly reduced accompanied by symptom improvement) was 93.3%. The 95% CI for the incidence that analyzed by the Clopper-Pearson method was 77.9% to 99.2%. One hundred and six patients were enrolled in the multicenter study. Fifty-five patients underwent body posture combined with pharmacotherapy. The age was 74(17) years (range:26 to 92). Thirty-nine patients were males and 16 were females. Fifty-one patients underwent pharmacotherapy alone. The age was 69(12) years (range:48 to 84). Thirty-seven patients were males and 14 were females. The length of body posture recorded in diary card was (15.7±2.3) hours(range:7.6 to 19.3 hours). The efficacy rate in the body posture combined with pharmacotherapy group and pharmacotherapy alone group were 83.6% (46/55) and 56.9% (29/51), respectively at 3 months. The result of the logistic regression analysis showed that the efficacy of body posture combined with pharmacotherapy group was better than that of pharmacotherapy alone group ( OR=3.88,95% CI:1.57 to 9.58, P=0.003). Surgery rate in the body posture combined with pharmacotherapy group and pharmacotherapy alone group were 5.5% (3/55) and 21.6% (11/51) respectively. The result of Logistic regression showed that the pharmacotherapy alone group was more likely to be converted to surgery ( OR=0.21,95% CI:0.05 to 0.80, P=0.023). At the 6 months, no recurrence of cases was found in the body posture combined with pharmacotherapy group. However, the recurrence rate of pharmacotherapy alone group was 6.3% (3/48), there was no significant difference between the two groups ( P>0.05). Conclusion:The effect of body posture combined with pharmacotherapy for chronic subdural hematoma is better than that of pharmacotherapy alone.

To investigate the current situation and demand for specialized development of allergy nursing in 88 hospitals in Guangdong Province, as well as to provide evidence for the management of allergy nursing. From August to December 2023, a cross-sectional study was conducted to analyze the core managers of allergy nursing-related work in eighty-eight hospitals in Guangdong Province supported by the Allergy Nursing Branch of the Guangdong Nurses Association and the Nursing Group of the Allergy Branch of the Guangdong Medical Association. An online survey was conducted using the Delphi method to compile a questionnaire on the current status and demand for specialized development of allergy nursing in Guangdong Province. Chi-square tests and Fisher′s exact probability method were used to analyze the differences in the current status of specialty technology applications and the development and demand of specialized nursing between hospitals with or without allergy specialty departments. The results showed that among the eighty-eight hospitals in the nineteen prefecture-level cities surveyed in Guangdong Province, 17 hospitals (19%) had established allergy specialty departments, all of which were top-tier tertiary hospitals. The establishment duration [ M ( Q1, Q3)] was 3 (1, 10) years. The implementation rates of skin prick test (94%, 16/17), in-vitro examination (82%, 14/17), nasal endoscopy (82%, 14/17), subcutaneous specific immunotherapy (82%, 14/17), multidisciplinary consultation (65%, 11/17), fractional exhaled nitric oxide detection (47%, 8/17), skin patch test (41%, 7/17), and autologous serum skin test (35%, 6/17) in hospitals with allergy department were higher than those in hospitals without allergy department [59%, 42/71 ( χ 2= 7.462, P=0.006)], [54%, 38/71 ( χ 2= 4.717, P=0.030)], [51%, 36/71 ( χ 2= 5.599, P=0.018)], [34%, 24/71 ( χ 2= 13.177, P<0.001)], [24%, 17/71 ( χ 2= 10.505, P=0.001)], [24%, 17/71 ( χ 2= 5.540, P=0.019)], [11%, 8/71 (Fisher′s exact test, P=0.008)], [11%, 8/71 (Fisher′s exact test, P=0.025)]. In the development plan of allergy nursing in the next two years, hospitals that had set up allergy departments had a higher proportion in adding nurses (82%, 14/17, χ 2= 13.177, P<0.001), formulating specialized nurse training plans (71%, 12/17, χ 2= 3.980, P=0.046), formulating group standards or clinical guidelines (41%, 7/17, Fisher′s exact test, P=0.046), and formulating assessment standards (53%, 9/17, Fisher′s exact test, P=0.002). The demand for all aspects of allergy specialty nursing knowledge and skills accounted for more than 50%, and it was not related to whether the hospital set up an allergy department. In conclusion, the specialized development of allergy nursing in Guangdong province is still in its early stages, and there is a great demand for improving knowledge and skills in allergy. Further standardization of specialty construction is needed, along with the training of allergic nurses, to promote the comprehensive development of allergic reaction nursing specialties from point to area.

