1.A comparison of neuroendoscopic minimally invasive surgery and traditional extraventricular drainage for severe hypertensive intraventricular hemorrhage:a single-center retrospective study

Objective To analyze the safety and efficacy of neuroendoscopic minimally invasive surgery and traditional extraventricular drainage in the treatment of severe hypertensive intraventricular hemorrhage.Methods The clinical data of 50 cases with neuroendoscopic ventricular hematoma evacuation(endoscopy group)and 44 cases with traditional ventricles external puncture drainage(drainage group)from July 2020 to July 2023 were retrospectively analyzed,and the hematoma clearance rates,classification of activities of daily living(ADL)scale,incidence of hydrocephalus,secondary bleeding,intracranial infection,and pulmonary infection were observed between the two groups of patients.Results After surgery,the proportion of patients with hematoma clearance rate>60%and ADL grades Ⅰ,Ⅱ,and Ⅲ in the endoscopy group were significantly higher than those in the drainage group[the proportion of patients with hematoma clearance rate>60%:88.0%(44/50)vs.47.7%(21/44),χ2=17.794,P<0.001;the proportion of individuals with ADL grades Ⅰ,Ⅱ,and Ⅲ:94.0%(47/50)vs.77.3%(33/44),respectively,χ2=5.459,P=0.019],the incidence of complications in endoscopy group was significantly lower in the drainage group[8.0%(4/50)vs.34.1%(15/44),χ2=9.879,P=0.002].Conclusion Compared with traditional ventricular puncture drainage surgery,neuroendoscopic minimally invasive surgery for the treatment of severe hypertensive intracerebral hemorrhage with ventricular casting can achieve better treatment outcomes,a higher hematoma clearance rate,and fewer postoperative complications.
2.A case of anti-neutrophil cytoplasmic antibodies-associated vasculitis with ocular masses as the main manifestation

A case of anti-neutrophil cytoplasmic antibodies (ANCA)-associated vasculitis (AAV) with ocular masses as the main manifestation was reported. The patient was a middle-aged female, the initial symptom was eye swelling, pulmonary nodules were found before eye surgery, and further examination revealed proteinuria, hematuria and renal insufficiency. Renal pathology showed ANCA-associated glomerulonephritis. The final diagnosis was eye, kidney and lung lesions caused by AAV. Treatment with glucocorticoids and cyclophosphamide resulted in improvement in eye, kidney, and pulmonary lesions. Atypical clinical manifestations of AAV may lead to delayed diagnosis, and attention should be paid to the exclusion of AAV for ocular masses of unknown cause.

In 2016, a national one million general population cohort project was set up in China for the first time in "Precision Medicine Research" Key Project, National Key Research and Development Program of China, which consists of general population cohorts in seven areas in China. As one of the seven major areas in China, northeastern China has unique climate and specific dietary patterns, and population aging is serious in this area. And the burden of chronic and non-communicable diseases ranks tops in China. Therefore, it is of great significance to establish a large general population cohort in northeastern China to explore the area specific exposure factors related to pathogenesis and prognosis of chronic and non-communicable diseases, develop new prevention strategies to reduce the burden of the diseases and improve the population health in northeastern China. In July 2018, the general population cohort study in northeastern China was launched, the study includes questionnaire survey, health examination and blood, urine and stool sample collection and detection in recruited participants. By now, the cohort has covered all age groups, and the baseline data of 115 414 persons have been collected. This paper summarizes the design and practice of the general population cohort study in northeastern China to provide reference for related research in China.

6.Research progress on the relationship between ultra-processed foods intake and metabolic syndrome

Objective To analyze the epidemiological evidence on the relationship between ultra-processed foods intake and metabolic syndrome. Methods CNKI, Wanfang database, PubMed and Web of Science Core Collection database were searched to obtain literature, as of June 10, 2020, about ultra-processed foods intake and metabolic syndrome. The relevant literature was reviewed and analyzed. Results A total of 4 epidemiological studies about the relationship between ultra-processed foods intake and metabolic syndrome were found, of which three studies suggested a positive correlation, and one study suggested no correlation. Conclusion There may be a positive correlation between ultra-processed foods intake and metabolic syndrome, but the research results available are still controversial. Further research is needed to explore the relationship between ultra-processed foods intake and metabolic syndrome.
7.Risk factors predicting conservative treatment failure of acute calculous cholecystitis in pregnant women

Objective To explore the predictor of invalid conservative treatment of acute calculous cholecystitis (ACC) in pregnant women.Methods A total of 107 pregnant patients with ACC in a tertiary hospital from Jan.2010 to Jun.2017 were studied retrospectively.Results Univariate analysis revealed that body temperature > 37.3 ℃ (x2 =4.721,P < 0.05),gallbladder dilation (x2 =4.764,P < 0.05),TBiL (t=-2.518,P<0.05),ALT (t=-2.860,P<0.05),AST (t=-3.014,P<0.05),hospital stay (x2 =9.769,P < 0.05),rate of pregnancy termination (x2 =7.349,P < 0.05) were all significantly higher in conservative therapy failure group than those in the conservative therapy success group.Multivariate analysis showed that gallbladder dilation,TBiL,body temperature > 37.3 ℃,AST were the independent risk factors predicting invalid conservative management.Conclusion Gallbladder dilation,body temperature > 37.3 ℃,high level of TBiL and AST are the early predictors for the invalid conservative management for ACC in pregnant women.
8.Clinical study on modified Shenghua decoction combined with motherwort granules in treatment of postpartum placenta remnants

Objective To study the effects of modified Shenghua decoction combined with motherwort granules in treatment of postpartum placenta remnants.Methods136 selected patients with postpartum placenta remnants admitted into the hospital from June 2015 to October 2016 were divide into control group (68cases) and research group (68 cases).Control group was given motherwort granules and research group took modified Shenghua decoction on the basis of control group.The changes of endometrial thickness, residual area of placenta, TCM symptom scores, level of human chorionic gonadotropin (β-HCG), survival quality scores, menstruation recovery time, vaginal bleeding duration, cure time, occurrence of adverse reactions of two groups before and after treatment were compared, and efficacy of two groups were evaluated.ResultsAfter treatment, there was statistical difference in the endometrial thickness, residual area of placenta, TCM symptom scores, β-HCG level, survival quality scores, pain scores compared between the two groups (P<0.05);The menstruation recovery time, vaginal bleeding duration, cure time of research group were significantly lower than those of control group (P<0.05);There was statistical difference in the clinical efficacy compared between the two groups (P<0.05);There was no statistical difference in the incidence of adverse reactions between the two groups.ConclusionModified Shenghua decoction soup combined with motherwort granules for treating patients with postpartum placenta remnants can effectively relieve clinical symptoms, increase efficacy, reduce patients' pain, promote their recovery and improve survival quality, which has significant clinical advantages and can be further generalized and applied.
9.Long-term outcome after cardioverter-defibrillator implantation in patients with Brugada syndrome