Objective To investigate the predictive value of acute ischemic stroke associated pneumonia score (AIS-APS) combined with systemic immune inflammation index (SII) in the early clinical outcome of stroke-associated pneumonia (SAP). Methods A retrospective study method was conducted involving 287 AIS patients who were diagnosed and treated in Zhujiang Hospital,Southern Medical University from January 2022 to December 2023. The patients were divided into SAP group (79 cases) and non-SAP group (208 cases) according to whether SAP occurred. Collect the following data, include the patient's basic information[gender, age, body mass index (BMI),smoking history],previous history[atrial fibrillation, congestive heart failure, chronic obstructive pulmonary disease (COPD),diabetes history],laboratory test results[biochemical indicators:blood glucose, triglycerides (TG),total cholesterol (TC),high-density lipoprotein cholesterol (HDL-C),low-density lipoprotein cholesterol (LDL-C),glycated hemoglobin (HbA1c) and blood indicators:white blood cell count (WBC),neutrophilic count (NEU),lymphocyte count (LYM),platelet count (PLT)],SII,National Institutes of Health Stroke Scale (NIHSS) score at admission,AIS-APS,the difference of the above indexes between the two groups with different prognosis was compared. The risk factors for SAP were screened by Logistic regression analysis, and the predictive value of AIS-APS and SII for SAP was evaluated by receiver operator characteristic curve (ROC curve). Patients were followed up for 90 days to analyze the association between AIS-APS and SII and early outcomes. Use Pearson correlation analysis to analyze the correlation between SII and AIS-APS risk levels and patient prognosis. Results Compared with non-SAP group, patients with atrial fibrillation, congestive heart failure,COPD and proportion of patients with smoking history and TG,WBC,NEU,SII,NIHSS and AIS-APSY were significantly higher in SAP group[atrial fibrillation:13.92% (11/79) vs. 7.12% (15/208),congestive heart failure:32.92% (26/79) vs. 26.44% (55/208),COPD:34.17% (27/79) vs. 24.52% (51/208),smoking history:15.18% (12/79) vs. 7.69% (16/208),TG (mmol/L):1.79 (1.31,2.53) vs. 1.40 (1.03,1.99),WBC (×109/L):8.98 (6.78,9.15) vs. 6.02 (4.29,8.17),NEU (×109/L):5.20 (4.03,7.99) vs. 4.69 (3.31,5.53),SII:1145.9 (895.50,1346.3) vs. 545.80 (385.50,639.10),NIHSS score:6.00 (3.00,11.00) vs. 2.00 (1.00,4.75),AIS-APS:28.02 (14.29,33.17) vs. 22.98 (19.78,28.15),all P<0.05],while LYM was significantly decreased[×109/L:1.13 (0.95,2.25) vs. 1.62 (1.29,2.67),P<0.05]. Logistic regression analysis showed that atrial fibrillation, congestive heart failure,COPD,TG,SII and AIS-APS were all independent risk factors for AIS occurrence of SAP[odds ratio (OR) were 1.258,1.040,1.338,1.583,1.631 and 1.407,respectively,95% confidence interval (95%CI) were 1.093-1.210,1.006-1.109,1.145-1.274,1.205-1.874,1.504-1.759,and 1.267-1.673,respectively;P values were 0.002,0.045,0.037,0.034,0.010,0.018,respectively]. ROC curve analysis shows:SII,AIS-APS and their combined detection had predictive value for the occurrence of SAP. When SII and AIS-APS were combined, area under the curve (AUC) increased to 0.894,the sensitivity increased to 83.67%,and the specificity increased to 88.65%,P=0.012,which was significantly better than SII and AIS-APS alone. At 90 days of follow-up,SII and AIS-APS risk levels were found to be strongly associated with the prognosis of AIS patients, with higher SII and AIS-APS risk levels predicting poor prognosis. Pearson correlation analysis showed that both SII and AIS-APS risk levels were positively correlated with the prognosis of AIS patients (r values were 0.906 and 0.418,respectively,both P<0.05). Conclusion AIS-APS combined with SII can effectively predict the risk of SAP and is closely related to the early clinical outcomes of patients.