Objective To observe the long-term outcome of implantable cardioverter-defibrillator (ICD) implantation in Brugada syndrome patients and to explore how to reduce the frequency of ICD inappropriate schocks.Methods This study included 14 symptomatic patients (mean age (44.3 ± 8.3)years old;all males) with Brugada syndrome implanted with ICD in our hospital between 1998 and 2012,and these patients were followed up routinely every 6 months.The initial ICD parameters were set according to conventional experience.The ventricular tachycardia (VT) zone was programmed to ventricular rate 150-188 bpm/cycle length (CL) 400-320 ms and the ventricular fibrillation (VF) zone was programmed to ventricular rate≥ 188 bpm/CL≤320 ms.The total events were recorded by ICD.The ICD parameters revision was made by electrophysiological (EP) experts in case of inappropriate shocks.Results Patients were followed up for mean (43.0 ± 28.3) months.A total of 297 VF/VT events were recorded by ICD.Electrophysiological experts found that 90% (178/198) episodes were true VF (CL 130-250 ms) among of 198 VF episodes and 147 VF episodes were terminated by one shock and 21 VF events were terminated by two or more shocks,and the rest 10 VF terminated spontaneously.Only 9% (9/99) VT events were true VT (CL 320-360 ms) among of 99 VT episodes.Eight VT episodes were converted by antitachycardia pacing therapy (ATP) and the other one terminated spontaneously.The rest 90 VT episodes (91%) were supraventricular arrhythmias (SVT,CL 340-390 ms).About 90% inappropriate shocks can be reduced by Wavelet discrimination function and optimal programming (VF zone ventricular rate ≥222 bpm/CL ≤ 270 ms and/or VT zone ventricular rate 167-222 bpm/CL 270-360 ms) according to the characteristics of arrhythmia of individual patient.Conclusion ICD can effectively prevent sudden cardiac death and syncope in high-risk patients with Brugada syndrome.The most common complication is inappropriate shock due to SVT.Optimal ICD programming with Wavelet discrimination function can effectively reduce the frequency of inappropriate shock rate.
10.Effects of renal denervation on left atrial fibrosis in rats with isoproterenol induced chronic heart failure

Objective To investigate the effects of renal denervation (RDN) on left atrial fibrosis in rats with chronic heart failure.Methods Sixty healthy male Sprague Dawley rats were randomly assigned to control group (n =10,intraperitoneal injection with 5 mg/kg normal saline daily for 3 consecutive weeks),sham group (n =25) and RDN group (n =25).Rats in sham and RDN group were intraperitoneally injected with 5 mg/kg isoproterenol daily for 3 consecutive weeks.RDN and sham RDN procedure was implemented at week 5.The renal arteries and veins were not isolated and the nerves were left intact in sham group.The experiment ended at week 10.Cardiac function,diastolic interventricular septal thickness (IVSD) and left atrial dimension (LAD) were evaluated by echocardiography at baseline,week 5 and 10.The rats of all three groups were sacrificed at week 10 and the left atrial tissue was used for following analysis:fibrosis was detected by Masson staining,plasma BNP was measured by ELISA kit,the protein expression of Ang Ⅱ,TGF-β1,MMP2 and collagen Ⅰ was determined by Western blot.Results (1) Cardiac function:compared with control group,LVEF decreased (P < 0.01),IVSD (P < 0.01) and LAD (P <0.01) increased significantly in the sham and RDN group at week 5.Compared with sham group at week 10,LVEF and IVSD significantly improved (P < 0.05) and LAD tended to be smaller (P >0.05) in RDN group.(2) The degree of left atrial tissue fibrosis:Masson staining (collagen volume fraction,CVF) showed significantly decreased fibrosis of left atrial tissue in RDN group compared with that in sham group (P < 0.01).(3) Plasma BNP level:ELISA assay revealed that plasma BNP in sham group was significantly increased compared with that in control group (P < 0.05) and was similar between RND group and control group at week 10.(4) Protein expression of AngⅡ,TGF-β1,MMP2 and collagen Ⅰ in rats left atrial:Western blot analysis demonstrated that the expression of Ang Ⅱ,TGF-β1,MMP2 and collagen Ⅰ was significantly down-regulated in RDN group compared to sham group (all P < 0.05) but still significantly higher than in control group (all P < 0.05).Conclusions RDN can effectively attenuate the left atrial fibrosis in rats with isoproterenol induced chronic heart failure.The attenuation of left atrial fibrosis by RDN in these rats may be attributed to improved cardiac function and downregulated pro-fibrogenic factors (Aug][,TGF-β1,MMP2 and collagen Ⅰ).
